Custom design of a temporomandibular joint prosthesis: A kinematic approach evaluated by finite element analysis.

STATEMENT OF PROBLEM: Designing temporomandibular joint (TMJ) prostheses that accurately replicate natural kinematic movement and stress distribution remains a challenge because standardized methods for determining the prosthetic kinematic center are lacking. Current designs often rely on empirical placements without considering individualized kinematic parameters, which can compromise functionality and longevity. PURPOSE: The purpose of this study was to establish an optimal prosthetic kinematic center location for a custom TMJ prosthesis based on invariant TMJ anatomic landmarks to improve

Prediction of time to prosthesis implantation as a function of joint anatomy in patients with developmental dysplasia of the hip.

BACKGROUND: Developmental dysplasia of the hip (DDH) can lead to pain and premature secondary osteoarthritis at an early stage. Joint-preserving osteotomy is an established solution to this problem. In contrast, a conservative approach would result in pain persistence, ultimately raising the patients question for a possible date of expected prosthesis implantation. The aim of the study was to identify the relationship between the dysplastic hip anatomy and the time of prosthesis implantation in order to enable prognostic predictions in younger patients with symptomatic DDH. MATERIALS AND METH

Early infection of hip joint prosthesis by Clostridium difficile in an HIV-1 infected patient.

Anaerobes are less frequently described as causative pathogen of prosthetic joint infection (PJI). We report the first case of early PJI after hip arthroplasty due to Clostridium difficile in a diabetic and HIV-1 infected patient with bacteremia. Our patient was successfully treated through surgical debridement and prosthesis retention combined with targeted antibiotic therapy.

Management of chronic unilateral temporomandibular joint dislocation with a mandibular guidance prosthesis: a clinical report.

Recurrent or chronic dislocation of the temporomandibular joint is relatively rare and often results in facial asymmetry, impairment of function, and discomfort. Although manual reduction is the primary choice of treatment, patients presenting with recurrent or prolonged dislocations require conservative and surgical methods to limit forward movement of the mandibular condyle. This clinical report presents a 75-year-old woman with severe mandibular deviation and subsequent facial asymmetry caused by a chronic unilateral temporomandibular joint dislocation that was treated with a mandibular gui

[Results after arthroplasty of the metacarpophalangeal joints with uncemented, unconstrained HM prosthesis in rheumatoid patients].

PURPOSE: In 80 % of patients with rheumatoid arthritis, the metacarpophalangeal (MP) joints are involved with increasing destruction and loss of function. Silicone arthroplasty of the MP joints leads to a limited range of motion, an increase in osteolysis and fractures of the implants. The cementless, unconstrained design of the HM prosthesis is a new concept for replacing the MP joints of rheumatoid patients. Short-term and midterm results are shown. METHOD AND MATERIAL: In a prospective study, 38 HM prostheses were implanted, 28 in patients with rheumatoid arthritis, four with osteoarthrosi

Outcome of penicillin-susceptible streptococcal prosthetic joint infection treated with debridement and retention of the prosthesis.

Debridement with retention of the prosthesis was the initial treatment modality for 19 cases of penicillin-susceptible streptococcal prosthetic joint infection that occurred in 18 patients who presented to the Mayo Clinic (Rochester, Minnesota) during 1969-1998. All of the cases of prosthetic joint infection occurred >30 days after implantation of the prosthesis, which was well fixed at the time of debridement. The median duration of symptoms before debridement was 4 days (range, 1-10 days). Treatment failure (defined as relapse of infection with the original microorganism) occurred in 2 cases

Targeting regulation of stem cell exosomes: Exploring novel strategies for aseptic loosening of joint prosthesis.

Periprosthetic osteolysis is a major long-term complication of total joint replacement. A series of biological reactions caused by the interaction of wear particles at the prosthesis bone interface and surrounding bone tissue cells after artificial joint replacement are vital reasons for aseptic loosening. Disorder of bone metabolism and aseptic inflammation induced by wear particles are involved in the occurrence and development of aseptic loosening of the prosthesis. Promoting osteogenesis and angiogenesis and mediating osteoclasts and inflammation may be beneficial in preventing the aseptic

[Long-term results after the surgical treatment of saddle joint arthrosis with Swanson's silastic prosthesis].

The use of a silastic implant in resection arthroplasty of the carpometacarpal joint of the thumb allows the joint space to be preserved. With the Swanson type arthroplasty relief of pain, good function, as well as stability and improved pinch force can be obtained, avoiding the consequence of carpal instability by narrowing of the joint space associated with resection of the trapezium alone or in combination with tendon interposition arthroplasty. The two main problems associated with Swanson arthroplasty are prosthetic dislocation and longterm stability of the prosthesis itself. The joint su

Continuous Hyaluronic Acid Supply by a UHMWPE/PEEK Interlocking Scaffold for Metatarsophalangeal Joint Prosthesis Lubricating Applications.

Wear is one of the main causes of prothesis failure in hemiarthroplasty of small joints such as the metatarsophalangeal joint, to which lubrication improvement is a promising solution. Inspired by the natural joint structure and lubrication mechanisms, we developed a novel composite strategy for metatarsophalangeal joint hemiarthroplasty. An ultrahigh-molecular-weight polyethylene (UHMWPE) lubrication layer is interlocked within a 3D-printed poly(ether ether ketone) (PEEK) scaffold, and hyaluronic acid (HA) is introduced for hydrophilic modification of the UHMWPE matrix. HA encapsulated in the
