[Free re-innervated gracilis muscle transfer to restore flexion or extension of the fingers: about three cases].

Free re-innervated gracilis muscle transfer is used to restore fingers or wrist flexion or extension after severe forearm injuries and when none muscle transfer is possible. The long-term function after this procedure is evaluated for three patients treated in our unit. This microsurgical transfer is technically demanding but allows good fingers mobility and good autonomy for daily activities and working.

Age-related differences in myocardial metabolism affects response to ischemia. Age in heart tolerance to ischemia.

Difficulties persist in providing optimum myocardial protection to neonatal hearts undergoing congenital cardiac repair. Controversy on actual ischemic sensitivity of neonatal hearts compared to adult hearts may depend on species, age selected, and conditions of the experimental protocol. In 1985, our laboratory began to investigate this area using the time to ischemic contracture (TIC) model popularized by Hearse and Wechsler and reported that neonates developed TIC in a significantly shorter time than adult hearts. The neonatal heart had rapid lactate accumulation and early rapid decline in

Acute Compartment Syndrome of the Leg.

INTRODUCTION: Acute compartment syndrome (ACS) is well known among orthopaedic surgeons. The timely diagnosis and management of ACS is crucial to avoiding its sequelae, including renal failure, ischemic contractures, and limb loss. Despite its relative importance, ACS poses a challenge to many residents and clinicians as diagnosis relies largely on clinical judgment. METHODS: Timely diagnosis and thorough compartment release are essential to optimizing outcomes in ACS. This video highlights a clinical case in which compartment syndrome of the leg was considered, diagnosed, and surgically mana

McArdle disease: what do neurologists need to know?

McArdle disease (also known as glycogen storage disease type V) is a pure myopathy caused by an inherited deficit of myophosphorylase, the skeletal muscle isoform of the enzyme glycogen phosphorylase. The disease exhibits clinical heterogeneity, but patients typically experience exercise intolerance, that is, reversible, acute crises (early fatigue and contractures, sometimes with rhabdomyolysis and myoglobinuria) triggered by static muscle contractions (e.g. lifting weights) or dynamic exercise (e.g. climbing stairs or running). In this Review, we discuss the main features of McArdle disease,

Surgical management of the hand in scleroderma.

Disabling deformity of the hand is a hallmark feature of the person afflicted with scleroderma. However, existing literature provides little guidance to operative treatment for the wide spectrum of hand derangement. Although arthrodesis is generally recommended for severe flexion contractures of the interphalangeal joints, other surgical procedures such as arthroplasty, excision of painful calcinosis, and digital sympathectomy have been employed sparingly, undoubtedly due to potentially hazardous soft tissue conditions. Based on experience with 70 scleroderma patients requiring 272 hand operat

The outcomes of salvage surgery for vascular injury in the extremities: a special consideration for delayed revascularization.

A seven years retrospective study was performed in 45 consecutive vascular injuries in the extremities to investigate the pattern of injuries, managements and outcomes. Motor-vehicle accidents were the leading cause of injuries (80%), followed by industrial injuries (11.1%) and iatrogenic injuries (4.4%). Popliteal and brachial artery injuries were commonly involved (20%). Fifteen (33.3%) patients had fractures, dislocation or fracture dislocation around the knee joint and 6 (13.3%) patients had soft tissue injuries without fracture. Traumatic arterial transection accounted for 34 (75.6%) case

Priapism as the initial presentation of chronic myeloid leukemia in a patient with cerebral palsy: A case report.

Priapism is defined as a penile erection lasting more than four hours without stimulation. The majority of cases involve ischemic priapism, in which there is partial or complete absence of cavernosal arterial flow. Priapism is uncommon, with an estimated annual incidence of approximately 1.5 cases per 100,000 men. In chronic myeloid leukemia (CML) patients, leukostasis may occur, which can lead to priapism in 1-2% of cases. We present the case of a 21-year-old patient with a history of cerebral palsy who was brought to the emergency department by his family with priapism persisting for 13 hour

Abnormalities of the myocytes in ischaemic cardiomyopathy.

Acute myocardial ischemia and subsequent reperfusion result in biochemical and ionic changes in cardiac myocytes which cause contracture of the muscle and a reduced contractile force. Whether changes observed in single myocytes isolated from ischaemic ventricles are a direct consequence of the acute insult, or develop more slowly due to subsequent alterations in load and neurohumoural environment, is controversial. Myocytes from ischemic hearts have a similar contraction amplitude to those from non-failing hearts at physiological or maximally activating levels of ca2+. This could be partly due

Free myocutaneous flap transfer to treat congenital Volkmann's contracture of the forearm.

The purpose of this study is to report our experience with free functional muscle transfer procedures for the late sequelae of the rare condition of congenital Volkmann's ischaemic contracture of the forearm. Four children, with an average age of 9.5 years (range 1.5-17), were treated and were followed for a mean of 6 years (range 1-14). Two patients had dorsal forearm contractures, and two had both flexor and extensor forearm contractures. We carried out free functional muscle transfers to replace the flexor or extensor muscles. The functional result was assessed according to the classificati

Neonatal presentations of neuromuscular disorders.

The term 'neuromuscular diseases' defines disorders of the extended motor unit. Newborns with disorders of peripheral nervous system (PNS) (motor neurons, nerve roots, plexuses, peripheral nerves, neuromuscular junction, and skeletal muscles) present most frequently with hypotonia, weakness, contractures, respiratory and feeding difficulties. Challenge in the newborn period is, hypotonia may also occur with more common central causes such as; electrolyte disturbances, sepsis, hypoxic-ischemic encephalopathy (HIE), congestive heart failure, and inborn errors of metabolism. Moreover, newborns wi

Partial artificial heart (ALVAD) use with subsequent cardiac and renal allografting in a patient with stone heart syndrome.

The abdominal left ventricular assist device (ALVAD) is an order of magnitude more effective than conventional intra-aortic balloon pumping (IABP) in unloading and providing circulatory support to the failing left ventricle. This is a report of a unique case which demonstrates that in the absence of pulmonary vascular obstruction or constriction, the ALVAD can substitute for both left and right heart function. A 21-year-old patient with a congenital bicuspid aortic valve developed acute valvular endocarditis which rapidly progressed to congestive heart failure. An operation was undertaken, the

Long-term HIF-1α transcriptional activation is essential for heat-acclimation-mediated cross tolerance: mitochondrial target genes.

An important adaptive feature of heat acclimation (HA) is the induction of cross tolerance against novel stressors (HACT) Reprogramming of gene expression leading to enhanced innate cytoprotective features by attenuating damage and/or enhancing the response of "help" signals plays a pivotal role. Hypoxia-inducible factor-1α (HIF-1α), constitutively upregulated by HA (1 mo, 34°C), is a crucial transcription factor in this program, although its specific role is as yet unknown. By using a rat HA model, we studied the impact of disrupting HIF-1α transcriptional activation [HIF-1α:HIF-1β dimerizati

The application of the IMB model combined with feedback-based health education in the rehabilitation of limb function in stroke patients.

This study evaluates the impact of an information-motivation-behavioral skills model (IMB model) health education intervention combined with feedback on rehabilitation outcomes for ischemic stroke patients with hemiplegia, targeting improvements in motor function, psychological state, disease awareness, and quality of life. A total of 152 ischemic stroke patients with hemiplegia were divided into an experimental group receiving the IMB-based intervention and a control group receiving standard education. Both interventions lasted 3 months. Rehabilitation outcomes were measured using indicators

Voluntary multiplet discharge after the activation test for spasmophilia as an electromyographic sign of a mild form of this disease.

The electromyographic (EMG) diagnosis of spasmophilia is based on the appearance in a susceptible muscle of spontaneous burst (multiplet) discharges after an activation procedure consisting usually in 10 minutes ischemia followed by a pause of a length that varies with the author but does not exceed 5 minutes, and finally by a hyperventilation, again not exceeding 5 minutes. In investigations carried out many years ago we observed that a number of subjects with symptoms of spasmophilia showed no postischemic spontaneous bursts, but responded with bursts to voluntary contraction. In this work w

Vascular injuries and their sequelae in pediatric supracondylar humeral fractures: toward a goal of prevention.

Between 1988 and 1994, 128 consecutive children with grade III supracondylar humeral fractures presented for treatment at our hospital. Seventeen had absent or diminished (detected with Doppler but not palpable) radial pulses on initial examination. Fourteen of these 17 children recovered pulse (palpable) after reduction and stabilization of their fractures. The remaining three had persistent absence of radial pulse. Each of these three children was explored immediately and found to have a significant vascular injury requiring repair. Two of the 14 children who had initially regained their pul

Phosphoglycerate kinase deficiency: another cause of recurrent myoglobinuria.

A 14-year-old boy had myoglobinuria and renal failure after intense exercise; a year earlier he had experienced a milder episode. There was no consanguinity and no family history of neuromuscular diseases or hemolytic anemia. Strength was normal. Forearm ischemic exercise caused prolonged contracture with no rise of venous lactate. Muscle morphology showed only a mild increase of lipid droplets. Glycogen concentration was normal. Muscle phosphoglycerate kinase (PGK) activity was 5% of the normal mean, and all other glycolytic enzymes were normal. The residual PGK activity of muscle was heat st

pGlu-serpinin protects the normotensive and hypertensive heart from ischemic injury.

Serpinin peptides derive from proteolytic cleavage of Chromogranin-A at C-terminus. Serpinin and the more potent pyroglutaminated-serpinin (pGlu-Serp) are positive cardiac β-adrenergic-like modulators, acting through β1-AR/AC/cAMP/PKA pathway. Because in some conditions this pathway and/or other pro-survival pathways, activated by other Chromogranin-A fragments, may cross-talk and may be protective, here we explored whether pGlu-Serp cardioprotects against ischemia/reperfusion injury under normotensive and hypertensive conditions. In the latter condition, cardioprotection is often blunted beca
