Preventing symptomatic vitamin D deficiency and rickets among Indigenous infants and children in Canada.

Vitamin D deficiency (VitDD) rickets and other manifestations of severe VitDD, such as cardiomyopathy and hypocalcemic seizures, continue to be diagnosed in Canada. Breastfed Indigenous infants, particularly those living in northern communities, are disproportionately impacted, although formula-fed infants are not exempt in cases where the mother's vitamin D status is critically low. This statement deals with the prevention of rickets and hypocalcemia due to VitDD for Indigenous children, and revises an earlier document from the Canadian Paediatric Society. An assessment of the risk for VitDD

Burden of disease from Helicobacter pylori infection in western Canadian Arctic communities.

BACKGROUND: Indigenous communities across the circumpolar north have elevated H. pylori (Hp) prevalence and stomach cancer incidence. We aimed to describe the Hp-associated disease burden among western Canadian Arctic participants in community-driven projects that address concerns about health risks from Hp infection. METHODS: During 2008-2013, participants underwent Hp screening by urea breath test and gastroscopy with gastric biopsies. We estimated Hp prevalence and prevalence by Hp status of endoscopic and histopathologic diagnoses. RESULTS: Among 878 participants with Hp status data, Hp

Genetic diversity of hepatitis B virus genotypes B6, D and F among circumpolar indigenous individuals.

Hepatitis B virus (HBV) infection is highly prevalent in circumpolar indigenous peoples. However, the clinical outcome is extremely variable, such that while hepatocellular carcinoma (HCC) is uncommon in Canadian Inuit, the incidence of HCC is slightly higher in Greenlanders than in Danes, and it is especially high in Alaskan Native people infected with HBV genotypes F (HBV/F) and C (HBV/C). These differences may be associated with the genomic variability of the predominant HBV genotype in each group. The purpose of this study was to determine the rate, nature and regional susceptibility of HB

How systemic racism results in poorer outcomes for First Nations, and what First Nations are doing about it: the example of kidney health.

BACKGROUND: End-stage kidney disease continues to disproportionally impact the lives of First Nations peoples. Systemic racism is a key determinant, and manifests as differential access to determinants of health (housing, employment, access to care) and differential care. This paper discusses how different models of primary healthcare operating in rural and remote Manitoba communities results in different outcomes for patients identified as being at risk of kidney disease. METHODS: This study is a partnership between researchers from the First Nations Health and Social Secretariat of Manitoba

Manitoba First Nation peoples' use of hospital-based mental health services: trends and solutions.

OBJECTIVES: The objective of this article is to document patterns and trends of in-hospital mental health service use by First Nations (FN) living in rural and remote communities in the province of Manitoba. METHODS: Our sample included all Manitoba residents eligible under the Manitoba Health Services Insurance Plan living on FN reserves and those living in rural and remote communities from 1986 to 2014. Using administrative claims data, we developed multi-level models that describe hospitalization for mental health conditions shown responsive to primary healthcare interventions. We aggregat

Barriers and facilitators to the utilization of cancer screening services in two Arctic Indigenous communities in Canada.

BACKGROUND: Indigenous communities in Canada have high age-standardized rates of cancer mortality. Remote communities in Canada's northern territories have a high proportion of Indigenous residents and disproportionately low utilization rates of screening services for breast, cervical, and colorectal cancers, which could contribute to delayed cancer diagnosis and less favourable clinical outcomes. Knowledge is limited regarding the under-utilization of cancer screening services. OBJECTIVE: This study identified factors contributing to the underutilization of breast, cervical, and colorectal c

Housing conditions and respiratory morbidity in Indigenous children in remote communities in Northwestern Ontario, Canada.

BACKGROUND: Rates of lower respiratory tract infection (LRTI) among First Nations (FN) children living in Canada are elevated. We aimed to quantify indoor environmental quality (IEQ) in the homes of FN children in isolated communities and evaluate any associations with respiratory morbidity. METHODS: We performed a cross-sectional evaluation of 98 FN children (81 with complete data) aged 3 years or younger, living in 4 FN communities in the Sioux Lookout region of Northern Ontario. We performed medical chart reviews and administered questionnaires. We performed a housing inspection, including

First Nation and Métis Needs Assessment for Pre-Surgical Rehabilitation and Education of Hip and Knee Total Joint Replacements in Saskatchewan: Uplifting Indigenous Knowledges.

BACKGROUND: Arthritis impacts approximately one in five Canadians, with hip and knee osteoarthritis being the most prevalent and can require total joint replacement (TJR) surgeries. In Saskatchewan, patients access pre-surgical assessments for hip and knee replacements at urban multidisciplinary clinics, and rural and remote residents face significant barriers to accessing these services. Indigenous Peoples experience disproportionally higher rates of arthritis and additional barriers accessing appropriate care. OBJECTIVES: The objective of this project was to identify the unique needs, stren

COVID-19 impacts on decarceration for Indigenous, Black, and other racialized people in Ontario, Canada: an interrupted time series study.

BACKGROUND: The COVID-19 pandemic response in many jurisdictions included efforts to depopulate correctional facilities. In the context of the overrepresentation of Indigenous and Black people in Canadian correctional facilities, we aimed to assess COVID-19 impacts on decarceration by race and Indigenous identity in Ontario, Canada. METHODS: We accessed correctional administrative data for all people incarcerated in provincial correctional facilities in Ontario, Canada between 2015 and 2022. We categorized people using self-reported data into one of five identity groups: Indigenous, non-Indig

"There's no better medicine than my outdoors": understanding the importance of physical activity in rural and remote First Nations communities in Northern British Columbia, Canada.

BACKGROUND: While Canadian First Nations communities actively participate in sport, cultural, and land-based activities, they face various barriers to physical activity. Despite calls for increased support at all government levels, access to suitable programs for those with chronic conditions in particular remains limited. As part of an ongoing partnership between the University of British Columbia and Carrier Sekani Family Services (CSFS), this qualitative study aimed to explore current values, perceived barriers, and potential facilitators of physical activity among people living with chroni

Indigenous Perspectives on Infant Feeding in Women Living With HIV Developing Community-Driven Consensus in Canada: A Qualitative Study.

Indigenous women living with HIV (IWLWH) are compelled to navigate a colonial health care system that fails to acknowledge First Nations, Inuit, and Métis cultural practices related to childbirth and infant feeding. Infant feeding for IWLWH presents an even greater challenge because the Canadian infant feeding guidelines for WLWH recommend exclusive formula feeding to reduce the risk of vertical transmission through breast milk. Our study explored the knowledge, experiences, and values of Indigenous mothers living with HIV through an online Sharing Circle, where participants shared personal st

From knowledge to action: protocol for a mixed-methods evaluation of First Nations-led knowledge mobilisation on prenatal opioid exposure.

INTRODUCTION: First Nations communities in Canada are disproportionately impacted by prenatal opioid exposure (POE) and neonatal abstinence syndrome (NAS). In response, we developed a research partnership with 13 First Nations communities in Ontario. Phase I of the research project, initiated in 2018, included the development of mixed-methods reports on the impact of POE for each community. This protocol outlines the evaluation of phase II, during which nine communities individually co-designed and implemented community-specific knowledge mobilisation (KMb) plans informed by findings from phas

Nurturing Meaningful Intergenerational Social Engagements to Support Healthy Brain Aging for Anishinaabe Older Adults.

The emergence of Alzheimer's disease and related dementias (ADRD) in Indigenous populations across Canada is of rising concern, as prevalence rates continue to exceed those of non-Indigenous populations. The Intergenerativity Model, guided by Indigenous Ways of Knowing, nurtures a psychosocial approach to promoting healthy brain aging and quality of life. Community-based participatory action methods led by interviews, focus groups, and program observations aid in identifying the barriers to and facilitators of success for intergenerational social engagements in the Anishinaabe community of Wii

Diabetes during pregnancy and perinatal outcomes among First Nations women in Ontario, 2002/03-2014/15: a population-based cohort study.

BACKGROUND: In Canada, increasing numbers of women, especially First Nations women, are affected by diabetes during pregnancy, which is a major risk factor for adverse maternal and neonatal outcomes. The aim of this study was to examine temporal trends in pregnancy outcomes and use of health care services in a population-based cohort of First Nations women compared to other women in Ontario according to diabetes status during pregnancy. METHODS: Using health administrative databases, we created annual cohorts of pregnant women from 2002/03 to 2014/15 and identified those with preexisting diab

Interviewing Indigenous adults reporting historical child sexual abuse: The effect of question types on eliciting descriptive answers and details.

BACKGROUND: International evidence-based best practice for police interviewers of vulnerable groups, such as Indigenous populations, recommends encouraging interviewees to give a full uninterrupted account, followed by open-ended questions, to optimise memory and avoid contaminating information. However, most research examining the applicability of interview strategies on information gain has been conducted in western cultures. OBJECTIVE: There is currently little extant quantitative research on questioning in police interviews with Indigenous complainants. The primary objective of this resea

Systematic review of rheumatic disease epidemiology in the indigenous populations of Canada, the United States, Australia, and New Zealand.

OBJECTIVE: Past publications have highlighted an excess rheumatic disease incidence and prevalence in indigenous populations of Canada (First Nations, Inuit, and Métis), and the United States of America (Alaska Native and American Indian). We have updated these reviews and expanded the scope to include New Zealand (Maori) and Australia (Aborigine) indigenous populations. METHODS: We performed a broad search using medical literature databases, indigenous specific online indexes, and government websites to identify publications reporting the incidence and/or prevalence of arthritis conditions (
