Reducing Neonatal Hypothermia in Premature Infants in an Israeli Neonatal Intensive Care Unit.

BACKGROUND: Neonatal hypothermia (< 36°C) has been associated with both neonatal morbidity and mortality. OBJECTIVES: To develop a multifactorial approach to reduce the incidence of neonatal hypothermia at admission to the neonatal intensive care unit. METHODS: The approach involved a detailed quality improvement (QI) plan, which included the use of occlusive wrapping and exothermic mattresses as well as higher delivery and operating room environmental temperatures. The improvement plan was implemented over a 10-month period. Retrospective comparison to the same 10-month period during the pr

Neonatal respiratory care in Vietnam: surfactant use and clinical practices in a large neonatal intensive care unit.

BACKGROUND: Neonatal mortality remains a significant health challenge, particularly in low- and middle-income settings, where respiratory distress is a major contributor to mortality. Characterizations of respiratory care practices in the neonatal intensive care unit (NICU), in lower-middle-income contexts in south-east Asia, are notably sparse in the literature. This study aimed to describe the management practices, morbidity, and mortality of newborns requiring respiratory support at a large level III NICU in Hanoi, Vietnam. METHODS: This prospective, descriptive observational study was cond

Survival at nine neonatal intensive care units in São Paulo, Brazil. Paulista Collaborative Group on Neonatal Care.

A collaborative effort to assess factors affecting newborn survival at neonatal intensive care units (NICUs) was made by studying 1948 newborns admitted to nine NICUs in the city of São Paulo between 1 June and 30 November 1991. Data on the study subjects were obtained using a standardized form. This was the first activity undertaken by a network of neonatologists (the Paulista Collaborative Group on Neonatal Care) dedicated to jointly evaluating and improving neonatal care in that city. The study results showed an overall mortality of 59 deaths per 1,000 neonates, with survival improving as g

The danger of diazoxide in the neonatal intensive care unit.

BACKGROUND: The most common cause of persistent hypoglycemia in infancy is hyperinsulinemic hypoglycemia. When conservative measures fail, providers often use medications to treat persistent hypoglycemia. Diazoxide is first-line therapy for neonatal hypoglycemia and works by inhibiting insulin secretion. Diazoxide is associated with fluid retention, and less commonly with respiratory decompensation and pulmonary hypertension. Case reports documenting these severe adverse events exist in the literature, although the overall incidence, risk factors, and timing for these effects in a newborn are

Neuroimaging to guide neuroprognostication in the neonatal intensive care unit.

PURPOSE OF REVIEW: Neurological problems are common in infants admitted to the neonatal intensive care unit (NICU). Various neuroimaging modalities are available for neonatal brain imaging and are selected based on presenting problem, timing and patient stability. RECENT FINDINGS: Neuroimaging findings, taken together with clinical factors and serial neurological examination can be used to predict future neurodevelopmental outcomes. In this narrative review, we discuss neonatal neuroimaging modalities, and how these can be optimally utilized to assess infants in the NICU. We will review commo

Screening for Postpartum Depression in a Neonatal Intensive Care Unit.

BACKGROUND: Postpartum depression (PPD) is a common mental health condition that affects approximately 13% of postpartum women in the United States. However, the prevalence in the neonatal intensive care unit (NICU) has been shown to be higher. Routine screening for maternal PPD is recommended at infant primary care well-child visits up to 6 months of age. The absence of a routine PPD screening program in a level IV NICU created a large gap in screening in an at-risk population. PURPOSE: To develop and implement a screening program in the NICU to increase the identification of maternal PPD sy

Neonatal Staphylococcus aureus acquisition at a tertiary intensive care unit.

BACKGROUND: In this study, we explored the role of colonization in health care workers (HCWs) in transmission of methicillin-susceptible Staphylococcus aureus (MSSA) to neonates at a level IV neonatal intensive care unit (NICU). METHODS: All available screening and clinical MSSA isolates, from the period March 2015 through April 2016, isolated from HCWs and neonates at the level IV NICU, were included. MSSA isolates were initially genotyped using spa typing, and for the most prevalent spa types, whole-genome sequencing (WGS) was performed. RESULTS: From March 2015 through April 2016, 159 neo

[Serratia marcescens outbreak in Neonatal Intensive Care Unit: Guayaquil, Ecuador].

We report a Serratia marcescens outbreak occurred in the NICU of a pediatric hospital in Guayaquil, Ecuador. Nine cases of infection were detected, from which septicemia was developed in 55.5%. The index case was a newborn derived from another institution with septic arthritis caused by the outbreak strain. The infection rate was 17.6% and mortality rate was 33.3%. All isolates were resistant to aminoglycosides and susceptible to third generation cephalosporins and carbapenems. Clonality analysis by pulsed-field gel electrophoresis (PFGE) revealed the presence of two closely related clones con

Adverse events and other incidents in neonatal intensive care units.

The occurrence of avoidable adverse events (AEs) represents a problem of quality of care that is responsible for the increase in monetary and social costs, causing suffering to the patient, their family members and the professional involved. This situation is aggravated when it involves newborns (NBs) with very low birth weights and shorter gestational ages, admitted to neonatal intensive care units (NICU). The scope of this study is to understand more about these incidents and adverse events in NICUs. The article aims to identify the occurrence of incidents, with and without injury that have
