Journey of the Nasal Septum Into the Lungs: A Unique Complication of Cocaine Abuse.

Cocaine is a powerful addictive stimulant drug which is known to have a wide range of adverse effects across the spectrum of organ systems. Pulmonary complications have been widely reported. Intranasal inhalation has its own sequelae of complications. It is also quite common that patients who use cocaine also use other drugs. Polysubstance abuse may result in varying detrimental effects. Here, we report a unique case of nasal septal aspiration as a complication of intranasal cocaine inhalation with concomitant alcohol abuse. It is hypothesized the patient perforated his nasal septum due to chr

Necrotizing toxic laryngitis following combined polysubstance smoking of crack cocaine and heroin.

INTRODUCTION: Thermal injury of the upper aerodigestive tract related to inhalational drug abuse is uncommon but increasingly recognized. While airway injury associated with crack cocaine inhalation has been reported, the otolaryngologic manifestations of inhaled heroin vapor ("chasing the dragon") remain poorly described. CASE SUMMARY: We report the case of a 39-year-old woman presenting with progressive dysphonia and dyspnea. She had a history of polysubstance smoking including tobacco, cannabis, crack cocaine, and heroin vapor inhalation. Flexible fiber-optic nasopharyngolaryngoscopy demon

Toluene, halothane, 1,1,1-trichloroethane and oxazepam produce ethanol-like discriminative stimulus effects in mice.

Drug discrimination procedures were used to compare the discriminative stimulus properties of ETOH and volatile inhalants. Mice were trained to discriminate between i.p. injections of ETOH (1 g/kg) and saline in a two-lever operant task. Stimulus generalization was examined after 20-min inhalation exposures to toluene (300-5400 ppm), halothane (500-8000 ppm) and 1,1,1,-trichloroethane (TCE; 125-14000 ppm). Oxazepam (10-30 mg/kg i.p.) also was tested. Concentration- or dose-related increases in ETOH-lever responding were observed with toluene, halothane, TCE and oxazepam. Shared discriminative

An Exploratory Study of Inhalers and Injectors Who Used Black Tar Heroin.

AIMS: To undertake an exploratory study to examine the characteristics of patients in narcotic treatment programs who started their use of black tar heroin either as inhalers or as injectors and to compare them with those who started as inhalers but shifted to injecting. Other studies in this area have used subjects using other forms of heroin more amenable to inhaling. PARTICIPANTS, DESIGN, AND MEASUREMENT: A purposive sample of 199 patients in 6 methadone programs in Texas were interviewed in 2002-2003 using a structured instrument. FINDINGS: At admission to treatment, those who were heroin

From Poppers to Peril: Recreational Nitrite-Induced Methemoglobinemia.

Inhaled amyl nitrites, once used to alleviate chest pain, are more commonly now being used for their euphoric effects. The recreational use of inhaled nitrites can lead to methemoglobinemia, a life-threatening condition that hinders the utilization of oxygen in the body. In 2021, 2.2 million people aged 12 or older used inhalants in the past year. Methemoglobinemia has favorable outcomes with treatment. We present the case of a 43-year-old man, with a medical history of type 2 diabetes mellitus and hypertension presenting with bluish discoloration of the fingers that progressed to his tongue a

Fatal poisoning by butane sniffing: A forensic analysis and immunohistochemical detection of myocardial hypoxic damage.

Although less widespread than twenty years ago, inhalant abuse remains an on-going problem, whose incidence among U.S. teenagers and young adults ranges from 10 to 15%. Death due to the inhalation of vapor from halogenated hydrocarbons is a well-known phenomenon, yet few cases of fatal butane-gas poisoning have been described. Many cases of volatile substance abuse in prison populations have been reported: drug-addicted inmates often resort to this alternative practice when unable to get their habitual drugs of abuse. A similar pattern occurs especially among adolescents. The study herein desc

Interhemispheric insular and inferior frontal connectivity are associated with substance abuse in a psychiatric population.

Substance abuse is highly comorbid with major psychiatric disorders. While the neural underpinnings of drug abuse have been studied extensively, most existing studies compare drug users without comorbidities and healthy, non-user controls. Such studies do not generalize well to typical patients with substance abuse disorders. Therefore, we studied a population of psychiatric inpatients (n = 151) with a range of mental illnesses. Psychiatric disorders were diagnosed via structured interviews. Sixty-five percent of patients met criteria for at least one substance use disorder. Patients were recr

Assessment of reinforcement enhancing effects of toluene vapor and nitrous oxide in intracranial self-stimulation.

RATIONALE: Despite widespread abuse, there are few validated methods to study the rewarding effects of inhalants. One model that may have utility for this purpose is intracranial self-stimulation (ICSS). OBJECTIVES: This study aims to compare and contrast the ICSS reward-facilitating effects of abused inhalants to other classes of abused drugs. Compounds were examined using two different ICSS procedures in mice to determine the generality of each drug's effects on ICSS and the sensitivity of the procedures. METHODS: Male C57BL/6J mice with electrodes implanted in the medial forebrain bundle

An aetiological survey of burns in abusers of various kinds of drugs admitted to the tabriz sina hospital burns ward in iran.

A five-year prospective study (March 2003-March 2008) of burn victims hospitalized in a major burns centre in Iran was conducted in order to survey the aetiology and outcome of burns in patients who were drug addicts. Three hundred and thirty patients addicted to drugs were identified and stratified by age, sex, burn size, presence or absence of inhalation injury, kinds of abuse agents, and cause of burn. The mean patient age was 27.9 yr, and the male:female ratio was 7.6:1. There were 60 deaths overall (18.18%), the majority (47) among patients with flame burns. The mortality rate was signifi

Immunohistochemical study of rat spermatogenesis after toluene-inhalation.

After prolonged toluene-inhalation (for 20 days) at the common abuse density (1500 ppm for 4 h per day), the effect of toluene on spermatogenesis in rats was investigated. Body weight was significantly decreased in the toluene group (P < 0.05). However, the weights of the testis and epididymis were maintained. To confirm whether the toluene-inhalation influences testis and epididymis as a stress factor; anti-70kD heat-shock protein (HSP70) and c-fos gene product (c-Fos) were observed. To observe the change of the cell division and the proliferation in spermatogenesis, proliferating cell nuclea

The role of the laboratory in the investigation of solvent abuse.

1 A wide range of compounds which may be abused by inhalation such as butane, the halons, 1,1,1-trichloroethane, trichloroethylene and toluene can be detected and identified in blood specimens by means of headspace gas chromatography. Quantitative analysis of a number of the less volatile compounds of interest may also be accomplished. 2 The measurement of the urinary concentrations of benzoic and hippuric acids (metabolites of toluene) and of the toluric acids (metabolites of the xylenes) by high performance liquid chromatography may provide useful information. In general, a hippurate/creatin

Inhibition of gap junction currents by the abused solvent toluene.

Abused inhalants are a large class of compounds that are inhaled for their intoxicating and mood altering effects. They include chemicals with known therapeutic uses such as anesthetic gases as well as volatile organic solvents like toluene that are found in paint thinners and adhesives. Because of their widespread commercial use and availability, inhalants are often among the first drugs that children encounter and use of these compounds is often associated with adverse acute and long-term consequences. The cellular and molecular sites of action for abused inhalants is not well known although

Extensive Cerebral Venous Thrombosis Secondary to Recreational Nitrous Oxide Abuse.

Nitrous oxide, colloquially known as "whippets," is a commonly abused inhalant by adolescents and young adults. There are limited data describing the adverse effects of this abuse. We present a 16-year-old girl with no medical history who presented to the emergency department for confusion, hallucinations, weakness, and headaches. Imaging revealed extensive cerebral thrombosis. She had no prior history of venous or arterial thrombosis. Hypercoagulability workup demonstrated an elevated homocysteine level. She was treated with effective anticoagulation and vitamin B12 folate supplementation. To

Electroconvulsive therapy in a middle-aged adult with cortical atrophy.

A 48-year-old male with treated hypothyroidism presented with a long-standing history of depression that had been refractory to treatment. A computed tomography scan of the brain revealed atrophy in the frontal regions. This finding is significant because the patient has no known history of traumatic brain injury, inhalant or alcohol abuse, or prolonged exposure to gasoline. On the basis of the results of the brain imaging in conjunction with the patient's clinical presentation and neuropsychological testing, frontotemporal dementia was considered in the diagnosis and was thought to explain hi

Comparison of the hot-plate pain effect between three inhalation methods and subcutaneous injection of heroin.

Heroin, a widely abused opioid, is frequently consumed via inhalation; however, the majority of existing studies have focused on traditional administrations. This study aimed to compare the analgesic effects of heroin across different deliveries to elucidate the unique characteristics of inhalation. Two distinct inhalation exposure systems (nasal and systemic) were established and validated for stability. Liquid chromatography-tandem mass spectrometry was used to quantify blood concentrations of heroin and its metabolite 6-monoacetylmorphine following subcutaneous injection and three intratrac

Can inhaled heroin be a cause of bilateral globus pallidus ischemic injury?

INTRODUCTION: Bilateral globus pallidus lesions have been characteristically thought to be associated with global cerebral hypoperfusion and hypoxic changes. One of the important causative factors that need to be considered is the opiate drug abuse. CASE PRESENTATION: We present a case of a 50-year-old male who came to the hospital after being found down for 3 hours. He was found to have weakness of right upper and lower extremity. MRI showed bilateral globus pallidus infarcts. He later admitted to inhalation of heroin. Over the next few days, weakness started to improve and he was eventually

Talcum induced pneumoconiosis following inhalation of adulterated marijuana, a case report.

BACKGROUND: Talcosis, a granulomatous inflammation of the lungs caused by inhalation of talcum dust, is a rare form of pneumoconiosis. Besides inhalative occupational exposure, intravenous abuse of adulterated drugs is a major cause for this condition. Minerals such as talcum (magnesium silicate) and sand (predominant silicon dioxide) are used to increase both volume and weight of illicit substances. In intravenous heroin-abuse, talcosis is a well-known complication. Here we describe a case of talcosis caused by inhalative abuse of adulterated marijuana. CLINICAL HISTORY: A 29-year old man pr

Chemogenetic Excitation of Accumbens-Projecting Infralimbic Cortical Neurons Blocks Toluene-Induced Conditioned Place Preference.

Abuse rates for inhalants among adolescents continue to be high, yet preclinical models for studying mechanisms underlying inhalant abuse remain limited. Our laboratory has previously shown that, in male rats, an acute binge-like exposure to toluene vapor that mimics human solvent abuse modifies the intrinsic excitability of mPFC pyramidal neurons projecting to the NAc. These changes showed region (infralimbic; IL vs prelimbic; PRL), layer (shallow; 2/3 vs deep; 5/6), target (core vs shell), and age (adolescent vs adult) dependent differences (Wayman and Woodward, 2017).
