BACKGROUND: Elbow contracture is a sequelae of elbow trauma in pediatric patients. Arthroscopic contracture release has been shown to provide equivalent results to open contracture release with less associated morbidity and complications in the adult population. However, open contracture release is still commonly utilized in pediatric patients. The goal of this study is to determine the clinical results and safety profile of arthroscopic elbow contracture release in the pediatric population.
METHODS: A retrospective review of all patients 18 years of age and younger who underwent arthroscopic

Osteoarthritis is an incapacitating disease characterized by pain and a progressive decrease in joint mobility. The implantation of mesenchymal stem cells (MSCs) has shown promising results for its treatment. The challenge remains to keep the cells longer at the site of action, increasing their therapeutic potential. The aim of this study was to evaluate the effectiveness of the Qtracker® 655 nanocrystal marking on allogeneic synovial membrane (SM) MSCs, encapsulated in alginate hydrogel, evaluating the migration of these cells. The 10 radiocarpal joints were submitted to arthroscopic surgery

BACKGROUND: Calcitonin gene-related peptide (CGRP), a neuropeptide released from trigeminal nerves, is implicated in the underlying pathology of temporomandibular joint disorder (TMD). Elevated levels of CGRP in the joint capsule correlate with inflammation and pain. CGRP mediates neurogenic inflammation in peripheral tissues by increasing blood flow, recruiting immune cells, and activating sensory neurons. The goal of this study was to investigate the capability of CGRP to promote peripheral and central sensitization in a model of TMD.
RESULTS: Temporal changes in protein expression in trige

PURPOSE: Valgus deformity presents a particular challenge in total knee arthroplasty. This condition regularly leads to contractures of the lateral capsular ligament complex and to overstretching of the medial ligamentous complex. Reconstruction of the knee joint kinematics and anatomy often requires lateral release. However, data on how such release weakens the stability of the knee are missing in the literature. This study investigated the effects of sequential lateral release on the collateral stability of the ligament complex of the knee in vitro.
METHODS: Ten knee prostheses were implant

BACKGROUND: This study evaluated the preoperative distractive stress radiographs in order to quantify and predict the extent of medial release according to the degree of varus deformity in primary total knee arthroplasty.
METHODS: We evaluated 120 varus, osteoarthritic knee joints (75 patients). The association of the angle on the distractive stress radiograph with extent of medial release was analyzed. The extent of medial release was classified into the following 4 groups according to the stage: release of the deep medial collateral ligament (group 1), release of the posterior oblique ligam

Adhesive capsulitis is characterized by limited range of motion (ROM) due to adhesions within the glenohumeral joint capsule. The pathophysiology is suspected to involve an inflammatory reaction progressing to fibrotic contracture. Active fibroblastic proliferation can be found on histologic analysis, with some transformation to myofibroblasts, ultimately creating collagen in the form of a thick band. Clinically, adhesive capsulitis is classified into phases: freezing, frozen, and thawing. Risk factors include diabetes, hyperthyroidism, prior fractures, shoulder and cervical spine surgery, and

PURPOSE OF THE STUDY: Operations of hallux valgus deformity are very well known in orthopaedic surgery. The important part of these procedures is the release of soft tissue on the lateral side of the first metatarsophalangeal joint. Soft tissue procedures became routinely used in the 1920s and 1930s and several techniques of soft tissue release have been published in the past seventy years. We found several inaccuracies between the anatomy published and the routine clinical performance. The purpose of the study was to describe the anatomy of the first metatarsophalangeal joint's lateral part,

BACKGROUND: Volar dislocation of the metacarpophalangeal (MCP) joint of the thumb is an exceptionally rare injury due to the joint's inherent stability provided by the volar plate, collateral ligaments, and dorsal capsule. Chronic or neglected dislocations are even more uncommon and pose considerable surgical challenges because of fibrosis, soft tissue contracture, and adaptive bony changes.
CASE PRESENTATION: We report the case of a 12-year-old boy who presented with a nine-year history of progressive deformity and restricted motion of the left thumb following blunt trauma with a wooden stic

Non-Dupuytren's contracture, a cord-like structure formed because of trauma or surgery, rarely requires surgery. An 81-year-old woman underwent flexor tendon sheath release for right middle finger snapping and flexor tenolysis for postoperative complications. At the referral, a cord existed between the base of the middle finger and the mid-palm, and a 50° extension deficit of the metacarpophalangeal joint was noted. We performed a third operation. Limited fasciectomy could not achieve full extension; therefore, flexor tenolysis and joint mobilization were additionally performed intraoperativel

Our observations support the view of Millesi that camptodactyly is due to an imbalance between the flexor and extensor forces acting upon the proximal interphalangeal joint. In 21 consecutive operations the insertion of the lumbrical muscle was abnormal. The muscle inserted into either the superficialis tendon (in three cases), the capsule of the metacarpophalangeal joint (in 15), or the extensor expansion of the adjacent finger (in four). The fourth palmar interosseous muscle was examined in the last 10 patients and was abnormal in five. It was absent in two patients, small in one, and insert

HYPOTHESIS AND BACKGROUND: An elbow contracture in a young person can be a devastating problem. Significant contractures will lead to functional loss of the extremity. Appropriately performed contracture release can have profound implications on the overall well-being of the patient. The purpose of this study was to report improvements in sagittal-plane range of motion and the complication rate following an anterior elbow release for flexion contractures in patients 21 years or younger.
METHODS: We performed a retrospective review of 27 patients with a median age of 16.8 years who were treate

Congenital dislocation of the knee is a rare disorder. Late presentation of congenital dislocation of the knee at an older age is a therapeutic challenge. A 12-year-old girl presented to us with congenital dislocation of the knee and with complaints of limp, short limb, and pain on weight bearing. Two-stage surgery was performed with quadricepsplasty followed by gradual distraction in the first stage and repeat quadricepsplasty, anterior capsular release, and open reduction in the second stage. The result was fair to good with a stable and painless knee on walking.
