PURPOSE: To report our results of open reduction internal fixation with volar mini plate and screw fixation for unstable dorsal fracture dislocations (DFDs) of the proximal interphalangeal (PIP) joint.
METHODS: We performed a retrospective review of 13 consecutive DFDs of the PIP joint treated with volar mini plate and screw fixation, measuring both clinical and radiological outcomes.
RESULTS: The age range of our patients was 15 to 56 years (average, 33 y). Six injuries were related to work, 5 to sports, and 2 to motor vehicle accidents. Of the 13 DFDs, 6 were comminuted. Articular involvem

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2011eng
Acute acromioclavicular joint dislocations (ACDs) may be treated arthroscopically with flip buttons. This extra-articular fixation is easy to implant and is well tolerated. Between 2007 and 2009, 20 ACD patients (2 women and 18 men; mean age, 32 years) had surgery by the arthroscopic TightRope technique (Arthrex, Naples, FL). The main complication of this technique that has been reported is the partial loss of reduction at follow-up due to clavicular osteolysis under the superior flip button. We describe 4 cases with loss of reduction due to rupture of the sutures running across the buttons: 2

Four patients with proximal interphalangeal joint (PIP) dorsal dislocation without fracture had been treated at Chang Gung Memorial Branch (Kaohsiung and Keelung) Hospital from June 1987 to Jan. 1989. All four cases were open dislocations, only one patient received immediate diagnosis and treatment, while the other 3 cases were delayed diagnosis and treatment. Open reduction, repair of the ruptured ligaments and joint capsule, reinsertion of the disrupted volar plate were given in these 4 patients. The patient with immediate surgical treatment received rehabilitation program, the other three p

A 15-day-old female child is presented with multiple joint dislocations who died during the neonatal period. She had additional features of metaphyseal dysplasia, deficient calcification of vault of the skull, growth retardation, a natal tooth, lymphoedema and facial dysmorphism. This may constitute a new syndrome of multiple joint dislocations with metaphyseal dysplasia.

In a retrospective study, 31 patients were identified over a period of 25 years as having sustained traumatic knee dislocations without associated fractures. Three patients had early amputation secondary to vascular complications. Ten popliteal artery disruptions and five peroneal nerve palsies were diagnosed. At the time of follow-up evaluation (average, 40 months), 16 patients were examined. Operatively treated patients (n = 6) tended to have better motion (129 degrees) than nonoperatively treated patients (108 degrees). Varus instability was seen in only two patients treated nonoperatively.

In acetabular fractures, the size of a significant posterior wall fragment remains undefined as it affects joint stability. The purpose of this study was to quantitatively evaluate fragment size and hip stability in cadaveric specimens after serial osteotomies. Also, the role of the posterior capsule, in the various osteotomies, was evaluated for changes in hip stability. We found that fragments involving 25% or less of the acetabulum are insignificant, i.e., do not affect joint stability, while fragments involving 50% or more are significant. The significance of transitional fragments (25-50%

In case of persistent swelling and painful limitation of mobility, "distorsion" can be accepted as the definitive diagnosis, only if osseous and ligamentous injuries of the wrist and carpus have been ruled out with sufficient certainty. According to current knowledge multiform injuries of the carpus originate from a three dimensional impact of forces, which besides the well-known hyperextension and ulnar deviation of the wrist include intercarpal supination as a rotatory component. Depending on the effective forces and due to the biomechanical strength of the restraining ligaments (which chang

Type Tossy III dislocations of the acromioclavicular joint (AC) are treated surgically in German-speaking countries. There is considerable rotation and tilting in the AC joint with elevation or abduction of the shoulder joint over 90 degrees. Thus, in the majority of countless surgical procedures, the abduction and elevation are restricted or immobilized for long periods in abduction strapping or other types of movement restriction. The AC joint plate developed by Prof. Rahmanzadeh enables early functional, complete physical therapy since the plate permits rotation and tilting. Over a period o

A 45-year-old man presented with severe pinch-point crush injury to his left foot. Plain radiographs revealed dislocation of the first metatarsophalangeal joint and dorsolateral dislocation of the basal phalanx and sesamoids. The first tarsometatarsal joint was subluxed in the plantar direction and the second to fourth tarsometatarsal joints were subluxed dorsally. The sesamoids were displaced dorsolateral to the metatarsal head. There was a longitudinal tear of the joint capsule at the medial margin of the medial sesamoid, which was sutured together with the abductor hallucis tendon and colla

Background: The Rockwood classification is the most widely used radiographic framework for grading acromioclavicular (AC) joint dislocations. However, native anatomical variation may be associated with differences in the radiographic appearance of AC joint dislocations. Purpose: To evaluate the association between the acromioclavicular joint coronal orientation angle (AJCO) and Rockwood grade and to characterize the relationship between native coronal orientation and radiographic grade. Methods: This retrospective observational radiographic study included 198 adults with acute AC joint disloca

Radiocarpal dislocations are uncommon injuries that occur even more rarely in association with intercarpal dislocations. All the reported examples of such intercarpal dislocations are volar. We here describe a patient with a unique dorsal radiocarpal dislocation with an additional dorsal perilunar dislocation. Radiographic identification of this injury has important implications for its successful treatment.

Because of the anatomical configuration of the tarso-metatarsal joints with their strong ligamentous connections, pure dislocations in this region are rare. We describe our experience of 24 cases of Lisfranc dislocation. Classification is according to Wilson. In the long term, functional and radiological results appear to depend on the accuracy of reduction. For good anatomical results, immediate closed or, if necessary, open reduction and stabilization by percutaneous Kirschner wires is required. Because of the severity of soft tissue damage, decompression with dorsal skin incisions should al

Volar fracture-dislocations of the proximal interphalangeal joint are rare injuries caused by an axial force on a flexed digit resulting in an avulsion fracture of the dorsal lip of the middle phalanx with volar dislocation of the joint. This volar subtype is analogous to the more common dorsal subtype with a mirror image fracture on the dorsal lip of the middle phalanx. The main significance in this type of injury lies in the disruption of the extensor mechanism at the central slip. The goals of treatment, apart from restoring a congruent and stable joint, is to restore the extensor mechanism

Proximal interphalangeal (PIP) joint fracture dislocations are challenging injuries to treat. Multiple and varied treatments have been proposed. We present the use of cerclage wiring as a helpful technique in these challenging scenarios. The technique has the benefit of securing fracture fragments from the volar or dorsal base of middle phalanges or a comminuted fracture involving the entire articular surface. We report on the use of cerclage wires in eight patients (average: 43 years of age). Three patients had volar base fractures, three dorsal base fractures, and two impacted fractures invo

Diagnosis and treatment of injuries to the elbow joint place high demands on the treating physicians. The complex anatomy of the elbow joint enables a wide range of movement and complex functions in everyday and professional life. These must be restored in the event of injury.Clinical and imaging-based diagnostic procedures serve to classify typical injuries of this joint according to a large variety of different pathologies. These include fractures of the distal humerus, the proximal ulna, and the radial head, as well as dislocations and dislocation fractures. The following article gives an ov

OBJECTIVE: Recurrent dislocations of a total hip joint replacement are a difficult problem to address. We present a technique for reconstructing the ischiofemoral ligament of the hip to prevent recurrent posterior dislocations.
METHODS: This is a case series of 5 patients with a reconstruction performed using either a bone-patellar tendon-bone graft autograft or allograft.
RESULTS: At an average follow up of 47 months following the reconstruction, there were no redislocations.
CONCLUSION: Ischiofemoral hip ligament reconstruction is a viable technique in addressing recurrent dislocations of

PURPOSE: To evaluate the outcomes of extension block pinning used to treat unstable dorsal fracture dislocations of the proximal interphalangeal (PIP) joint. The factors affecting the functional outcome were analyzed.
METHODS: A series of 53 patients with 55 dorsal fracture dislocations of the PIP joint treated with closed reduction and extension block pinning were retrospectively reviewed. Additional percutaneous intramedullary fracture reduction (16 cases) or open fracture reduction (4 cases) had been performed. The radiological and clinical evaluations were included.
RESULTS: At a mean fo
