Surgical and nonsurgical treatment options in patients with movement disorders of the platysma.

PURPOSE: Movement disorders of the platysma may constitute both an aesthetic and a functional problem for the affected patients, who almost always have hyperkinesis. This article describes the different treatment options for movement disorders of the platysma. PATIENTS AND METHODS: The therapeutic options that we used involved surgical and nonsurgical strategies (33 patients), including neurectomy of the cervical branch of the facial nerve, myectomy of the platysma, and the injection of botulinum toxin A. After therapy, patients were evaluated through self-assessment of the clinical outcome,

Magnesium deficiency in the pathogenesis of mitral valve prolapse.

Idiopathic mitral valve prolapse (MVP) is the commonest valvular disorder in industrialized nations. It is predominantly a familial condition, showing Mendelian dominance with delayed and variable penetrance. Although hyperkinesis and hypertrophy of the left ventricle have been described in MVP, its histopathology, somatic morphology and genetics support the leading theory that MVP results from a hereditary disorder of connective tissue. Latent tetany (LT) due to chronic Mg deficit (Mg-D) occurs in over 85% of MVP cases; MVP complicates 26% of LT. Mg-D can explain many clinical features of the

Dietary management of nystagmus.

Two case reports illustrate the therapeutic response of congenital nystagmus to a diet eliminating synthetic food colors, synthetic food flavors, the antioxidant preservatives butylated hydroxytoluene (BHT) and butylated hydroxyanisole (BHA), and a small group of foods thought to contain a natural salicylate radical. A brief discussion of the hyperkinetic syndrome is offered with the proposal that a variety of neurologic and neuromuscular disturbances (grand mal, petit mal, psychomotor seizures; La Tourette syndrome; autism; retardation; the behevioral component of Down's syndrome; and oculomo

The intestine in allergic diseases.

In this review I have described the pathophysiology of allergic disorders of the gastrointestinal tract. Situations where the intestine cannot be a complete barrier to foreign allergens and antigens were discussed and etiological factors of gastrointestinal allergy were detailed. Clinical features of gastrointestinal allergy include diarrhea, vomiting, abdominal pain and colic, intestinal hemorrhage and malabsorption as well as symptoms and signs outside the gastrointestinal tract such as chronic rhinitis and asthma in the respiratory system, urticaria, angioedema and eczema as dermatological

Orthostatic Hyperkinetic Disorders in Subacute Sclerosing Panencephalitis - A Novel Observation.

BACKGROUND: Subacute sclerosing panencephalitis (SSPE) is a progressive, often fatal inflammatory encephalitis caused by persistent measles virus infection of the central nervous system. The neurological manifestations of SSPE are well characterised, encompassing cognitive decline, periodic myoclonic jerks, and often relentless progression. CASE REPORT: We report a case of a 14-year-old male with SSPE who developed bilateral lower limb shaking and knee buckling, with associated loss of quadriceps tone on standing. These features resulted in recurrent falls and necessitated supported ambulatio

Tetrabenazine for the treatment of chorea and other hyperkinetic movement disorders.

Tetrabenazine (TBZ; Xenazine) is a potent, selective, reversible depletor of monoamines from nerve terminals. TBZ inhibits the vesicular monoamine transporter type 2 which, in humans, is expressed nearly exclusively in the brain. TBZ is rapidly metabolized in the liver by carbonyl reductase to stereoisomers of hydrotetrabenazine, some of which are potent inhibitors of vesicular monoamine transporter type 2. Initially developed in the 1950s for schizophrenia, since the 1970s several publications have reported on the efficacy of TBZ in the treatment of various hyperkinetic movement disorders. Al

Does the use of phenobarbital as an anticonvulsant permanently exacerbate hyperactivity?

Data is presented on 12 patients in whom phenobarbital, when used as an anticonvulsant medication, either precipitated hyperactivity or permanently exacerbated existing hyperactivity. Discontinuation of the medication resulted in a decrease in hyperactivity in all patients. None of the patients returned to a pre-drug level of activity. The effect of phenobarbital on the reticular system is discussed. It is important for clinicians to carefully consider the potential side-effects of the use of phenobarbital in children in whom overactivity is a problem. Ascertaining whether hyperactivity has be

A Case of Peripartum Cardiogenic Shock Resulting From Reverse Takotsubo Cardiomyopathy.

Takotsubo cardiomyopathy (TCM) is characterized by transient left ventricular (LV) dysfunction that classically presents as apical ballooning with basal hyperkinesis. A recognized variant, reverse TCM, conversely is characterized by basal and mid-ventricular hypokinesis with apical sparing, often triggered by a catecholamine surge. We present a case of a woman who underwent an emergency cesarean section complicated by the development of cardiogenic shock due to TCM. She had rapid clinical improvement with restoration of systolic function and was ultimately treated with guideline-directed medic

Non-nystagmus hyperkinetic eye movement disorders.

BACKGROUND: While nystagmus is an involuntary ocular movement relatively frequent, well described, and easily recognized by most practitioners, non-nystagmus hyperkinetic eye movement disorders (NHEMD) are less obvious and can be overlooked. NHEMD may arise from intrinsic abnormalities in ocular muscles and oculomotor nerves, brain and brainstem lesions, systemic diseases, dysimmune disorders, drugs or can even be functional in nature. Given that some treatable disorders initially manifest with NHEMD, their knowledge becomes crucial. AIMS: This review aims to offer a practical and comprehensi

[Case of cerebellar hemorrhage complicated with Takotsubo cardiomyopathy - usefulness of plasma brain natriutetic peptide measurement for the diagnosis].

A 94-year-old woman was admitted to our hospital because of altered mental status and cerebellar ataxia of left upper and lower extremities. A brain CT scan revealed a right cerebellar hemorrhage approximately 15 cc. Plasma brain natriuretic peptide (BNP) value on admission was 1,064.6 pg/ml. Twelve-lead ECG revealed negative T-wave in V3-V5. Transthoracic echocardiology confirmed an ejection fraction of 35%, and left ventricular apical akinesia and basal hyperkinesis were seen. Plasma BNP value was dramatically declined in the subacute phase of cerebellar hemorrhage. On the 14th day, echocard

Advanced imaging findings in a cat with left ventricular apical aneurysm.

An eight-year-old, male neutered, domestic shorthair cat presented with severe anemia. Two-dimensional echocardiography identified severe left ventricular apical dilation with wall thinning and akinesia. The basal portion of the left ventricle showed equivocal hypertrophy and subjective hyperkinesis. Speckle tracking echocardiography showed marked abnormalities in all deformation planes (longitudinal, circumferential, radial strain) and twist. Three-dimensional Bull's eye plot reconstruction was also performed. Post-mortem evaluation showed a pale and paper-thin left ventricular apex and histo

[Botulinum toxin in otorhinolaryngology].

BACKGROUND AND OBJECTIVES: Through its anticholinergic effect, botulinum toxin is a suitable therapeutic option for dysfunctions of the muscular and the autonomic nervous system. PATIENTS/METHODS: Beside the classical indications like facial hyperkinesis (i.e. blepharospasm, hemifacial spasm), the treatment of complex dystonias (oromandibular dystonia, spasmodic dystonia, cervical dystonia), gustatory sweating, hypersalivation and crocodile tears is successful. Botulinum toxin is an alternative treatment of tension type headache and migraine. A new indication of botulinum toxin application ma

Spasmodic dysphonia combined with insufficient glottic closure by phonation.

The authors describe the case history of a patient who suffered from symptoms deriving from two different origins. The patient's voice was spasmodic dysphonia-like interrupted and pressed. At the same time, his voice was powerless, too. The reason for this was that besides the spasmodic dysphonia caused by hyperkinesis, an incomplete closure of the vocal cords during phonation in the middle third was present. It was caused by the atrophy of the vocal cords. In order to eliminate the symptoms, initially we injected 25 IU Botox into the left vocal cord transcutaneously under the direction of EMG

A review on approach to a twitchy tongue in neurology.

BACKGROUND: Several etiologies are responsible for presentation of a twitching tongue in clinical practice. Some of these etiologies cause an isolated hyperkinetic tongue muscle, and some others cause it along with other signs and symptoms. OBJECTIVES: The present paper aims to review the causes, pathology, and presentations reported with twitchy tongue. An anatomical basis of the etiologies responsible for presentation of a twitchy tongue and hyperkinetic movement disorders of this muscle is pursued. METHOD: The reporting of this systematic review was guided by the standards of the Preferre

Subjective experience of treatment, side-effects, mental state and quality of life in chronic schizophrenic out-patients treated with depot neuroleptics.

Attitude towards treatment, side-effects, mental state and quality of life was assessed in 53 chronic schizophrenic out-patients on maintenance treatment with depot neuroleptics. It was found that 60% of the patients viewed depot medication positively, while only 8% viewed it negatively. Only 70% of patients complained about side-effects, even though 94% had scored as having them. Hypokinesia and hyperkinesis were the side-effects least noticed by the patients, but most noticed by the treating physician, while the opposite was the case with psychic side-effects. Only 49% of patients thought th
