Development and validation of a model to predict platelet response to romiplostim in patients with lower-risk myelodysplastic syndromes.

Low endogenous erythropoietin levels and limited red blood cell transfusion history can predict response to erythropoiesis-stimulating agents in anaemic patients with myelodysplastic syndromes (MDS). The relationship between endogenous thrombopoietin (THPO) levels and platelet response to romiplostim is unknown. Variables including baseline endogenous THPO levels, transfusion needs, and platelet response were analysed in a randomized trial of 250 thrombocytopenic, lower-risk MDS patients (International Prognostic Scoring System low/intermediate-1). A predictive scoring system was developed bas

Breaking the Silence: A Mixed-Methods Study of Barriers and Solutions to Incident Reporting in Critical Care Units.

BACKGROUND: Incident reporting is a cornerstone of patient safety, particularly in critical care settings where patients are highly vulnerable to adverse events. Despite international efforts to promote safety reporting, underreporting persists, especially in low- and middle-income countries. Nurses, as frontline providers, are central to detecting and reporting incidents; however, psychological, organizational, and cultural factors often limit their engagement in reporting processes. AIM: To explore barriers to incident reporting among nurses working in critical care units and identify nurse

Waste respirator processing system for public health protection and climate change mitigation under COVID-19 pandemic: Novel process design and energy, environmental, and techno-economic perspectives.

The ongoing COVID-19 pandemic leads to a surge on consumption of respirators. This study proposes a novel and effective waste respirator processing system for protecting public health and mitigating climate change. Respirator sterilization and pre-processing technologies are included in the system to resist viral infection and facilitate unit processes for respirator pyrolysis, product separation, and downstream processing for greenhouse gas (GHG) emission reduction. We evaluate the system's environmental performance through high-fidelity process simulations and detailed life cycle assessment.

A Geometric Classification of Jaw Deformities.

In the United States, the most widely used classification system for jaw deformities is the one provided by the International Classification of Diseases, Clinical Modification (ICD-CM), a taxonomy scheme that is based on the World Health Organization's International Classification of Diseases (ICD). The last iteration of ICD-CM, version 10, sorts jaw deformities according to geometry, into 3 groups: anomalies of jaw size, anomalies of jaw-cranial base relationship, or unspecified. Yet these deformities can affect 6 different geometric attributes: size, position, orientation, shape, symmetry, a

Detecting Directional Coupling in Network Dynamical Systems via Kalman's Observability.

Detecting coupling in network dynamical systems from time series is an open problem in the physics of complex systems. In this Letter, we tackle this issue from a control-theoretic perspective. Drawing inspiration from Kalman's notion of observability, we argue the presence of directional coupling between two units, X→Y, when X is detected as an internal state from the measurement of Y. We illustrate this approach on a series of analytically tractable systems, showcasing how it overcomes limitations of state-of-the-art methods for network inference.

Nail disorders as clues to systemic disease.

Disorders of the nail unit can provide important clues to underlying systemic diseases. Onychodystrophy or nail abnormalities include altered nail color, shape, or texture with morphology and growth patterns often related to local anatomic factors such as edema, vascular supply, and neurologic innervation. Associated pain or loss-of-function can also affect daily activities. Because nail changes may be due to a range of systemic diseases, the authors provide a systems-based broad overview of the common nail findings in patients with internal disease.

Assembled pH-Responsive Gastric Drug Delivery Systems Based on 3D-Printed Shells.

Gastric acid secretion is closely associated with the development and treatment of chronic gastritis, gastric ulcers, and reflux esophagitis. However, gastric acid secretion is affected by complex physiological and pathological factors, and real-time detection and control are complicated and expensive. A gastric delivery system for antacids and therapeutics in response to low pH in the stomach holds promise for smart and personalized treatment of stomach diseases. In this study, pH-responsive modular units were used to assemble various modular devices for self-regulation of pH and drug deliver

From the TPR pathway and the neuroimmune unit to the cutaneous HPA axis: a neuro-endo-immune cross-talk in rosacea.

The pathogenesis of rosacea is complex, and most of the pathogenesis is not completely clear, but the nervous system, the internal system and the immune system dysregulation is an important factor mediating the pathogenesis of rosacea, yet the three are not a single system's role, it is the mutual influence, the interaction together mediate the pathogenesis of rosacea. The skin is essentially a whole of neural, internal and immune functions. From the neurological aspect, the skin realizes information transmission by means of neural pathways and receptors, and releases neurotrophins and neurope

Can financial retention incentives slow health worker migration? PPP-adjusted salary differentials in Zimbabwe's donor-dependent health system.

BACKGROUND: Large-scale migration of health workers from low-income countries continues to undermine health system capacity, particularly in contexts characterised by fiscal constraint and donor dependence. While retention incentives are widely used to mitigate attrition, their ability to offset international wage differentials remains contested. Zimbabwe provides a salient case, having implemented a long-standing Health Worker Retention Scheme (HWRS), largely financed by external partners, to stabilise critical cadres within the public health sector. This study aimed to quantify PPP-adjusted
