New concepts of vitamin D functions.

Besides its classical actions in calcium metabolism, it is clear that the hormonal form of vitamin D has many new functions, which have only been discovered as a result of following the appearance of its receptor. Furthermore, because of the vitamin D-based endocrine system, the use of vitamin D compounds in treating a variety of diseases has been expanded. We now know that 1.25-(OH)2D3 not only stimulates the intestine to absorb calcium and phosphorus, the bones to mobilize calcium and phosphorus, and the kidney to cause increased renal reabsorption of calcium, but also directly suppresses th

Pathophysiological importance of bile cholesterol reabsorption: hepatic NPC1L1-exacerbated steatosis and decreasing VLDL-TG secretion in mice fed a high-fat diet.

BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is one of the most common liver diseases worldwide, although its pathogenesis remains to be elucidated. A recent study revealed that hepatic Niemann-Pick C1-Like 1 (NPC1L1), a cholesterol re-absorber from bile to the liver expressed on the bile canalicular membrane, is an exacerbation factor of NAFLD. Indeed, transgenic mice with hepatic expression of human NPC1L1 under a liver-specific promoter (L1-Tg mice) developed steatosis with a high-fat diet (HFD) containing cholesterol within a few weeks. However, the mechanism underlying diet-induc

Exploring diet effects on the profile, excretion and enterohepatic circulation of bile acids in the chicken.

Beyond their role in lipid digestion, the physiological function, distribution and composition of bile acids (BAs) in poultry remain insufficiently understood. We investigated bile acid metabolism in broiler chickens as well as laying hens in response to different diets. Two experiments were conducted: the first was a digestibility study of both genotypes fed either a highly digestible or challenge diet to assess the effect on fat digestibility and fecal bile acid excretion. In broilers, the highly digestible corn-soy diet improved fat digestibility and increased plasma triglycerides and non-e

Pharmacology of Mammalian Na+-Dependent Transporters of Inorganic Phosphate.

Inorganic phosphate (Pi) is an essential component of many biologically important molecules such as DNA, RNA, ATP, phospholipids, or apatite. It is required for intracellular phosphorylation signaling events and acts as pH buffer in intra- and extracellular compartments. Intestinal absorption, uptake into cells, and renal reabsorption depend on a set of different phosphate transporters from the SLC20 (PiT transporters) and SLC34 (NaPi transporters) gene families. The physiological relevance of these transporters is evident from rare monogenic disorders in humans affecting SLC20A2 (Fahr's disea

Exome sequencing reveals cubilin mutation as a single-gene cause of proteinuria.

In two siblings of consanguineous parents with intermittent nephrotic-range proteinuria, we identified a homozygous deleterious frameshift mutation in the gene CUBN, which encodes cubulin, using exome capture and massively parallel re-sequencing. The mutation segregated with affected members of this family and was absent from 92 healthy individuals, thereby identifying a recessive mutation in CUBN as the single-gene cause of proteinuria in this sibship. Cubulin mutations cause a hereditary form of megaloblastic anemia secondary to vitamin B(12) deficiency, and proteinuria occurs in 50% of case

Fibroblast growth factor 23 as a phosphotropic hormone and beyond.

Fibroblast growth factor 23 (FGF23) is produced by bone and reduces serum phosphate by inhibiting proximal tubular phosphate reabsorption and intestinal phosphate absorption. Excess actions of FGF23 cause several kinds of hypophosphatemic rickets/osteomalacia while deficient actions of FGF23 result in hyperphosphatemic tumoral calcinosis. In addition, FGF23 has been shown to prevent the development of hyperphosphatemia during the progression of chronic kidney disease-mineral and bone disorder. Epidemiological studies have indicated that high FGF23 levels are associated with unfavorable events

Disruption of Claudin-2 and Trpv6 Results in Hypocalcemia, Hypercalciuria, and Altered Bone Microarchitecture in Mice.

Transepithelial calcium absorption occurs via a transcellular or paracellular pathway. The intestinal transcellular pathway relies, at least partially, on the apical calcium channel transient receptor potential vanilloid 6 (TRPV6), whereas paracellular calcium (re)absorption occurs through tight junction proteins, including claudin-2 (CLDN2). Both CLDN2 and TRPV6 are also expressed in the kidney. CLDN2 contributes to proximal tubular calcium reabsorption, whereas the functional relevance of TRPV6 remains unclear. Cldn2-/- knockout mice exhibit reduced intestinal calcium permeability and hyperc

Disorders of Calcium Metabolism: Hypocalcemia and Hypercalcemia.

Calcium (Ca+2) is a divalent cation that plays a critical role in numerous body functions such as skeletal mineralization, signal transduction, nerve conduction, muscle contraction, and blood coagulation. Ca+2 metabolism is linked to magnesium (Mg+2) and phosphate metabolism. Ca+2 homeostasis is dependent on intestinal absorption, bone turnover, and renal reabsorption. The hormonal regulators of these processes are the parathyroid hormone (PTH), calcitriol {1,25-dihydroxyvitamin D [1,25(OH)2D]}, and serum ionized Ca+2. Cloning of the Ca+2-sensing receptor (CaSR) has greatly advanced the unders

Metabolic consequences of ileal interruption of the enterohepatic circulation of bile acids.

The enterohepatic circulation of bile acids comprises a tightly regulated process of hepatic bile acid secretion, intestinal reabsorption and transport back to the liver. Disruption of this process has significant consequences for gastrointestinal, liver and whole body homeostasis and therefore offers opportunities for therapeutic intervention. In this review we discuss the effects of (pharmacological) interruption of the enterohepatic circulation at different levels. Recently, several studies have been published on ileal interruption of the enterohepatic circulation of bile acids, targeting t

Effects and mechanisms of nobiletin on gut motility by coordinating bile acid signaling via dual ileal-colonic axes in STC model mice.

Slow transit constipation (STC) is a functional gastrointestinal disorder with complex etiology and limited therapeutic options. Nobiletin (NOB), a natural polymethoxyflavone derived from citrus, exhibits multiple pharmacological activities. However, its effects on STC remain unknown. Using a loperamide-induced STC mouse model, this study demonstrates that NOB administration significantly alleviates constipation symptoms, improves intestinal propulsion, and restores plasma neurotransmitter balance. NOB also enhanced intestinal barrier function by upregulating colonic MUC2 and tight junction pr

Essential role for TRPM6 in epithelial magnesium transport and body magnesium homeostasis.

Magnesium is an important cofactor for many biological processes such as protein synthesis, nucleic acid stability and neuromuscular excitability. The extracellular magnesium concentration is regulated tightly by the extent of intestinal absorption and renal excretion. Despite their critical role in magnesium handling, the molecular mechanisms mediating transepithelial transport are still not understood completely. Recently, genetic studies in patients with primary hypomagnesaemia and secondary hypocalcaemia (HSH), a combined defect of intestinal magnesium absorption and renal magnesium conser

TRPV6 and Cav1.3 Mediate Distal Small Intestine Calcium Absorption Before Weaning.

BACKGROUND & AIMS: Intestinal Ca2+ absorption early in life is vital to achieving optimal bone mineralization. The molecular details of intestinal Ca2+ absorption have been defined in adults after peak bone mass is obtained, but they are largely unexplored during development. We sought to delineate the molecular details of transcellular Ca2+ absorption during this critical period. METHODS: Expression of small intestinal and renal calcium transport genes was assessed by using quantitative polymerase chain reaction. Net calcium flux across small intestinal segments was measured in Ussing chambe

Food additives: Assessing the impact of exposure to permitted emulsifiers on bowel and metabolic health - introducing the FADiets study.

Emulsifiers are common components of processed foods consumed as part of a Western diet. Emerging in vitro cell-line culture, mouse model and human intestinal tissue explant studies have all suggested that very low concentrations of the food emulsifier polysorbate 80 may cause bacterial translocation across the intestinal epithelium, intestinal inflammation and metabolic syndrome. This raises the possibility that dietary emulsifiers might be factors in conditions such as coronary artery disease, type 2 diabetes and Crohn's disease. The potential mechanism behind the observed effects of this em

Pharmacokinetic study of the enterohepatic circulation of acetaminophen glucuronide in rats.

A pharmacokinetic model of the enterohepatic circulation of acetaminophen glucuronide was investigated in rats with particular attention to a lag time between biliary excretion and reabsorption. The plasma drug data obtained after acetaminophen glucuronide injection into the various sites of the gut confirmed that there is a lag time in the enterohepatic circulation and that the lag time is due to the intestinal transit period of the conjugate to the site of the hydrolysis. The value of the lag time was fairly close to that reported previously in the rat. Based on the result, a compartment mod

Increased Milk Protein Concentration in a Rehydration Drink Enhances Fluid Retention Caused by Water Reabsorption in Rats.

A fluid-retention effect is required for beverages that are designed to prevent dehydration. That is, fluid absorbed from the intestines should not be excreted quickly; long-term retention is desirable. Here, we focused on the effect of milk protein on fluid retention, and propose a new effective oral rehydration method that can be used daily for preventing dehydration. We first evaluated the effects of different concentrations of milk protein on fluid retention by measuring the urinary volumes of rats fed fluid containing milk protein at concentrations of 1, 5, and 10%. We next compared the f

[A study of calcium metabolism in calcium-containing stone formers].

To clarify the pathogenesis of calcium-containing urinary stone, calcium metabolism of the 113 patients with calcium-containing stone was studied by fasting and oral calcium loading test. The stone patients were classified into four groups. Those were normocalciuria (NC, n = 60), absorptive hypercalciuria-1 (AH-1, n = 26), absorptive hypercalciuria-2 (AH-2, n = 16) and renal hypercalciuria (RH, n = 11). In the AH-1 group, hypercalciuria resulted from enhancement of intestinal calcium (Ca) absorption. The increase in serum Ca from absorbed Ca increased renal filtered load of Ca. Urinary excreti

Hypophosphataemic Rickets: Diagnosis Algorithm-How Not to Make a Mistake.

Hypophosphataemic rickets is a heterogeneous group of entities characterized by rickets or osteomalacia due to a phosphate deficit caused mainly by decreased renal reabsorption. They are also characterized by defective intestinal absorption of calcium and rickets or osteomalacia unresponsive to cholecalciferol. These metabolic alterations lead to growth retardation, bone pain and deformities, and short stature. For a correct diagnosis and treatment of all forms of rickets, the basic aspects of pathophysiology of the calcium-phosphorus metabolism and the relevance of the bone-kidney axis modula

A 79-year-old male with a Bricker loop and chronic renal failure was admitted to hospital because progressive dyspnoea. This was due to severe hyperchloraemic metabolic acidosis. Hyperchloraemic acidosis can occur if urinary diversions are constructed from the colon or ileum. Contact between intestinal mucosa and urine may cause reabsorption of ammonium and chloride, and secretion of bicarbonate. Hyperchloraemic acidosis is rarely seen with an incontinent ileal loop due to its small absorbing surface area and the rapid drainage of urine from the loop. Growth hormone and bone.

Bone and mineral metabolism is regulated by numerous hormones and local growth factors. Growth hormone (GH), directly and/or indirectly through local IGF1 and IGF2 production, stimulates bone turnover as it increases osteoblast number and function. It also stimulates bone resorption through unknown mechanisms. GH (by IGF1) stimulates the renal 25-hydroxyvitamin D-1 alpha-hydroxylase activity and therefore enhances calcium and phosphate absorption in the intestine. It also increases the maximal renal tubular reabsorption of phosphate. The overall effect of GH on bone therefore seems beneficial
