[Alloimmune neonatal granulocytopenia caused by anti-NA1 antibodies].

Two cases of neonatal alloimmune granulocytopenia due to fetomaternal incompatibility with granulocyte-specific antigen NA1 are presented. The diagnosis was based on the detection of anti-NA1 alloantibodies in maternal and neonates sera in leukoaglutination and immunofluorescence tests with granulocytes. Both children (twins) had severe granulocytopenia and unfortunately died.

Alloimmune neonatal neutropenia--a neglected diagnosis?

Alloimmune neonatal neutropenia is due to maternal IgG neutrophil-specific antibodies that cross the placenta to sensitize fetal neutrophils and cause neutropenia in the neonate. Within the first few weeks of life, the infant may succumb to localized or generalized bacterial infections. Haematologically, the mother is normal, but the infant is severely neutropenic. The incidence of this condition is one in 2000 neonates, but it can be overlooked easily if specialized testing is not performed in neonates with unexplained neutropenia. It is generally a self-limiting disorder, but severe infectio

Immunopathology of guinea pig autoimmune enterocolitis induced by alloimmunisation with an intestinal protein.

An enterocolitis has been induced in guinea pigs by alloimmunisation with a mucosal protein. A single dose of immunogen fails to provoke a synthesis of precipitant antibodies, but a high percentage of animals, injected with two or more doses, develop these antibodies. A specific cell mediated immune response was already detectable 30 days after a single dose of immunogen. At later periods and after multiple doses of immunogen positive results were still found, although in a lesser percentage of the animals. The pathology was characterised by the appearance of multiple mucosal ulcerations, cong

Purification and characterization of a liver cell membrane specific antigen.

Antibodies against human liver cell membranes were raised in rabbits by immunizing 105,000 g precipitate of normal human liver homogenates (insoluble fraction). After absorption with non-liver antigens such as human spleen, kidney, lung homogenates and serum, the anti-liver cell membrane specific antigen (LCM) IgG fraction was prepared. The antibody reacted with both 105,000 g supernatant of the liver homogenate (soluble fraction) and Triton X-100 extract of the insoluble fraction, and with the surface of Chang liver cells. Using affinity columns to conjugate this antibody and other antibodies

Immune tolerance: Parallels between pregnancy and kidney transplantation.

Pregnancy represents a unique immunological paradox in which the maternal immune system must tolerate the semi-allogeneic fetus expressing paternal antigens. The success of pregnancy depends on a highly coordinated network of immune tolerance mechanisms, encompassing the placental barrier, chemokine-guided leukocyte migration, restricted alloantigen presentation, regulatory T cells, tolerogenic natural killer cells, immune checkpoint molecules, and both hormonal and metabolic adaptations. In contrast, achieving long-term acceptance of transplanted organs-referred to as "operational tolerance"-

HNA-3 antibodies and increased risk of recurrent rejection in kidney transplant recipients.

BACKGROUND: There is an increasing number of biopsies with histological features of antibody mediated rejection but lacking the detection of anti-human leucocyte antigen (HLA) antibodies or C4d deposition. Anti-Human neutrophil antigens-3 (HNA-3) have been reported in severe and fatal cases of Transfusion-Related Acute Lung Injury and Neutropenia Alloimmune Neonatal, a phenomenon mediated by antibody endothelial activation. This study investigated the association between the presence of pre-formed anti-HNA-3 antibodies and the development of a recurrent episode of acute rejection (AR) after ki

SMA20/PMIS2 Is a Rapidly Evolving Sperm Membrane Alloantigen with Possible Species-Divergent Function in Fertilization.

Immunodominant alloantigens in pig sperm membranes include 15 known gene products and a previously undiscovered Mr 20,000 sperm membrane-specific protein (SMA20). Here we characterize SMA20 and identify it as the unannotated pig ortholog of PMIS2. A composite SMA20 cDNA encoded a 126 amino acid polypeptide comprising two predicted transmembrane segments and an N-terminal alanine- and proline (AP)-rich region with no apparent signal peptide. The Northern blots showed that the composite SMA20 cDNA was derived from a 1.1 kb testis-specific transcript. A BLASTp search retrieved no SMA20 match from

Immunoprotection of cellular transplants for autoimmune type 1 diabetes through local drug delivery.

Type 1 diabetes mellitus (T1DM) is an autoimmune condition that results in the destruction of insulin-secreting β cells of the islets of Langerhans. Allogeneic islet transplantation could be a successful treatment for T1DM; however, it is limited by the need for effective, permanent immunosuppression to prevent graft rejection. Upon transplantation, islets are rejected through non-specific, alloantigen specific, and recurring autoimmune pathways. Immunosuppressive agents used for islet transplantation are generally successful in inhibiting alloantigen rejection, but they are suboptimal in hind

Production of recombinant humanized monoclonal anti-human neutrophil antigen (HNA) antibodies with potential applicability as standard antibodies.

BACKGROUND: Antibodies against human neutrophil antigen (HNA) are involved in the pathogenesis of neonatal alloimmune neutropenia, autoimmune neutropenia, and transfusion-related acute lung injury. The present methods for anti-HNA antibody identification strongly depend on the presence of standard antisera with known allo/isospecificities. Here, we aimed to produce recombinant humanized antibodies to HNA from available mouse monoclonal antibodies (MoAbs). STUDY DESIGN AND METHODS: RNAs were extracted from available hybridoma cells producing mouse anti-HNA antibodies recognizing HNA-1a (TAG-1)

Critical evaluation of a possible role of HLA epitope matching in kidney transplantation.

Human leukocyte antigen (HLA) matching is one of the cornerstones of organ allocation in deceased-donor kidney transplantation. Increased numbers of HLA allele mismatches are associated with a higher risk of immunological rejection, de novo donor-specific HLA antibody development and graft failure. HLA epitopes are defined as the specific portions of HLA molecules to which antibodies and T-cell receptors bind with their paratopes. The same epitope can be present on different HLA alleles. Therefore, HLA matching at the epitope instead of allele level theoretically offers a more precise assessme

A study of human neutrophil antigen genotype frequencies in Hong Kong.

BACKGROUND: Alloantibodies against human neutrophil antigens (HNA) are associated with a variety of clinical conditions. Over the past decade, the allelic and genotypic frequencies of the five HNA systems have been evaluated. Although the HNA system is less polymorphic than human leukocyte antigens (HLA), significant differences in the genotypic and allele frequencies still exist in different populations, even those living in close proximity. OBJECTIVES: To delineate HNA genotypic and allele frequencies to provide vital information on estimating the risk of HNA-associated diseases for our loc

Immunoregulatory networks in sickle cell alloimmunization.

Red blood cell (RBC) transfusions are critical for treatment and prevention of complications of sickle cell disease (SCD), and most SCD patients will receive 1 or more transfusions by age 20. However, SCD alloimmunization remains a serious complication of transfusions that can lead to life-threatening acute and delayed transfusion reactions. Alloimmunization rates are higher in SCD patients most likely due to RBC antigenic differences between largely white donors vs mainly African-American recipients and frequency of transfusions. However, it remains unclear why some but not all SCD patients d

Risks associated with red blood cell transfusions: potential benefits from application of pathogen inactivation.

BACKGROUND: Red blood cell (RBC) transfusion risks could be reduced if a robust technology for pathogen inactivation of RBC (PI-RBCs) were to be approved. MATERIALS AND METHODS: Estimates of per-unit and per-patient aggregate infectious risks for conventional RBCs were calculated; the latter used patient diagnosis as a determinant of estimated lifetime exposure to RBC units. Existing in vitro data for the two technologies under development for producing PI-RBCs and the status of current clinical trials are reviewed. RESULTS: Minimum and maximum per-unit risk were calculated as 0.0003% (1 in

Allosensitization in heart transplantation: an overview.

Transplant candidates might manifest circulating antibodies against human leukocyte antigens and nonhuman leukocyte antigens, a condition termed allosensitization. The presence of these antibodies decreases a given candidate's possible donor pool, thereby prolonging the time to transplantation. They are also associated with poorer posttransplant outcomes including increased morbidity and mortality. With the increasing use of ventricular assist devices as a bridge to transplantation, the prevalence of allosensitized transplant candidates has increased. This has implications for transplant progr

Subgrouping of A and AB blood groups in Indian blood centres: is it required?

Anti A1 antibody in the serum of A2 and A2B individuals is rare but when present can have laboratory and clinical significance. Routine subgrouping of all A and AB blood groups in blood centres in India is difficult due to economic constraints and has always been a point of debate. This study thus brings out the prevalence of anti A1 antibody and the clinical significance related to its presence. The results of the study showed a low prevalence of anti A1 antibody and when present, it had a low thermal amplitude and titre. Further, no blood group discrepancy or problems during compatibility te

Immunomodulation by blood transfusions.

Blood transfusions can affect the immune response in two opposite ways. They may either lead to immunization or to tolerance induction. Immunization is reflected by the induction of HLA alloantibodies and T cell activation while the induction of tolerance is suggested by the enhanced graft survival in transfused versus non-transfused recipients. The immunological mechanism leading to downregulation of the alloimmune response is not clear. One possible explanation is the induction of a Th2 response by non-professional antigen presentation by the transfused blood cells. On the other hand, eviden

The treatment of bleeding in hemophilic patients with inhibitors with recombinant factor VIIa.

Recombinant factor VIIa (rFVIIa) has proven efficacy in the treatment of patients with alloantibodies and autoantibodies to factors VIII and IX. More than 80% of over 2,000 reported bleeding episodes treated with rFVIIa have been controlled in a variety of clinical circumstances. Its theoretical lack of activation of the coagulation system prompted many clinicians to undertake elective surgical procedures using rFVIIa in addition to on-demand treatment of minor bleeding episodes. Hemostatic efficacy has been good for most of these surgical procedures, without serious adverse events. rFVIIa is
