Optimal care for mother and baby in severe postpartum mental illness.

BACKGROUND: Severe postpartum mental illnesses (PPSMI), including postpartum psychosis, bipolar disorder, and major depression, affect approximately 1-2 per 1000 deliveries and can lead to serious consequences including suicide, infanticide, and long-term disruptions in the mother-infant relationship. In many low-resource settings, the absence of specialized mother-baby units (MBUs) necessitates alternate models of care. METHODS: This narrative review synthesizes current evidence and clinical guidelines on managing PPMI in outpatient and inpatient settings. Key focus areas include rational ps

Denial of pregnancy: obstetrical aspects.

Between 1987 and 1990 27 women were observed who professed they did not know they were pregnant until term or until premature contractions set in. The aim of this study was to evaluate obstetric history and pregnancy outcomes and assess defence mechanisms and coping strategies which contribute to negation of pregnancy. In 11 women pregnancy was denied until delivery, five of these had breech presentations. In nine women denial ended between 27 and 36 weeks and in seven women between 21 and 26 weeks of gestation. Three of the four fetal deaths that occurred and two of the three cases of prematu

Epidermal growth factor advances some aspects of development but retards others in both rats and hamsters.

The present experiments were undertaken to confirm the recent suggestion that epidermal growth factor (EGF) can have both retarding and accelerating effects on development, using a greater number of developmental indices than hitherto; to extend such studies to another species, the golden hamster, and to compare the responses of males and females. On each of days 0-3, one male and one female rat pup from each of 16 litters of 6 pups were injected subcutaneously with human EGF (0.5 micrograms/g body weight), one male and one female with vehicle, and the remaining two pups were not injected. Health in the early contact period: a contemporary example from Papua New Guinea.

In 1984 a rare opportunity arose to document the effects of contact on a previously isolated population in Papua New Guinea. The Hagahai, a small group of hunter-horticulturalists, remained hidden from government and mission influence until the early 1980s. Prior to that time, indirect contact through trade with neighboring peoples facilitated the entry of introduced infectious diseases. In late 1983 the Hagahai sought medical aid at a mission station, an event which accelerated their contact with the common epidemic diseases of the highlands. A wide variety of genetic, linguistic, ethnographi

Sexual rights and gender roles in a religious context.

A common feature of the five faiths discussed in this article is change over time. This allowed diversity within the religions, and generally led to a degree of liberalization, but in some faiths or their sects, resulted in a narrow or rigid interpretation. For example, the golden Vedic Period of Hinduism in India when "women were worshipped" gradually faded, ushering in the social evils of female feticide, child marriage, and "sati." The advent of Islam in the seventh century banned female infanticide, but has been unable to abolish many tribal pre-Islamic customs such as female genital mutil

Child abuse: measuring a global problem.

Child abuse and neglect include four distinct conditions: physical abuse, neglect, emotional abuse and sexual abuse. The magnitude of the problem is not well defined. The lack of epidemiological data limits the extent to which sound public health and social welfare policies and intervention programmes could be developed, implemented and evaluated. In recent years two approaches have been developed to document the magnitude and nature of child abuse and neglect: regional or national case registers; and a screening instrument for suspected cases of child abuse and neglect (SCAN) for use in healt

Pictorial review of the postmortem computed tomography in neonaticide cases.

Neonaticide is defined by the deliberate killing or homicide of a child within 24 h of its birth. In this context, three fundamental questions are generally asked of the forensic pathologist: what is the cause of death of the neonate? Was the child viable (i.e., what is the gestational age of the neonate)? Finally, was the neonate stillborn or liveborn?Postmortem imaging can help answer these questions by conducting (1) a complete lesional analysis of the body and the placenta, (2) an estimation of the gestational age by measuring the lengths of the diaphyseal long bones, and (3) an analysis o

Fraught with the utmost danger: the object relations of mothers who kill their children.

The author explores the psychodynamics of maternal filicide from an object relations perspective. Among psychotic women, the murder of the child reflects a critical interplay among the mother's neurobiology, constitution, developmental experiences, and complex internal object world. Two types of personality structure are discussed. For the disorganized type, the psychodynamic scenario involves attempts to contend with the danger of massive internal breakdown. For the organized type, the scenario involves attempts to contend with the danger of persecution and annihilation. For these women, phys

Neonaticide: an appropriate application for therapeutic jurisprudence?

Might therapeutic jurisprudence, a perspective that attempts to study interaction between the legal and mental health disciplines, be brought to bear effectively with respect to neonaticide, the murder of a newborn infant in the first 24 hours of its life? This is a crime that leads to sentencing that is now rarely therapeutic, rehabilitative, or corrective. An examination of the crime, its motives, and its perpetrators precedes a discussion of ways in which the mental health viewpoint in this matter might be brought to the active attention of the courts in order to promote sentencing that is

Antisocial personality by proxy: the Norton-Sims syndrome.

An antisocial personality disorder by proxy is defined by a proposed set of diagnostic criteria and a general description of proxy and perpetrator characteristics. Subtypes, dynamics, and features of this proposed disorder are described, and five-factor personality model (Costa & Widiger, 1994) loadings for the proxy and perpetrator are hypothesized. The relationship to abuse trauma and the five-factor personality model are discussed along with implications and suggestions for future research.

Psychiatric Morbidity and Correlates in Postpartum Women in a Tertiary Care Hospital.

BACKGROUND: A range of psychological disorders occur in women in the postpartum period apart from the traditional blues, postpartum depression and psychosis. These include obsession of infanticide, PTSD, morbid preoccupations regarding child birth and disorders of mother-infant relationships, though they are under emphasized. METHODS: it is a cross-sectional study conducted in the tertiary maternity care hospital. A total of 152 study subjects were interviewed on MINI (Mini International Neuropsychiatric Inventory) and GAF (Global Assessment of Functioning) within 2 weeks after delivery. RES

Clear and compelling evidence: the Polish tribunal on abortion rights.

On 25 July 2001 the Polish Federation for Women and Family Planning organised a Tribunal on Abortion Rights in Warsaw, to publicize the negative consequences of the criminalization of abortion in Poland. A panel of Polish and foreign experts heard the testimonials of seven Polish women's experiences under the 1993 "Anti-Abortion Act". Only two of the seven women were able to tell their stories in person. One died in 2001, at the age of 21, of an unsafe abortion. One is legally blind after having carried her last pregnancy to term. One is in prison for infanticide, which in all likelihood was c

Mutual effect of Schistosoma mansoni infection and pregnancy in experimental C57 BL/6 black mice.

Sixty female C57BL/6 mice were infected with 40 Schistosoma mansoni cercariae each. Seven weeks later, they were mated with normal syngeneic males. Uninfected mice (30) were bred in parallel, and both groups were bred several more times with daily records of pregnancy, delivery and number of offsprings. The number of pregnancy was 146, with 50 survived infants (34.2%) in contrast to 121 pregnancy with 93 survived infants (76.8%) in controls. The outcome of pregnancy was 13% abortion, 10.9% maternal death and 41.7% infanticide. The weight of offspring at 2 and 4 weeks of age was significantly l

Homicidal parents.

This paper describes a series of 13 cases of parents who have killed their children. A review of the literature suggests that child murder is infrequent and committed in most instances by the parents. Most attention has been directed to the universal phenomenon of child abuse. The killing of a child in our culture is viewed much more seriously than the killing of a newborn infant, legally defined as infanticide. Only a few authors have reported on the former, and their studies tend to demonstrate that a higher proportion of these crimes are perpetrated by mothers. Homicidal behaviour in parent

Between a rock-a-bye and a hard place: mood disorders during the peripartum period.

Mood disorders including major depressive disorder and bipolar disorder are common during and after pregnancy. Timely identification and appropriate management of mood episodes is essential to maximize maternal well-being and minimize adverse outcomes. Failure to do so results in maternal suffering and impaired child bonding, and has the potential for devastating outcomes including suicide and infanticide. Women are routinely screened for unipolar depression during or after pregnancy but not for bipolar disorder, in spite of the fact that childbirth is associated with a major risk for onset or

The ecology of motherhood: the structuring of lactation costs by chacma baboons.

1. Data from a long-term study of Papio hamadryas ursinus (L.) in the De Hoop Nature Reserve, Western Cape, South Africa, were used to test the assumptions and predictions of Altmann's model of maternal time budgets. 2. Female baboons' feeding time was below model predictions, and there was no evidence for a consistent increase in feeding time with infant age. In addition, female feeding time was not significantly higher than observed baseline feeding times for nonlactating females. 3. Female baboons reduced activity levels in the first few months post-partum, as reflected in significant incre

Acts, omissions, intentions and motives: a philosophical examination of the moral distinction between killing and letting die.

Health care professionals frequently justify moral decisions by appealing to the acts and omissions distinction and the principle of double effect. These principles are often quoted and criticised in the nursing literature, particularly in relation to active and passive euthanasia. Authors are, however, inclined to acknowledge the fact that they are a philosophical minefield, but fall short of providing a substantial critical analysis. Those who uphold the acts and omissions distinction claim that we are morally less responsible for our omissions than for our acts. The principle of double effe
