Coaptation of Cutaneous Nerves for Intractable Stump Pain and Phantom Limb Pain after Upper Limb Amputation.

BACKGROUND: Various surgical treatments have been advocated for stump pain and phantom limb pain after limb amputation but the most effective is unknown. We report a case of intractable stump pain and phantom limb pain of the upper limb, which was successfully treated by end-to-end coaptation of the cutaneous nerves after multimodal treatment failures. CASE DESCRIPTION: A 39-year-old man was referred to our department with a history of severe stump neuroma-related pain and phantom limb pain of his right upper limb. He had undergone multiple treatments over 26 years including medication, nerve

Subcutaneous BoNT/A Injection for Intractable Pain and Disability in Complex Regional Pain Syndrome: A Case Report.

We treated a 51-year-old woman with refractory Complex Regional Pain Syndrome type I (CRPS-I) involving her left hand and forearm with subcutaneous injections of BoNT/A. The injections were performed every 3 months, with a total of six treatments. Each treatment was able to effectively improve pain and motor impairment; however, the duration of the effect was limited to only a few months. BoNT/A could improve patients' quality of life with CRPS; however, extensive clinical studies are needed to determine its role in clinical practice.

MR-guided Laser Interstitial Thermal Therapy mesencephalotomy for medically intractable malignant pain.

Stereotactic mesencephalotomy is a neurosurgical technique designed to sever spinothalamic pain transmission pathways for medically intractable pain. This report presents the first case of Magnetic Resonance-guided Laser Interstitial Thermal Therapy (MRgLITT) mesencephalotomy for severe malignant pain due to metastatic melanoma. The procedure significantly reduced the patient's pain, with a postoperative visual analog scale (VAS) score decreasing from > 7 to < 3. No adverse effects were observed. The case underscores the potential of MRgLITT mesencephalotomy as a precise, minimally invasive op

Erector spinae plane block for intractable, nonsurgical abdominal pain: a scoping review.

Abdominal pain is one of the most common presenting chief complaints in the emergency department. Erector spinae plane block (ESPB) is an ultrasound-guided nerve block with proven effectiveness in treating visceral and somatic abdominal pain. Despite the increasing popularity of ESPB, its role in the management of nonsurgical abdominal pain has not yet been characterized. Our scoping review aims to synthesize current knowledge on the safety and efficacy of ESPB in the management of patients experiencing intractable, nonsurgical abdominal pain. We searched PubMed and Scopus to evaluate the exis

Intraoperative Neurophysiology for Optimization of Percutaneous Spinothalamic Cordotomy for Intractable Cancer Pain.

BACKGROUND: Percutaneous ablation of the cervical spinothalamic tract (STT) remains a therapeutic remedy for intractable cancer pain. However, it is accompanied by the risk of collateral damage to essential spinal cord circuitry, including the corticospinal tract (CST). Recent studies describe threshold-based mapping of the CST with the objective of motor bundle preservation during intramedullary spinal cord and supratentorial surgery. OBJECTIVE: To assess the possibility that application of spinal cord mapping using intraoperative neuromonitoring in percutaneous cordotomy procedures may aid

Proclaim™ DRG Neurostimulator System for the management of chronic, intractable pain.

Chronic pain is consistently listed as one of the most costly and disabling health problems worldwide. In an effort to treat these suffering individuals, significant amounts of time and energy have been devoted to discover safe and effective pain relieving treatments. Dorsal root ganglion stimulation is the newest treatment modality to be created for chronic intractable pain. In this manuscript, we review the history and development, published research and safety profile of the Proclaim™ DRG Neurostimulator System (Abbott, TX, USA). At last, we offer our outlook on future developments with dor

Programming Parameters and Techniques in Trigeminal Ganglion Stimulation for Intractable Facial Pain.

OBJECTIVES: Atypical facial pain syndromes are challenging disorders to manage and often incur limited benefit with surgery for classical trigeminal neuralgia presentations, such as microvascular decompression or ablative procedures. Neurostimulation of the trigeminal ganglion and peripheral nerves can be effective at treating atypical presentations of trigeminal facial pain affecting the V1-3 dermatomes, and the surgical techniques are well described. The stimulation parameters, however, have thus far received limited description; we therefore sought to describe programming strategies. MATER

Microendoscopy-guided percutaneous cordotomy for intractable pain: case series of 24 patients.

OBJECTIVE: The aim of this study was to show that microendoscopic guidance using a double-channel technique could be safely applied during percutaneous cordotomy and provides clear real-time visualization of the spinal cord and surrounding structures during the entire procedure. METHODS: Twenty-four adult patients with intractable cancer pain were treated by microendoscopic-guided percutaneous radiofrequency (RF) cordotomy using the double-channel technique under local anesthesia. A percutaneous lateral puncture was performed initially under fluoroscopy guidance to localize the target. When t

Sacral nerve stimulation as a treatment modality for intractable neuropathic testicular pain.

Chronic testicular pain, or "chronic orchalgia," is defined as testicular pain 3 months or longer in duration that significantly interferes with the daily activities of the patient. For patients failing to respond to conservative treatment, microsurgical denervation of the spermatic cord, epididymectomy, and vasovasostomy have all shown a degree of relief. However, these are all invasive procedures and no treatment has proven efficacy when these options fail. We present a case of a male who presented with over a decade of chronic right-sided testicular pain secondary to recurrent epididymitis.

Thoracoscopic renal denervation for intractable autosomal dominant polycystic kidney disease-related pain.

The authors report a case of intractable autosomal dominant polycystic kidney disease-related pain associated with normal renal function, treated with renal denervation. Renal denervation was performed via a thoracoscopic approach. The good medium-term result suggests that thoracoscopic sympatho-splanchnicectomy would be an attractive procedure for pain management in autosomal polycystic kidney disease.

Epidural clonidine analgesia for intractable cancer pain. The Epidural Clonidine Study Group.

Although the vast majority of patients with cancer pain receive effective analgesia from standard therapy, a few patients, particularly those with neuropathic pain, continue to experience severe pain despite large doses of systemic or intraspinal opioids. Animal studies suggest intraspinal alpha 2-adrenergic agonists may be effective in such cases. Eighty-five patients with severe cancer pain despite large doses of opioids or with therapy-limiting side effects from opioids were randomized to receive, in a double-blind manner, 30 micrograms/h epidural clonidine or placebo for 14 days, together

Spinal cord stimulation for chronic, intractable pain: superiority of "multi-channel" devices.

Spinal cord stimulation has evolved over the past 20 years into an easily implemented technique, with low morbidity, for the treatment of intractable, chronic pain in properly selected patients. We report our experience with a series of 62 patients implanted between 1983 and 1987, with percutaneous and laminectomy electrodes, and with single- and "multi-channel" (programmable, multi-contact) devices. Fifty had chronic, intractable low back and leg pain ("failed back surgery syndrome," lumbar arachnoid fibrosis), five had spinal cord injuries, and seven "peripheral" pathology or stump pain. Sta

Retrograde adriamycin sensory ganglionectomy: novel approach for the treatment of intractable pain.

Selective sensory ganglionectomy by means of retrograde suicide transport of adriamycin was performed on 3 patients with neuropathic pain in the areas of the trigeminal and intercostal nerves, producing significant pain relief, particularly from hyperalgesic pain. Adriamycin ganglionectomy is considered as a less invasive and highly selective pain treatment, which may possibly become an alternative for surgical ganglionectomy or rhizotomy.

Childhood hospitalization and chronic intractable pain in adults: a controlled retrospective study.

Three groups of patients have been studied in order to elucidate the relationship between childhood hospitalization and chronic intractable pain in adults. The groups were: patients referred to a pain clinic, psychiatric patients with a depressive illness, and patients attending a rheumatology clinic. The findings suggest that early hospitalization is related to the genesis of both depressive illness and intractable pain: in the former occurring in the preschool years and in the latter, during school age. The significance of these relationships is discussed.

Case Report: Cryoanalgesia for intractable thoracic pain due to chest wall tumor.

INTRODUCTION: Postoperative pain in thoracic surgery is a major concern, often delaying recovery. Cryoanalgesia, using pressurized nitrous oxide, offers long-term pain relief by temporarily blocking peripheral nerve function. It takes effect within 72 hours, so an intercostal nerve block is used for immediate pain relief. Cryoanalgesia is effective for severe pain, with minimal side effects and no drug interactions. CASE REPORT: A 63-year-old man with severe thoracic pain, unresponsive to opioids, was diagnosed with a typical carcinoid tumor after a CT scan revealed lesions in his lung, thora

Pituitary Targeting for Intractable Cancer Pain: A Systematic Review and Current Recommendations.

BACKGROUND: Most patients suffering from cancer will face pain during the course of their disease. Pain is a source of major discomfort and alteration of quality of life. Pituitary targeting, initially reported as hypophysectomy, was first proposed to control endocrine cancer. Although this technique did not improve the overall tumoral control, it rapidly showed an interesting effect on cancer pain related to bone metastases. Due to the complications, this technique gradually decreased and finally fell into disuse. Recently, some authors have shown a regained interest in pituitary targeting us

Open thoracic cordotomy for intractable cancer pain: a how I do it.

BACKGROUND: Some cancers of the lower extremity involve nerves and plexuses and can produce extreme drug-resistant noceptive pain. In these cases, open thoracic cordotomy can be proposed. METHOD: This procedure involves disruption of the spinothalamic tract, which sustains nociceptive pathways. After placement in the prone position, selection of the side to be operated on (contralateral to the pain), and dura exposure, microsurgery is used to section the anterolateral spinal cord quadrant previously exposed by gently pulling on the dentate ligament. CONCLUSION: Open thoracic cordotomy is a m
