Toxic shock syndrome. Evaluation of national surveillance data using a hospital discharge survey.

Data from the Commission on Professional and Hospital Activities, Professional Activities Study were used to estimate the incidence and geographic distribution of hospitalized patients with toxic shock syndrome (TSS) in the United States in 1981 and 1982. There were 1004 discharges with a diagnosis of TSS from hospitals participating in the study. These discharges could not be validated using clinical criteria. Using these data, the projected incidences for the United States were 0.78/100,000 and 0.84/100,000 total population in 1981 and 1982, respectively. Female patients, 15 to 19 years of a

Work-related injuries in a rural emergency department population.

OBJECTIVE: To describe the spectrum of work-related injury evaluated in a rural ED population. METHODS: An ED-based injury surveillance system (EDBISS) was used to collect injury data for all ED patients seen over a 1-year period. A patient was classified as injured if his or her record contained an ED log injury code, an ICD-9 N-code between 800 and 995 in any diagnostic field, an E-code, or an entry in the trauma registry. An injury was considered work-related if the patient reported that the injury had occurred while at work. RESULTS: Work-related injuries accounted for 1,539/12,321 (12.5

Impact of Limited Access to Surgical Care in Medically Underserved Communities.

Summary/BackgroundMedically underserved communities and ethnic minorities constitute a significant portion of the vulnerable population within the United States. Recent changes in the health care structure, rising inflation with a decline in median household income, and the SARS-CoV-2 pandemic have disproportionately impacted communities of low socioeconomic status. Healthcare providers and federal organizations must be aware of how these factors influence access to surgical care to tailor treatment, interventions, and policies better to meet the needs of these populations.MethodWe systematica

Multicenter genomic analysis reveals environmental hotspots and near-clonal subclusters of Acinetobacter baumannii in veterinary hospitals in Beijing, China.

INTRODUCTION: Acinetobacter baumannii is a major healthcare-associated opportunistic pathogen with exceptional environmental persistence. However, its ecological distribution and potential transmission pathways in veterinary hospital environments remain poorly characterized. METHODS: Samples were collected from multiple departments and surface types across eight veterinary hospitals in Beijing from September to December 2020. A. baumannii isolates were recovered and subjected to antimicrobial susceptibility testing and whole-genome sequencing. Environmental risk factors were assessed using ge

Current State of Brazilian Neurosurgery: Evaluation of Burden of Care and Case Volume.

BACKGROUND: Low- and middle-income countries (LMICs) bear a heavier burden of neurosurgical diseases than high-income countries. Brazil, a growing middle-income country, holds promise as a health care market. However, there exists a lack of information to characterize the state of neurosurgical practice and access to care in Brazil. This study aimed to characterize neurosurgical practice in Brazil and identify barriers to care. METHODS: A collaborative survey was developed with Brazilian neurosurgeons and distributed by the Brazilian Society of Neurosurgery. The survey gathered demographic in

Distribution of pathogens and antimicrobial resistance in ICU-bloodstream infections during hospitalization: a nationwide surveillance study.

Changing microorganism distributions and decreasing antibiotic susceptibility over the duration of hospitalization have been described for the colonization or infection of selected organ systems. Few data are available on bacteremias in the intensive care unit (ICU) setting. We conducted a nationwide study on bloodstream infection (BSI) using data from the Swiss Centre for Antibiotic Resistance (ANRESIS). We analyzed data on BSI detected in the ICU from hospitals that sent information on a regular basis during the entire study period (2008-2017). We described specific trends of pathogen distri

Nosocomial transmission of Legionella pneumophila to a child from a hospital's cold-water supply.

Human Legionella infections mainly consist of community-acquired and nosocomial pneumonia and rarely affect children. We describe a nosocomial infection with Legionella pneumophila, serogroup 1, subgroup OLDA, in an immunocompromized 2-y-old girl at a paediatric clinic. L. pneumophila identical to that of the patient was found in the hospital's cold-water but not in the hot-water distribution system. Transmission of Legionella to the girl most probably occurred by Legionella-contaminated cold water mixed and heated by water from the hot-water system. Mixing of hot and cold water probably occur

Rural Surgical Workforce Policy in the US: Insights from a National Symposium.

Surgical workforce shortages represent a consequential but underrecognized threat to healthcare access, hospital viability, and health system stability in rural communities across the US. Although rural health policy has historically focused on primary care shortages and hospital closures, the role of surgical care as a foundational component of rural health systems has received comparatively limited attention. Drawing on insights from the 2025 Access in Rural Surgery Symposium hosted by Creighton University and CHI Health, this policy analysis applies the Health Policy Triangle framework to e

Spatial Distribution of COVID-19 Hospitalizations and Associated Risk Factors in Health Insurance Data Using Bayesian Spatial Modelling.

The onset of COVID-19 across the world has elevated interest in geographic information systems (GIS) for pandemic management. In Germany, however, most spatial analyses remain at the relatively coarse level of counties. In this study, we explored the spatial distribution of COVID-19 hospitalizations in health insurance data of the AOK Nordost health insurance. Additionally, we explored sociodemographic and pre-existing medical conditions associated with hospitalizations for COVID-19. Our results clearly show strong spatial dynamics of COVID-19 hospitalizations. The main risk factors for hospit

The Neurologist's Guide to Drug Pricing in the United States.

As newer, innovative neurology drugs enter the US health care system, neurologists should consider the cost of these treatments in addition to their efficacy, safety, and tolerability. To do so thoughtfully requires an understanding of how prescription drugs are priced in the United States. The process of drug pricing is linked to the distribution supply chain and the many stakeholders involved. Stakeholders include pharmaceutical manufacturers; wholesalers; pharmacies; pharmacy benefit managers; payers, including health insurers; hospital systems; neurologists and other clinicians; and patien

Bullous pemphigoid and comorbidities: a case-control study in Portuguese patients.

BACKGROUND: Although rare, bullous pemphigoid (BP) is the most common autoimmune blistering disease. Recent studies have shown that patients with bullous pemphigoid are more likely to have neurological and psychiatric diseases, particularly prior to the diagnosis of bullous pemphigoid. OBJECTIVE: The aims were: (i) to evaluate the demographic and clinical features of bullous pemphigoid from a database of patients at a Portuguese university hospital and (ii) to compare the prevalence of comorbid conditions before the diagnosis of bullous pemphigoid with a control group. METHODS: Seventy-seven

[A case of primary brain-stem injury recovered from persistent vegetative state after L-dopa administration].

A 51-year-old male was transferred to our hospital just after traffic accident. On admission, the patient was comatose (Glasgow Coma Scale of 6) and showed a left hemiparesis with a left oculomotor nerve palsy. Computed tomography demonstrated a traumatic subarachnoid hemorrhage without mass lesion. Magnetic resonance imaging showed high intensity lesions on the left dorsolateral midbrain and the right cerebral peduncle. The distribution of lesions implied diffuse axonal injury involving dopaminergic systems such as the substantia nigra and the ventral tegmental area. After several months of c

Detection of antibiotic-resistant bacteria and their resistance genes in wastewater, surface water, and drinking water biofilms.

Abstract In view of the increasing interest in the possible role played by hospital and municipal wastewater systems in the selection of antibiotic-resistant bacteria, biofilms were investigated using enterococci, staphylococci, Enterobacteriaceae, and heterotrophic bacteria as indicator organisms. In addition to wastewater, biofilms were also investigated in drinking water from river bank filtrate to estimate the occurrence of resistant bacteria and their resistance genes, thus indicating possible transfer from wastewater and surface water to the drinking water distribution network.
