A Rare Case of Hydromorphone-Induced Angioedema Effectively Managed by a Difficult Airway Response Team.

Hydromorphone, unlike other opioids associated with histamine release, has never been reported to cause angioedema. We report a rare case of hydromorphone-induced angioedema in a 34-year-old woman with history of deep venous thrombosis and pulmonary embolism who presented with leg swelling and pain after trauma. Hydromorphone was administered with subsequent rapid development of stridor and edematous changes of the tongue, uvula, and surrounding mucosa. The difficult airway response team was activated, and the airway was secured by emergent awake fiberoptic intubation in the operating room. Af

A pilot evaluation of a hydromorphone dose substitution policy and the effects on patient safety and pain management.

Hydromorphone is a potent opioid analgesic commonly utilized in the hospital setting for the management of acute pain. Initial dose recommendations range from 0.1 to 2 mg of hydromorphone for opioid-naïve patients. This creates a challenge to optimally dose hydromorphone in opioid-naïve patients with the goals of avoiding opioid toxicities while also providing adequate pain management. In order to minimize the incidence of opioid toxicity, a community hospital implemented a dose-substitution hydromorphone policy that allows practitioners to automatically use a lower dose of hydromorphone withi

Tremors and agitation following low-dose intravenous hydromorphone administration in a patient with kidney dysfunction.

OBJECTIVE: To report a case of tremors and agitation associated with the administration of low doses of intravenous hydromorphone in a patient with acute kidney injury in the setting of chronic kidney disease. CASE SUMMARY: A 91-year-old man was admitted for a left intertrochanteric hip fracture. On hospital days 1 and 2, the patient received hydromorphone 1 mg intravenously for pain. By hospital day 3, the patient had received a total of 3.5 mg of hydromorphone. He then became tremulous and agitated, which worsened as the dose was increased. During hospital day 4, the hydromorphone dosage wa

Intrathecal hydromorphone for cesarean delivery: in search of improved postoperative pain management: a case report.

Currently, morphine and fentanyl are the most commonly used intrathecal opioids for the postoperative pain management of patients who underwent cesarean delivery. Unfortunately, the analgesic benefits of these 2 drugs tend to fall into different extremes based on lipid solubility. Intrathecal hydromorphone may provide more consistent analgesia because its lipid solubility falls between that of the other 2 opioids. A 22-year-old woman with a 39-week intrauterine pregnancy, gravida 2, para 1, came in for a scheduled second-time cesarean delivery. Her preoperative history included a morphine alle

Double-blind injectable hydromorphone versus diacetylmorphine for the treatment of opioid dependence: a pilot study.

Using data from the North American Opioid Maintenance Initiative study, a Phase III randomized and parallel arm trial, this pilot study is aimed at testing if treatment response with injectable hydromorphone differs compared to diacetylmorphine in the treatment of long-term opioid addiction. A total of 140 long-term, treatment-refractory opioid-dependent individuals received either injectable diacetylmorphine (n = 115) or hydromorphone (n = 25), in a double-blind fashion, over 12 months. At the end of the study, none of the participants in the hydromorphone group thought they were definitely r

[Pain reduced and quality of life improved by sustained-release hydromorphone. Results of an observational study].

METHOD: A total of 487 patients with severe pain of varying pathogenesis received sustained-release hydromorphone (Palladon) in an observational study. After an initial examination, two follow-ups took place on the 3rd and 7th day and a final examination after about 21 days. RESULTS: The intensity of the pain, measured using a numerical rating scale from 0 to 10 (none--most severe pain), decreased by 65.3% from 7.2 at the start to 2.5 at the final examination. The quality of life, characterized by the patients themselves assessing the variables of sleep, vitality, mood, social contacts, activ

Retrospective study of the use of hydromorphone in palliative care patients with normal and abnormal urea and creatinine.

An uncontrolled retrospective study was conducted looking at the use of oral controlled-release hydromorphone in palliative care patients. Over a 2-year period 55 patients were switched to hydromorphone therapy, and the efficacy and outcomes were assessed. Urea and electrolyte measurements were also recorded at the time of opioid switch and renal impairment defined as urea > 10.5 mmol/l and/or creatinine > or = 101 mmol/l. This group of 29 patients with abnormal urea and/or creatinine (Group 1) was compared with the remaining 26 patients (Group 2) who had normal urea and creatinine. The major

Opioid Prescribing Changes in Advanced Cancer Before and After Withdrawal of Hydromorphone Slow-Release Tablets.

AIM: Ongoing disruptions to opioid supply in Australia have raised ongoing concerns about their impact on cancer pain management. The withdrawal of hydromorphone slow-release (SR) tablets from the Australian market has presented challenges to advanced cancer pain management. This study aimed to determine how this affected the utilisation of alternative slow-release opioids in patients with advanced cancer. METHODS: A retrospective cohort study was conducted at a quaternary cancer centre, comparing opioid prescribing patterns across two 12-month periods: before hydromorphone SR tablet disconti

Hydromorphone ameliorates postoperative pain and depressive disorder in women receiving cesarean section under spinal anesthesia.

BACKGROUND: Pain and depressive mood disorders during cesarean sections negatively impact both mothers and infants. Studies have shown that hydromorphone has positive effects on both pain management and depressive mood disorders. This study examines how hydromorphone influences postoperative pain and depressive disorders in parturients undergoing cesarean sections under spinal anesthesia. METHODS: This single-center, controlled, randomized trial involved 130 patients. Parturients in the intervention (H) group received patient-controlled intravenous analgesia (PCIA) with hydromorphone combined

Hydromorphone Protects against CO2 Pneumoperitoneum-Induced Lung Injury via Heme Oxygenase-1-Regulated Mitochondrial Dynamics.

Various pharmacological agents and protective methods have been shown to reverse pneumoperitoneum-related lung injury, but identifying the best strategy is challenging. Herein, we employed lung tissues and blood samples from C57BL/6 mice with pneumoperitoneum-induced lung injury and blood samples from patients who received laparoscopic gynecological surgery to investigate the therapeutic role of hydromorphone in pneumoperitoneum-induced lung injury along with the underlying mechanism. We found that pretreatment with hydromorphone alleviated lung injury in mice that underwent CO2 insufflation,
