[Anatomical study of the ramification and intramuscular distribution of the intercostal nerves in man].

Minute dissection was performed on the ventral primary rami of the thoracic and the first lumbar nerves in the left half of an adult male cadaver in order to obtain more detailed data of the nerve supply to the muscles of the thorax and abdomen. The ribs, costal cartilages, sternum and vertebral column were removed to facilitate dissection. The origin, course and intramuscular distribution of the muscular branches of each nerve were examined in precise detail. The main findings are as follows: 1. The intercostal nerves The collateral branches were found in approximately 1/3 of the segments exa

Timing of activation of different inspiratory muscles during incremental inspiratory loading in healthy adults: A cross-sectional study.

PURPOSE: To evaluate whether the onset and duration of electromyography (EMG) activity of different inspiratory muscles vary during an incremental inspiratory threshold loading (ITL) in healthy adults and whether it is associated with dyspnea and inspiratory mouth pressure (Pm) at task failure. METHODS: Twelve healthy adults (30 ± 7 years, six females) performed incremental ITL starting at warm-up (7.6 ± 1.7 cmH2O), followed by 50 g increments every two minutes until task failure in this cross-sectional study. EMG onset (relative to inspiratory flow) and activity duration of the costal diaphr

Mechanical properties of respiratory muscles in primates.

We examined the in-vitro mechanical characteristics of the diaphragm and parasternal intercostal muscles in adult baboons. At optimal length (Lo) the parasternal intercostal muscles were consistently faster than the diaphragm as judged by a shorter time-to-peak tension and a reduced twitch-to-tetanus ratio. However, maximal absolute force at Lo, corrected for cross-sectional area, revealed that the inherent strengths of both inspiratory muscles were identical (31.5 +/- 3.2 N/cm2 for diaphragm vs 29.5 +/- 1.1 N/cm2 for parasternal intercostals). Moreover, at submaximal stimulation rates, the pa

Elbow flexion strength and contractile activity after partial ulnar nerve or intercostal nerve transfers for brachial plexus injuries.

We compared the outcomes of 23 partial ulnar nerve and 15 intercostal nerve transfers for elbow flexion reconstruction in patients with C56 or C567 brachial plexus injuries using manual muscle power, dynamometric measurements of elbow flexion strength and electromyography. The range of elbow flexion and muscle strength recovery to Grade 3 or 4 were comparable between the two groups. The patients with C567 injuries had significantly stronger eccentric contraction after the partial ulnar nerve transfer than after the intercostal nerve transfer (p < 0.05). Electromyography of individual muscles d

[Spinal cord ischemia caused by dissecting aortic aneurysm in a patient with acute exacerbation of pulmonary emphysema].

A 61-year-old man with pulmonary emphysema was admitted due to acute exacerbation of chronic respiratory failure and a complaint of chest pain. A chest CT scan on admission showed aneurysmal dissection from the ascending aorta to the descending aorta. Analgesia was noted below the fourth thoracic vertebra, which supplies the accessory respiratory muscles including the intercostal muscles. Even after recovery from circulatory failure, his chest muscles were weak and he could not be removed from mechanical ventilation. An autopsy revealed ischemia of the spinal cord at the T4 level. In contrast,

Respiratory muscle response to flail chest.

We have previously shown that flail chest in the dog causes an inspiratory inward displacement of the ribs and an increased inspiratory activity in the external intercostal muscles, and we have speculated that this increased activity is due to an increased spindle afferent activity. The present studies were designed to test this hypothesis. Twenty-nine supine anesthetized dogs were studied, and flail was produced surgically by fracturing ventrally and dorsally two to four contiguous ribs on the right side of the chest. Although flail elicited an increased inspiratory activity in the external i

Intercostal-lumbar-spinal nerve anastomoses for cord transection. A preliminary investigation.

Since 1978, 10 intercostal nerve-cauda equina anastomoses and 11 intercostal nerve-lumbar anastomoses were performed for thoracolumbar spinal cord transection. Three to four pairs of intercostal nerves above the traumatic level of the cord were anastomosed with two to four pairs of cauda equina or lumbar nerve roots below the traumatic level, using microsurgical technique. The surgical techniques used for the latter 11 cases were performed through an extraperitoneal approach. The operative technique is described in detail. These 21 cases have been followed-up for six to 48 months. Electromyogr

Intercostal electromyographic responses to multi-site chest wall vibration at an optimal frequency.

BACKGROUND: High-frequency mechanical vibration facilitates respiratory muscle activity via muscle spindle afferents. Although synchronised multi-site chest wall vibration enhances ventilation, the associated intercostal electromyographic (EMG) responses and their contribution to ventilatory changes remain unclear. This study investigated the effects of multi-site chest wall vibration on intercostal muscle activation and ventilation in healthy adults. METHODS: Twenty healthy adults underwent phase-synchronised chest wall vibration delivered to four or eight intercostal sites during inspiratio

Propriospinal myoclonus: a neurophysiologic analysis.

We are reporting a neurophysiologic analysis of two patients presenting with thoracoabdominal spontaneous muscle jerks. Polymyographic recordings showed myoclonic bursts with onset in the upper rectus abdominis or lower intercostal muscles followed by rostral propagation to the upper intercostal and caudal propagation to the abdominal muscles by slowly conducting pathways. Jerk-locked back-averaging did not show time-locked cortical or premovement potentials. Peroneal somatosensory evoked response, C-reflex, and intercostal nerve conduction were normal. These findings suggest a generator for t

Rehabilitation, Using Guided Cerebral Plasticity, of a Brachial Plexus Injury Treated with Intercostal and Phrenic Nerve Transfers.

Recovery after surgical reconstruction of a brachial plexus injury using nerve grafting and nerve transfer procedures is a function of peripheral nerve regeneration and cerebral reorganization. A 15-year-old boy, with traumatic avulsion of nerve roots C5-C7 and a non-rupture of C8-T1, was operated 3 weeks after the injury with nerve transfers: (a) terminal part of the accessory nerve to the suprascapular nerve, (b) the second and third intercostal nerves to the axillary nerve, and (c) the fourth to sixth intercostal nerves to the musculocutaneous nerve. Spinal localization of the intercostal motoneurones innervating the upper thoracic spaces.

At the rostral level of the thorax, the intercostal muscles participate both in postural and respiratory functions to a variable degree depending upon the considered muscle: external intercostal, intercartilaginous, internal intercostal, and triangularis sterni. In order to determine if these physiological properties are related to a special organization at the spinal cord level, we have used the retrograde transport of horseradish peroxidase as a tool for studying the spinal distribution of intercostal motor cells in the adult cat. Results suggest that the intercostal motoneurones could be di

Simulation of acute spinal cord injury: effects on respiration.

The respiratory effects of acute spinal injury and paralysis are difficult to study. Urgent medical needs of human spinal cord injury victims usually preclude study, while induction of spinal cord lesions in awake animals is not feasible ethically. We utilized controlled, segmental infusion of epidural anesthetic in awake, highly trained, implanted canines to reversibly simulate the effects of thoracic and cervical (paraplegic and quadriplegic) spinal cord injury. We studied six animals, an average of 29 days after implantation with electromyogram and sonomicrometry transducers in transversus

Histamine induced airway response in pre-school children assessed by a non-invasive EMG technique.

The aim of the study was to investigate the association between surface electromyographic (EMG) activity of the diaphragm and intercostal muscles, and clinical symptoms (wheeze, cough, increased respiratory rate and prolonged expiration) during bronchial challenge testing and after administration of salbutamol in asthmatic pre-school children. A histamine challenge test was performed in 20 asthmatic pre-school children. The histamine dose at the appearance of 1 or more clinical symptoms was defined as the maximum histamine provocation dose (PDcs). The clinical symptoms were recorded with a mic

The craniosacral progression of muscle development influences the emergence of neuromuscular junction alterations in a severe murine model for spinal muscular atrophy.

AIMS: As 4-day-old mice of the severe spinal muscular atrophy (SMA) model (dying at 5-8 days) display pronounced neuromuscular changes in the diaphragm but not the soleus muscle, we wanted to gain more insight into the relationship between muscle development and the emergence of pathological changes and additionally to analyse intercostal muscles which are affected in human SMA. METHODS: Structures of muscle fibres and neuromuscular junctions (NMJs) of the diaphragm, intercostal and calf muscles of prenatal (E21) and postnatal (P0 and P4) healthy and SMA mice were analysed by light and transm

Respiratory muscle activity and fatigue response during respiratory endurance testing in patients with spinal muscular atrophy.

OBJECTIVE: To explore respiratory muscle activity and fatigue response during a respiratory endurance test (RET) in patients with spinal muscular atrophy (SMA). METHODS: Fifty-five participants with SMA types 2 and 3 performed a respiratory endurance test (RET) at 20 %-70 % of their maximum inspiratory mouth pressure (PImax). We recorded surface electromyography (sEMG) of the diaphragm, intercostal, and scalene muscles and evaluated muscle activity at onset and during the test. Respiratory muscle fatigability (RMF) was defined as the inability to complete 60 consecutive breaths during the RET

Evaluation of an intercostal myoneurovascular transposition as a lower esophageal neosphincter.

Previous work has shown promising results for an intercostal myoneurovascular transposition in the prevention of gastroesophageal reflux following esophagectomy. A first study evaluated the intercostal transposition procedure and compared it with the Nissen fundoplication using a rabbit model of gastroesophageal reflux. Group A underwent partial cardiomyectomy to produce gastroesophageal reflux. Group B underwent cardiomyectomy, and intercostal transposition around the gastric cardia. Group C underwent Nissen fundoplication and cardiomyectomy. All animals had preoperative and 1-week and 4-week

Effect of acute inflation on the mechanics of the inspiratory muscles.

When the lung is inflated acutely, the capacity of the diaphragm to generate pressure, in particular pleural pressure (Ppl), is impaired because the muscle during contraction is shorter and generates less force. At very high lung volumes, the pressure-generating capacity of the diaphragm may be further reduced by an increase in the muscle radius of curvature. Lung inflation similarly impairs the pressure-generating capacity of the inspiratory intercostal muscles, both the parasternal intercostals and the external intercostals. In contrast to the diaphragm, however, this adverse effect is large

Intercostal lung herniation secondary to thoracotomy: a case report.

Intercostal lung herniation is defined as a protrusion of the lung parenchyma through a defect in the intercostal muscles between adjacent ribs. The authors report a case of intercostal pulmonary hernia in a 45-year-old male patient, with smoking habit (30 packs-year), presented to the emergency department with dyspnea. He had the history of pulmonary emphysema complicated with a total right pneumothorax in 2015 treated by mini-thoracotomy with bullectomy and pleural abrasion. In 2019, he was admitted to hospital for left chest pain. The computed tomography (CT) scan of the chest revealed a bi

Glutathione metabolic responses to loaded breathing: variation among respiratory muscles.

Several studies have shown that loaded breathing elicits an oxidation of reduced glutathione (GSH) to oxidized glutathione (GSSG) within the diaphragm, but the effects of loaded breathing on GSH and GSSG levels in other respiratory muscles have not been examined. The present experiment examined this issue by using decerebrate unanesthetized rats in which a large inspiratory resistive load was applied until respiratory arrest. Subsequently, muscle samples from the triangularis sterni, diaphragm (Dia), parasternal intercostal (PI), upper rib cage lateral intercostal, lower rib cage lateral inter
