Visual Guide to Opportunistic Infections in People With HIV.

Modern antiretroviral therapy has led to a dramatic reduction in HIV-associated opportunistic infections and mortality, but late diagnosis of HIV and poor linkage to care persist and lead to a worse prognosis. HIV care is now focused on early therapy initiation and virologic control, and new practitioners may have less experience and fewer educational resources for diagnosis and management of opportunistic infections. In this special issue, we outline the clinical presentation, diagnosis, and management of opportunistic infections in the modern antiretroviral therapy era alongside clinical pho

Hearing aids: family practitioner referral patterns.

This study aims to determine whether the implementation of a direct referral service in which family practitioners received feedback on the appropriateness of their patient selection had any impact on the referral pattern. Six months after the establishment of such a service to which general practitioners were requested to refer patients who satisfied the Hearing Aid Council and Therapist Technicians and Scientists in Audiology group criteria, 40 consecutive patients were analysed. 40 age matched controls referred concurrently to the routine ENT clinic for hearing aid provision were compared.

Association between glycaemic control and common infections in people with Type 2 diabetes: a cohort study.

AIM: To investigate the impact of glycaemic control on infection incidence in people with Type 2 diabetes. METHODS: We compared infection rates during 2014 in people with Type 2 diabetes and people without diabetes in a large primary care cohort in the UK (the Royal College of General Practitioners Research and Surveillance Centre database). We performed multilevel logistic regression to investigate the impact of Type 2 diabetes on presentation with infection, and the effect of glycaemic control on presentation with upper respiratory tract infections, bronchitis, influenza-like illness, pneum

Evaluation of an evidence-based, nurse-driven checklist to prevent hospital-acquired catheter-associated urinary tract infections in intensive care units.

Catheter-associated urinary tract infections account for 40% of all health care-associated infections. An evidence-based, nurse-driven daily checklist for initiation and continuance of urinary catheters was implemented in 5 adult intensive care units. Measures of compliance, provider satisfaction, and clinical outcomes were recorded. Compliance with the checklist was 50 to 100%: catheter-associated urinary tract infections decreased from 2.88 to 1.46 per 1000 catheter days and catheter days decreased in 2 intensive care units.

Risk factors for hospital-acquired non-catheter-associated urinary tract infection.

BACKGROUND AND PURPOSE: Nosocomial urinary tract infection in patients with no Foley catheter [non-catheter-associated urinary tract infection (non-CAUTI)] has been a serious health issue that is associated with an increase in the cost of care, morbidity, and mortality. Identifying the risk factors of non-CAUTI would help determine patients at high risk and prevent complications. This study aims to identify the risk factors of non-CAUTI. METHOD: This study was conducted in four hospitals in three Middle Eastern countries: Jordan, Qatar, and Saudi Arabia. A convenience sample of 189 participan

Current dental chemotherapeutics, Part 2.

Periodontal diseases are the result of bacterial infections. Antimicrobial and other chemotherapeutic agents are effective adjuncts to mechanical therapy in helping practitioners limit the progression of disease. Topical antimicrobials used as mouthrinses can control supragingival plaque. Studies demonstrate that irrigation devices enhance the effectiveness of antimicrobials. Antibiotics have been shown to be of value in treating juvenile periodontitis, and rapidly advancing and refractory periodontitis. The use of tetracyclines to promote healing by stabilizing collagen and the use of nonster

Cervical necrotizing fasciitis originating with a periapical infection.

BACKGROUND: Necrotizing fasciitis is a rapidly spreading, soft-tissue infection involving the subcutaneous tissues. Necrotizing fasciitis originating from a dental-related source is rare. Practitioners should be aware that this infection could occur in patients who are immunocompromised and in patients who are healthy. Practitioners must treat this disease aggressively with surgical debridement and intensive medical support. CASE DESCRIPTION: The authors present a case report of a man with poorly controlled diabetes mellitus in whom a periapical infection progressed into a maxillofacial space

[Microbiology of periodontitis. 1. The infective nature of periodontitis].

In the last years, guided tissue regeneration (GTR) and implantology have opened new possibilities for practical periodontologists in the treatment of advanced cases. For both techniques the successful therapy of the local infection of the pocket is a prerequisite. In the near future rapid tests for microbiological diagnoses and slow release devices for antibiotics will be available to achieve this aim easier. The following article will present the microbiological aspects of periodontitis and plaque control and show their relevance for the practitioner.

Identifying and treating common and uncommon infections in the patient with diabetes.

Patients with diabetes represent a unique group of individuals who appear more prone than others to developing infections. Several mechanisms have been proposed to explain the association between diabetes and infections. However, few conclusive studies exist and considerable debate continues regarding evidence for this predisposition. Despite this lack of documented proof, many health care practitioners acknowledge that a number of infections are overrepresented in patients with diabetes. Some of these infections appear to occur exclusively in patients with diabetes, especially patients who ar

Treatment of Severe Infections Due to Metallo-Betalactamases Enterobacterales in Critically Ill Patients.

Metallo-beta-lactamases-producing (MBL) Enterobacterales is a growing problem worldwide. The optimization of antibiotic therapy is challenging. The pivotal available therapeutic options are either the combination of ceftazidime/avibactam and aztreonam or cefiderocol. Colistin, fosfomycin, tetracyclines and aminoglycosides are also frequently effective in vitro, but are associated with less bactericidal activity or more toxicity. Prior to the availability of antibiotic susceptibility testing, severe infections should be treated with a combination therapy. A careful optimization of the pharmacok

[Food-borne streptococcal epidemics].

Approximately two days after a high school final year farewell party a striking number of pupils and teachers became ill with pharyngitis. The regional public health medical office carried out a questionnaire study with a control group of younger pupils. The regional food control unit studied the preparation and handling at food. Some bacterial cultures received from general practitioners were identified by the regional clinical-microbiological laboratory. Seventy-six percent of the 216 exposed pupils who answered (94% response rate) against 15% of 238 answering in the control group (83% respo

The role of certified foot and nail care nurses in the prevention of lower extremity amputation.

The primary purpose of this article is to outline the role, opportunities, and benefits of the certified foot and nail care nurse (CFCN). Current demographic trends toward an aging population and associated health issues such as diabetes mellitus, arthritis, peripheral arterial disease, dementia, and loss of visual acuity hinder self-care of the feet and nails. The resulting foot and nail care needs create opportunities for CFCN to promote health and prevent adverse outcomes such as infection and limb loss. Routine foot and nail assessment, clinically appropriate care, and individualized patie

Hepatitis B in general practice: epidemiology, clinical and serological features, and control.

In an outbreak of hepatitis B virus infection in a south Wales village 31 cases were identified over 16 months. Spread of the infection was by sharing needles and by sexual contact. Twenty seven patients were known to have symptoms, of whom nine had an anicteric illness. Generally the illness was mild with no deaths, and there was only one chronic carrier. Two patients were only transiently positive for hepatitis B surface antigen. The outbreak was controlled by vigorous contact tracing and counselling. Despite an enormous increase in workload for the general practitioner and problems includin

Society-Based Infection Prevention and Control Guidelines After COVID-19: A Delphi Study.

Background: Current infection prevention and control guidelines, protocols, and practices for public, outside healthcare facilities are not adequate as evidenced by increasing rates of infections and outbreaks. This study proposed to formulate a consensus on the guidelines that would then govern future public health-related infection prevention and control practices. Methods: A three-round Delphi technique was used to generate experts' consensus on the development and required modifications of the current IPC guidelines to address public safety. Eighteen infection prevention and control practi

The effectiveness of an automated algorithm as a tool for investigating the cause of anaemia in undiagnosed patients from general practitioners.

BACKGROUND: The Dutch guideline algorithm for the analysis of anaemia in patients of general practitioners (GPs) was programmed in a Clinical Decision Support system (CDS-anaemia) to support the process of diagnosing the cause of anaemia in the laboratory. This study investigates the diagnostic yield of the automated anaemia algorithm compared to that of the manual work up by the GP. METHODS: This retrospective population-based study consisted of 2697 people ≥18 years. Anaemia was defined according to the Dutch College of General Practitioners (DCGP) guideline. Causes of anaemia were based on

Developing national outcome standards for the management of gonorrhoea and genital chlamydia in genitourinary medicine clinics.

BACKGROUND: Measuring clinical performance within a framework of clinical governance is increasingly important for monitoring improvements in patient care. Standards for quality indicators must, however, be achievable and evidence based. We describe an approach to the development of national standards for measuring outcomes of care for gonorrhoea and genital chlamydia in genitourinary medicine clinics. METHODS: Two standards for each infection, one reflecting quality of case management and one reflecting partner management were chosen by consensus. A systematic review of published and unpubli

Dutch paratuberculosis programme history, principles and development.

Organised disease control started in the Netherlands in the 18th century with governmental attempts to eradicate cattle plague. At the beginning of the 20th century, the dairy industry and cattle breeding organisations initiated a programme to control infectious diseases by means of a complex system of rewards and penalties. This was also the reason for establishing the Animal Health Service in Friesland in 1919. The history of programmes to control paratuberculosis in sheep, goats, and cattle in various countries is described. The vaccination of young animals seems to be an effective measure
