Many hops, many stops: care-seeking "loops" for diabetes and hypertension in three urban informal settlements in the Mumbai Metropolitan Region.

BACKGROUND: The burden of Non-Communicable Diseases (NCDs) in urban informal settlements across Lower and Middle Income Countries is increasing. In recognition, there has been interest in fine-tuning policies on NCDs to meet the unique needs of people living in these settlements. To inform such policy efforts, we studied the care-seeking journeys of people living in urban informal settlements for two NCDs-diabetes and hypertension. The study was done in the Mumbai Metropolitan Region, India. METHODS: This qualitative study was based on interviews with patients having diabetes and hypertension

Selecting self-and-follower goal-aware leadership styles across sectors: a decision support approach.

Current leadership research falls short of addressing multidimensional performance criteria such as employee engagement, team productivity, innovation, job security, and sustainability in a balanced manner. Instead, it largely focuses solely on follower-focused approaches and fails to systematically assess leaders' own goal awareness. This situation increases the need for holistic and data-driven decision-making models for leadership style selection across various sectors. Studies conducted on this objective are quite limited in the literature. The aim of this study is to determine the most ap

Logging-related fatalities in the Eastern Black Sea region of Türkiye: a forensic-epidemiological analysis.

BACKGROUND: Logging is widely recognized as one of the most hazardous industries. Despite the prominence of this sector in Türkiye's Eastern Black Sea region, comprehensive forensic investigations of logging-related deaths are limited. METHODS: This retrospective study examined 102 logging-related fatalities identified among 4,878 forensic autopsies performed between 2013 and 2023 by the Recep Tayyip Erdoğan University. Demographic, occupational, environmental, seasonal, and medical response characteristics were extracted from autopsy reports and supplemented with information from police and

Reaching Forest Workers with Yellow Fever Vaccine Through Engagement of the Private Sector in Central African Republic.

Background/Objectives: Yellow fever (YF) outbreaks continue to affect populations that are not reached by routine immunization services, such as workers at a high risk of occupational exposure to YF. In the Central African Republic (CAR), YF cases were detected in districts characterized by the presence of workers in forest areas. We developed an innovative approach based on a local partnership with private companies of the extractive industry to administer YF vaccine to workers in remote areas during the response to an outbreak. Methods: The planning stage of the campaign included the mapping

Evaluation of the physicochemical and hygienic quality of cow's milk and its derivatives in El Jadida city Morocco.

BACKGROUND: Despite the efforts and investments made for traceability and the guarantee of a fair and safe milk product, the informal sector threatens the safety of milk. In fact, during this circuit, the product does not undergo any treatment and therefore presents serious risks for the health of the consumer. In this context, studies have been carried out on samples of peddled milk and products derived from it. OBJECTIVE: The purpose of this study is to evaluate the relevance of the informal circuit in Morocco's Doukkala region (El Jadida Province) by physicochemical and microbiological inv

Online sperm donors: the impact of family, friends, personality and risk perception on behaviour.

As informal sperm donation becomes more prevalent worldwide, understanding donor psychology and interactions is critical in providing effective policy, equitable legislative frameworks and frontline health support to an ever-growing number of global participants. We analyse data of informal sperm donors who were members of the connection website PrideAngel to identify the role and effect of several factors, e.g. kinship, social networks, personality, and risk perception, on behaviour. A key strength of the study is the ability to analyse various factors, such as the level and history of inform

Awakening from the listeriosis crisis: Food safety challenges, practices and governance in the food retail sector in South Africa.

BACKGROUND: The recent listeriosis outbreak in South Africa brought food safety concerns to the fore in terms of both policy and practice. These concerns encompass both health and nutrition aspects, as well as the economy, because the food system in South Africa contributes significantly to economic growth and food security. However, the food sector is challenged with food safety risks, such as foodborne diseases, food fraud and a general lack of effective enforcement of regulation. The inability of government to effectively regulate the food sector is a contributing factor to increased food s

Institutional assembly under constraints: pathways to universal health coverage in Brazil, Rwanda, and China.

This paper examines how Brazil, Rwanda, and China have pursued universal health coverage (UHC) beyond classical health system models developed in high-income settings. We introduce the concept of institutional assembly under constraints, offering a complementary framework for understanding UHC pathways in developing countries, where no single trajectory universally applies. Brazil's health system ensures free access at the point of care through tax-based public financing, but limited government spending has produced a dualised system of public and private provision. Rwanda extends coverage to

Economic approaches to measuring labor exploitation in agriculture: A systematic literature review of indicators and methodologies.

Agriculture remains highly vulnerable to labor exploitation, affecting almost two million workers worldwide and drawing increasing attention, particularly within the framework of Sustainable Development Goal 8 of the United Nations 2030 Agenda. However, the informal nature of exploitation and fragmented data complicate its analysis. This review examines methodological approaches and multidimensional indices used to assess labor conditions in the agri-food sector. Out of 1528 documents screened from Web of Science and Scopus, 56 articles were analyzed, revealing a predominance of quantitative m

Innovating affordable neonatal care equipment for use at scale.

The care of small and sick neonates requires biomedical technologies, such as devices that can keep babies warm (radiant warmers and incubators), resuscitate (self-inflating bags), track growth (weighing scales), treat jaundice (phototherapy units) and provide oxygen or respiratory support (hoods, continuous positive airway pressure (CPAP) devices and ventilators). Until the 1990s, most of these products were procured through import at a high cost and with little maintenance support. Emerging demand and an informal collaboration of neonatologists, engineers and entrepreneurs has led to the pro

Health seeking behaviour and self-treatment for common childhood symptoms in rural Guatemala.

This study was conducted in the Guatemalan highland department of Sacatepequez, in a sample of 146 rural women insured by the Social Security system. It examined their health care behaviour when their children presented common childhood symptoms such as diarrhoea, fever, cough and worms. The mothers generally sought help and treatment advice from an older woman in the family, and did so more often for diarrhoea (82%) and fever (64%) than for cough (43%) or worms (28%). Obtaining advice in a pharmacy or from a drug seller ranked second (range: 8%-38%, depending on the symptom), before the procu

COVID-19, a blessing in disguise for the Tech sector: Evidence from stock price crash risk.

In this paper we document that although COVID-19 has brought uncertainties to the overall economy, the Technology (tech) sector is the systematic beneficiary of the pandemic. Using a quasi-natural setup, we find a significant notion that the Stock Price Crash Risk (SPCR) of firms within the Tech sector decreases during the COVID-19 pandemic compared to the recent past and firms belonging to other sectors. Our analyses further reveal that firms in the Tech sector with stronger external monitoring and better information environment receive an even greater advantage from the pandemic. Overall, ou

Towards integrated cross-sectoral surveillance of pathogens and antimicrobial resistance: Needs, approaches, and considerations for linking surveillance to action.

Pathogenic and antimicrobial-resistant (AMR) microorganisms are continually transmitted between human, animal, and environmental reservoirs, contributing to the high burden of infectious disease and driving the growing global AMR crisis. The sheer diversity of pathogens, AMR mechanisms, and transmission pathways connecting these reservoirs create the need for comprehensive cross-sectoral surveillance to effectively monitor risks. Current approaches are often siloed by discipline and sector, focusing independently on parts of the whole. Here we advocate that integrated surveillance approaches,

Alcohol use and mental health distress correlates among artisanal gold miners in Ghana.

Despite rising concerns about alcohol use in artisanal mining, a focused investigation into its psychosocial drivers and mental health implications within sub-Saharan Africa's informal mining sector remains scarce. This study investigates the prevalence, motivations, and the association between alcohol use and mental health outcomes (depression, anxiety, and stress) among artisanal miners in Ghana. Using community based cross-sectional survey data from 664 miners, alcohol use was assessed using the Alcohol Use Disorders Identification Test, while symptoms of anxiety, stress, and depression wer

What are the challenges for antibiotic stewardship at the community level? An analysis of the drivers of antibiotic provision by informal healthcare providers in rural India.

In many low- and middle-income countries, providers without formal training are an important source of antibiotics, but may provide these inappropriately, contributing to the rising burden of drug resistant infections. Informal providers (IPs) who practise allopathic medicine are part of India's pluralistic health system legacy. They outnumber formal providers but operate in a policy environment of unclear legitimacy, creating unique challenges for antibiotic stewardship. Using a systems approach we analysed the multiple intrinsic (provider specific) and extrinsic (community, health and regula

A Case Study about Joining Databases for the Assessment of Exposures to Noise and Ototoxic Substances in Occupational Settings.

Evaluating risks associated with multiple occupational exposures is no easy task, especially when chemical and physical nuisances are combined. In most countries, public institutions have created databases, which gather extensive information on occupational exposures or work-related diseases. Unfortunately, these tools rarely integrate medical and exposure information, and, above all, do not take into account the possible adverse effects of co-exposures. Therefore, an attempt to exploit and join different existing databases for the assessment of the health effects of multiple exposures is desc

A cross-sectional study of the availability and price of anti-malarial medicines and malaria rapid diagnostic tests in private sector retail drug outlets in rural Western Kenya, 2013.

BACKGROUND: Although anti-malarial medicines are free in Kenyan public health facilities, patients often seek treatment from private sector retail drug outlets. In mid-2010, the Affordable Medicines Facility-malaria (AMFm) was introduced to make quality-assured artemisinin-based combination therapy (ACT) accessible and affordable in private and public sectors. METHODS: Private sector retail drug outlets stocking anti-malarial medications within a surveillance area of approximately 220,000 people in a malaria perennial high-transmission area in rural western Kenya were identified via a census

Were medicine quality and pharmaceutical management contributing factors in diminishing artemisinin efficacy in Guyana and Suriname?

BACKGROUND: Recent studies in Guyana and Suriname unveiled diminished efficacy of artemisinin derivatives based on day-3 parasitaemia. The migrant characteristics of the population at risk and the potential development of resistance pose a serious health threat in the region. Assessment of factors that may have contributed to this situation is warranted, and analysis of the data generated in those countries on quality and pharmaceutical managements of anti-malarials may contribute to a better understanding of this occurrence. METHODS: Data on malaria medicine quality and pharmaceutical manage

Adolescent sexual offenders: a self-psychological perspective.

Following a request for assistance in formulating a treatment philosophy for adolescent sexual offenders, a qualitative study of seven adolescent offenders was designed with a view to elaborating pre-offense, and post-offense dynamics. The point of departure was the hypothesis that sexual offending had relation to object relations. It was further hypothesized that offenders' object relations and self-development had been disfigured in childhood and adolescent development. The developmental theories of Mahler, Stern, Winnicott, and Kohut were reviewed in order to shed light on the connection be
