BACKGROUND: Efficient allocation of operating room (OR) time is critical in trauma centers, where unpredictable volumes lead to wasted resources. Forecasting trauma volume is challenging, but integrating exogenous factors with historical patterns may enable proactive scheduling tailored to daily operative demand. This study develops time-series and machine learning models to predict daily orthopaedic trauma operative caseloads.
METHODS: Data for patients undergoing operative management for orthopaedic trauma from January 2012 to December 2023 were aggregated. Models predicted daily caseload i

Jasmine is an 18-year-old Black female bringing her infant to the pediatrician for a newborn weight check. She asks her pediatrician's opinion about hormonal contraceptive injections, sharing that they were strongly recommended after she gave birth. The recommending health care provider told her, "We don't want you to end up back here any time soon." Rosita, a 16-year-old Latina female, visits her pediatrician for a well check. She reports a history of vaginal sex with 2 male partners and agrees to have a hormonal subcutaneous implant placed to avoid pregnancy. After 4 months of bothersome spo

Caring for the trauma patient requires an in-depth knowledge of the pathophysiology of trauma, the ability to rapidly diagnose and intervene to reverse the derangements caused by shock states, and an aptitude for the use of advanced monitoring techniques and perioperative point-of-care ultrasonography (P-POCUS) to assist in diagnosis and delivery of care. Historically, anesthesiology has lagged behind in wholly embracing this technology. P-POCUS has the potential to allow the trauma anesthesiologist to diagnose numerous injuries, quickly guide the placement of central vascular catheters and in

The carotid cavernous fistula has historically been associated with extensive facial trauma as a result of direct or indirect forces. Most fistulas of traumatic origin develop as a result of a fracture through the base of the skull, which produces a force causing laceration of the internal carotid artery in the region where it approximates the cavernous sinus. We report a case in which apparently minimal head trauma resulted in the development of a carotid cavernous fistula. The rather innocuous presentation of this complication requires particular attention by the caregiver in assessing the p

Trauma, including both historical adversity and acute injury, is highly prevalent in the general population. Over 50% of adults report adverse childhood experiences, and surgical patients frequently present with unaddressed psychological distress, which is significantly compounded by maxillofacial injuries and the potentially triggering nature of the care environment. Standard biomedical training typically lacks the necessary competencies for diagnosing, preventing, and managing these complex psychosocial needs. This review proposes and details a comprehensive, evidence-based framework for int

Venous thromboembolism (VTE) is a significant contributor to morbidity and mortality among patients with severe trauma. Historically, prophylactic inferior vena cava filters (IVCFs) were used in high-risk trauma patients with suspected risk factors for VTE, including prolonged immobilization, and concurrent contraindication to anticoagulation. Mounting data regarding the efficacy of IVCF in this cohort, as well as concerns regarding morbidity of an in situ IVCF, have challenged this practice paradigm. In this review, we discuss the comanagement of VTE and trauma, including anticoagulation and

Chest wall trauma is rapidly evolving and now represents a multidisciplinary field with incredible growth in research and surgical intervention; however, even with more than 800 publications on chest wall trauma to date, surgical indications are not black and white. Injury patterns need to be better defined and outcome measurements need to evolve for accurate longer term functional assessment of patients if this field of surgery is to move beyond historical indications for operative intervention. This essay will communicate what is known about operative indications in a way that stratifies the

Trauma to the globe is a common problem seen in the emergency department that may lead to significant permanent visual disability. Common causes of globe trauma include motor vehicle crashes, assaults, falls, and sports-related and occupational injuries. Rapid collection of accurate historical data, expedient physical examination, evaluation, and treatment are of paramount importance in preserving visual capacity. This article summarizes the initial diagnostic and therapeutic approach, important prognostic indicators, and delayed complications of the traumatized globe. Accurate diagnosis and p

Hemorrhage remains the leading cause of death from potentially survivable injuries in both military and civilian trauma. Whole blood (WB) has repeatedly proven to be the most physiologically coherent resuscitation strategy for life-threatening hemorrhage, yet modern transfusion systems have historically prioritized inventory efficiency and component fractionation over biologic fidelity. This syllabus synthesizes contemporary military and civilian evidence for WB resuscitation, with emphasis on fresh WB (FWB) obtained through walking blood banks (WBBs) and low-titer group O WB (LTOWB), the pred

Primary fibromyalgia syndrome (PFS) is a common form of nonarticular rheumatism with chronic and generalized musculoskeletal aching and stiffness, accompanied by tender points at characteristic sites in the absence of an underlying condition. No satisfactory criteria for its diagnosis, based on appropriate controlled studies, have yet been proposed. We undertook such a study which included a consecutive series of clinically diagnosed PFS and compared them with 3 control groups--mild rheumatoid arthritis, localized fibromyalgia secondary to trauma and normal controls. The concept of psychological trauma implies that people experiencing traumatic stress are wounded, thus relating to the metaphor of a physical injury. Although this notion is widely accepted by clinicians and researchers in mental health, there is evidence of a broad range of metaphorical idioms for extremely aversive experiences or catastrophic events across different cultures. In this ethnopsychological study, we aimed to investigate and contrast culturally shared metaphors for trauma among four distinct cultural groups: two indigenous communities (Pitaguary from Brazil, Adivasis from India)

Lunatic asylums formed part of the civic infrastructure that was constructed out of British colonists invading and subsequently colonising unceded, Indigenous Australian lands during the eighteenth and nineteenth century. This historical narrative examined nineteenth century primary and secondary sources including, patient lists, medical files, and government correspondence, to provide insight into the experiences of Indigenous Australians admitted to Australia's earliest lunatic asylums. Awareness that lunatic asylums formed part of the structure imposed during colonisation, provides nurses a

The disproportionate negative impact of the COVID-19 pandemic on Native communities is a result of transgenerational traumas-mental and physical-which have been ongoing and developing for centuries. This article considers 19th-century American visual and narrative representations of Native experiences of and responses to transgenerational trauma. This article also suggests ethical implications for Native American health of interpreting those representations and suggests an obligation to look on 19th-century White American artists' romanticizations of Native experiences with humility.

Florida's motorcycle insurance framework can separate individual risk-taking from the financing of catastrophic trauma care. When meaningful first-party medical coverage is absent or inadequate, unreimbursed costs may be shifted to hospitals, physicians, public programs, insured families, and taxpayers. This editorial argues that the resulting arrangement can function as an implicit subsidy of a privately chosen motor-vehicle risk and reviews historical Florida and contemporary out-of-state evidence describing that potential burden. Because the current statewide magnitude, payer distribution,

This article focuses on the concept of 'accident neurosis', popularised by neurologist Henry Miller in studies published in 1961. It aims to realise two goals. First, it introduces Miller's concept of accident neurosis to the broader history of trauma-to a field, that is, more preoccupied with military traumata and clear-cut psychiatric aetiologies. Secondly, I use Miller's studies, and the considerable legacy they created, to reflect on how historians of trauma construct historical narratives, asking whether there is sufficient appreciation of the ways in which events seem to leak into or ret

Peritoneum is a serous membrane with a significant fibrinolytic potential, playing an important role in the abdominal response to trauma. Peritoneum takes part in the formation and degradation of postoperative adhesions. The sequence of changes during the adhesion formation is indispensable in the healing of peritoneal trauma. Presented paper describes the short historical update of mesothelial research and review of contemporary knowledge over the peritoneal function with special regard to its fibrinolytic activity. The factors influencing the fibrinolytic capacity of peritoneum were discusse

BACKGROUND: Historically, resilience has often been conceptualized as the sustained lack of symptoms following trauma exposure. In line with a novel conceptualization of resilience as being dynamic over lifespan, determined by interacting biological and environmental factors, we examined the VA Mid-Atlantic Post Deployment Mental Health Repository (PDMH) comprised of 3876 US Military Veterans with and without PTSD diagnoses.
METHODS: We performed regression modelling to study the relationship between resilience (measured with Connor Davidson Resilience Scale; CD-RISC), posttraumatic stress di

Canada's Correctional Investigator has found that mental health disorders, alone or in combination with alcohol and drug abuse, challenge public health and safety. Trauma is a key contributor among Canada's inmate population. Therapy dogs can assist in supporting individuals with mental health, addiction, and trauma concerns. This case report presents the work of a St. John Ambulance therapy dog in a trauma-informed approach to prisoner health. The Substance Abuse and Mental Health Services Administration articulates six evidence-based trauma principles for service providers; safety; trustwort
