Roles of NLRP3 inflammasome in intervertebral disc degeneration.

Intervertebral disc degeneration (IVDD) is one of the leading causes of low back pain and one of the most common health problems in the world. The nucleotide-binding oligomerization domain-like receptor family pyrin domain-containing-3 (NLRP3) inflammasome, as a pattern recognition receptor, has been shown to be associated with the pathological processes of many diseases in recent years. With the exploration of the mechanism of IVDD, recent studies have shown that activation of the NLRP3 inflammasome is associated with intervertebral disc (IVD) inflammation, pyroptosis, extracellular matrix de

Targeting STING attenuates ROS induced intervertebral disc degeneration.

OBJECTIVE: DNA damage induced by ROS is considered one of the main causes of nucleus pulposus (NP) cells degeneration during the progression of intervertebral disc degeneration (IVDD). cGAS-STING pathway acts as DNA-sensing mechanism for monitoring DNA damage. Recent studies have proved that cGAS-STING contributes to the development of various diseases by inducing inflammation, senescence, and apoptosis. This work explored the role of STING, the main effector of cGAS-STING signaling pathway, in NP degeneration. METHOD: Immunohistochemistry was conducted to measure STING protein levels in the

[Degenerative intervertebral disc processes : Current aspects of diagnosis].

Starting with the smallest functional unit, the spinal segment including the centrally located intervertebral disc, the spine and pelvis including the sacroiliac and hip joints form a functional unit. The discs play an important role in the complex interaction between disc, osseous and ligamentous structures and the adjacent muscles. Disc pathologies are very frequently associated with changes of the adjacent endplates of the vertebral bodies and can be depicted at an early stage using magnetic resonance imaging (MRI). The focus of the diagnostics and assessment of degenerative disc pathologie

Mechanoreceptors in Diseased Cervical Intervertebral Disc and Vertigo.

STUDY DESIGN: We collected the samples of cervical intervertebral discs from patients with vertigo to examine the distribution and types of mechanoreceptors in diseased cervical disc. OBJECTIVE: The aim of this study was to determine whether mechanoreceptors are distributed more abundantly in cervical discs from patients with cervical spondylosis, and whether they are related to vertigo. SUMMARY OF BACKGROUND DATA: Previous limited studies have found that normal cervical intervertebral discs are supplied with mechanoreceptors that have been considered responsible for proprioceptive functions

Intervertebral disc development and disease-related genetic polymorphisms.

The intervertebral disc (IVD) comprises a gelatinous inner core (nucleus pulposus; NP) and concentric rings (annulus fibrosus; AF). The NP, an important structure for shock absorption in the vertebrate spinal motion segment, can be traced back to the notochord in ontogenetic lineage. In vertebrates, the notochord undergoes mucinoid changes, and had been considered vestigial until recently. However, observed correlations between IVD degeneration and back pain in humans have renewed interest in the IVD in biomedical fields. Beyond its mechanical contribution to development, the notochord is also

Intervertebral disc degeneration: evidence for two distinct phenotypes.

We review the evidence that there are two types of disc degeneration. 'Endplate-driven' disc degeneration involves endplate defects and inwards collapse of the annulus, has a high heritability, mostly affects discs in the upper lumbar and thoracic spine, often starts to develop before age 30 years, usually leads to moderate back pain, and is associated with compressive injuries such as a fall on the buttocks. 'Annulus-driven' disc degeneration involves a radial fissure and/or a disc prolapse, has a low heritability, mostly affects discs in the lower lumbar spine, develops progressively after a

Diversity of intervertebral disc cells: phenotype and function.

The intervertebral disc (IVD) is a moderately moving joint that is located between the bony vertebrae and provides flexibility and load transmission throughout the spinal column. The disc is composed of different but interrelated tissues, including the central highly hydrated nucleus pulposus (NP), the surrounding elastic and fibrous annulus fibrosus (AF), and the cartilaginous endplate (CEP), which provides the connection to the vertebral bodies. Each of these tissues has a different function and consists of a specific matrix structure that is maintained by a cell population with distinct phe

Notochordal intervertebral disc cells: sensitivity to nutrient deprivation.

OBJECTIVE: The nucleus pulposus (NP) of the intervertebral disc develops from the notochord. Humans and other species in which notochordal cells (NCs) disappear to be replaced by chondrocyte-like mature NP cells (MNPCs) frequently develop disc degeneration, unlike other species that retain NCs. The reasons for NC disappearance are unknown. In humans, the change in cell phenotype (to MNPCs) coincides with changes that decrease nutrient supply to the avascular disc. We undertook this study to test the hypothesis that the consequent nutrient stress could be associated with NC disappearance. METH

Juvenile intervertebral disc calcification: recognition, management, and pathogenesis.

Juvenile intervertebral disc calcification is an uncommon disorder of childhood, characterized by calcification of the nucleus pulposus of one or more intervertebral discs. Calcification may remain dormant or subsequently become symptomatic. The symptoms include fever, malaise, and neck pain and are associated with an elevated erythrocyte sedimentation rate and, occasionally, leukocytosis. Although disc protrusion occurs in 38% of patients, neurological signs are distinctly uncommon. We report the case of a patient with a herniated T2-T3 calcified intervertebral disc and compressive myelopathy

Thoracic intervertebral disc protrusion with spinal cord compression.

Four patients presenting with spinal cord compression and spinal nerve root irritation are described. The clinical and radiological features are described and the contribution of CT scanning to accurate diagnosis of this condition is highlighted. Review of the literature indicates that the natural history of this disorder is one of steadily progressive neurological dysfunction. Surgical decompression of the lesion, although hazardous, is the only therapeutic option. Brief comment is made on the neurosurgical approaches currently in vogue.

Anterior spinal fusion or deranged lumbar intervertebral disc.

Ninety-seven patients who underwent anterior interbody fusion for lumbar disc derangement were examined two to 15 years after the operation. Complete relief of back pain was obtained in 60% and of sciatica in 85%; marked improvement of back pain was obtained in 29% and of sciatica in 10%. The fusion rate was 63% and was only slightly correlated with symptomatic improvement. Many complications were encountered, most of which were transient.

Microstructural imaging of the cartilaginous endplate in the intervertebral disc.

1 OBJECTIVE: Intervertebral disc (IVD) degeneration is a major risk factor for the development of chronic low back pain (LBP), affecting millions globally, particularly as they age. IVD degeneration can promote inflammation, and reduce spinal stability leading to compression of nerve roots. Research has primarily focused on the annulus fibrosus (AF) or nucleus pulposus (NP), with an unmet need to explore the physiology of the cartilaginous endplate (CEP) and its microstructural assessment in IVD degeneration. Imaging the microstructure of the CEP, a relatively thin layer between the IVD and ve

[Biomechanics of the intervertebral disc : Consequences of degenerative changes].

The intervertebral disc represents the flexible connection between two adjacent vertebral bodies. Intervertebral discs, therefore, give the spine its enormous range of motion. At the same time, intervertebral discs distribute the load evenly over the bony vertebral bodies to ensure load transfer from the upper body to the pelvis, provide sufficient stability, and absorb shocks during everyday movements as well as under extreme loads. The two tissue regions of the intervertebral disc, the central fluid-rich core (nucleus pulposus) and the lamellae of the outer fibrous ring (annulus fibrosus) ex

A Brief Review of the Degenerative Intervertebral Disc Disease.

IntroductionThe degenerative processes of the intervertebral disc represent an important cause of morbidity in everyday clinical practice, exerting burden on patients and clinicians treating them. Numerous factors may initiate degenerative processes, which most commonly affect the nucleus pulposus and ultimately influence the biomechanics of the whole spine.AimThis paper provides an overview from the literature about the process, causes and mechanisms of disc degeneration and the associated factors.MethodsThe scientific literature was reviewed through PubMed, Medline and Science Direct. The cell biology of intervertebral disc aging and degeneration.

Intervertebral disc degeneration, which mimics disc aging but occurs at an accelerated rate, is considered to be related to neck or low back pain and disc herniation. Degenerated discs show breakdown of the extracellular matrix and thus fail to bear the daily loadings exerted on the spine. Rather than a passive process of wear and tear, disc degeneration is an aberrant, cell-mediated response to progressive structural failure due to aging and other environmental factors such as abnormal mechanical stress. With aging and degeneration, disc cells undergo substantially biologic changes, including
