Forty-two tibial and 5 fibular stress fractures in 34 patients with knee arthritis were radiologically classified into intraarticular malunited and ununited fractures, and extraarticular impending, acute, united, malunited, and ununited fractures. Depending on fracture type, total knee arthroplasty was performed using long-stem tibial component, metal augments, corrective osteotomy, or in routine fashion. At a mean follow-up of 36 months, the mean Knee Society knee score improved from 36.7 points preoperatively to 90.3 points; function score improved from 24 to 86.2 points. All fractures had u

Fifty patients with isolated open tibial shaft fractures were reviewed to determine the incidence and type of knee ligament injuries sustained. Eighteen patients (36 per cent) had at least one ligament injury in the ipsilateral knee; eight had multiple ligament injuries. Only four patients (22 per cent) were diagnosed as having a ligament injury at the time of initial management and the remaining 14 patients were diagnosed at the time of review for this study. There is a high incidence of ipsilateral knee ligament injuries in open tibial shaft fractures; the knee should be thoroughly examined

Epiphysiolysis and epiphyseal fractures of the distal femur and proximal tibia are an extremely rare entity, but due to their far-reaching consequences with associated functional restrictions of the knee joint, they must be recognized and treated thoroughly. Complete and correct diagnosis is essential and, diagnostically speaking and in addition to standard x‑rays in two planes, the threshold for cross-sectional imaging examination techniques should be low. A conservative treatment attempt is possible for undisplaced fractures, but surgical retention and stabilization using wires and screws is

Fractures around the knee joint are per se challenging to treat, often regardless of the patient's age. Osteosynthesis remains the gold standard for fracture management; however, geriatric patients are often unable to comply with postoperative weight-bearing restrictions on the osteosynthetically treated limb. This can result in a prolonged hospital stay and particularly in aged patients to an increased risk of internal complications. Secondary total knee arthroplasty (TKA) is associated with both higher complication and revision rates compared to primary TKA for acute fractures. The advantage

CLINICAL/METHODICAL ISSUE: Subchondral insufficiency fractures (SIF) occur without adequate trauma to the weight-bearing joint surfaces due to repeated physiological loading and occur predominantly in older people.
STANDARD RADIOLOGICAL METHODS: Magnetic resonance imaging (MRI). Radiographs.
METHODOLOGICAL INNOVATIONS: MRI is the best method for the detection, follow-up, and assessment of the prognosis of subchondral insufficiency fractures.
PERFORMANCE: The typical MRI findings of SIF are bone marrow edema, a subchondral hypointense fracture line, and possibly a subchondral hypointense are

Pathologic fractures secondary to metastatic disease are an increasingly prevalent problem. Such patients require multidisciplinary collaboration to optimize clinical outcomes. An established algorithm for clinical, laboratory, and radiographic work-up will ensure that each patient achieves the best outcome while avoiding catastrophic complications. Metastatic disease to the region of the knee is less commonly encountered than in other regions of the body, but it presents unique difficulties that merit discussion. Part one of this two-part article series will discuss the appropriate work-up of

BACKGROUND: One of the rare complications of primary total knee arthroplasty is intra-operative fracture. Intra-operative fracture during revision knee arthroplasty has been well-documented but there is limited literature on fractures occurring during primary knee arthroplasty. We conducted a systematic review of the literature to compare and contrast the various studies to clearly define the predisposing factors, incidence, and characteristics of the fracture itself and to arrive at a consensus on the management and prevention of intra-operative fractures during primary knee arthroplasty.

BACKGROUND: Calcaneal insufficiency fracture (IF) following total knee arthroplasty (TKA) is a rare disorder. This study aimed to examine the prevalence and clinical findings of calcaneal IF following TKA.
METHODS: We retrospectively reviewed 3,585 consecutive patients undergoing primary TKA between 2012 and 2017 in four hospitals. Calcaneal IF following TKA was diagnosed by plain radiography or magnetic resonance imaging. First, we investigated the prevalence and clinical findings of calcaneal IF following TKA. Second, we classified calcaneal IF into three types based on its location: type 1

Tibial shaft fractures distal to total knee arthroplasty are rare, but they are likely to become more common with the increasing number of arthroplasty procedures being performed. These fracture patterns have been treated in the past either with closed reduction and casting/bracing or with open reduction internal fixation using plates. Weight-bearing precautions in the elderly patient population can affect patient disposition, and weight bearing on extramedullary fixation can lead to early hardware failure. We present a series of nailing techniques that can be used for tibial fractures distal

Retrograde nailing is an attractive method for stabilisation of femoral shaft fractures in cases of polytrauma, ipsilateral pelvic, acetabular, tibial and femoral neck fractures, bilateral femoral fractures, obese and pregnant patients. However, retrograde nailing may result in complaints about the knee. The purpose of this study was to evaluate the incidence and risk factors of lasting knee pain, and its relation to knee function deficits. We conducted a retrospective study of 75 patients in whom 82 femoral shaft fractures were treated with retrograde intramedullary nailing. Complaints of pai

Tibial periprosthetic fractures (PPF) following total knee arthroplasty (TKA) are uncommon but clinically important complications with rising incidence parallel to the growing volume of arthroplasty procedures. Although the overall 10-year cumulative rate of PPF after primary TKA is approximately 1.7%, tibial fractures account for a small proportion yet are associated with substantial morbidity, reoperation, and infection risk. These injuries occur more frequently in revision TKA, females, and individuals who have osteoporosis or compromised bone stock. Additional risk factors include nonosteo

AIMS: Total knee arthroplasty (TKA) allows immediate weightbearing and avoids challenges associated with secondary arthroplasty following conservative management or failed fixation of tibial plateau fractures (TPFs). Metaphyseal sleeves may overcome the limitations of conventional implants by addressing issues of deficient bone stock and inadequate proximal fixation in the fracture zone.
METHODS: We conducted a retrospective case series of patients undergoing TKA with metaphyseal sleeves for acute TPFs at a single tertiary centre between January 2019 and June 2025. Demographic details, injury

Metastatic disease affecting the proximal tibia is rarer than disease affecting the femur; however, it presents unique challenges that the orthopaedic surgeon must address to ensure successful outcomes and return the patient to an ambulatory status. The essential workup for evaluating and treating these patients is addressed briefly in this review as a summary of the information is provided in part 1 of this series. Part 2 of this series will focus on the surgical treatment of these lesions, which can be complex and is not as well described in the literature. Procedures ranging from open reduc

BACKGROUND: Periprosthetic fractures (PPF) following total knee (TKA) and hip arthroplasty (THA) have become more common over the years. The aim of the present study was to assess morbidity and mortality following surgery for PPF of hip and knee.
PATIENTS AND METHODS: Altogether, 124 patients (mean age: 77 years; 77.4% female) with PPF of the hip (n=97) and knee (n=27), treated between 2005 and 2017 at a level-1 trauma centre, were retrospectively included. In order to assess risk factors for postoperative morbidity, Fine and Gray's model was used to compensate for death as the competing even

Intra-operative fractures in knee revision surgery are relatively rare and have not been well studied. They may occur during joint exposition, removal of the prosthesis or cement, or implantation of trial or original components. The fractures affect both the metaphyseal area and diaphysis of the tibia and femur. Tibial fractures are slightly more common than femur fractures. On the femur, the medial condyle is most frequently affected, followed by the femur diaphysis. The use of non-cemented stems is associated with a greater risk of intra-operative diaphyseal fractures than that of cemented s

BACKGROUND: The incidence of periprosthetic fractures after total joint arthroplasty (TJA) is rising due to an increasing number of TJAs performed annually and the growing elderly population. In many elderly patients with periprosthetic fractures, the bone strength is lowered due to the deterioration of bone quality and a decrease in bone quantity; rigid fixation of the fracture is difficult. It is a challenging operation for orthopedic surgeons. The usefulness of circular external fixation for periprosthetic fractures has been reported in several case studies. The aim of this study was to inv

BACKGROUND: The ideal thromboprophylaxis regime following lower limb arthroplasty and proximal femur fractures remains controversial. Guidelines disagree on the type of chemical prophylaxis, its dose or duration. This article describes a method of monitoring venous thromboembolism (VTE) rates following Total Hip (THA), Total Knee Arthroplasty (TKA) and surgery for hip fractures (NOF#).
METHODS: Over 3 years, all patients investigated for VTE were analysed using Picture Archiving Communications System (PACS). All positive scans were then cross-referenced using PACS and local registry data to s
