BACKGROUND: Diastolic mitral regurgitation (MR) occurs when left ventricular pressure exceeds left atrial pressure during diastole, resulting from a reverse atrioventricular gradient. Diastolic MR is occasionally detected in patients with structural mitral valve abnormalities, left ventricular dysfunction, atrioventricular block, or severe aortic regurgitation. However, there are no reports on diastolic MR associated with interatrial left-to-right shunts.
CASE SUMMARY: Case 1 was a 66-year-old man diagnosed with an atrial septal defect and diastolic MR between the E wave and A wave. Case 2 wa

A case is reported of a 59 year old woman who presented with palpitations. Electrocardiographic studies revealed atrial fibrillation and atrioventricular block. Echocardiography and magnetic resonance imaging showed a right atrial cystic mass attached to the interatrial septum. The patient underwent surgical excision of the mass. Histopathological findings were of a cystic tumour of the atrioventricular nodal region. This is the second report of this condition diagnosed antemortem and treated successfully with surgical excision.

UNLABELLED: Atrial septal aneurysm (ASA) is an uncommon lesion which often associates other cardiac abnormalities such as patent foramen ovale or mitral valve prolapse. More than half of the cases with ASA associate an interatrial shunt which can explain cerebral embolism and consecutive ischemic stroke.
CASE SUMMARY: We present two cases which were admitted to our hospital with minor complaints. Personal histories excluded cardiac disease, high blood pressure, smoking, and included ischemic stroke in both cases. Physical examination revealed no abnormalities. Electrocardiogram showed right b

BACKGROUND: The atrial and ventricular response observed immediately after cessation of ventricular induction/entrainment pacing is commonly analyzed to discriminate atrial tachycardia from other supraventricular tachycardias during electrophysiologic studies. However, the response in fast-slow atrioventricular nodal reentrant tachycardia (F/S-AVNRT) remains poorly investigated.
OBJECTIVE: The purpose of this study was to analyze the atrial and ventricular activation patterns after ventricular pacing in F/S-AVNRT.
METHODS: We enrolled 28 patients with F/S-AVNRT incorporating a typical slow p

AIMS: Biventricular pacing (BVP) can improve haemodynamics in patients with dilated cardiomyopathy (DCM) and left bundle branch block by reducing interventricular delay (IVD). Since in DCM interatrial delay (IAD) and IVD frequently coexist, the aim of this study was to test the hypothesis that IVD reduction associated with IAD produces an imbalance between the programmed right atrioventricular (AV) delay and the effective AV delay on the left, and that interatrial septum pacing (IASP) combined with BVP overcomes this adverse effect.
METHODS AND RESULTS: IAD, IVD, left and right mechanical atr

Wegener's granulomatosis is a systemic necrotizing granulomatous vasculitis of small- to medium-sized vessels typically affecting upper and lower airways, lungs, and kidneys. Cardiac involvement is less common and conducting tissue involvement is extremely rare. Cardiac manifestations are often not clinically apparent, but are associated with increased mortality. We report the case of a 36-year-old female with Wegener's thought to be in remission, presenting in complete atrioventricular (AV) block, with echocardiographic evidence of basal interatrial septum and basal lateral left atrial wall t

A new technique to block conduction from both atria to the atrio-ventricular node (A-V node) was devised through anatomical considerations of the junctional area. To assess the validity of the technique, a series of experiments were performed in mongrel dogs. Under cardiopulmonary bypass, the interatrial septum was incised along the tendon of Todaro, followed by cryosurgical ablation of the approach to the A-V node. Postoperative electrophysiological studies revealed complete conduction block between both atria and the A-V node. A spontaneous A-V nodal rhythm was produced. Initial activation w

OBJECTIVE: Current fetal cardiac intervention for restrictive atrial septum is invasive and technically demanding. We investigated the feasibility of computer-assisted high-intensity focused ultrasound (HIFU) for cardiac intervention on the atrial septum of a beating heart.
METHODS: To create an interatrial communication in the beating heart of nine anesthetized rabbits, the heart was exposed surgically and placed under a water-filled tank, in contact with the tank's membranous base. A HIFU transducer (3.3 MHz) coupled with a diagnostic ultrasound probe (10.0 MHz) was placed in the tank over

A variety of atrial dysrhythmias including paroxysmal atrial tachycardia, atrial tachycardia with 2nd-grade atrioventricular block, atrial fibrillation, and atrial flutter developed in a 5-year-old Quarter Horse gelding. Quinidine and propranolol were not successful in restoring normal sinus rhythm. Sinus rhythm was re-established during digoxin therapy, but later reverted to atrial dysrhythmia. At necropsy, multiple, discrete pale areas were found on both atria and the interatrial myocardium. Histologic examination of these lesions demonstrated myocytolysis and replacement by fibrous connecti

AIM: To optimize recommendations for programming of the rate-adaptive atrioventricular (AV) delay.
METHODS AND RESULTS: Optimal AV delay (AVD(opt)) is the net effect of the pacemaker-related interatrial conduction time (IACT), duration of the left-atrial electromechanical action (LA-EAC(long)) and duration of left-ventricular latency (S(V)-EAC(short)). It can be calculated by AVD(opt) = IACT + LA-EAC(long)-S(V)-EAC(short). We measured these three components in 20 DDD pacemaker patients (EF >45%) with the third degree AV block (AVB) at rest and submaximal ergometric exercise load of 71 +/- 9 W

Electrophysiologic studies with recordings of multiple intracavitary electrograms were performed in two patients with atrial dysrhythmias. In Case 1 the arrhythmic pattern in the surface electrocardiogram resembled atrial flutter. Electrophysiologic studies revealed the arrhythmia to be paroxysmal left atrial tachycardia, with separation of left and right atrial components of the P wave by an isoelectric period secondary to marked interatrial conduction delay. In Case 2 the surface electrocardiogram indicated paroxysmal atrial tachycardia with block. Electrophysiologic studies revealed right a

BACKGROUND: The aim of this paper is to present our own experience related to frequency of symptomatic tachy- and bradyarrhythmias in patients after transcatheter closure of interatrial defects (ASD) and patent foramen ovale (PFO) using Amplatzer plugs.
METHODS: Transcatheter closure of interatrial communications with Amplatzer devices was carried out on 739 patients in our center. Only patients with new symptomatic arrhythmias (who required pharmacotherapy, cardioversion or pacemaker implantation) were included in to the study. All patients who had had arrhythmias prior to ASD closure, such

A 63-year-old man was admitted to our emergency department with the complaint of syncope. An electrocardiogram showed complete atrioventricular block with a low ventricular rate. He was hospitalized and a temporary transfemoral pacemaker was inserted under direct fluoroscopy urgently. Immediate postprocedural electrocardiogram in the paced mode showed right bundle branch block morphology that led to suggestion of abnormal lead location. A transthoracic echocardiogram revealed that the pacemaker lead was directly inserted from the right atrium into the left ventricle through the junction of the

BACKGROUND: P-wave parameters, readily obtainable from standard 12-lead ECGs, have been associated with atrial fibrillation (AF), ischemic stroke, and other cardiovascular conditions. Left atrial cardiomyopathy (AtCM), characterized by atrial fibrosis and functional impairment, is considered a central substrate in the development of AF and embolic stroke of undetermined source. This study examines the relationship between P-wave parameters and left atrial hemodynamics and evaluates their potential diagnostic utility in identifying AtCM.
METHODS: We conducted a monocentric, prospective study i

We report a case of a 58 year old gentleman with prior history of catheter ablation for persistent atrial fibrillation (AF). His baseline ECG showed sinus rhythm with a broad and notched P-wave in lead II and biphasic P-wave (positive/negative) in leads III and aVF previously described as advanced interatrial block. A redo ablation procedure was performed due to AF recurrence. An iatrogenic isolation of the coronary sinus (CS) was observed during ablation with marked narrowing and loss of the terminal negative component of the P-wave on the surface ECG.

BACKGROUND: After acute myocardial infarction (AMI), it is common to observe new-onset atrial fibrillation (NOAF), which is often related to a negative prognosis. Some P-wave variables (P-wave duration [PWD], P-wave amplitude, and interatrial block [IAB]), reflecting the process of electrical and structural remodeling, could predict the risk of atrial fibrillation (AF). This study aimed to assess the predictive value of P-wave variables for post-AMI NOAF.
METHODS: We retrospectively analyzed 1581 AMI patients with no prior AF, using follow-up data from January 2023 to January 2024. P-wave var

OBJECTIVES: This study sought to quantify characteristics of atrial conduction disorders in patients with right atrial (RA) volume overload.
BACKGROUND: Patients with an interatrial shunt are prone to developing atrial fibrillation (AF), which may be related to conduction disorders occurring due to atrial stretch.
METHODS: Thirty-one patients undergoing surgery for an interatrial shunt (49 ± 14 years of age) underwent epicardial sinus rhythm mapping of the RA, Bachmann's bundle (BB), and left atrium (LA). Conduction delay (CD) was defined as interelectrode conduction time (CT) of 7 to 11 ms

Cystic tumor of the atrioventricular node is an extremely rare primary tumor of the heart. Here we report a case of a 41-year-old female who presented with dizzy and palpitations. Electrocardiography revealed third-degree atrioventricular block. Echocardiography and Chest computed tomography showed a mass attached to the interatrial septum. Intraoperative frozen pathological examination revealed that the mass was the cystic tumor of the atrioventricular node. The diagnosis was proved by histopathology and immunohistochemistry. This is the first case that was diagnosed dependent on frozen patho
