Determinants of health insurance coverage among women in Mauritania: a multilevel analysis.

BACKGROUND: Health insurance has been documented as one of the primary methods of financing healthcare for Sustainable Development Goals (SDGs) by 2030. Yet, there is a dearth of evidence on the determinants of health insurance coverage among women in Mauritania. We examine the factors associated with health insurance coverage among women in Mauritania using a nationally representative survey dataset. METHODS: We analyzed secondary data from the 2019-2021 Mauritania Demographic and Health Survey. A weighted sample of 15,714 women of reproductive age (15-49 years) was included in the study. Mu

Health insurance, perceived threat, and posttraumatic stress after suspected acute coronary syndrome.

OBJECTIVE: Threat perceptions during evaluation for acute coronary syndrome (ACS) in the emergency department (ED) predict posttraumatic stress symptoms (PSS). It is unknown how health insurance status affects threat perceptions. We tested whether lacking health insurance is associated with higher threat perceptions and PSS in patients with suspected ACS in the ED and whether threat perceptions mediate associations between lack of health insurance and subsequent PSS. METHOD: Patients in the Columbia University Irving Medical Center ED with suspected ACS enrolled in an observational cohort stu

Use of big data from health insurance for assessment of cardiovascular outcomes.

Outcome research that supports guideline recommendations for primary and secondary preventions largely depends on the data obtained from clinical trials or selected hospital populations. The exponentially growing amount of real-world medical data could enable fundamental improvements in cardiovascular disease (CVD) prediction, prevention, and care. In this review we summarize how data from health insurance claims (HIC) may improve our understanding of current health provision and identify challenges of patient care by implementing the perspective of patients (providing data and contributing to

Incidence of Campylobacter-Associated Guillain-Barré Syndrome Estimated from Health Insurance Data.

Guillain-Barré syndrome (GBS) is sometimes preceded by Campylobacter infection. We estimated the cumulative incidence of Campylobacter-associated GBS in the United States using a retrospective cohort design. We identified a cohort of patients with an International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) diagnosis code of "intestinal infection due to Campylobacter" (008.43) using MarketScan Research Databases for 2004-2013. Campylobacter patients with an encounter for "acute infective polyneuritis" (AIP; ICD-9-CM 357.0) were identified. Patients with an inpa

Factors That Influence Enrolment and Retention in Ghana' National Health Insurance Scheme.

BACKGROUND: The government of Ghana introduced the National Health Insurance Scheme (NHIS) in 2004 with the goal of achieving universal coverage within 5 years. Evidence, however, shows that expanding NHIS coverage and especially retaining members have remained a challenge. A multilevel perspective was employed as a conceptual framework and methodological tool to examine why enrolment and retention in the NHIS remains low. METHODS: A household survey was conducted after 20 months educational and promotional activities aimed at improving enrolment and retention rates in 15 communities in the C

Using Nation-Wide Health Insurance Claims Data to Augment Lyme Disease Surveillance.

OBJECTIVE: Lyme disease (LD) is the most commonly reported tick-borne illness in North America. To improve LD surveillance, we explored claims data as an adjunct data source for monitoring trends in Lyme disease incidence. METHODS: We retrospectively analyzed claims from a nationwide US health insurance plan, identifying patients with newly diagnosed LD in 13 high-prevalence states over two time periods, 2004-2006 and 2010-2012. RESULTS: The average LD case incidence as estimated by using claims data in 2010-2012 (75.67 per 100,000 person-years, n = 3474) was 1.50 times higher than 2004-2006

Smoking among pregnant women with Medicaid insurance: are mental health factors related?

Smoking during pregnancy is the single most modifiable risk factor for poor birth outcomes, yet it remains prevalent among low-income women. This study examined factors associated with continued smoking and quitting among pregnant women. A total of 2,203 Medicaid-eligible pregnant women were screened at their first enhanced prenatal services visit for risk factors including demographics, health behaviors (smoking, alcohol and drug use), mental health (history of mental health disorders, current depressive symptoms), and stress. Smoking status was divided into non-smokers, quitters (quit smokin

Equity in paying for health care services under a national insurance system.

The debate over the future of the health care delivery system evolves around the policy issue of what constitutes a fair distribution of the medical services which are considered essential to prolonging life, curing disease, and relieving pain. A case can be made that a socially equitable distribution implies that consumption of medical services is independent of the consumer's income and payment for them unrelated to utilization. The present paper examines to what extent the provisions for financing a national health insurance system are likely to advance or hinder the fair distribution of he

The impact of national health insurance on the utilization of health care services by pregnant women: the case in Taiwan.

OBJECTIVES: Substantially increased funding for health care services occurred in Taiwan after the implementation of a national health insurance plan in 1995. This study attempts to examine the impact of this national health insurance plan on the utilization of prenatal and intrapartum care services. METHODS: Nationally representative surveys of all pregnant women in Taiwan in 1989 (1,662 participants) and in 1996 (3,626 participants) were included in the analysis. We first compared the distribution of birth characteristics between the two surveys. We then calculated the rate of utilization of

State-Funded Health Insurance Expansion for Noncitizen Immigrants and Hospital Uncompensated Care.

IMPORTANCE: Undocumented and many noncitizen immigrants are largely excluded from federally funded health insurance programs, creating substantial gaps in health coverage and financial strain on safety-net hospitals that disproportionately serve these populations through uncompensated care. Several states have implemented state-funded health coverage expansions for immigrants, but the financial implications of these policies remain poorly understood. OBJECTIVE: To examine whether Health Benefits for Immigrant Seniors (HBIS) and Health Benefits for Immigrant Adults (HBIA) programs in Illinois

Pursuing Affordability, Fairness, and Efficiency in the U.S. Commercial Health Insurance Market.

The high cost of health insurance and health care services in the US is affecting not only the uninsured and lower income households, but is beginning to reach into the middle class. Current health insurance benefit designs often fail to address two forms of market failure: poor consumer information and distorted prices. These market failures distort the market for health care services, and contribute to unaffordable health insurance. When combined with the pooling function of health insurance, those sources of market failure also result in income redistributions that arguably are unfair. Usin

Who Are We Missing: Examining Health Insurance Coverage for Expanded Carrier Screening.

PURPOSE: Our study aimed to examine the effect of coverage criteria utilized by payors on the detection of an individual's carrier status. METHODS: We performed a retrospective chart review of patients and their reproductive partners at a single institution who had a positive carrier screening result to collect demographic information, carrier screening results, and family medical history. We estimated the proportion of individuals who would not have met coverage criteria for carrier screening that would detect their full carrier status based on policies used by major healthcare payors in Cal
