What is the analgesic ceiling dose of ketorolac for treating acute pain in the ED?

Recent research has suggested that ketorolac has an analgesic ceiling effect, meaning that despite increased dosages, the patient obtains no additional pain relief and is more likely to suffer adverse drug reactions. This article describes the outcomes of these studies and the recommendation to use the lowest possible dose for the shortest time when treating patients with acute pain.

Ketorolac use and risk of bleeding after appendectomy in children with perforated appendicitis.

BACKGROUND: Ketorolac is an opioid sparing agent commonly used in children. However, ketorolac may be avoided in children with peritonitis owing to a possible increased risk of bleeding. METHODS: A retrospective cohort study of healthy children 2-18 years who underwent appendectomy for perforated appendicitis was performed using the Pediatric Health Information System (2009-2019). Multivariable logistic regression was used to evaluate the association between perioperative ketorolac use and postoperative blood transfusions within 30 days of surgery, adjusting for patient and hospital level fac

Antinociceptive Interaction and Pharmacokinetics of the Combination Treatments of Methyleugenol Plus Diclofenac or Ketorolac.

Although nonsteroidal anti-inflammatory drugs (NSAIDs) are one of the main types of drugs used to treat pain, they have several adverse effects, and such effects can be reduced by combining two analgesic drugs. The aim of this study was to evaluate the nociceptive activity of methyleugenol combined with either diclofenac or ketorolac, and determine certain parameters of pharmacokinetics. For the isobolographic analysis, the experimental effective dose 30 (ED30) was calculated for the drugs applied individually. With these effective doses, the peak plasma concentration (Cmax) was found and the

Postoperative Ketorolac Administration Is Not Associated with Hemorrhage in Cranial Vault Remodeling for Craniosynostosis.

UNLABELLED: Nonsteroidal anti-inflammatory drugs have been used as part of multimodal postoperative analgesic regimens to reduce the necessity of opioids. However, due to its effect on platelet function, there is a hesitation to utilize ketorolac postoperatively. The goal of this study is to analyze our experience utilizing ketorolac in patients who underwent major cranial vault remodeling (CVR) for craniosynostosis with an emphasis on postoperative hemorrhage and complications. METHODS: A retrospective review was performed for all patients undergoing CVR for craniosynostosis from 2013 to 201

Ketorolac Use and Anastomotic Leak in Elective Colorectal Surgery: A Detailed Analysis.

BACKGROUND: Recent population-level analyses have linked ketorolac use to adverse outcomes. However, its use is also associated with decreased opioids and faster return of bowel function. OBJECTIVE: This study aims to assess the association between ketorolac and anastomotic leak. We hypothesize that receiving at least 1 dose of ketorolac will not be associated with anastomotic leak in elective colorectal surgery. DESIGN: This is a retrospective, observational cohort study of a prospectively collected data base. Anastomotic leak rates and other patient outcomes were adjusted for patient-level

Prevention of hearing loss in experimental pneumococcal meningitis by administration of dexamethasone and ketorolac.

Pneumococcal meningitis remains a significant cause of morbidity, particularly sensorineural hearing loss. Recent literature has suggested that a vigorous host immune response to Streptococcus [corrected] pneumoniae is responsible for much of the neurologic sequelae, including deafness, after bacterial meningitis. This study used a rabbit model of hearing loss in experimental pneumococcal meningitis to evaluate the therapeutic effect of two anti-inflammatory agents, dexamethasone and ketorolac, coadministered with ampicillin. Both adjunctive drugs minimized or prevented sensorineural hearing l

Analgesic effects of amlodipine and its interaction with morphine and ketorolac-induced analgesia.

1. The antinociceptive effects of amlodipine, administered subcutaneously (s.c.), intracerebroventricularly (i.c.v.) and intrathecally (i.t.) were examined with the acetic acid writhing and tail-flick tests in mice. Amlodipine was also tested in combination with morphine and ketorolac. Isobolographic analyses were used to define the nature of functional interactions between amlodipine and morphine or ketorolac. 2. The s.c. (0.1, 1.25, 2.5, 5 and 10 mg/kg), i.c.v. Comparison of analgesic effect of locally and systemically administered ketorolac in mastectomy patients.

BACKGROUND: Ketorolac is a parenteral nonsteroidal antiinflammatory drug (NSAID). Two features have limited its clinical utility: tendency to elicit kidney failure and inability to produce complete analgesia. Because most NSAIDs are weak acids (pKa 3-5) and become concentrated in acidic tissues, such as injured and inflamed tissues, we hypothesized that local administration may enhance its analgesic efficacy while lowering the potential for systemic complications. METHODS: We conducted a randomized, placebo-controlled study of 60 group I-II (American Society of Anesthesiology criteria) mastec

Postoperative analgesia for outpatient arthroscopic knee sugery with intraarticular bupivacaine and ketorolac.

Intraarticular (IA) local anesthetics are often used for the management and prevention of pain after arthroscopic knee surgery. Systemic ketorolac is also useful in the management of these patients. Ketorolac, a nonsteroidal antiinflammatory drug (NSAID), alters the sensitivity of peripheral nociceptors by reducing the local concentration of allogenic chemicals which are activated by peripheral tissue injury. It is interesting to speculate that placing a NSAID at the site of injury might result in more profound pain relief. However, IA ketorolac has not been evaluated in arthroscopic patients.

Effects of ketorolac on postoperative analgesia and ventilatory function after laparoscopic cholecystectomy.

Ketorolac, a nonsteroidal anti-inflammatory drug, is alleged to produce postoperative analgesia without opioid-related side effects. Patients undergoing laparoscopic cholecystectomy were assigned randomly to receive either ketorolac or a placebo (saline) according to a double-blind protocol. Preoperative (baseline) pulmonary function was evaluated using a Respiradyne II monitor. Patients received midazolam, 2 mg, and 2 mL of either ketorolac, 60 mg (n = 31), or saline (n = 29), 20-40 min before surgery. Anesthesia consisted of thiopental, 4-5 mg/kg, and vecuronium, 0.1 mg/kg, for induction, an

Ketorolac as a premedicant for coronary artery bypass surgery patients with normal ventricles.

Premedication of patients requiring cardiac surgery should provide adequate analgesia, sedation and anxiolysis for the stress and pain associated with preoperative preparation and placement of monitoring catheters. Ideally, these effects would be achieved without producing respiratory depression and hypoxia, which could be life-threatening to patients at risk for myocardial ischemia. Ketorolac, a nonsteroidal, antiinflammatory agent, has previously been shown to provide postoperative pain relief comparable to that provided by morphine, without respiratory depression. This study compared the in

Hemodynamic effects of two intravenous doses of ketorolac tromethamine compared with morphine.

The cardiovascular effects of intravenous administration of ketorolac tromethamine 10 and 90 mg were measured and compared with those of morphine sulfate 10 mg in 18 anesthetized patients after major vascular surgery. Morphine caused a significant reduction in mean arterial pressure, left cardiac work index, and left ventricular stroke work index. Stroke volume index, cardiac output, and pulmonary artery pressures decreased to a lesser extent. Ketorolac at either dose was not associated with any significant change in cardiac and hemodynamic parameters. The data suggest that even high doses of

Sonochemical Formation of Copper/Iron-Modified Graphene Oxide Nanocomposites for Ketorolac Delivery.

A feasible sonochemical approach is described for the preparation of copper/iron-modified graphene oxide nanocomposites through ultrasonication (20 kHz, 18 W cm-2 ) of an aqueous solution containing copper and iron ion precursors. Unique copper-, copper/iron- and iron-modified graphene oxide nanocomposites have a submicron size that is smaller than that of pristine GO and a higher surface area enriched with Cu2 O, CuO, and Fe2 O3 of multiform phases (α-, β-, ϵ-, or γ), FeO(OH), and sulfur- or carbon-containing compounds. These nanocomposites are sonochemically intercalated with the nonsteroida

Enantiomer-specific ketorolac pharmacokinetics in young women, including pregnancy and postpartum period.

Racemic ketorolac clearance (CL) is significantly higher at delivery, but S-ketorolac disposition determines the analgesic effects. The aim of this study was to investigate the effect of pregnancy and postpartum period on enantiomer-specific (S and R) intravenous (IV) ketorolac pharmacokinetics (PKs). Data in women shortly following cesarean delivery (n=39) were pooled with data in a subgroup of these women that was reevaluated in the later postpartum period (postpartum group, n=8/39) and with eight healthy female volunteers. All women received single IV bolus of 30 mg ketorolac tromethamine.
