A Single-Item Screening Tool for the Assessment of Hoarding: Preliminary Observations.

OBJECTIVE: Hoarding disorder is defined as persistent difficulty discarding or parting with possessions, regardless of their actual value. Hoarding is associated with biopsychosocial distress and reduced quality of life, and although it is often associated with obsessive-compulsive spectrum disorders, hoarding is also encountered in cases of neurodegeneration. Assessments of hoarding behavior traditionally involve a comprehensive evaluation that may be challenging in clinical settings. The authors developed a simplified hoarding screen for patients with neurobehavioral disorders. METHODS: The

Attitudes to possessions in emerging adults: Predictors of hoarding behaviours and beliefs.

OBJECTIVE: Although hoarding symptoms are chronic and the average onset is late adolescence, younger cohorts have received little attention in research. Given the insidious symptom trajectory of hoarding and the unsatisfactory treatment outcomes in clinical groups, comprehensive research focusing on younger participants may reveal insights and suggest early intervention opportunities. DESIGN: Cross-sectional data were collected online from an emerging adult sample. METHOD: A total of 316 participants (aged 18-25) reported on hoarding symptoms, executive functioning, attention deficit/hyperac

A comparison between community and treatment-seeking samples of hoarding disorder.

OBJECTIVE: Hoarding disorder studies are primarily based on persons who seek treatment and demonstrate good insight. The aim of the present study is to evaluate whether there are differences between community and treatment-seeking samples of individuals with hoarding disorder (HD). METHODS: Fourteen people with HD from the community and twenty treatment-seeking people with HD were assessed by a battery of instruments to evaluate HD features and other associated characteristics. RESULTS: Compared to the treatment-seeking sample, the HD community sample was older, had poorer insight, and had a

Emotion dysregulation and hoarding symptoms: A systematic review and meta-analysis.

OBJECTIVES: Much of the research on hoarding is focused on cognition and behavior, with less focus on emotion and its regulation. METHOD: A comprehensive search yielded nine studies (out of 5581) from which to draw data for the current study. Across the eight studies (nine independent effect sizes) which provided data for 1595 total participants (Meanage  = 34.46, SD = 8.78; 64.26% females). RESULTS: Emotion dysregulation had a medium association with hoarding symptoms (r = 0.43). The effect was strong (r = 0.61) in some populations and weaker (r = 0.19) in others. However, it was higher in

Group cognitive and behavioral therapy and bibliotherapy for hoarding: a pilot trial.

BACKGROUND: Group cognitive behavioral treatments (GCBTs) for hoarding have produced modest benefits. The current study examined whether the outcomes of a specialized GCBT improve upon bibliotherapy (BIB) for hoarding, as part of a stepped care model. We also explored whether additional home assistance enhanced GCBT outcomes. METHODS: Hoarding patients (n = 38) were randomized and completed one of three conditions: (1) GCBT with nonclinician home assistants (GCBT+HA; N = 11), (2) GCBT without HA (CGBT; N = 14), and (3) BIB (N = 13). All GCBT participants received 20 weekly group sessions and

Hoarding, hermitage, and the law: why we love the collyer brothers.

Interest in hoarding behavior has intensified, as it works its way through DSM-V deliberations and treatment models. Meanwhile, both documentarians and fiction writers have embraced accounts of individuals with disposophobia and romanticized versions of the Collyer brothers, the Hermits of Harlem. In this article, I examine the range of media and professional attention given to hoarders and their problems and then focus on a potential role for forensic mental health professionals. The psycholegal problems of hoarders include health and zoning code violations that evolve into criminal charges,

Group cognitive and behavioral treatment for compulsive hoarding: a preliminary trial.

BACKGROUND: Time-limited group cognitive behavioral treatments (GCBT) for obsessive-compulsive disorder have demonstrated improvement in target symptoms. One small sample study of GCBT specifically for hoarding problems also showed benefit. This study examines the efficacy of a specialized GCBT for compulsive hoarding on a larger sample. METHODS: Thirty-two clients diagnosed with hoarding participated in five groups. Four groups met once weekly for 2 hour over 16 weeks (n=27) and one group met for 20 weeks (n=5). All participants had two individual 90-min home sessions. Self-report assessment

Diogenes syndrome in a patient with obsessive-compulsive disorder without hoarding.

Diogenes syndrome (DS) is characterized by self-neglect, classically portrayed by a filthy personal appearance, dirty home, and hoarding of rubbish. We report a patient with DS who presented obsessive-compulsive disorder and Tourette syndrome in the absence of hoarding. We suggest that hoarding may be a symptom of the conditions that are frequently comorbid with DS, but is not one of its fundamental features.

Daily Functioning in Low-Income Older Adults With Hoarding Disorder.

OBJECTIVES: Hoarding disproportionately affects older adults and has been linked with functional and cognitive impairment. However, existing work investigating is limited by methodological issues (e.g., absence of diagnostic interviewing) and little attention to low-income individuals or older adults. The present study sought to address these limitations in a large sample of low-income older adults diagnosed with hoarding disorder. METHODS: We administered measures of functional disability, hoarding severity, and cognitive functioning to 169 low-income older adults with hoarding disorder. RE

Psychiatric comorbidities in hoarding disorder: a structured clinical interview study.

OBJECTIVE: Hoarding disorder (HD) is a psychiatric condition characterized by persistent difficulty in discarding possessions, leading to significant clutter and functional impairment. HD is often comorbid with other psychiatric disorders, but previous findings of these comorbidities are inconsistent due to limited structured assessments. This study investigates the psychiatric comorbidities in individuals with HD using structured, clinician-administered interviews. METHODS: Between 2016 and 2022, 68 individuals diagnosed with HD were recruited from an outpatient clinic at Kyushu University H

Investigating associations between hoarding symptoms and affective and cognitive empathy.

OBJECTIVES: Hoarding is associated with poor interpersonal functioning, such as social isolation and difficulties in forming relationships, but the reasons for these social problems are not well understood. Previous studies have identified empathy as an important precursor to social functioning, particularly for clinical disorders characterized by social deficits. The aim of the current study was therefore to investigate associations between measures of cognitive and emotional empathy and hoarding symptoms. METHOD: A total of 278 participants recruited via MTurk completed online self-report q

Cognitive behavioral therapy for hoarding disorder: An updated meta-analysis.

BACKGROUND: Hoarding disorder (HD) is a new disorder in DSM-5. While cognitive-behavioral therapy (CBT) is seen as the gold standard approach to treatment, the literature in this field is still emerging. METHODS: The aim of the present study is to synthesize the current treatment outcome literature on CBT for HD, as well as secondary depressive symptoms, using a meta-analytic approach. Due to a lack of controlled trials only within-group effect sizes were calculated. RESULTS: Sixteen studies were included in the meta-analysis (n = 505; mean age = 56 years; mean percentage female participants

General public health considerations for responding to animal hoarding cases.

Animal hoarding is an under-recognized problem that exists in most communities and adversely impacts the health, welfare, and safety of humans, animals, and the environment. These guidelines address public health and worker safety concerns in handling situations where animal hoarding or other dense concentrations of animals have caused unhealthy and unsafe conditions. Because animal hoarding situations are often complex, a full response is likely to be prolonged and require a cross-jurisdictional multiagency effort. Each animal hoarding case has unique circumstances related to the types and nu

Sex-specific clinical correlates of hoarding in obsessive-compulsive disorder.

Little is known about whether the clinical correlates of hoarding behavior are different in men and women with obsessive-compulsive disorder (OCD). In the current study, we evaluated the association of hoarding with categories of obsessions and compulsions, psychiatric disorders, personality dimensions, and other clinical characteristics separately in 151 men and 358 women with OCD who were examined during the OCD Collaborative Genetics Study. We found that, among men but not women, hoarding was associated with aggressive, sexual, and religious obsessions and checking compulsions. In men, hoar

Hoarding behaviour: special features and complications in real-world clinical practice.

OBJECTIVE: Hoarding behaviour is a common but poorly characterised problem in real-world clinical practice. Although hoarding behaviour is the key component of Hoarding Disorder (HD), there are people who exhibit hoarding behaviour but do not suffer from HD. The aim of the present study was to characterise a clinical sample of patients with clinically relevant hoarding behaviour and evaluate the differential characteristics between patients with and without HD. METHODS: This study included patients who received treatment at the home visitation program in Barcelona (Spain) from January 2013 th
