Intraoperative Neurophysiological Monitoring During Adult Brainstem Glioma Resection: Association Between Electrophysiological Changes and Early Neurological Outcome.

Intraoperative neurophysiological monitoring (IONM) is widely used during brainstem surgery to reduce the risk of neurological injury, although its prognostic value in adult brainstem glioma (BSG) surgery remains unclear. We retrospectively analyzed 22 consecutive adults who underwent surgery for BSGs with multimodal IONM between 2010 and 2023. Monitoring included somatosensory evoked potentials (SSEPs), motor evoked potentials (MEPs), brainstem auditory evoked potentials (BAEPs), corticobulbar MEPs (CoMEPs), and cranial nerves electromyography. The primary endpoint was the association between

Influence of remazolam and propofol on intraoperative neurophysiological monitoring during spinal surgery: A prospective randomized study.

BACKGROUND: Propofol, traditionally used for spinal surgery anesthesia under intraoperative neurophysiological monitoring (IONM), can interfere with IONM signals. Remazolam offers quick onset, minimal circulatory and respiratory effects, and resistance to flumazenil reversal, yet its impact on IONM signaling remains uncertain. Herein, we evaluated the impact of these 2 anesthetics on IONM during spinal surgery. METHODS: Eighty patients who had undergone spinal operation were enrolled and divided into group R and group P, with 40 cases in each group. Group R used remazolam while group P used p

Intraspinal epidermoid and dermoid cysts-tumor resection with multimodal intraoperative neurophysiological monitoring and long-term outcome.

BACKGROUND: Intraspinal epidermoid/dermoid cysts are very rare, benign tumors arising from pathological displacement of epidermal cells into the spinal canal. Literature data about the long-term outcome after microsurgical resection with multimodal intraoperative neurophysiological monitoring (IONM) are lacking. We analyzed one of the largest case series with special regard to intraoperative characteristics and long-term outcome after IONM-aided surgery. METHOD: All 12 patients (m:f = 1.4:1) who underwent microsurgical tumor resection with multimodal IONM for intraspinal epidermoid/dermoid tu

Intraoperative neurophysiological monitoring during complex spinal deformity cases in pediatric patients: methodology, utility, prognostication, and outcome.

INTRODUCTION: Complex spinal deformity (CSD) problems in pediatric patients result from a wide variety of congenital, acquired, neoplastic, or traumatic abnormalities that result in a combination of spinal deformity and spinal cord impingement. While these problems are rare, decompression, correction, instrumentation, and fusion are quite hazardous. Intraoperative neurophysiological monitoring (IONM) seems particularly beneficial in these patients. METHODOLOGY: Somatosensory evoked potentials, transcranial electrical motor evoked potentials (MEPs), direct waves, and electromyography were used

[Surgical treatment of anterior visual pathway gliomas in children using intraoperative neurophysiological monitoring and MR tractography].

BACKGROUND: Surgical resection of anterior visual pathway gliomas carries a high risk of postoperative visual deficit. The goals of surgical treatment are maximum safe removal of tumor and histological verification. Diffusion tensor imaging (DTI) with reconstruction of visual pathways and intraoperative monitoring of visual evoked potentials (VEP) were proposed to reduce the risk of potential visual dysfunction. OBJECTIVE: To demonstrate two cases of surgical treatment for anterior visual pathway gliomas using preoperative MR tractography of visual pathways and intraoperative VEP monitoring.

Intraoperative Neurophysiological Monitoring in Spina Bifida and Tethered Cord Surgeries: A Systematic Review of Clinical Outcomes.

Spina bifida and tethered cord syndrome (TCS) are forms of spinal dysraphism requiring surgical intervention to prevent progressive neurological decline. Intraoperative neurophysiological monitoring (IONM) provides real-time feedback, allowing surgeons to minimize intraoperative injury during these procedures. This systematic review analyzed 19 primary studies published between 2015 and 2025 examining the role of IONM techniques, including somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), electromyography (EMG), and bulbocavernosus reflex (BCR), in spina bifida and tethere

Intraoperative neurophysiological monitoring during spine surgery with total intravenous anesthesia or balanced anesthesia with 3% desflurane.

Total intravenous anesthesia (TIVA) with propofol and opioids is frequently utilized for spinal surgery when somatosensory evoked potentials (SSEPs) and transcranial motor evoked potentials (tcMEPs) are monitored. Many anesthesiologists would prefer to utilize low dose halogenated anesthetics (e.g. 1/2 MAC). We examined our recent experience using 3% desflurane or TIVA during spine surgery to determine the impact on propofol usage and on the evoked potential responses. After institutional review board approval we conducted a retrospective review of a 6 month period for adult spine patients who

Persistent hemiplegia with normal intraoperative neurophysiological monitoring in supratentorial neurosurgery: a case report and review of literature.

Intraoperative neurophysiological monitoring (IONM) is needed for evaluating and demonstrating the integrity of the central and peripheral nervous system during surgical manoeuvres that take place in proximity to eloquent motor and somatosensory nervous structures. The integrity of the monitored motor pathways is not always followed by consistent clinical normality, particularly in the first hours/days following surgery, when surgical resection involves brain structures such as the supplementary motor areas (SMA). We report the case of a patient who underwent surgical excision of a right front

Intraoperative neurophysiologic monitoring in idiopathic scoliosis surgery: a retrospective observational study of new neurologic deficits.

PURPOSE: Patients with adolescent idiopathic scoliosis undergoing corrective surgery are at risk for iatrogenic spinal cord injury and subsequent new neurologic deficits (NNDs). Intraoperative neurophysiologic monitoring (IONM) has been used to identify spinal cord injury; however, available data showing that IONM leads to improved clinical outcomes are inconclusive. This exploratory study aimed to examine the incidence of NNDs after idiopathic scoliosis surgery in two pediatric institutions in Canada with a focus on IONM use. METHODS: Charts of pediatric patients (10-18 yr) with adolescent i

Benefit of Intraoperative Neurophysiological Monitoring in a Pediatric Patient with Spinal Dysmorphism, Split Cord Malformation, and Scoliosis.

Intraoperative neurophysiological monitoring (IONM) consists of a group of neurodiagnostic techniques that assess the nervous system's functional integrity during surgical operations. A retrospective analysis of a pediatric female patient was conducted who underwent 12 operations for the correction of scoliosis, tethered cord, and split spinal cord wherein IONM played an important role. From age 3 to 6, she underwent six procedures including a release of the tethered cord, resection of the filum terminale, removal of a T11-T12 bony spur, release of L3 adhesions, repair of subcutaneous meningoc

Intraoperative Neurophysiologic Monitoring for Prediction of Postoperative Neurological Improvement in a Child With Chiari Type I Malformation.

INTRODUCTION: Although many surgical treatment strategies for Chiari malformation type I (CM-I) have been reported, the most appropriate surgical technique remains controversial. It is wholly ascribable to the complicacy of pathological condition in CM-I. Recently, intraoperative neurophysiologic monitoring (INM) is becoming prevalent in spinal surgery. Indeed, motor-evoked potentials (MEPs) monitoring and somatosensory-evoked potentials (SSEPs) monitoring are standard tools to minimize the risk of neurologic injury and postoperative deficits. The most recent study suggested that multimodality

Intraoperative neurophysiological monitoring (IONM) in neurosurgery: A critical appraisal of established practices, ongoing controversies, and future trajectories.

Intraoperative neurophysiological monitoring (IONM) has evolved from a novel technique into an evidence-based standard treatment method for high-risk neurosurgical and spinal surgeries. Its effectiveness is based on two interrelated pillars: optimized multimodal monitoring, mainly including motor and somatosensory evoked potentials (MEPs/SSEPs) as well as electromyography (EMG), and total intravenous anesthesia (TIVA) combined with a precise neuromuscular blockade-based anesthesia protocol. IONM significantly reduces neurological function damage during surgeries for spinal deformities, intrame
