Corneal damage after glass intraocular lens implantation.

We present three cases of corneal damage in two patients after implantation of an anterior chamber, iris-supported, intraocular lens (IOL) with a glass optic and polyamide haptics. We found bullous keratopathy in all three eyes. Penetrating keratoplasty, IOL removal, and anterior vitrectomy were performed in all cases. We believe the bullous keratopathy was caused by long-term iritis and pseudophakodonesis.

Acyclovir and vidarabine in the treatment of ulcerative herpes simplex keratitis.

In a masked controlled study, we treated 41 patients who had active herpes simplex corneal ulcers with either 3% acyclovir of 3% vidarabine ointment five times daily for 14 days. There was no statistically significant difference between the two drugs with reference to mean healing time, efficacy of healing, development of stromal keratitis or iritis, post-treatment visual acuity, or adverse reaction.

[Clinical analysis of recurrences of herpes simplex keratitis].

The rates of recurrence in 180 cases of herpes simplex keratitis in 1, 2 and 3 years after the primary attack were found to be 20%, 31.5% and 34.5% respectively, irrespective of sex and types of the lesions. The recurrence rate was significantly higher in cases complicated with corneal neovascularization or iritis and slightly higher in children and older patients. The factors involved and possible measures for reducing the recurrence rate were discussed.

Early Transcriptomic Response of Human Iris Stromal Cells During Herpes Simplex Virus Entry Reveals Interplay Between Cell Glycocalyx and Viral Exploitation.

Herpes simplex virus type 1 (HSV-1) initiates infection through sequential interactions with host receptors, yet the early transcriptional responses driving HSV-mediated iritis remain poorly understood. Given the clinical burden of HSV-induced anterior uveitis and the lack of targeted therapies, we sought to define the initial host response to infection. We performed temporal transcriptomic profiling of primary human iris stromal (HIS) cells at 1, 3, and 6 h post-infection. HSV-1 triggered rapid and extensive gene expression changes, with early activation of IL-17, TNFα, MAPK, and NF-κB signal

Safety and efficacy of manual small incision cataract surgery for brunescent and black cataracts.

AIMS: To evaluate the safety, visual outcome and complications of manual small incision cataract surgery (MSICS) in the treatment of patients with brunescent and black cataract (BBC). METHODS: In a non-randomised interventional case series, 102 consecutive patients with BBC underwent cataract extraction by MSICS, with staining of the anterior capsule by trypan blue. RESULTS: Of the 102 eyes with BBC, MSICS was performed through superior scleral tunnel (SST) in 31 eyes (30.4%) and through temporal scleral tunnel (TST) in 71 eyes (69.6%). The main intraoperative complication was posterior caps

[The meaning of antinuclear antibodies and of the ASL O titer in rheumatic diseases (author's transl)].

Iritis, Iridocyclitis, Uveitis and Scleritis are sometimes caused by rheumatic diseases. Antinuclear antibodies of 64 patients with high ASL O titer and Antinuclear antibodies of 24 patients with negativ ASL O titer were investigated. The results demonstrate the existance of autoantibodies in many cases. These may be caused by autoimmuno-diseases or could be related directly to ophthalmic disorder.

[Reiter's disease in an infant (author's transl)].

A case of Reiter's disease in a 15-month-old child is presented. The manifestation of this syndrome in childhood is an extraordinary event, so far few cases only have been observed. The symptoms of the syndrome are urethritis, arthritis and iritis. A specific infection could not be demonstrated. The typical clinical features are discussed and compared with earlier reports. Speculations on the etiology are made.

Behçet's Disease Presenting with Acute Transverse Myelitis: MRI Findings and Review of the Nosology. A Case Report.

Spinal cord involvement, either isolated or together with brain, in Behçet's disease (BD) has been reported. In these cases the existence of the disease was previously known or the classical triad of disease such as oral and genital ulcers with uveitis/iritis was present. Here we describe a 22-year-old man in whom acute transverse myelitis diagnosed with MRI was the first finding of BD.

Extracranial carotid artery aneurysm due to Behcet's disease.

Behçet's disease is a chronic systemic inflammatory disorder associated with recurrent oral and genital ulcers and iritis. Vascular lesions are encountered in 7%-29% of patients, gravely affecting the course of the disease. Extracranial carotid aneurysms due to Behçet's disease are extremely rare. We describe a surgically treated case of Behçet's disease in a 28-year-old man who presented with a rapidly enlarging left common carotid artery aneurysm.

Common eye disorders: six patients to refer.

Thorough ocular history taking and physical examination are essential to establish a diagnosis in patients presenting with eye conditions. Some conditions require ophthalmologic referral to avoid serious complications and even vision loss. These include corneal ulcers, retinal detachment, iritis, glaucoma, retinal artery occlusion, and endophthalmitis. Because primary open-angle glaucoma can have an insidious onset and cause irreversible damage, funduscopic examination should be a part of every complete physical examination.

Varicella dendritic keratitis.

A 7-year-old boy and a 3-year-old girl suffered from unilateral disciform keratitis and iritis associated with varicella. While they were treated with topical corticosteroid, idoxuridine, and atropine drops, dendritic lesions typical of herpes zoster appeared four months after the onset of eruptive skin lesions. Using the direct immunofluorescent method, we showed varicella-zoster virus antigen in the epithelial cells scraped from the dendritic lesion.

[Thrombosis of the inferior vena cava in Behçet disease].

Behçet's disease is considered to be a systemic process caused by a nonspecific vasculitis with a diversity of symptoms. It is defined by the coexistence of aphthous stomatitis, genital ulcerations, inflammatory conditions of the eye such as iritis and dermatological changes. Cardiovascular involvement includes both arteries and veins. We report an unusual case of thrombosis of the inferior vena cava with extensive collateral circulation of the superficial abdominal and paravertebral veins due to Behçet's disease diagnosed by phlebography and magnetic resonance imaging.

Ocular manifestations of syphilis.

Since the mid-1970s, the number of new cases of infectious syphilis has begun to increase. Ocular manifestations of secondary and late syphilis occur. The optometric role in the management of infectious syphilis is one of detection, consultation with internists and neurologists for systemic treatment, topical treatment of anterior uveitis, and monitoring for recurrence of disease. This report describes an elderly patient presenting with recurrent, bilateral iritis and optic atrophy as initial clinical manifestations of syphilitic infection.

Transient glaucoma as a manifestation of mumps. A case report.

A case report of a 43-year-old man who during convalescence after mumps (parotitis epidemica) developed bilateral glaucoma associated with redness of his eyes but no other ocular manifestations. The chamber angles were open. No signs of scleritis or iritis were present. The best treatment was found to be prednisolone topically and acetazolamide orally. After ten days the intraocular pressure was normalized and after a fortnight all treatment could be discontinued.

Ciliary injection: a differential diagnosis for the patient with acute red eye.

This case presents the challenge of how to accurately assess the unilateral painful red eye with ciliary injection. The diagnosis can be clarified with careful clinical observation and a few basic clinical tests. Physical examination should focus on visual acuity; presence of pain; location of redness; shape, size, and reaction of the pupil; and the IOP, if it can be obtained safely. If a slit lamp is available, the diagnosis can be made more definitively. Because the diagnosis in a situation like this is usually made by an eye specialist, a clinical presentation of unilateral painful red eye
