An exploration of neural predictors of treatment compliance in cognitive-behavioral group therapy for hoarding disorder.

A persistent and influential barrier to effective cognitive-behavioral therapy (CBT) for patients with hoarding disorder (HD) is treatment retention and compliance. Recent research has suggested that HD patients have abnormal brain activity identified by functional magnetic resonance (fMRI) in regions often engaged for executive functioning (e.g., right superior frontal gyrus, anterior insula, and anterior cingulate), which raises questions about whether these abnormalities could relate to patients' ability to attend, understand, and engage in HD treatment. We examined data from 74 HD-diagnose

The things that make us: self and object attachment in hoarding and compulsive buying-shopping disorder.

Excessive or maladaptive object attachment is the defining feature of Hoarding Disorder (HD) and the acquisition process within Compulsive Buying-Shopping Disorder (CBSD). In recent years, the relationship of object attachment within HD and CBSD to individual's self-concept has become the focus of direct research. On the basis of this literature, it seems that an underdeveloped, ambivalent, or discrepant self-concept could be a vulnerability for dysfunctional object attachment behavior. Further, the importance of objects in buttressing and extending self-concept, both individually and in relat

Factors Affecting the Referral Rate of the Hoarding Disorder at Primary Mental Health Care in Quebec.

Hoarding disorder (HD) places an important burden on people with HD, on their family members and society. In this paper we evaluate help-seeking in HD at primary mental health, measured in referral rate, together with its individual, environmental and structural correlates. We conducted an aggregate study by combining existing official data with our own survey data at the catchment area level. We found a mean annual referral rate of 1.58 (SD = 1.79) cases of HD in primary mental health facilities per 10,000 of adult population. The referrals rate correlated with socio-demographic characteristi

Brexpiprazole Treatment in a Child With Autism Spectrum Disorder, Attention-Deficit/Hyperactivity Disorder (ADHD), and Trauma-Associated Hoarding Symptoms: A Case Report.

Children with autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) frequently present with multiple psychiatric comorbidities, often accompanied by severe emotional dysregulation and behavioral disturbances that complicate diagnosis and treatment. We describe a child with comorbid ASD and ADHD who also met diagnostic criteria for hoarding disorder and separation anxiety disorder and exhibited trauma-related symptoms. The patient showed minimal response to psychotherapy and conventional pharmacological interventions. Although comprehensive multidisciplinary support

Impact of Hoarding and Obsessive-Compulsive Disorder Symptomatology on Quality of Life and Their Interaction With Depression Symptomatology.

Hoarding disorder (HD) is a psychiatric condition characterized by difficulty discarding items and accumulation of clutter. Although studies have established the negative impact of HD and compulsive hoarding behavior, fewer have examined the impact on quality of life (QoL) of hoarding behavior independent of obsessive-compulsive disorder (OCD). Moreover, specific aspects of QoL such as success in work/academics or satisfaction with interpersonal relationships have not been well-investigated. In this study, we examined, in a sample of 2100 adult participants obtained from Amazon Mechanical Turk

Patient Perspectives on What Alleviates Hoarding Disorder Symptoms and Improves Wellbeing: A Systematic Review of Qualitative Data.

Hoarding disorder is a debilitating psychological disorder that is difficult to treat. Poor treatment response to current interventions indicates that new perspectives are needed. This systematic review examines qualitative and case study research on patient-centred strategies for reducing hoarding disorder symptoms and improving wellbeing. We conducted a comprehensive search of PsycInfo, Medline Complete, CINAHL, SocINDEX and Scopus in May 2025 to identify peer-reviewed qualitative research and case studies, published in English that examined hoarding disorder patients' first-hand experiences

An Examination of the Association Between Subjective Distress and Functional Connectivity During Discarding Decisions in Hoarding Disorder.

BACKGROUND: Individuals with hoarding disorder (HD) demonstrate exaggerated subjective distress and hyperactivation of cingulate and insular cortex regions when discarding personal possessions. No prior study has sought to determine whether this subjective distress is associated with specific profiles of abnormal brain function in individuals with HD. METHODS: We used multimodal canonical correlation analysis plus joint independent component analysis to test whether five hoarding-relevant domains of subjective distress when deciding to discard possessions (anxiety, sadness, monetary value, im

Treatment outcomes in older adults with hoarding disorder: The impact of self-control, boredom and social support.

OBJECTIVES: Hoarding disorder in older adults often develops in the context of co-occurring psychosocial maladies, and treatment response tends to be suboptimal. This preliminary investigation explored several ageing-related factors and their relationship to hoarding symptom severity (HSS), and examined treatment response to 15 sessions of cognitive behavioural therapy (CBT) with in-home support. METHODS: Twenty-nine participants (Mage = 67) completed self-report questionnaires measuring HSS, self-control, indecisiveness, depression, loneliness, social support and boredom, before (T1) and aft

The experience of leaving a valuable object: An investigation of emotional processes related to Hoarding disorder features.

One of the core features of hoarding is a significant resistance to discarding objects, which is fueled by dysfunctional beliefs and unwarranted negative emotions that hoarders tend to feel when disposing of their possessions. To our knowledge, longitudinal studies investigating the psychological effects that people who hoard experience after separating from their valuable possessions have yet to be conducted. Our study's principal aim was to explore psychological processes that individuals with high hoarding features (n = 53; 49.1%) and individuals with low hoarding features (n = 55; 50.9%) e

Long-term changes of hoarding symptoms among adults with attention-deficit/hyperactivity disorder (ADHD) treated with methylphenidate.

UNLABELLED: Hoarding disorder (HD) has been consistently linked to attention-deficit/hyperactivity disorder (ADHD), and ADHD patients show high rates of HD comorbidity. However, no studies have investigated the effects of ADHD medications on hoarding symptoms in patients with a primary diagnosis of ADHD. Thus, the aim of this naturalistic study was to investigate the long-term changes of hoarding symptoms among adult ADHD patients after at least one year of treatment with methylphenidate. METHODS: 60 newly diagnosed adult ADHD outpatients underwent a psychometric assessment which included hoa

The developmental origins of hoarding disorder in adolescence: a longitudinal clinical interview study following an epidemiological survey.

Hoarding disorder (HD) is hypothesized to originate in childhood/adolescence but little is known about the presentation of hoarding symptoms in youth and their natural history. In this longitudinal study, we tracked and conducted in-depth psychiatric interviews with twins who participated in an epidemiological survey and screened positive on a measure of hoarding symptoms at age 15. Twins screening positive for clinically significant hoarding symptoms at age 15 (n = 42), their co-twins (n = 33), a group of screen negative twins (n = 49), and their parents underwent a clinical assessment a medi

Hoarding in children and adolescents with obsessive-compulsive disorder: prevalence, clinical correlates, and cognitive behavioral therapy outcome.

Hoarding, common in pediatric obsessive-compulsive disorder (OCD), has specific clinical correlates and is associated with poor prognosis. However, there are few studies of hoarding in pediatric OCD. This study estimates the occurrence of hoarding symptoms in a sample of children and adolescents with OCD, investigating possible differences in demographic and clinical variables between pediatric OCD with and without hoarding symptoms. Furthermore, the study investigates whether hoarding symptoms predict poorer treatment outcomes after cognitive behavioral therapy (CBT). The study sample compris

Unbending mind: Individuals with hoarding disorder do not modify decision strategy in response to feedback under risk.

Cognitive-behavioral models of hoarding disorder emphasize impairments in information processing and decision making in the genesis of hoarding symptomology. We propose and test the novel hypothesis that individuals with hoarding are maladaptively biased towards a deliberative decision style. While deliberative strategies are often considered normative, they are not always adaptable to the limitations imposed by many real-world decision contexts. We examined decision-making patterns in 19 individuals with hoarding and 19 healthy controls, using a behavioral task that quantifies selection of de

Neurocognitive Profile of Late Life Hoarding Disorder Using the NIH Toolbox Cognition Battery as an In-Home Assessment.

OBJECTIVE: Adults with hoarding disorder report high levels of subjective cognitive impairment which have largely gone undetected in formal testing in laboratory settings; this discrepancy may be due to the testing environment. METHOD: This study examined associations between cognitive performance and hoarding severity in a sample of older adults diagnosed with hoarding disorder (N = 52) using the NIH Toolbox Cognition Battery administered in participants' homes. RESULTS: Participants demonstrated a cognitive profile that included weaknesses in attention, executive function, and processing s

Hoarding Symptoms in Children and Adolescents With Obsessive-Compulsive Disorder: Clinical Features and Response to Cognitive-Behavioral Therapy.

OBJECTIVE: Although adult hoarding disorder is relatively common and often debilitating, few studies have examined the phenomenology of pediatric hoarding. We examined the clinical phenomenology and response to cognitive-behavioral therapy (CBT) treatment in youths with a diagnosis of obsessive-compulsive disorder (OCD) with and without hoarding symptoms. Age was tested as a moderator across analyses, given prior findings that the impact of hoarding symptoms may not become apparent until adolescence. METHOD: Youths (N = 215; aged 7-17 years) with OCD pursuing evaluation and/or treatment at a

Treating hoarding disorder with compassion-focused therapy: A pilot study examining treatment feasibility, acceptability, and exploring treatment effects.

OBJECTIVES: Hoarding disorder (HD) was recognized as a psychiatric disorder in 2013. Existing literature suggests room for improvement in its treatment. The current pilot study aimed to provide an initial evaluation on the potential of compassion-focused therapy (CFT) as an intervention for HD, with the primary aim being assessing its feasibility and acceptability, and the secondary being evaluating its effects. DESIGN: Both CFT and a second round of the current standard of treatment and cognitive behavioural therapy (CBT) were investigated in the current study as follow-up treatment options

Exploring the effects of cognitive behavioral therapy on cognitive control circuit and behavioral task performance in hoarding disorder.

Hoarding disorder (HD) is characterized by difficulty discarding or parting with possessions and clutter, which causes distress or impairment in important areas of functioning. Cognitive behavioral therapy (CBT) for HD has shown promise; however, little is known about the brain mechanisms underlying symptom reduction. We previously reported the robust clinical effects of the Buried in Treasures workshop (BIT)-a skills-based group which incorporates CBT principles-augmented with uncluttering home visits (BIT+), in a waitlist-controlled trial involving adults with HD. This study examined neural
