Metastatic amelanotic melanoma to the mandible mimicking benign odontogenic neoplasm.

Metastatic lesions to the jaws are rare and represent about 1% of all malignancies in the oral cavity. They are not usually considered as part of the differential diagnosis due to their paucity and unusual clinico-pathological features leading to diagnostic challenges. We report on a rare case of metastatic melanoma to the anterior mandible in a 58 year old Caucasian female that mimicked benign odontogenic tumour. The lesion posed a diagnostic challenge as it clinically and radiographically presented as a well-defined radiolucency with minimal bony destruction and no pigmentation or ulceration

[«Man-in-the-barrel» syndrome: atypical manifestation of giant cell arteritis].

INTRODUCTION: «Man-in-the-barrel» syndrome refers to diplegia of the upper extremities in which mobility of the head and lower limbs is preserved. Brachial plexitis that presents as «man-in-the-barrel» syndrome is an unusual manifestation of giant cell arteritis. We report a case of C5-C6 plexitis as part of the clinical features of a patient with giant cell arteritis. CASE REPORT: A 70-year-old male with a two-month history of weight loss, headache, facial pain and jaw claudication, associated with a persistent elevation of acute phase reactants and bilateral brachial plexopathy, with no evi

Mixed odontogenic tumors in children and adolescents.

The typical forms of mixed odontogenic tumors in the jaws of children and adolescents are ameloblastic fibroma, ameloblastic fibroodontoma, complex odontoma, and compound odontoma. Ameloblastic fibromas (AFs) are true, mixed, soft tissue neoplasms, deriving from the proliferation of both odontogenic epithelium and mesenchyma. They have a potential for both recurrence and malignant transformation. Ameloblastic fibroodontomas (AFOs) may be regarded as hamartomas, which exhibit epithelial, mesenchymal, and abundant hard tissue components of the developing teeth. Odontomas are calcifying benign ha

Immunohistochemical expression of p53, MDM2, Ki-67 and PCNA in central giant cell granuloma and giant cell tumor.

Central giant cell granuloma (CGCG) is a reactive bone lesion that occurs mainly in the jaws. The giant cell tumour (GCT) is a benign locally aggressive neoplasm located near the articular end of tubular bones. Both lesions are characterised histologically by multinucleated giant cells in a background of ovoid to spindle-shaped mesenchymal cells. There is a basic question whether both lesions are separate entities or variants of the same disease. The study of cell cycle-associated proteins may give insights into clarifying such question. The expression of these proteins is also important to de

Efficacy and safety of cold forceps biopsy for diminutive pharyngeal neoplasms: Single-center, prospective pilot study.

BACKGROUND: Management of diminutive pharyngeal neoplasms is controversial. Thus, we conducted a single-center, prospective pilot study to investigate the efficacy and safety of endoscopic excision with cold forceps biopsy (CFB) of these lesions. PATIENTS AND METHODS: Thirty-nine lesions endoscopically diagnosed with narrow-band imaging as pharyngeal neoplasms of 3 mm or smaller were excised with CFB using jumbo biopsy forceps (cap diameter 2.8 mm, jaw volume 12.4 mm3 ). The primary outcome was endoscopically determined local remnant/recurrence rate 3 months after CFB. The secondary outcomes

Central Mucoepidermoid Carcinoma in the Mandible Mimicking Dentigerous Cyst: A Report of a Rare Case.

Mucoepidermoid carcinoma is a rare neoplasm of the salivary gland of which the intraosseous variety is commonly observed with a female predilection and the affected side is more commonly in the mandible. It is usually perceived as an asymptomatic swelling that increases in volume over a few months to a year. They more frequently present as a cortical bulging and are mostly discovered as an accidental finding in a routine radiograph as a well-defined unilocular or multilocular radiolucency resembling an odontogenic cyst. The most widely accepted treatment is radical surgical resection due to it

Solitary plasmacytoma of mandible: A rare entity.

Plasmacytoma is a discrete, unifocal, monoclonal neoplastic proliferation of the plasma cells. It may present as one of the three distinct clinical entities: multiple myeloma (MM), solitary plasmacytoma of bone and extramedullary plasmacytoma. Solitary bone plasmacytoma accounts for 3% of all plasma cell neoplasms with approximately 50% of the cases transforming into MM. It is frequently seen in vertebrae and secondarily in long bones. Its presence in jaws is extremely rare and when it is seen, retromolar area, angle and ramus of the mandible are most common sites of occurrence. A comprehensiv

Intraosseous Hemangioma of the Zygomatic Bone: A Rare Maxillofacial Case in a 57-Year-Old Woman.

Central hemangioma is one of the rare lesions of the jawbones, with a prevalence ranging between 0.5% and 1%. It more commonly occurs in the vertebral column and cranial bones, with rare occurrences in the jaws. The World Health Organization classifies hemangioma as a true neoplasm of endothelial origin; however, some authors believe that hemangiomas are hamartomas. Diagnosis of this lesion can be challenging due to its varying clinical and radiographic characteristics. Treatment is also difficult due to the presence of an extensive vascular network in the affected area. In this article, we pr

Eosinophilic Granuloma of Mandible: A Diagnostic Challenge.

Eosinophilic Granuloma is the mildest and localized form of Langerhans Cell Histiocytosis and is characterized by clonal proliferation of Langerhans cells. It is a rare disease, accounting for less than 1% of all the osseous neoplasms. It has predilection for the axial skeleton and incidence in jaws is just 7.9%. It lacks pathognomonic clinical and radiographic trait and hence is difficult to make a correct diagnosis without histopathological and immunohistochemical examination. This report describes a case of Eosinophilic Granuloma of mandible in 30 years old male who presented with complain

Jaw angiosarcoma. First case, with massive intraosseous localization, described in Italian literature.

Angiosarcoma (AS) is a rare non-epithelial malignant neoplasm arising from neoplastic vascular degeneration of endothelial cells. It usually occurs in soft tissue and skin. The incidence, according to American authors, is 1% of all soft tissue sarcomas. About 50% of AS is localized in head and neck region (scalp and face skin) and represents less than 1% of all malignancies of this district; the primitive intra- oral localization is rare, even rarer intraosseous development of AS in jaw bones. The Authors report a case of a mandibular intraosseus angiosarcoma with different peculiarities: the

Bone Tumors of the Jaw - the "Blind Spot" for Radiologists Experienced with Tumors? - Part I.

Primary bone tumours of the jaw are rare tumoral entities and do substantially differ from other bone tumours of the human body with respect of their frequently encountered unusual radiological appearances. The reason for that may be confined to the co-existence of two closely neighbored but different anatomical structures (i.e., tooth-forming apparatus and jaw bones with adjacent gingiva) and some tumour pathologies which are nearly excusively encountered in the jaw bones only (e.g., ameloblastoma, ossifying fibroma, ghost cell carcinoma). This paper would like to highlight some basic principl

Pathogenic Mitochondrial DNA Mutation Load Inversely Correlates with Malignant Features in Familial Oncocytic Parathyroid Tumors Associated with Hyperparathyroidism-Jaw Tumor Syndrome.

While somatic disruptive mitochondrial DNA (mtDNA) mutations that severely affect the respiratory chain are counter-selected in most human neoplasms, they are the genetic hallmark of indolent oncocytomas, where they appear to contribute to reduce tumorigenic potential. A correlation between mtDNA mutation type and load, and the clinical outcome of a tumor, corroborated by functional studies, is currently lacking. Recurrent familial oncocytomas are extremely rare entities, and they offer the chance to investigate the determinants of oncocytic transformation and the role of both germline and som

A comparison of piezoelectric surgery and conventional techniques in the enucleation of cysts and tumors in the jaws: a systematic review and meta-analysis.

BACKGROUND: Despite the comprehensive classifications provided by the WHO, the most common lesions include radicular cysts, dentigerous cysts, odontogenic keratocysts, ameloblastomas, and odontomas. The piezoelectric technique has shown effectiveness in removing intraosseous pathologies by relying on ultrasonic microvibrations, which help preserve soft and vascular tissues. Precision in manipulating intraosseous pathology can impact the prognosis and improve the surgical procedure by controlling hemorrhage and promoting microscopic benefits. While previous research has compared the advantages

Multiple endocrine neoplasia syndromes.

The multiple endocrine neoplasia (MEN) syndromes are rare autosomal-dominant conditions that predispose affected individuals to benign and malignant tumors of the pituitary, thyroid, parathyroids, adrenals, endocrine pancreas, paraganglia, or nonendocrine organs. The classic MEN syndromes include MEN type 1 and MEN type 2. However, several other hereditary conditions should also be considered in the category of MEN: von Hippel-Lindau syndrome, the familial paraganglioma syndromes, Cowden syndrome, Carney complex, and hyperparathyroidism jaw-tumor syndrome. In addition, researchers are becoming

Pathogenesis of bone metastasis: a review.

BACKGROUND: Metastasic deposits from malignancies frequently lodge in the skeleton, including the jaw bones. METHOD: A review of the literature was performed in order to provide a coherent overview on the pathogenesis of bone metastasis. RESULTS: Bone metastasis follows complex molecular interactions that enable tumor cells to detach from the primary site, invade the extracellular matrix, intra-vasate, extra-vasate, and proliferate within bone. They induce local bone changes that could manifest radiologically as either osteolytic or radiodense. In addition to the direct bone changes, maligna

Exceptional Evolution of a Squamous Odontogenic Tumor in the Jaw: Molecular Approach.

A squamous odontogenic tumor (SOT) is an epithelial locally benign neoplasia derived from the periodontium of the jaws. It is considered a lesion of low incidence. Predominantly, it affects the mandible, although both jaw bones may be involved. Here, we discuss the malignant clinical evolution of an SOT lesion in an 80-year-old female patient. The patient exhibited an expansive triangular lesion at the inferior right quadrant. Surgery was performed and an SOT was diagnosed (2019). Two years after, the lesion grew, and the analysis of the biopsy revealed SOT malignization with pleomorphic atypi

Gingival Squamous Cell Carcinoma: Diagnostic Challenges and Clinical Implications.

Oral squamous cell carcinoma (OSCC) is one of the highly invasive malignancies globally, accounting for the vast majority of all oral cancers, that is, more than 90%. Carcinomas of the gingiva represent a distinct subgroup of OSCC, constituting approximately 12% of all OSCC cases. These neoplasms might mimic the inflammatory and reactive state of the periodontium, often leading to a delay in diagnosis and hence, poorer prognosis. A case of gingival squamous cell carcinoma in a 48-year-old female patient has been reported here, in which the patient had no risk factors for the development of thi
