The mechanisms governing the accumulation of granulocytes in inflammatory lesions are poorly understood. Using a sensitive method of sequential 111In-granulocyte scintigraphy, we recorded the speed of focal 111In-granulocyte accumulation in 70 patients with non-osseous inflammatory and infectious foci, with special reference to the influence exerted by the duration of disease, patient age, body temperature, antibiotic therapy and initial trapping of granulocytes in the lungs. Unexplained false-positive accumulation of In-111 leukocyte in the gastrointestinal tract has been reported previously. In a pediatric population, uptake in the submandibular gland was noted in 19 of 42 children. This is believed to be a normal finding in children that is not seen in adults. The extensive morbidity and mortality of intra-abdominal abscess is mainly due to the delay in a diagnosis. The diagnostic accuracy of 111In-labelled mixed leucocytes with gamma imaging has been investigated in 100 consecutive patients including 34 following surgery and 36 with inflammatory bowel disease. The diagnostic utility of pentetate indium trisodium in 111 CSF studies, technetium Tc 99m brain scans, and computerized tomographic (CT) scans was evaluated in eight patients in whom coccidioidal meningitis developed following a dust storm in the Central Valley of California. The 111In flow studies and the CT scans demonstrated hydrocephalus in five patients with clinical findings suggesting this complication. Ventriculitis has not previously been diagnosed before death in patients with coccidioidal meningitis; however, it was demonstrated in two patients by the technetium Tc 99m brain scan.

An eleven year old boy presented with headaches and dizziness associated to micturition. On radiologic imaging, he was found with a bladder mass. The biochemical work up was suggestive of pheochromocytoma. An OctreoScan (111In-pentreotide) was used to rule out metastatic extension or other extra-adrenal locations of the pheochromocytoma. OctreoScan data correlated well with other radiologic studies, operative findings and with the final diagnosis, validating its use on pediatric patients.

Indium-111-octreotide, a somatostatin analog is the first commercially available receptor-based agent. It shares the binding site of naturally occurring somatostatin and is suitable for imaging somatostatin receptor-bearing tumors. An 111In-octreotide scan performed on a 6-wk old girl showed increased uptake in a subcutaneous cavernous hemangioma, which should be added to the list of lesions that can be visualized on an octreotide scan.

Heart disease is a rare but important complication of polymyositis. Diagnosis of myocardial disease is usually based on non-specific clinical, electrocardiographic, and echocardiographic data. This paper reports a case of polymyositis with myocardial disease diagnosed by myocardial imaging with radiolabelled antibody to myosin, a specific marker of the necrotic myocardial fibre.

A proportion of syngeneic female rats inoculated intramuscularly with a lethal T-cell lymphoblastic (Roser) leukaemia are cured by a single intraperitoneal injection of bischloroethylnitrosourea (BCNU) (Carmustine) (10 mg kg-1) given towards the end of the preleukaemic phase (day 7). Additional therapy on day 4, using intravenous leukaemia cells lethally labelled with the radionuclide 114Inm, enhanced the overall cure rate by 30%. Gastrointestinal transit times were measured in 12 patients with progressive systemic sclerosis.
