Hypoplastic left heart syndrome in situs inversus totalis.

A very rare case of a newborn with hypoplastic left heart syndrome and situs inversus totalis is herein reported. Successful surgical treatment of this cardiac malformation was accomplished at our institution and consisted of: modified Norwood stage I procedure, with direct anastomosis of the transected main pulmonary artery to the aortic arch, without any prosthetic material interposition and right ventricle to pulmonary artery polytetrafluoroethylene shunt.

Hypoplastic left heart syndrome: factors influencing therapeutic choice.

Parent's choices among therapeutic options for their infants born with hypoplastic left heart syndrome are difficult and controversial. Currently, management options include surgical reconstruction, cardiac transplantation, and comfort measures only. We retrospectively reviewed medical records of 47 patients (1989-1999) to create a database of clinical features of infants who received either an operation or comfort care only. Eleven families were interviewed by means of a structured questionnaire pertaining to their experience and reasons for their choice. Of the 47, 20 were prenatally diagnos

Hypoplastic left heart syndrome: Prognosis and management options.

Few congenital heart malformations have raised as many surgical, ethical, social and economic issues as the therapy for infants diagnosed with hypoplastic left heart syndrome. Before the 1980s, this complex malformation was associated with 95% mortality within the first month of life. In the past two decades, palliative surgery and cardiac transplantation have become management options, in addition to comfort care for the infant. These innovations have forced parents and physicians to make difficult decisions because the long term results of the additional treatment options are not known. This

Isolated hypoplastic left heart syndrome in three siblings.

BACKGROUND: Reports of recurrent isolated hypoplastic left heart syndrome have been infrequent, and the genetic basis of this occurrence is not well understood. CASE: A 32-year-old woman with a noncontributory medical and family history terminated her first pregnancy after the fetus was diagnosed with hypoplastic left heart syndrome. Chromosomes were normal and no extracardiac anomalies were found at autopsy. In one subsequent pregnancy, a healthy live-born neonate was delivered, but in two other pregnancies isolated hypoplastic left heart syndrome was diagnosed again. CONCLUSION: Parents wi

Nature of coarctation in hypoplastic left heart syndrome.

The study was designed to establish the underlying pathology of aortic coarctation in hypoplastic left heart syndrome. Aortic coarctation in patients with hypoplastic left heart syndrome is a significant problem when a Norwood procedure or heart transplantation is performed. Previous reports have claimed that the obstruction was merely a branch-point phenomenon and that ductal tissue was absent. Five heart specimens with hypoplastic left heart syndrome and aortic coarctation were examined histologically. The distal segment of the aortic arch, together with the arterial duct, were dissected and

Modified Norwood procedure for hypoplastic left heart syndrome.

BACKGROUND: Since June 1994, we have used a modification of the Norwood technique in 13 patients presenting with the hypoplastic left heart syndrome or similar variants. METHODS: This technique involves coarctation repair, arch reconstruction, and creation of a neo-ascending aorta using autologous great vessel tissue only. Pulmonary blood flow is provided by a central shunt of 3.0- to 4.0-mm Gore-Tex. RESULTS: The mean age and weight at operation were 15 days (range, 1 to 77 days) and 3.2 kg (range, 1.7 to 4.6 kg), respectively. The mean circulatory arrest time was 32 minutes (range, 25 to 5

Care of infants with hypoplastic left heart syndrome.

Hypoplastic left heart syndrome (HLHS) is a common congenital heart defect that is universally fatal without surgical intervention. Two distinctly different surgical options, heart transplantation and reconstruction surgery, have been successful in treating infants with HLHS. This article reviews the anatomy and pathophysiology of HLHS. Preoperative care of infants with HLHS is discussed. The care required by infants with HLHS after heart transplantation is the same as that required by other infants who need heart transplant and has been reported. The primary focus of this report is postoperat

Hypoplastic left heart syndrome: experience with palliative surgery.

Aortic atresia is a form of hypoplastic left heart syndrome always complicated by associated severe hypoplasia of the ascending aorta and various degrees of mitral valve and left ventricular hypoplasia. At present it is a universally fatal lesion in early infancy. This is a report of a new palliative procedure for hypoplastic left heart syndrome that has resul;ed in early ongoing survival of two infants with aortic atresia. On the basis of experience with a third patient, an operation for future physiologic correction is proposed.

Radiologic patterns of the hypoplastic left heart syndrome.

The hypoplastic left heart syndrome is a malformative complex which unfortunately can not be treated surgically. This cardiopathy is the result of a serious embryogenic error and takes into consideration the different anatomic varieties described here. Borderline cases are also noted especially isthmic coarctation of the aorta or aortic stenosis in the neonatal period. Though several years ago, the diagnosis of the hypoplastic left heart syndrome was made only on pathology, it is now possible to make this diagnosis using clinical, laboratory and radiologic examinations. However, they are cases

Medical Therapies for Heart Failure in Hypoplastic Left Heart Syndrome.

Significant surgical and medical advances over the past several decades have resulted in a growing number of infants and children surviving with hypoplastic left heart syndrome (HLHS) and other congenital heart defects associated with a single systemic right ventricle (RV). However, cardiac dysfunction and ultimately heart failure (HF) remain the most common cause of death and indication for transplantation in this population. Moreover, while early recognition and treatment of single ventricle-related complications are essential to improving outcomes, there are no proven therapeutic strategies

Hypoplastic left heart syndrome: "to treat or not to treat".

OBJECTIVE: To review our experience in managing infants with hypoplastic left heart syndrome (HLHS) and to establish a consistent approach in counseling families based on our experience and on review of medical literature. METHODS: Infants were divided into three group periods based on significant changes in the approach to treatment. After a diagnosis of HLHS was made, a multidisciplinary team met the family to discuss different management options: compassionate care, active treatment by heart transplantation, or Norwood staged surgery. RESULTS: Parents of 58 infants (58%) opted for compass

The natural history of the hypoplastic left heart syndrome.

In a fetus, examined initially at 22 weeks gestation, we identified the echocardiographic features of a dilated, hypertrophied and poorly contracting left ventricle. The presumptive diagnosis was critical aortic stenosis. Subsequent scans at 32 weeks and at term showed that the left ventricle had not grown since the first study such that the left ventricle had developed the appearance of a hypoplastic and densely echogenic chamber. Thus, in some forms of the hypoplastic left heart syndrome, the left ventricle can be of normal size or even dilated in early pregnancy. This may mean that the more

Whole Genome Sequencing in Hypoplastic Left Heart Syndrome.

Hypoplastic left heart syndrome (HLHS) is a genetically complex disorder. Whole genome sequencing enables comprehensive scrutiny of single nucleotide variants and small insertions/deletions, within both coding and regulatory regions of the genome, revolutionizing susceptibility-gene discovery research. Because millions of rare variants comprise an individual genome, identification of alleles linked to HLHS necessitates filtering algorithms based on various parameters, such as inheritance, enrichment, omics data, known genotype-phenotype associations, and predictive or experimental modeling.
