Permissive and obligatory roles of NO in cerebrovascular responses to hypercapnia and acetylcholine.

Inhibition of nitric oxide (NO) synthesis attenuates the hypercapnic cerebrovasodilation or the increases in cerebral blood flow (CBF) produced by acetylcholine (ACh), either topically applied or endogenously released in neocortex by stimulation of the basal forebrain cholinergic system. We investigated whether exogenous administration of NO, using NO donors, can reverse the attenuation of these responses by NO synthase (NOS) inhibitors. In halothane-anesthetized, ventilated rats the frontoparietal cortex was exposed and superfused with Ringer. CBF was monitored at the super fusion site by las

Hyperosmolality alters the ventilatory response to acute hypercapnia and hypoxia.

Acute hyperosmolality in the Pekin duck results in an extracellular acidosis and hypercarbia without any stimulation of ventilation. The development of the extracellular acidosis is accompanied by the concurrent development of an intracellular alkalosis systemically which has been hypothesized to depress ventilation (Kasserra et al., J. Appl. Physiol., 1993). In order to investigate this apparent suppression of ventilation, the ventilatory response to various respiratory challenges (CO2, O2, K+) was studied both before (normosmotic) and after (hyperosmotic) hypertonic sucrose infusion. Increas

Responses to hypoxia and hypercapnia in infants of substance-abusing mothers.

Because infants of substance-abusing mothers (ISAM) have an increased risk of sudden infant death syndrome and have abnormal sleeping ventilatory patterns, we studied the effects of mild hypoxia during quiet sleep on ventilatory pattern, heart rate, and arousal in 23 healthy ISAM (mean +/- SEM: 9.0 +/- 0.49 weeks of age) and 15 healthy, similarly aged, control infants. Hypercapnic challenges were performed in six ISAM and eight control subjects. Hypoxic arousal responses were elicited by rapidly decreasing inspired oxygen tension to 80 mm Hg for 3 minutes or until arousal occurred. Failure to

Interpretation of the alveolar-arterial oxygen difference in patients with hypercapnia.

According to conventional wisdom the difference between alveolar and arterial O2 tensions, the AaPO2, should distinguish between hypoxemia caused by alveolar hypoventilation and hypoxemia caused by alveolar hypoventilation complicated by other abnormalities of gas exchange. To test this concept we have calculated the AaPO2 from arterial blood gas measurements, breathing air, in 23 patients with hypercapnia, hypoxemia, and advanced obstructive lung disease (mean FEV1 = 0.88 L). We found that AaPO2 varied inversely with PaCO2 (r = -0.83, p less than 0.001). In five of these patients with the mos

Interaction of hypoxia and hypercapnia on respiratory drive in patients with COPD.

The objective of this study was to compare the response of respiratory drive to progressive hypoxia under eucapnic and hypercapnic conditions in patients with severe COPD. Twenty-five patients with severe COPD and 13 nonsmoking young men were studied. The pressure in the occluded airway measured 0.1 second after the onset of inspiration was used as an index of respiratory drive. The occlusion pressure was measured at levels of SaO2 between 97 and 85 percent while eucapnic. The PETCO2 was then increased 10 mm Hg and the study repeated. The response of respiratory drive to hypoxia as measured by

Endogenous opioids and ventilatory responses to hypercapnia in normal humans.

Though administration of opioid peptides depresses ventilation and ventilatory responsiveness, the role of endogenous opioid peptides in modulating ventilatory responsiveness is not clear. We studied the interaction of endogenous opioids and ventilatory responses in 12 adult male volunteers by relating hypercapnic responsiveness to plasma levels of immunoactive beta-endorphin and by administering the opiate antagonist naloxone. Ventilatory responsiveness to hypercapnia was not altered by pretreatment with naloxone, and this by itself suggests that endogenous opioids have no role in modulating

Effects of hypercapnia and hypoxia on abdominal expiratory nerve activity.

We examined the effects of progressive hypercapnia and hypoxia on the efferent neural activity in a whole abdominal expiratory nerve (medial branch of the cranial iliohypogastric nerve (L1) in anesthetized, paralyzed dogs. To eliminate effects of phasic lung and chest-wall movements on expiratory activity, studies were performed in the absence of breathing movements. Progressive hyperoxic hypercapnia and isocapnic hypoxia were produced in the paralyzed animals by allowing 3-5 min of apnea to follow mechanical ventilation with 100% O2 or 35% O2 in N2, respectively; during hypoxia, isocapnia was

[Analysis of pulmonary vasomotor responses to alveolar hypoxia and hypercapnia].

In acute experiments on cats responses of the lung vessels to hypoxic and hypercapnic stimuli and the latters effect on minor circle hemodynamics were studied as well as the role of changes of the blood acid-alkaline balance and volume of ventilation in potentiation and blocking of the above responses. The alveolar hypoxia and hypercapnia being induced a reversed dependence between the relative mass of the lung area and reduction of its blood flow vasdseined Hyperventilation with hypoxic gas mixture inhibited the hypoxic vasoconstriction in respective lung area which seems to suggest a shift t

Stimulus-dependent sex differences in dyspnoea: Mechanical loading vs. hypercapnia.

Dyspnoea is a multidimensional sensation of breathing discomfort that varies in sensory and affective experience. During exercise, healthy adults commonly report dyspnoea sensations of 'air hunger' and 'work/effort', with females more frequently reporting air hunger at maximal exertion. Interpreting these sex differences and their underlying mechanisms is challenging due to the complex physiological responses associated with exercise. Therefore, this study used laboratory models to target respiratory stimuli and evaluate sex-differences in perceptual and physiological responses to dyspnoea. In

Menstrual cycle and oral contraceptives influence cerebrovascular dynamics during hypercapnia.

Women experience fluctuating orthostatic intolerance during the menstrual cycle, suggesting sex hormones may influence cerebral blood flow. Young (aged 18-30) healthy women, either taking oral contraceptives (OC; n = 14) or not taking OC (NOC; n = 12), were administered hypercapnic gas (5%) for 5 min in the low hormone (LH; placebo pill) and high hormone (HH; active pill) menstrual phases. Hemodynamic and cerebrovascular variables were continuously measured. Cerebral blood velocity changes were monitored using transcranial doppler ultrasound of the middle cerebral artery to determine cerebrova

Hypercapnia attenuates ventilator-induced lung injury through vagus nerve activation.

PURPOSE: To investigate the role of vagus nerve activation in the protective effects of hypercapnia in ventilator-induced lung injury (VILI) rats. METHODS: Male Sprague-Dawley rats were randomized to either high-tidal volume or low-tidal volume ventilation (control) and monitored for 4h. The high-tidal volume group was further divided into either a vagotomy or sham-operated group and each surgery group was further divided into two subgroups: normocapnia and hypercapnia. Injuries were assessed hourly through hemodynamics, respiratory mechanics and gas exchange. Protein concentration, cell coun

Effect of hypercapnia on spleen-related haemoglobin increase during apnea.

BACKGROUND: Splenic contraction associated with apnea causes increased haemoglobin concentration and haematocrit (Hct), an effect that may promote prolonged breath-holding. Hypoxia has been shown to augment this effect, but hypercapnic influences have not been investigated previously. METHODS: Eight non-divers performed three series of apneas on separate days after inspiration of oxygen with different carbon dioxide (CO₂) levels. Each series consisted of three apneas 2 minutes apart: one with pre-breathing of 5% CO₂ in oxygen (O₂, 'Hypercapnia'); one with pre-breathing of 100% O₂ ('Normocapni

Permissive hypercapnia to decrease lung injury in ventilated preterm neonates.

Lung injury in ventilated premature infants occurs primarily through the mechanism of volutrauma, often due to the combination of high tidal volumes in association with a high end-inspiratory volume and occasionally end-expiratory alveolar collapse. Tolerating a higher level of arterial partial pressure of carbon dioxide (PaCO2) is considered as 'permissive hypercapnia' and when combined with the use of low tidal volumes may reduce volutrauma and lead to improved pulmonary outcomes. Permissive hypercapnia may also protect against hypocapnia-induced brain hypoperfusion and subsequent periventri

Peripheral chemoreceptor contributions to sympathetic and cardiovascular responses during hypercapnia.

We tested the hypothesis that integrated sympathetic and cardiovascular reflexes are modulated by systemic CO2 differently in hypoxia than in hyperoxia (n = 7). Subjects performed a CO2 rebreathe protocol that equilibrates CO2 partial pressures between arterial and venous blood and that elevates end tidal CO2 (PET(CO2)) from approximately 40 to approximately 58 mmHg. This test was repeated under conditions where end tidal oxygen levels were clamped at 50 (hypoxia) or 200 (hyperoxia) mmHg. Heart rate (HR; EKG), stroke volume (SV; Doppler ultrasound), blood pressure (MAP; finger plethysmograph),

[A patient who manifested various symptoms following severe accidental hypercapnia].

A case of severe accidental hypercapnia during anesthesia is presented. A 44-year-old woman underwent laparotomy under general anesthesia. Forty minutes after the start of the operation, BP rose slightly and HR increased from 110 to 140 x min-1. Then ST segment depression was noted on ECG monitor. Therefore, nitrous oxide was discontinued for 20 minutes. Frequent oxygen supply with oxygen flush was needed to inflate the collapsed bag. The operation was concluded without additional clinical problems. The patient remained unconscious after the anesthetics were discontinued. Cyanosis was observed

Pulmonary development in growing guinea pigs exposed to chronic hypercapnia.

The role of lung stretch in causing pulmonary hyperplasia was studied in weanling male guinea pigs breathing air or 5% CO2 (in 22% O2, 73% N2) for 4 weeks. By the end of the exposure, oxygen consumptions were similar for both groups, but tidal volume and minute ventilation doubled in the hypercapnic group compared to controls. Arterial and venous blood gases reflected compensatory increases in plasma bicarbonate in animals breathing 5% CO2. The two groups did not differ in growth rate, lung volume or weight, alveolar surface area, anatomically estimated pulmonary diffusing capacity, or lung ce

Carotid body hypercapnia does not elicit ventilatory acclimatization in goats.

The carotid body (CB) perfusion model utilizes surgical vascular ligations to allow isolated blood supply to a single in situ CB in awake goats. The contralateral CB was excised. By use of an extracorporeal pump-oxygenator system the blood gas composition perfusing the CB can be controlled independently from that of the systemic arterial system including the brain. Using this model we compared the responses of systemically normoxic goats to CB hypercapnia and CB hypoxia. In 6 goats CB stimulation with hypercapnic-normoxic blood (mean PcbCO2 = 78 Torr, mean PcbO2 congruent to 100 Torr) produced

Diaphragm length and breathing pattern changes during hypoxia and hypercapnia.

In this study diaphragmatic length changes were measured during quiet breathing and during augmentation of breathing with hypoxia and hypercapnia in supine anesthetized dogs. The breathing pattern and the VT-TI relationship during hypoxia were different than those during hypercapnia. The crural diaphragm shortened more than the costal diaphragm with both stimuli, and the amount of shortening in relation to the tidal volume implied that there was considerable distortion of the chest wall during hyperventilation. The velocity of shortening of both parts of the diaphragm at similar levels of vent
