Evolution of understanding.

From the time of Hippocrates to the early 19th century, knowledge advanced but that was an uneven process. Anatomy was basically defined by Galen and remained cast in stone until the early 16th century. Neuroanatomy was described by Galen but had little practical value, as brain surgery was not possible. The anatomy of the cranium was known and was largely correct. Care was taken to avoid the frontal air sinuses and the venous sinuses and the temporal region. The role of the brain in consciousness was not understood. It was considered the seat of the soul but there was a lack of understanding

Historical awareness of the brainstem: From a subsidiary structure to a vital center.

The historical trajectory of the discovery of the brainstem as a vital center has been rarely explored. Focusing on its tracts and neurons first, anatomists much later expanded their understanding of the brainstem with the discovery of groups of nuclear networks that affected major vital functions. Comparative anatomists (i.e., Edinger) postulated a primordial paleopallium that indirectly implied the centrality of the brainstem and a neopallidum with its differentiation and specialization up to Homo sapiens Methods that governed the discovery of the brainstem were (1) comparative anatomy, (2)

Pathfinders in oncology from the end of the Middle Ages to the beginning of the 19th century.

During the 3 centuries reviewed herein, all that was known regarding cancer was limited to Italy, France, Germany, and England. Practically nothing was written about cancer in the rest of the known world, including the newly discovered geographic areas in the Americas, Africa, and Australia. The 7 pioneer physicians considered in this review lived through some of the most turbulent geopolitical, social, and religious upheavals that Europe had experienced during the Renaissance. Despite their diverse social, political, and religious backgrounds and disparate personalities, they were united in t

Teratology in cultural documents and today.

Teratology is the science of congenital malformations. The incidence of birth defects amounts to 2-3%, but it doubles postnatal owing to the fact that many dysfunctions are not discernible at birth. Congenital malformations were already known in ancient cultures, records from Assyrian and Babylonian astrologists as well as from physicians and philosophers of the Hippocratic era are testifying it. In medieval times they were recognized as supernatural phenomenons, terata, from what the term TERATOLOGY derived. In the eyes of the superstitious people affected stillborns were regarded as monster,

Conscience: A Brief History.

The lived experience of human moral agency suggests that some actions favour human flourishing while others lead to division and conflict: the universal human desire for happiness is bound up with the effort to discern and do 'good' and to avoid 'evil'. Words exist to convey an experience, and the word 'conscience' has emerged as an attempt to describe the experience of moral discernment. We offer an account of the development of the word 'conscience'. Our treatment is divided into three sections: the first beginning around 600 BCE until the arrival of Christianity; the second will cover the C

Tourette Syndrome Through the Ages: A Historical and Contemporary Overview.

Persistent tic disorders (PTDs), including Tourette syndrome (TS), are neurodevelopmental disorders characterized by the presence of multiple motor and vocal tics for at least 1 year. Nearly 1.4 million Americans have PTDs, including TS. The prevalence is underreported because 50% of individuals who meet criteria for TS are suspected to be undiagnosed. The first clinical descriptions of TS were documented in the 1400s. It was not until 1885 that TS was formally characterized, and not until the 1960s that the understanding of TS shifted from psychological toward current neurobiologic theories.

[Gout in the past and uric arthritis today in Croatia] ].

In the past, gout was well known in these places, from Istria to Dubrovnik. Our very first dictionaries also witness about gout. Old doctors from Krapinske Toplice had also written about gout. Dr Edmund Mai, in his article published in "Lijecnicki vjesnik" in 1904 mentions "171 cases of uric arthritis". But, after that, for long 60 years there wasn't even a word about our patients, except surprise on very high incidence of the disease in the past. Even our handbooks had claimed that uric arthritis is not common in our country. In 1964 after 60 years of silence, the first article about our pati

The historical evolution of the understanding of the submandibular and sublingual salivary glands.

The discovery of the major salivary glands was primarily a story of the establishment of the glands major excretory ducts. Occurring during the Renaissance: religious, political, and philosophical considerations played a role in defining the structure's function. We describe the history and background of these political, religious, and philosophical factors. Next, we present a translation of the original texts, describing the establishment of the submandibular and sublingual salivary glands. We place these translations into historical context and comment on their naming propriety. Initially we

A brief history of cortical functional localization and its relevance to neurosurgery.

Modern cortical mapping is a cornerstone for safe supratentorial glioma resection in eloquent brain and allows maximal resection with improved functional outcomes. The unlocking of brain functionality through close observation and eventually via cortical stimulation has a fascinating history and was made possible by contributions from early physician-philosophers and neurosurgery's founding fathers. Without an understanding of brain function and functional localization, none of today's modern cortical mapping would be possible.

On the Origins of Pituitary Apoplexy.

This paper sketches the early history of pituitary apoplexy, a disorder later fully described in 1950 by Brougham, Heusner and Adams. Haemorrhage or necrosis in an adenoma causes a characteristic sudden drowsiness, stupor or coma, headache and stiff neck, ocular palsy, and impaired acuity with visual field loss owing to optic nerve or chiasmal compression. The associated endocrinopathy and management are described.

Endemic syphilis in Europe.

Nonvenereal syphilis (endemic syphilis) has existed in Europe since the 16th century. Main characteristics of the disease are its presence for a longer time in a specific territory and its transmission regardless of age and sex, mainly extragenitally in unsanitary living conditions. Nonvenereal syphilis was described under different names in almost all regions of Europe. The primary genital chancre was absent, and lesions were most frequently found in the mouth and affected mostly children. The disease spread in rural areas with poor economic and hygienic conditions. The disease was eradicated

Dyspepsia and non-ulcer dyspepsia: an historical perspective.

Dyspepsia or indigestion is one of the most common disorders that is managed by general practitioners and gastroenterologists. Non-ulcer dyspepsia can be defined as upper abdominal pain or nausea in patients in whom endoscopy reveals no evidence of peptic ulceration or gastric cancer. Non-ulcer dyspepsia is a heterogeneous disorder and can be the result of such diverse entities as the irritable bowel syndrome, duodenitis or gastro-oesophageal reflux, or may be idiopathic ("essential" dyspepsia). This review traces the development of modern thought on dyspepsia and non-ulcer dyspepsia, from the

Jan Fryderyk Knolle's forgotten dissertation on osseous syphilis, 1763.

In 1763 Jan Fryderyk Knolle, a Polish physician, wrote his doctoral thesis on osseous syphilis, De Ossium Carie Venerea, at the University of Leipzig, Saxony (now East Germany). Knolle described the pathology, clinical picture, and treatment of syphilis of the bones and emphasized the severity and destructiveness of bony lesions, which could occur even in the early stage of the disease. An interesting and detailed drawing showing osteomyelitis of the femur, which appears to be of syphilitic origin, was included in Knolle's dissertation. This illustration is an example of 18th century syphilolo

Illustrations of the cerebrovascular system in Florentius Schuyl's Latin editions (1662, 1664) of René Descartes' Treatise on Man.

The Treatise on Man (1662, 1664) by French philosopher René Descartes (1596-1650) is the primary source for Cartesian physiology, but the accompanying illustrations were created after Descartes' death by a group of Descartes' disciples who were forced to create illustrations for Descartes' unfinished, and often vague and confusing, text. The iconographic tradition originating in the French edition (L'Homme, 1664) has predominated since the 17th century, whereas the Latin editions (De Homine, 1662, 1664) and their illustrations remain little known. Dutch physician and botanist Florentius Schuyl

Treating epilepsy: a review of Polish historical sources.

The first surviving Polish publications on epilepsy were written in the 16th and 17th centuries. Many causes of epileptic seizures are quoted and they are divided into two categories: internal and external. Internal causes (causa interna) include imbalance in the basic bodily humors, that is, yellow bile, black bile, phlegm, and blood. According to medieval writers, the principal cause of epilepsy was vapor, a damp, cold volatile substance originating in the excessive production of one of the basic organismic liquids. Vapor allegedly stuck to the openings leading to the cerebral ventricles or
