BACKGROUND: Humerus fractures play a major role in daily clinical routine, and subsequent osteonecrosis is common after osteosyntheses.
OBJECTIVES: The current paper highlights the incidence and cause of osteonecrosis after proximal humerus fractures, with respect to presentation of anatomy and risk factors.
METHODS: Display of the literature and therapy options for humerus fracture and subsequent necrosis.
RESULTS: Humerus fractures are the seventh most frequent fractures in humans. The complication rate is 40%, and the described rate of necrosis is up to 34%. Accordingly, the surgical rev

Avascular necrosis (AVN) of the humeral head is an uncommon clinical entity which can result in significant morbidity for patients. There is a paucity of literature concerning humeral head AVN, which may be due to the relatively rarity of the condition and poorly understood nature. Despite being first described decades ago, the underlying pathophysiology leading to humeral head AVN is still poorly defined. While the staging of humeral head AVN is well described, not much is known about prognosticating factors to predict the eventual course. Most of the management options are based on that of f

CASE: A 70-year-old woman pedestrian struck by a motor vehicle presented with multiple orthopaedic injuries including a humeral head fracture dislocation with a large segment of humeral head located in the mediastinum. Thoracic surgery personnel performed a minimally-invasive video-assisted thoracoscopic extraction of the humeral head, and the patient underwent subsequent reverse total shoulder arthroplasty.
CONCLUSION: Intramediastinal displacement of the humeral head is a rare, yet serious traumatic injury that necessitates early recognition and comanagement with cardiothoracic or thoracic

The authors describe a surgical biological reconstruction of the humeral head with frozen autogenous allograft technique for the treatment of young patients with focal osteonecrosis of the humeral head. This represents a possible alternative, maybe even definitive for some patients, when compared to hemiarthroplasty or total shoulder arthroplasty. The technique consists of the fixation of a frozen autogenous allograft with previously-molded articular cartilage from the humeral head, after cleansing the osteonecrotic focus. Five patients under 50 years of age were treated, with three very satis

Humeral head bone defects (Hill-Sachs lesions) are caused by anterior shoulder dislocation with impaction of the posterosuperior humeral head onto the anterior glenoid rim. Frequently, these bony lesions are associated with glenohumeral instability, and large lesions may contribute to recurrent instability after failure of a soft tissue repair. To improve outcomes and minimize the risk of persistent instability, a thorough understanding of the biomechanics of humeral bone loss is required. Detection and quantification of clinically relevant humeral head bone loss are performed through an accur

OBJECTIVES: Arthroplasty for the proximal humerus problems secondary to fractures is troublesome, because of necrosis of the tubercles and the resulting insufficiency of the rotator cuff. The aim of this study was to investigate whether better results can be achieved with the differential use of anatomic and reverse shoulder prostheses, in comparison to the preoperative status.
METHODS: Fifty-five patients with secondary fracture prostheses due to sequelae of fractures of the humeral head were followed. Anatomic prostheses were implanted in 36 cases (fracture sequelae types 1 and 2 according

Cryptococcal skeletal infections can lead to significant morbidity and mortality and should be considered in the differential diagnosis of lytic osseous lesions. Clinical and radiological similarities to various disorders may lead to delay in initiation of antifungal treatment. This report describes a case of cryptococcal osteomyelitis of humeral bone in a 19-year-old female, emphasizing the clinical presentation, diagnostic approach and treatment options.

OBJECTIVE: To investigate indication, technical key during surgery and early results of humeral head replacement for the treatment of complicated proximal humerus fractures.
METHODS: Sixteen patients who received humeral head replacement were reviewed with a mean follow-up period of 6.8 months. Of the 16 patients, 15 suffered acute fractures with the mean interval between injury and surgery being 8 days. 1 case was a malunion after ORIF 1 year ago. The mean age for these patients were 58.9 years. Trauma series X-rays were taken at post-operation 2-week, 6-week, 10-week, 16-week 6-month and th

We undertook a prospective MRI study to measure the retroversion of the humeral head in 33 consecutive infants with a mean age of 1 year 10 months (3 months to 7 years 4 months) who had an obstetric brachial plexus lesion (OBPL). According to a standardised MRI protocol both shoulders and humeral condyles were examined and the shape of the glenoid and humeral retroversion determined. The mean humeral retroversion of the affected shoulder was significantly increased compared with the normal contralateral side (-28.4 +/- 12.5 degrees v -21.5 +/- 15.1 degrees, p = 0.02). This increase was found o

Percutaneous reduction and fixation of severe humeral head fractures would be the treatment of choice since it will not increase the risk of necrosis already inherent in these fractures. Nevertheless, the question arises of whether anatomical reduction is possible with the percutaneous technique and whether the reduced fracture can be adequately stabilized. It is important to study the fracture closely before the operation in order to determine the fracture type and identify the relationship of the individual fragments to each other. Radiographs taken in at least two planes are essential and a

A method for directly measuring the position of the humeral head on the face of the glenoid in different positions of abduction of the arm was developed. We studied three subject groups: 12 patients with normal shoulders (group 1), 15 patients with stage II impingement syndrome (group 2), and 20 patients with rotator cuff tears or stage III impingement (group 3). The study consisted of a series of anteroposterior roentgenograms in the plane of the scapula with the arm in neutral rotation. Roentgenograms were obtained at 20 degrees intervals as the arm was elevated in the plane of the scapula f

Specimens of cartilage removed from the shoulder joint of dogs with osteochondritis dissecans were compared with the articular-epiphyseal cartilage from the same location of healthy dogs. The pathologic cartilage lost the lamination pattern of articular-epiphyseal cartilage from healthy dogs. Chondrocytes of healthy and pathologic cartilage contained lipid inclusions. However, in pathologic samples, the lipid inclusions were more prominent, particularly in the superficial region of the presumptive articular cartilage. Lipids in the interterritorial matrix of the pathologic cartilage were obser

We present the case of a 22-year-old female patient with chondroblastoma in the right humeral head. To allow a gentle and anatomic resurfacing of the humeral joint surface and to avoid total joint arthroplasty in our young patient with high functional requirements, we implanted a HemiCAP® after intralesional curettage of the chondroblastoma. Our patient's excellent short-term functional outcome shows that our approach can be considered a very good therapeutic option.

BACKGROUND: Pyrocarbon (PyC) interposition shoulder arthroplasty can be used to treat advanced collapse of the humeral head after avascular necrosis.
OBJECTIVES: We examined outcomes for this bone-preserving implant which has a PyC coating and a novel biomechanical concept.
MATERIALS AND METHODS: For a minimum of 2 years, we followed 10 patients (4 men, 6 women, 55.6 ± 12.9 years) treated with a free interposition PyC arthroplasty ("snookerball") due to advanced humeral head collapse but with an intact glenoid and rotator cuff. Anteroposterior radiographs, the Constant score (CS), adjusted C

Osteochondritis dissecans (OCD) is a common joint disorder in knee, ankle and elbow, however it can be rarely found in glenohumeral joint. In this study, we report an asymptomatic case of humeral head OCD, which was detected incidentally following a trauma. X-rays showed an area of lucency around an oval bony fragment measuring about 1 cm on the superior aspect of the humeral head. However, the patient was pain-free and the shoulder range of motion was normal.

BACKGROUND: Humeral head osteonecrosis treatment varies depending on the stage and symptoms. Successful outcomes for humeral head core decompression for stage I/II disease in chronic steroid-induced (CSI) osteonecrosis have been reported, but fewer data exist for sickle cell disease (SCD) etiology. Resurfacing and hemiarthroplasty or total shoulder arthroplasty (TSA) are common for advanced collapse, with mixed results.
METHODS: We evaluate radiographic and functional outcomes after procedures for humeral head atraumatic avascular necrosis (HAAVN), decompression efficacy in CSI and SCD popula
