Persistent school dysfunction: unrecognized comorbidity and suboptimal therapy.

To determine reasons for continued school dysfunction in children previously diagnosed as having attention deficit hyperactivity disorder (ADHD) or enrolled in a special education program (spec. ed.), a retrospective chart review of patients referred for interdisciplinary evaluations at a tertiary center for hyperactivity and learning problems was completed. Interdisciplinary clinical recommendations were used to define reasons for treatment failure in 116 children with prior diagnosis of ADHD or spec. ed. placement. Results showed 45% of children enrolled in spec. ed. had previously undiagnos

Air Leak Syndrome After Percutaneous-endoscopic Rendezvous for Malignant Biliary Obstruction: A Case Report.

Endoscopic retrograde cholangiopancreatography (ERCP) is a key therapeutic tool for biliary access, but cannulation failure occurs in 5%-10% of cases. We report a 92-year-old woman with obstructive jaundice and complete distal common bile duct obstruction, in whom standard ERCP failed. A percutaneous-endoscopic rendezvous (PE-RV) approach successfully established biliary drainage via a transhepatic guidewire and stent placement. However, the patient developed pneumoperitoneum, tension pneumothorax, and subcutaneous emphysema following abrupt catheter removal. These manifestations are collectiv

Periprosthetic Joint Infection by Streptococcus bovis Reveals Hidden Colorectal Cancer: A Case Report.

Periprosthetic joint infection (PJI) caused by Streptococcus bovis (S. bovis) is rare but clinically significant due to its established association with colorectal neoplasia. Early recognition and interdisciplinary management are essential to ensure favorable outcomes. We report the case of a 68-year-old woman who presented with a chronic fistula and signs of active infection 20 years after uncemented total hip arthroplasty. Cultures from the wound identified S. bovis, prompting further evaluation. Imaging and laboratory tests supported a diagnosis of chronic PJI. A two-stage revision was perf

Comprehensive integrated NGS-based surveillance and contact-network modeling unravels transmission dynamics of vancomycin-resistant enterococci in a high-risk population within a tertiary care hospital.

Vancomycin-resistant E. faecium (VRE) are an important cause of nosocomial infections, which are rapidly transmitted in hospitals. To identify possible transmission routes, we applied combined genomics and contact-network modeling to retrospectively evaluate routine VRE screening data generated by the infection control program of a hemato-oncology unit. Over 1 year, a total of 111 VRE isolates from 111 patients were collected by anal swabs in a tertiary care hospital in Southern Germany. All isolated VRE were whole-genome sequenced, followed by different in-depth bioinformatics analyses includ

Interventional electrophysiology and cardiac resynchronization therapy: delivering electrical therapies for heart failure.

Implantable devices have become a readily available option for patients with heart failure. Not only do these patients develop bradycardia and ventricular tachycardia, but their ventricular dysfunction can often improve with cardiac resynchronization therapy. However, this is a complex and rapidly developing clinical science for which the physician chooses techniques and selects patients on the basis of the results of clinical trials, clinical experience, and rapidly evolving tools. The results depend on the interplay of these complex variables. Placement of the left ventricular lead has force

Pericardial Tamponade following Asymptomatic SARS-CoV-2 Infection: A Diagnostic Journey.

Background. Pericardial tamponade is a known life-threatening condition rarely reported in COVID-19 but has not been reported following asymptomatic SARS-CoV-2 infection. Its pathomechanism is still elusive. Case Summary. We report the case of a 66-year-old man with progressive shortness of breath and leg swelling due to new-onset heart failure and pericardial tamponade following asymptomatic SARS-CoV-2 infection. Ultrasound-guided placement of a pericardial drainage led to significant improvement of symptoms and revealed an exudative effusion. Throughout the diagnostic process, we were confro

Torturous path of an elastic gap band: Interdisciplinary approach to orthodontic treatment for a young patient who lost both maxillary central incisors after do-it-yourself treatment.

Spacing between the maxillary anterior teeth is a common concern for young patients and their parents. Patients often consider a diastema to be an annoying but minor problem; they might consult their general dentists for help, or try to address the issue themselves by applying elastic "gap bands." Moreover, parents, without any informed consent, sometimes accept this erroneous method as an easy and inexpensive treatment approach. A 9-year-old boy had severe acute periodontitis involving the maxillary central incisors caused by the placement of an elastic band and its apical migration. Despite

Children with self-injurious behavior.

Self-injurious behavior is a serious problem that is not uncommon among individuals with mental retardation. Medical and developmental characteristics of 97 children, adolescents, and young adults (age range 11 months to 21 years, 11 months) assessed and treated for self-injurious behavior in a specialized, interdisciplinary inpatient unit between 1980 and 1988 were reviewed. This population differed from those reported in previous studies in that it was of school age and predominantly community based. Severe or profound mental retardation was present in 82.5% of our patients. The causative di

Caudally directed posterior high-dose-rate brachytherapy for reirradiation of para-aortic lymph node recurrence: A technical innovation enabled by interventional radiology.

PURPOSE: Reirradiation for para-aortic lymph node (PALN) recurrence is technically challenging due to prior radiation exposure and the anatomical proximity of critical organs such as the small intestine and kidneys, making external beam radiation therapy (EBRT) difficult or unfeasible. High-dose-rate interstitial brachytherapy (HDR-IBT) offers a highly localized treatment alternative; however, it requires precise applicator placement to ensure safety and efficacy. METHODS AND MATERIALS: We report a case of PALN recurrence located between the aorta and inferior vena cava (IVC) in a patient pre

Neuronavigated placement of iliosacral screws in sacral fractures: an additional tool in the spine surgeon's arsenal.

Iliosacral screw fixation is a cornerstone technique for stabilizing posterior pelvic ring injuries, particularly vertical sacral wing fractures and sacroiliac joint disjunctions. Although traditionally performed under fluoroscopic guidance, conventional two-dimensional (2D) imaging exposes both patients and surgical teams to significant radiation and carries a non-negligible risk of screw malposition, with potential injury to neural and vascular structures. The integration of intraoperative three-dimensional (3D) neuronavigation represents a major technological advance that enhances the accur

Penetrating orbital floor injury by an undetected foreign body: a case report.

Orbital trauma can result in significant complications, particularly when accompanied by foreign body entrapment. Wooden foreign bodies are rare but carry a high risk of infection and chronic inflammation. In these cases, immediate surgical intervention is critical for restoring orbital anatomy and preventing complications. A 16-year-old male patient presented with a 2-month history of persistent pus discharge from his right cheek following facial trauma sustained from a fall. Initial management involved drainage of the abscess; however, the condition persisted. On examination, the patient exh

Bridging the "med-ed gap" for students with special health care needs: a model school liaison program.

A successful school experience is critical to the development of all children, particularly in the areas of academic achievement, regular school attendance, and social competency. Vulnerabilities in achieving each of these three goals have been documented among students with special health care needs (SSHCN), and ascribed to the influence of their health-related disabilities. Despite recognition of these vulnerabilities, barriers still exist to successful integration of SSHCN into educational settings. A key barrier to successful integration involves poor linkages between the health and educat
