Role of the hedgehog pathway in the formation, maintenance, and degeneration of intervertebral disc.

Low back pain (LBP), one of the most common health problems, is the leading cause of disability globally. Intervertebral disc degeneration (IDD) accounts for most LBP. However, the molecular mechanism underlying IDD remains unclear, and the existing treatment strategy for IDD is still limited. A growing body of evidences suggest that the Hedgehog (HH) pathway plays an essential role in the formation, maintenance, and degeneration of intervertebral discs (IVDs), with Sonic HH (SHH) being primarily involved in the development and maturation of the IVDs and a strong link between Indian HH(IHH) an

The role of sirtuins in intervertebral disc degeneration: Mechanisms and therapeutic potential.

Intervertebral disc degeneration (IDD) is one of the main causes of low back pain, which affects the patients' quality of life and health and imposes a significant socioeconomic burden. Despite great efforts made by researchers to understand the pathogenesis of IDD, effective strategies for preventing and treating this disease remain very limited. Sirtuins are a highly conserved family of (NAD+)-dependent deacetylases in mammals that are involved in a variety of metabolic processes in vivo. In recent years, sirtuins have attracted much attention owing to their regulatory roles in IDD on physio

Contribution of immune cells to intervertebral disc degeneration and the potential of immunotherapy.

Substantial evidence supports that chronic low back pain is associated with intervertebral disc degeneration (IDD), which is accompanied by decreased cell activity and matrix degradation. The role of immune cells, especially macrophages, in a variety of diseases has been extensively studied; therefore, their role in IDD has naturally attracted widespread scholarly interest. The IVD is considered to be an immunologically-privileged site given the presence of physical and biological barriers that include an avascular microenvironment, a high proteoglycan concentration, high physical pressure, th

The Pivotal Role of Nrf2 Signal Axis in Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IDD) is considered as a dominant contributor to low back pain (LBP), causing severe pain, limited range of lumbar motion, physical dysfunction, and restriction of social activity. However, the specific pathological mechanisms underlying IDD remain elusive, and effective strategies to delay the pathogenesis of IDD are still unclear and limited. In recent years, some studies have found that nuclear factor erythroid 2-related factor 2 (Nrf2), an important antioxidant transcription factor, may play crucial roles in the pathogenesis and progression of age-related d

Intervertebral disc degeneration and how it leads to low back pain.

The purpose of this review was to evaluate data generated by animal models of intervertebral disc (IVD) degeneration published in the last decade and show how this has made invaluable contributions to the identification of molecular events occurring in and contributing to pain generation. IVD degeneration and associated spinal pain is a complex multifactorial process, its complexity poses difficulties in the selection of the most appropriate therapeutic target to focus on of many potential candidates in the formulation of strategies to alleviate pain perception and to effect disc repair and re

Stem Cell Therapy and Exercise for Treatment of Intervertebral Disc Degeneration.

As part of the motor system, intervertebral disc (IVD) is a complicated tissue with multiple components. The degeneration of IVD may result in low back pain (LBP), which strongly impairs quality of life. Various causes are related to the degeneration of IVD, including cell senescence, hydration lost, and inflammation. Stem cells founded in different tissues have attracted the interest of the researchers and clinicians to study the implication of these cells in the treatment for tissue injury and degeneration. In this report, we will review the study of stem cells in the treatment for IVD degen

The role of IL-1β and TNF-α in intervertebral disc degeneration.

Low back pain (LBP), a prevalent and costly disease around the world, is predominantly caused by intervertebral disc (IVD) degeneration (IDD). LBP also presents a substantial burden to public health and the economy. IDD is mainly caused by aging, trauma, genetic susceptibility, and other factors. It is closely associated with changes in tissue structure and function, including progressive destruction of the extracellular matrix (ECM), enhanced senescence, disc cell death, and impairment of tissue biomechanical function. The inflammatory process, exacerbated by cytokines interleukin-1β (IL-1β)

Stem cell regeneration of degenerated intervertebral discs: current status.

Low back pain (LBP) is one of the most common musculoskeletal conditions, and intervertebral disc (IVD) degeneration is associated with most cases. Although many treatment options are available, they focus on the removal of symptoms rather than repair of the degenerate tissue. However, there is a growing interest in the potential of cell-based tissue engineering strategies for regeneration of the damaged IVD. To achieve this, investigators are now focusing on the use of mesenchymal stem cells (MSCs), which offer several advantages over more mature cell types. A number of problems must be overc

Matrix metalloproteinase-3 production by human degenerated intervertebral disc.

To assess its possible role in the pathophysiology of intervertebral disc degeneration, we investigated the production of matrix metalloproteinase-3 (MMP-3) using human intervertebral disc explant culture. Five normal and 10 degenerated disc specimens were used. The levels of MMP-3 released in the medium were measured with use of an enzyme immunoassay. The results showed that the level of MMP-3 in the degenerated group (0.57 microg/ml/mg wet weight; n = 10) was significantly higher than that of the control group (0.29 microg/ml/mg wet weight; n = 5) (p < 0.05). Immunostaining of MMP-3 revealed

Immunohistochemical analysis to detect a molecular signature in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is known as a primary contributor to low back pain, a debilitating condition which is the leading cause of disability worldwide. Traditionally, its assessment is based on clinical parameters, including magnetic resonance imaging (MRI). However, patients with similar radiological findings may have significantly different prognoses suggesting the involvement of patient-specific biomarkers and little-investigated molecules supporting the complexity of the pathophysiological microenvironment of the intervertebral disc (IVD). We conducted a study on IVD biopsi

Periostin as a diagnostic marker and therapeutic target for intervertebral disc degeneration.

The pathophysiology of intervertebral disc degeneration (IDD), a condition associated with significant morbidity, remains incompletely elucidated. Identification of key proteins that drive IDD progression is crucial for developing effective strategies to mitigate disease advancement. In this study, we discovered that periostin (POSTN) play an important role in IDD through the bioinformatic analysis, and confirmed its presentation in patient nucleus pulposus (NP) tissues and serum samples. Additionally, the serum POSTN level is positively correlated with IDD. To further investigate the function

The relationship between biomechanical factors and intervertebral disc degeneration: a review.

The intervertebral disc is an avascular structure composed of the nucleus pulposus, annulus fibrosus, and the superior and inferior cartilage endplates. Located between adjacent vertebrae, it connects them structurally, contributing to spinal stability, load-bearing, shock absorption, and protection of the spinal cord. Intervertebral disc degeneration (IDD) results from various factors, including mechanical overload, trauma, repetitive strain, pregnancy, genetic predisposition, and developmental abnormalities. In the context of increased occupational demands and lifestyle changes, the spine is
