BACKGROUND Richter's obturator hernia is a rare abdominal hernia that is difficult to diagnose. The purpose of this case report is to show an unusual presentation of a fatal Richter's obturator hernia that was accompanied by a femoral abscess. CASE REPORT An 89-year-old woman complained of sudden left coxalgia and a gait disorder but no abdominal symptoms. She had no history of trauma or surgery in the inguinal area. Twenty-three days after her first complaint of coxalgia, the patient was admitted in a coma with necrotizing fasciitis in the left inguinal area. The patient died of asystole due

PURPOSE: Symptom duration ≥ 72 h increases bowel resection risk in incarcerated obturator hernia, but onset is often imprecise, especially in patients without pain. We investigated whether hernia-sac computed tomography (CT) attenuation improves prediction beyond symptom duration.
METHODS: In this single-center retrospective cohort study of patients treated between 2006 and 2023, onset was determined from electronic medical records using the best available clinical information. On non-contrast CT, mean hernia-sac attenuation in Hounsfield units (HU) was measured using a circular region of int

INTRODUCTION AND IMPORTANCE: An obturator hernia is a rare condition accounting for less than 1% of abdominal wall hernias. Due to its deep pelvic location and nonspecific clinical presentation, it is frequently misdiagnosed, most often as an inguinal hernia. Delayed or incorrect diagnosis may result in bowel ischemia and perforation.
CASE PRESENTATION: We report the case of a 61-year-old man who underwent emergency open surgery at another institution for a presumed strangulated right inguinal hernia with placement of prosthetic mesh. No operative report was available regarding intraoperative

Obturator hernia (OH) is a rare but potentially life-threatening pelvic hernia associated with delayed diagnosis, bowel strangulation, and high mortality. We present a rare case of spontaneously reduced right obturator hernia documented by sequential computed tomography (CT) imaging and successfully treated with elective laparoscopic transabdominal preperitoneal (TAPP) mesh repair. A 67-year-old thin woman presented with acute lower abdominal pain associated with medial right thigh pain consistent with a positive Howship-Romberg sign. Initial non-contrast CT performed at a district hospital de

Obturator hernia, a rare condition that accounts for only 0.07-1% of all hernias, typically occurs in older adult, thin, multiparous women owing to weakening of the obturator membrane and increased intra-abdominal pressure. Conditions such as chronic constipation, chronic obstructive pulmonary disease, ascites, peritoneal dialysis (PD), and autosomal dominant polycystic kidney disease (ADPKD) are considered risk factors for an obturator hernia. Diagnosis is often delayed because of nonspecific symptoms, and computed tomography imaging is essential for early detection. We report the case of a 6

To evaluate the effectiveness of 4-hand reduction for obturator hernia with the guidance of sonography (FROGS) as a new treatment strategy for obturator hernia. Since November 2019, FROGS was performed for all patients with obturator hernia at our emergency department. We retrospectively compared the clinical data of 20 patients who underwent FROGS (after FROGS group) to those of 23 patients who did not (before FROGS group). All patients except one were female. The male-to-female ratio, age, duration of symptoms, lesion site, and predisposing factors did not significantly differ between groups

A modified technique for transposition of the internal obturator muscle was used to repair perineal hernias in 100 dogs. Complications and long-term results are described. The most important complications were wound infection (45%), faecal incontinence (15%), and perineal fistula (7%). These complications often occurred in combination. The recurrence rate of perineal hernia was 5%. Nine of the 15 patients with faecal incontinence had paresis of the external anal sphincter or faecal incontinence before surgery. We suggest that in numerous patients, faecal incontinence is a complication of the c

PURPOSE: We evaluated ultrasound-guided manual reduction as an initial management strategy for incarcerated obturator hernia, both as a bridge to elective surgery and as a pragmatic non-operative option in severely frail patients.
METHODS: We retrospectively analyzed consecutive patients who underwent attempted manual reduction for incarcerated obturator hernia between 2012 and 2025, excluding those with suspected bowel necrosis. The reduction success, associated factors, and subsequent outcomes were assessed.
RESULTS: Manual reduction was successful in 39/46 lesions (84.8%) without procedur

INTRODUCTION AND IMPORTANCE: Obturator hernia (OH) is a rare, potentially fatal cause of small bowel obstruction, mainly affecting elderly, thin females. Diagnosis is often delayed due to nonspecific symptoms, causing high morbidity and mortality. Left-sided OH is uncommon because of sigmoid colon protection. We present a left sided OH case managed with a novel appendiceal orifice decompression technique.
CASE PRESENTATION: An 80-year-old cachectic female (BMI 18 kg/m2) had five days of obstipation, bilious vomiting, abdominal distension, and pain. Clinical examination and abdominal radiograp

INTRODUCTION: An obturator hernia is an uncommon form of abdominal hernia that is difficult to diagnose due to its non-distinct presentation. This case investigates an emergency treatment of an obturator hernia presenting in a patient with an ipsilateral hip disarticulation in a 266-bed community hospital.
PRESENTATION OF CASE: A 53-year old man with a history of a left hip disarticulation 3-weeks prior presented to the emergency department with fever, nausea, vomiting, and diarrhea for the past 5-days. An elevated WBC and presence of gas within the hip stump on CT led to an emergency operati

OBJECTIVES: This retrospective study compared the outcomes of three surgical methods: traditional technique, internal obturator muscle flap, and sacroischial sling (TT, IOMF, and SS) for the treatment of canine perineal hernias. Postoperative complications associated with each technique were also compared.
MATERIALS AND METHODS: 87 dogs (86 males, 1 female) with perineal hernia were included in this study. Dogs were grouped based on the surgical technique used: TT (30 sites in 24 dogs), IOMF (30 sites in 26 dogs), and SS (53 sites in 37 dogs).
RESULTS: Surgical times were 36.8 ± 9.7 min for

A man in his 40's presented with vomiting and numbness in the right thigh. Eight months earlier, he had undergone robot-assisted radical cystectomy and pelvic lymphadenectomy for bladder cancer. Computed tomography (CT) revealed a caliber change and a closed loop of the small intestine in the right pelvis, prompting emergency diagnostic laparoscopy. Intraoperative findings showed that the small intestine had herniated into the space between the right obturator nerve and pelvic wall, resulting in strangulated intestinal obstruction. The necrotic small intestine was resected and reconstructed, w

Obturator hernia is a rare condition. Due to its deep pelvic location, surgical management is often challenging. We present the case of an elderly woman admitted with abdominal pain, vomiting, and no passage of stool or flatus for five days. Computed tomography revealed a right obturator hernia with an incarcerated ileal loop. A laparoscopic transabdominal preperitoneal (TAPP) repair was performed. After peritoneal incision and dissection of the Retzius and Bogros spaces, the incarcerated bowel loop was reduced. A self-gripping polyester mesh was placed to cover the myopectineal orifice and ob

Obturator hernia is a rare pelvic hernia that predominantly affects elderly, thin women. Despite advances in imaging and surgical techniques, the optimal surgical approach remains controversial. We retrospectively analyzed 10 patients who underwent emergency surgery for an obturator hernia at our institution between January 2010 and August 2024. All patients were female with a median age of 88.9 years (range: 84-97). Six patients (60%) required bowel resection. Seven underwent open repairs, and three underwent laparoscopic transabdominal preperitoneal (TAPP) repair. One patient with perforated

Obturator hernia is a rare clinical condition, mainly affecting the elderly and frail patients. It is often challenging to diagnose, and carries a significant morbidity and mortality risk. We aim to highlight the importance of timely and adequate diagnosis followed by early intervention that can be done as open or laparoscopic. Furthermore, through a literature review, we intend to compare two types of surgical approaches (open vs. laparoscopic). Herein, we present the case of an 83 years old female admitted as an emergency with right lower quadrant, colicky abdominal pain, followed with nause

Obturator hernia is a rare pelvic hernia that predominantly affects elderly, thin women and often presents as small bowel obstruction requiring emergency surgery. Although mesh repair reduces recurrence, placing mesh in a potentially contaminated field may be inadvisable when the acutely irreducible bowel is necrotic. We report an 87-year-old woman with an acutely irreducible right obturator hernia causing small bowel obstruction; intraoperative findings confirmed strangulation with focal bowel necrosis. Emergency repair was performed through a single-incision laparoscopic surgery (SILS) appro

BACKGROUND Herniation of the bowel through the obturator foramen is a rare cause of intestinal obstruction. It occurs more frequently in elderly, thin women and typically presents on the right side. Because of its deep pelvic location, diagnosis is often delayed, and emergency surgery is usually required. When reduction is difficult, alternative approaches, such as the double-incision technique, can facilitate safe and effective management. This report presents the case of a 67-year-old woman with acute right lower abdominal pain due to an incarcerated obturator hernia, managed with a double-i

A 63-year-old man who underwent robot-assisted laparoscopic low anterior resection and right lateral lymph node dissection (LLND) for rectal cancer presented with right thigh pain, nausea, vomiting, and abdominal pain on postoperative day 17. CT revealed dilated small bowel in the pelvis, and a small bowel loop was detected outside the internal iliac artery branch. Emergent laparoscopic surgery revealed the migration of the small bowel into the space beneath the right obturator nerve. The herniated bowel was reduced, and the obturator nerve was sharply dissected from the herniated bowel and pr

PURPOSE: Total extraperitoneal preperitoneal (TEP) repair is widely used for inguinal, femoral, or obturator hernia treatment. However, mesh repair is not often used for strangulated hernia treatment if intestinal resection is required because of the risk of postoperative mesh infection. Complete mesh repair is required for hernia treatment to prevent postoperative recurrence, particularly in patients with femoral or obturator hernia.
CASES: We treated four patients with inguinocrural and obturator hernias (a 72-year-old male with a right indirect inguinal hernia; an 83-year-old female with a

INTRODUCTION: Obturator hernia is an uncommon condition. With the widespread adoption of laparoscopic surgery, incidental detection of obturator hernias has become increasingly frequent. Bladder herniation through the obturator foramen is extremely rare, with only 14 cases reported to date. Herein, we describe a rare case of obturator bladder hernia successfully repaired via the laparoscopic transabdominal preperitoneal (TAPP) approach, highlighting a novel technical modification.
CASE PRESENTATION: A 74-year-old female was diagnosed with an obturator bladder hernia. The patient initially pre
