Alprazolam augmentation of the antipsychotic effects of fluphenazine in schizophrenic patients. Preliminary results.

Alprazolam was added, under double-blind conditions, to stable fluphenazine hydrochloride regimens in 12 symptomatic, chronically ill inpatients with schizophrenia. The addition of alprazolam was associated with significant, albeit modest, reductions in global psychosis, thought disorder, and paranoia ratings, with a return to pretreatment levels on discontinuation of alprazolam treatment. Improvement in "negative symptoms" such as emotional withdrawal paralleled the changes in "positive symptoms" but did not, in itself, reach statistical significance. There were no significant changes in grou

Familial glucocorticoid deficiency with achalasia of the cardia associated with mixed neuropathy, long-tract degeneration and mild dementia.

This paper describes the progress of two previously reported brothers with familial glucocorticoid deficiency, achalasia of the cardia, and alacrima. In their early 'teens both boys developed polyneuropathy with sensory, motor and autonomic components, Parkinsonism, and signs of both dorsal column and pyramidal tract damage. The older boy also showed signs of dementia. Red cell folate levels were markedly reduced but plasma and CSF folate were normal. Serum B12 and erythrocyte concentrations were at or below the lower limit of normal CSF levels of homovanillic acid and 5-hydroxyindole acetic a

Catha edulis chewing effects on treatment of paranoid schizophrenic patients.

BACKGROUND: The current study's aim is to evaluate the possible interaction effects of khat chewing on treatment of paranoid schizophrenic patients. PATIENTS AND METHODS: In the study group, 42 male subjects suffered from paranoid schizophrenia and were classified according to their khat chewing habits into two subgroups: either khat-chewer subgroup (SKc; n=21; r=11, h=10) or non-khat-chewer subgroup (SNKc; n=21, r=11, h=10). Each subgroup was further subdivided according to type of treatment into r (risperidone) and h (haloperidol). Healthy male subjects (37) were subdivided into healthy kha

Pharmacological approaches to treatment of hemiballism and hemichorea.

Hemiballism is an involuntary uncontrollable movement disorder with grave prognosis. Post stroke anatomical lesions of the subthalamic nucleus are the most frequent but not sole site. Increased cerebrospinal fluid homovanillic acid levels and successful management of hemiballistic symptoms with neuroleptics have suggested a dopaminergic overactivity as the neurochemical pathology. Diazepam, a gamma amino butyric acid (GABA) mimetic drug, has recently been reported to possess therapeutic efficacy. Hemiballism is a treatable condition which responds to neuroleptics and possibly GABA mimetic drug

Acting out hostility in normal volunteers: negative correlation with levels of 5HIAA in cerebrospinal fluid.

We examined relationships in normal volunteers between cerebrospinal fluid (CSF) concentrations of the serotonin metabolite 5-hydroxyindoleacetic acid (5HIAA) and scores on the Hostility and Direction of Hostility Questionnaire. There was a significant negative correlation between CSF 5HIAA levels and scores on the "urge to act out hostility" subscale, as well as a similar nonsignificant trend with CSF levels of homovanillic acid. These results suggest that among normal individuals reduced central serotonergic tone may be associated with impaired impulse control.

Gilles de la Tourette's syndrome: biochemical approaches.

Haloperidol, a dopamine receptor blocking agent, is the most effective therapy for Tourette's syndrome. In five patients with Tourette's syndrome, we found in the CSF an elevated probenecid-induced accumulation of HVA, the major metabolite of dopamine. This supports the hypothesis that Tourette symptoms are related to an increased firing of dopaminergic neurons in the central nervous system; haloperidol relieves these symptoms by blocking dopamine receptors. Some similarities of Tourette's syndrome to Lesch-Nyhan's syndrome prompted us to compare these two disorders, obtaining data from a larg

Elevated plasma monoamine metabolites in eating disorders.

Mean fasting plasma homovanillic acid (HVA) levels were elevated in females hospitalized with eating disorders (both anorexia and bulimia nervosa) compared with females hospitalized with adjustment disorder. Fasting plasma 3-methoxy-4-hydroxyphenylglycol (MHPG) was elevated in anorectic patients only. Five patients with eating disorder and psychosis had higher values for HVA and MHPG than the remainder of the patients with eating disorder.

Plasma catecholamine metabolites and treatment response at neuroleptic steady state.

Using either haloperidol or perphenazine in a fixed-dose protocol, plasma free homovanillic acid (HVA) and methoxyhydroxyphenethylglycol (MHPG) were decreased in 37 nonorganic psychotic inpatients at neuroleptic steady state (7-9 days) in comparison with pretreatment values. The data indicate that the magnitude of the decline in HVA and MHPG was associated with treatment response and not with neuroleptic plasma levels.

Dopamine transmission in DYT1 dystonia: a biochemical and autoradiographical study.

Indices of dopamine transmission were measured in the postmortem striatum of DYT1 dystonia brains. A significant increase in the striatal 3,4-dihydroxyphenylacetic acid/dopamine ratio was found. Quantitative autoradiography revealed no differences in the density of dopamine transporter or vesicular monoamine transporter-2 binding; however, there was a trend toward a reduction in D(1) receptor and D(2) receptor binding. One brain with DYT1 parkinsonism was similarly evaluated and marked reductions in striatal dopamine, 3,4-dihydroxyphenylacetic acid, and homovanillic acid content as well as the

Cerebrospinal fluid 5-HIAA concentrations correlate with cardiac uptake of 123I-MIBG during myocardial scintigraphy in drug naïve Parkinson's disease.

We examined the correlations between cerebrospinal fluid (CSF) concentrations of homovanillic acid (HVA) and 5-hydroxyindole acetic acid (5-HIAA) and imaging assessment scores, using 123I-Ioflupane SPECT and 123I-MIBG myocardial scintigraphy in 23 drug naïve PD patients. The CSF 5-HIAA concentration correlated with the H/M ratio of the delayed image (r = 0.458, p < 0.05) and the washout rate (r = - 0.642, p < 0.01) of 123I-MIBG myocardial scintigraphy. These correlations suggest some unclarified pathophysiological links between the central serotonergic and cardiac sympathetic systems.

Noradrenergic overactivity in chronic schizophrenia: evidence based on cerebrospinal fluid noradrenaline and cyclic nucleotide concentrations.

Concentrations of noradrenaline (NA), homovanillic acid, 5-hydroxyindoleacetic acid and cyclic nucleotides were determined in lumbar cerebrospinal fluid (CSF) from acute and chronic schizophrenics and various groups of psychiatric and non-psychiatric control subjects. Statistically significant increases in NA and cyclic adenosine monophosphate were found in CSF from chronic schizophrenics compared to all other groups. These results were shown by statistical analyses to be unrelated to medication. They may be interpreted as evidence for noradrenergic overactivity as a possible primary abnormali

Pretreatment plasma HVA predicts neuroleptic response in manic psychosis.

Elevated pretreatment plasma free homovanillic acid (HVA) predicted acute response to neuroleptic treatment in patients with manic psychosis. These findings suggest that plasma HVA may be a useful predictor of a successful short-term response in manic as well as schizophrenic psychoses, and that elevated pre-synaptic dopaminergic release may play a role in more than one group of psychotic disorders.

Narcolepsy: biogenic amine deficits in an animal model.

Concentrations of biogenic amine metabolites in discrete brain areas differed significantly between dogs with genetically transmitted narcolepsy and age- and breed-matched controls. Dopamine and 3,4-dihydroxyphenylacetic acid were consistently elevated in the brains of narcoleptic animals, while homovanillic acid was not. Narcoleptic animals consistently exhibited lower utilization of dopamine and higher intraneuronal degradation of dopamine but no uniform decrease in serotonin utilization. Hence neuropathology appears to be associated with genetically transmitted canine narcolepsy. The data i

(+/-)-CTC, a cyproheptadine derivative, possesses both antidopaminergic and anticholinergic activities which can be resolved, respectively, into its component (-)- and (+)-enantiomers. Both in vivo (antagonism of apomorphine-induced stereotypy, elevation of striatal homovanillic acid) and in vitro (inhibition of [3H]haloperidol binding), (-)-CTC was less active than haloperidol but more potent or equipotent compared to chlorpromazine. (+)-CTC was a more potent anticholinergic agent in vitro (inhibition of [3H]quinuclidinyl benzilate binding) than either thioridazine or clozapine, whereas in vi

Clinical pharmacokinetics of levodopa in parkinson's disease.

Although levodopa has provided a major advance in the treatment of parkinsonism, its maximum benefits have not yet been realised, in part because of its complicated pharmacokinetics. This review summarises that available pharmacokinetic data involving levodopa, especially as it relates to therapeutic response of parkinsonian patients. A large number of factors, including protein intake, gastric emptying time, pyridoxine ingestion, and dopa decarboxylase activity, affect plasma levels of levodopa attained following oral administration of this drug. Other variables influence the rate of brain up

Secondary neurotransmitter deficiencies in epilepsy caused by voltage-gated sodium channelopathies: A potential treatment target?

We describe neurotransmitter abnormalities in two patients with drug-resistant epilepsy resulting from deleterious de novo mutations in sodium channel genes. Whole exome sequencing identified a de novo SCN2A splice-site mutation (c.2379+1G>A, p.Glu717Gly.fs*30) resulting in deletion of exon 14, in a 10-year old male with early onset global developmental delay, intermittent ataxia, autism, hypotonia, epileptic encephalopathy and cerebral/cerebellar atrophy. In the cerebrospinal fluid both homovanillic acid and 5-hydroxyindoleacetic acid were significantly decreased; extensive biochemical and ge

Autosomal dominant GTP-CH deficiency presenting as a dopa-responsive myoclonus-dystonia syndrome.

The authors report a kindred in which GTP-CH deficiency resulted in a myoclonus-dystonia syndrome. The proband, a 17-year-old boy, presented with early-onset myoclonus and later, dystonia and bradykinesia. Blood prolactin was increased and CSF homovanillic acid, 5-hydroxyindoleacetic acid, and biopterin were all reduced. L-Dopa/carbidopa administration resulted in clinical improvement. In the paternal branch, the grandfather and three relatives had myoclonus-dystonia and resting or postural tremor of limbs. The authors found a missense mutation in the exon 6 of GCH-1 gene (K224R).

Effects of aging on cochlear monoamine turnover.

The aging of the cochlear dopaminergic system has been analyzed by quantifying the levels of dopamine (DA) and its metabolites (3,4-dihydroxyphenylacetic acid, DOPAC, and homovanillic acid, HVA) in adult rats aged 3, 12 or 24 months. The main results were an increase in DA, DOPAC and HVA basal concentrations in aged females with respect to the adults (3 or 12 months old), while just DA and DOPAC increased in aged males. A higher synthesis of DA in aged animals could support these findings, which could indicate some kind of compensatory mechanism related to presbycusis.

The biogenic amines, dopamine, serotonin, and noradrenaline, and their respective metabolites, homovanillic acid, 5-hydroxyindoleacetic acid, and hydroxymethoxyphenylglycol, were measured in selected brain regions obtained at postmortem examination from 4 patients, ages 12-30 years, with typical features of Rett syndrome. A 50% or greater reduction in each compound, except hydroxymethoxyphenylglycol, was observed in the substantia nigra from the 2 older patients, while the youngest patient had normal or nearly normal levels. Enhanced monoaminergic neurotransmission by desglycinamide-arginine-vasopressin in human subjects.

The effect of desglycinamide-arginine-vasopressin (DGAVP) on monoaminergic neurotransmission was studied in human subjects. Samples of cerebrospinal fluid (CSF) were collected during 240 min after DGAVP (2 mg) had been administered intranasally, and monoamine metabolites in CSF were measured with HPLC using electrochemical detection. Levels of homovanillic acid and 5-hydroxyindoleacetic acid increased (P less than 0.05) 150 min after DGAVP administration, whereas levels of 3-methoxy-4-hydroxyphenylglycol were stable during the time monitored. These results suggest that in human subjects the me
