Both oral contraceptives (OCs) and hormone replacement therapy (HRT) are widely used by healthy women, so that any increase in breast cancer has to be considered in relation to the avoidance of either pregnancy or menopausal symptoms. Use of OCs for more than four years before first full term pregnancy leads to a small but significant increase in risk of breast cancer (6.5 extra cases per 10,000 women aged 20-29) that persists for 10 years after cessation. Risk is not amplified by a positive family history and OC-induced breast cancers are more likely to be localised to the breast. For those t

It has been suggested that oestrogen replacement therapy is associated with risk of epithelial ovarian cancer of the endometrioid type. Using data from an Australian population-based case-control study, the relation between unopposed oestrogen replacement therapy and epithelial ovarian cancer, both overall and according to histological type, was examined. A total of 793 eligible incident cases of epithelial ovarian cancer diagnosed from 1990 to 1993 among women living in Queensland, New South Wales and Victoria were identified. These were compared with 855 eligible female controls selected at

Although the cardiovascular effects of hormone replacement therapy of menopause have been controversial for a long time, the benefit of this treatment currently appears to be well established, despite the methodological problems of most studies and the absence of definitive demonstration by a prospective randomized study. The mechanism of this favourable action appears to be multifactorial with, in particular, an improvement of the lipid profile. In the absence of any gynaecological contraindication, the combination of a natural oestrogen and a progestogen devoid of any androgenic effect there

We report our experience with 25 women previously treated for breast cancer who subsequently received hormone replacement therapy (HRT) for the relief of menopausal symptoms and the prevention of postmenopausal cardiovascular disease and osteoporosis. Two patients had in situ disease, 13 had stage I disease, 7 had stage II disease, 1 had stage III disease, and 2 had invasive cancer of undetermined stage. Seventeen patients (group I) began HRT less than 24 months after primary breast cancer therapy, and 8 patients (group II) began HRT more than 24 months after breast cancer therapy. The HRT-fre

The time of menopause provides an excellent opportunity for physicians to educate their patients toward preventive care that involves life-style modification and disease screening. It is also the time to consider appropriate hormone replacement therapy. Most postmenopausal women are candidates for such therapy, which has beneficial effects on serum cholesterol levels and other cardiovascular risk factors and protects against osteoporosis, urogenital atrophy, and vasomotor symptoms. A small number of patients have specific risk factors for or contraindications to hormone replacement therapy. In

Gonadotropin-releasing hormone agonists are effective in the treatment of endometriosis and myomas, both of which are estrogen-dependent processes, but there is a high clinical recurrence rate after therapy is discontinued. Long-term continuous therapy (2 years or more) has a cumulative effect on bone loss and causes other uncomfortable or harmful side effects. Noninvasive assessments of disease response in patients with myomas have shown that bone changes might be prevented and other side effects of long-term therapy can be alleviated by adding back small amounts of estrogen or progestin. Hormone replacement therapy (HRT) is given to relieve the climacteric symptoms of menopause. Use of HRT reduced after a report from the Women's Health Initiative linked it to an increased risk of breast cancer. This association has been confirmed in several other studies, including the Million Women Study. The risk of breast cancer is greater for formulations that contain both estrogen and progesterone, compared with estrogen alone. The breast cancer risk associated with HRT is higher for estrogen receptor-positive cancers than for estrogen receptor-negative cancers, and for low-grade cancers

Hormone replacement therapy (HRT) has had a chequered history ever since its initial use to manage menopausal symptoms. It is clear that it has many other effects and here we review its impact on the risk of breast cancer. A clear risk is seen for current uses of combined oestrogen/progestagen pills, but this returns to normal shortly after treatment cessation. The role of oestrogen only replacement therapy is less clear, but most studies find a weaker, but still positive, association in current users. Recent sharp reductions in HRT use have been correlated with declines in breast cancer incid

Thousands of women are treated each year for gynaecological cancers; many of these are already in menopause, while other younger patients will go into early menopause due to surgery, chemotherapy and/or radiotherapy to the pelvic region. The aim of this paper is to review the biological and clinical evidence in favour and against hormone replacement therapy (HRT) use after gynaecological cancers. With the exception of breast and endometrial cancer, there is no biological evidence that HRT may increase the recurrence risk. In women with previous endometrial cancer, HRT use is not supported by u

There is a growing use of hormone replacement therapy during menopause in the western world. Although most of epidemiologic studies have not found a cause-effect relation between this therapy and breast cancer, the fear of breast cancer related to this therapy limits its vast use. Controversy continues to surround the issue of the relationship between postmenopausal hormone therapy and the risk of breast cancer. Physicians, willing to prescribe this treatment, have often been asked by their patients if these hormones can cause breast cancer. Sufficient evidence exists to indicate the possibili

A review of the literature indicates that hormone replacement therapy (HRT) in post-menopausal women not only reduces the frequency of hot flushes and sweating episodes but also has a protective effect as regards the post-menopausal osteoporotic process. However, it has not yet proved possible to arrive at any conclusions concerning the overall effect of HRT on either ischaemic heart disease and other cardiovascular problems or the risk of developing malignant neoplastic disease. Nevertheless, there is evidence that oestrogen replacement in combination with a progestogen will reduce the risk o

The effects of growth hormone (GH) treatment on protein metabolism were studied in adults with GH deficiency (GHD). A double-blind, placebo-controlled trial of recombinant human GH, 0.018 IU/kg/day for 1 month followed by 0.036 IU/kg/day for 1 month, was performed with isotopic whole-body protein turnover studies at 0 and 2 months. In all, 18 adults with GHD (9 men, 9 women; mean age, 46.6 years; range, 30-56 years) were studied. Whole-body isotopic leucine turnover using L-[1-13C]leucine was assessed by measuring leucine Ra (a measure of protein degradation), non-oxidative leucine Rd (a measu

Menopause hormone replacement therapy (HRT) remains the main strategy for managing menopausal symptoms and preventing osteoporosis in postmenopausal women. However, its cardiovascular effects are complex and influenced by multiple factors. Early initiation of HRT within 10 years of menopause onset consistently demonstrates cardiovascular benefits, whereas delayed initiation may increase risks such as stroke and venous thromboembolism. Transdermal and bioidentical hormones generally show a safer cardiovascular profile compared to oral synthetic preparations. Current guidelines advocate for indi

This review examines hormone replacement therapy (HRT) in cases of surgical menopause following gynecological malignancies. It aims to capture current knowledge, summarize recent findings, and provide recommendations for clinical settings. Unlike natural menopause, surgical menopause occurs abruptly, without an adjustment period, and is associated with a notably higher risk of fractures, arthritis, cognitive decline, dementia, Parkinson's disease, and various metabolic disorders affecting glucose and lipid levels-all of which contribute to an increased risk of major cardiovascular events. In 2

Sex-related differences are found not only in the reproductive system but also across various biological systems, such as the cardiovascular system. Compared with premenopausal women, cardiovascular disease (CVD) tends to occur more frequently in adult men and postmenopausal women (Post-MW). Also, during the reproductive years, sex hormones synthesized and released into the blood stream affect vascular function in a sex-dependent fashion. Estrogen (E2) interacts with estrogen receptors (ERs) in endothelial cells, vascular smooth muscle, and the extracellular matrix, causing both genomic and no

OBJECTIVE: Intervertebral discs act as shock absorbers, thereby helping to reduce the risk of vertebral body fractures. Previous studies have shown that estrogen loss following menopause is associated with disc height reduction whereas treatment with hormone replacement therapy (HRT) helps to maintain disc height. This study reports the effect of HRT on disc height from a post hoc analysis of a prospective randomized clinical trial of the effect of HRT on bone density.
METHODS: A total of 355 healthy postmenopausal women aged (mean ± standard deviation) 55.4 ± 4.8 years were randomized to HRT
