Multi-level experiences of HIV-related stigma among people living with HIV: an institution-based qualitative study.

BACKGROUND: Stigma remains a pervasive barrier to the health and well-being of people living with HIV (PLHIV), affecting access to treatment and care. In Nepal, discriminatory attitudes and behaviors persist across all levels, often resulting in mental distress, social isolation, and delayed care-seeking. Guided by the Intersectional Stigma Framework, this study explored how lived HIV -related stigma operates across individual, interpersonal, community, and structural levels, and how existing social and institutional mechanisms shape the lived experiences of PLHIV. METHODS: A qualitative study

A Reproductive Justice Response on Social Egg Freezing.

Current bioethical reflections on social egg freezing appear to be dominated by a rights-based perspective, as illustrated by the recent publication by Gao and Liu in this journal. We argue that, to avoid theoretical blindness in this debate, a more reproductive justice-oriented perspective is necessary. In this article, we critically address two interrelated points in response to Gao and Liu's analysis. First, alongside their discussion of reproductive freedom, we introduce a reproductive justice framework to illustrate how intersectional forms of oppression may inhibit individuals from genui

A Bayesian spatial Poisson-lognormal model to examine pedestrian crash severity at signalized intersections.

Reducing nonmotorized crashes requires a profound understanding of the causes and consequences of the crashes at the facility level. Generally, existing literature on bicyclists and pedestrian crash models suffers from two distinct problems: lack of exposure/volume data and inadequacy in capturing potential correlations across various crash aspects. To develop a robust framework for pedestrian crash analysis, this research employed a multivariate model across multiple pedestrian crash severities incorporating a crucial piece of information: pedestrian exposure. A multivariate spatial (conditio

Identity management in the face of HIV and intersecting stigmas: A metasynthesis of qualitative reports from sub-Saharan Africa.

While stigma experienced by people living with HIV (PLWH) is well documented, intersectional stigma and additional stigmatized identities have not received similar attention. The purpose of this metasynthesis is to identify salient stigmatized intersections and their impact on health outcomes in PLWH in sub-Saharan Africa. Using Sandelowski and Barroso's metasynthesis method, we searched four databases for peer-reviewed qualitative literature. Included studies (1) explored personal experiences with intersecting stigmas, (2) included ≥1 element of infectious disease stigma, and (3) were conduct

"They helped me fly": an autoethnographic study of disability, gender, patriarchy, class, and FIRST CARE framework.

PURPOSE: This autoethnography examines the intersectional barriers of disability, gender, and class experienced by a woman with polio within a conservative and patriarchal Indian setting. It generates conceptual insight into how structural constraints and enabling systems shape educational and professional trajectories. MATERIALS AND METHODS: The study adopts an autoethnographic methodology grounded in the author's lived experiences. Through iterative narrative analysis, thematic abstraction, and theoretical engagement, the study develops the FIRST CARE framework, comprising nine interrelated

Intersecting impacts of ageing, migration, and socioeconomic disparities on health equity: a post-pandemic policy review.

BACKGROUND: The COVID-19 pandemic exposed and intensified structural inequities at the nexus of ageing, migration, and socioeconomic vulnerability. These overlapping disadvantages resulted in uneven health outcomes and highlighted systemic fragilities in health systems; yet, few policy reviews have integrated these demographic dimensions into a single analytical framework. OBJECTIVES: This review critically examines how ageing, migration, and socioeconomic disparities intersect to shape health equity during and after the pandemic. It identifies structural bottlenecks, adaptive responses, and

A One Health Gender Equality and Social Inclusion (OH GESI) Framework to strengthen equity goals of One Health implementation tools.

There is a persistent lack of gender and social inclusion in widely used One Health assessment, planning and implementation tools, underscoring the gap between high-level One Health policy commitments to equality and inclusion and real-world practice. Most One Health tools, including those produced by international agencies with high-level commitments, do not adequately address gender equality or social equity. To tackle this issue, the authors developed the One Health Gender Equality and Social Inclusion assessment framework (OH-GESI), which evaluates how tools support gender equality, empowe

Cardiovascular risk factors in immigrants: an intersectional real world data approach to understand health inequalities.

BACKGROUND: Cardiovascular risk factors (CVRFs) are shaped by social determinants of health. Lower CVRFs frequency in immigrants, as reported in the literature, may oversimplify a group affected by multiple and intersecting social inequities. Using the framework of intersectionality, we aim to analyse the prevalence of CVRFs using Real-World Data (RWD), in immigrants to better understand how overlapping social identities shape health outcomes. METHODS: A cross-sectional observational study was conducted in the immigrant population of the CARhES cohort (45,861) a population cohort of RWD of CV

Reducing or reproducing inequalities in health? An intersectional policy analysis of how health inequalities are represented in a Swedish bill on alcohol, drugs, tobacco and gambling.

BACKGROUND: According to post-structural policy analyses, policies and interventions aiming at reducing social inequalities have been found to be part in producing and reifying such inequalities themselves. Given the central role of health inequalities on the public health policy agenda globally it seems important to examine the way policy on health inequalities may potentially counteract the goal of health equity. The aim of this intersectional policy analysis, was to critically analyze the representation of health inequalities in a government bill proposing a national strategy on alcohol, dr

Digging into the Roots of the Problem: Advancing Trans-inclusive Perinatal Healthcare Through Indigenous Knowledge and Decolonising Practice.

Dismantling matrixes of domination that contribute to health inequities for queer and trans people requires acknowledging and attending to all of the oppressive structures which shape health outcomes. Within the context of colonised countries like Aotearoa New Zealand, this means attending to the role that colonisation and Eurocentric views of gender and sexuality play in queer and trans peoples' experiences of marginalisation within healthcare. In this paper we argue that intersectional decolonising approaches to gender and sexuality are central to transforming trans inclusion within healthca

Seeing ourselves in anatomy: Embedding equity, diversity, and inclusivity to transform learning in a UK BSc curriculum.

Embedding intersectional equity, diversity, and inclusion (EDI) in anatomy education is essential to reflect the growing diversity of student populations, support all learners, and prepare graduates for a more inclusive, socially responsive global workforce. Anatomical textbooks and models have typically focused on white, cis-gender, able-bodied individuals who perpetuates biases and underrepresentation, impacting learners' professional development. We embedded five workshops in the junior honors BSc anatomy curriculum, which all adopted an intersectional EDI approach. These were strongly anch

Extending multilevel analysis of individual heterogeneity and discriminatory accuracy to time-to-event outcomes: an application of survival MAIHDA to Korean health data.

BACKGROUND: Multilevel analysis of individual heterogeneity and discriminatory accuracy (MAIHDA) is a leading quantitative approach for intersectionality-informed health research, but most applications analyse binary or cross-sectional outcomes, ignoring event timing. We applied a multilevel survival (shared frailty) model within the MAIHDA framework to examine intersectional disparities in time-to-diagnosis of hypertension. METHODS: Using 2019 Korean Community Health Survey data (n=228 632), we defined intersectional strata by sex, education, income and residential area. Three survival speci

Where I'm Livin' and How I'm Feelin': Associations among community stress, gender, and mental-emotional health among Black Americans.

RATIONALE: Structural racism is a primary avenue for the perpetuation of racial health disparities. For Black Americans, both historically and contemporarily, the neighborhood context serves as one of the most striking examples of structural racism, with stressful neighborhood contexts contributing to the well-documented inequalities in psychological functioning among this population. OBJECTIVE: Thus, in this study, we adapted an intersectional-ecological framework to investigate the links between community stress and multiple dimensions of mental-emotional health for Black men and women. Time and time again: the reincarnations of coerced sterilisation.

The recently reported cases of coerced sterilisation of women at a privately operated immigration detention facility in the USA are egregious in their disregard for human dignity and professional ethics, but sadly not surprising. These abuses represent a continuation of efforts to control the reproductive capacity of women, fueled by racist and xenophobic motives. Physicians helped create and legitimise the pseudoscientific framework for the eugenics movement, which would implement forceful sterilisation as its tool of choice to eliminate undesirable traits that were thought to be biologically

Precarity and health: Theorizing the intersection of multiple material-need insecurities, stigma, and illness among women in the United States.

Material-need insecurities (including insecurities in basic resources such as income, food, housing, and healthcare) are widespread in the United States (US) and may be important predictors of poor health outcomes. How material-need insecurities besides food insecurity are experienced, however, remains under-researched, including how multiple material-need insecurities might intersect and converge on the individual. Here we used qualitative methods to investigate experiences with multiple material-need insecurities among 38 food-insecure women aged over 50 years living with or at risk for HIV

Efficient video-based traffic conflict prediction and interpretable risk analysis at signalized intersections via deep learning.

OBJECTIVES: To achieve accurate and real-time prediction of traffic conflicts at signalized intersections and identify their key contributing factors, thereby supporting proactive safety management and reducing accident risks. METHODS: This study proposes a novel multi-stage traffic-conflict prediction framework that integrates a real-time video image processing system and an advanced conflict-prediction model. Specifically, a real-time video analysis system integrating the YOLOv8 object detection framework and the OC-SORT algorithm for multi-vehicle tracking is first developed to extract key

Beyond static snapshots: gated recurrent units and graph attention networks for smarter multi-agent traffic control.

Existing multi-agent reinforcement learning (MARL) approaches for traffic signal control often fail to capture two critical characteristics of real-world urban traffic: the dynamic spatio-temporal propagation of congestion across intersections and the frequent imperfection of sensor data. To address these fundamental gaps, this paper introduces hybrid coordinated spatio-temporal graph reinforcement learning (HC-STGRL), a novel framework that integrates three complementary mechanisms-gated recurrent unit (GRU), graph attention network (GAT), dueling double deep Q-Network (3DQN). Each intersecti

Artificial Intelligence Governance in Anatomy: From Disclosure Toward Stewardship.

Artificial intelligence (AI) is increasingly influencing anatomical education, research, scholarly publishing, and the development of digital anatomical resources. Current discussions of AI governance in anatomy have largely focused on author use and disclosure. However, AI now operates across multiple stages of scholarly communication and increasingly intersects with anatomical images, body donation, and donor-derived resources. These developments raise discipline-specific questions about visual authenticity, provenance, confidentiality, secondary use, and the continuing responsibilities asso

El Dolor Es Lo Que Me Saluda: A Qualitative Study on Multilevel and Intersectional Factors Impacting Physical Activity Among Latino Persons With Chronic Spine Pain.

BACKGROUND: Latino persons with chronic spine pain (CSP) face distinct challenges to engaging in physical activity (PA) that may be explained by factors at multiple levels of the socioecological model and by intersecting oppressions. The objective of this study was to use the socioecological model and intersectionality framework to explore multilevel and intersecting determinants of PA among Latino persons with CSP using a qualitative approach. METHODS: A purposive sample of Latino persons with CSP living in the United States near the US-Mexican border completed a demographic survey and parti

St. Louis enhancing engagement and retention (STEER) in HIV/AIDS care: a participatory intersectional needs assessment for intervention and implementation planning.

BACKGROUND: Missouri is one of seven priority states identified by the Ending the HIV Epidemic Initiative, and St. Louis contains almost half of the people living with HIV (PLWH) in Missouri. As St. Louis has a marked history of structural racism and economic inequities, we utilized the Intersectionality Based Policy Analysis (IBPA) framework to guide a participatory needs assessment for planning and program development. METHODS: The planning team included researchers, the lead implementer from our community partner, and two community representatives, and had biweekly 60-90 min meetings for 1
