Non-surgical management of overuse knee injuries in runners.

Seventy-three injured runners (45 men, 28 women) who presented with 83 running-induced overuse knee injuries were managed conservatively. The regimens utilized focused on quadriceps and hamstring muscle retraining and the control of functional overpronation. Patello-femoral pain syndrome (42 cases) was by far the most common problem. The next most common problems were iliotibial band friction syndrome (10), patellar tendonitis (9) and popliteal tendonitis (5). The most frequent etiological factor was a sudden increase in training mileage. Following the initial visit and at regular intervals th

Rating systems in the evaluation of knee ligament injuries.

Many different methods of evaluating disability after knee ligament injury exist. Most of them differ in design. Some are based on only patients' symptoms. Other include patients' symptoms, activity grading, performance in a test, and clinical findings. The rating in these evaluating systems can be either numerical, as in a score, or binary, with yes/no answers. Comparison between a symptom-related score and a score of more complex design showed that the symptom-related score gave a more differentiated picture of the disability. It was also shown that the binary rating system gave less detaile

Occult knee ligament injuries associated with femoral shaft fractures.

Recognition of ligamentous injuries of the knee in the acute phase is essential for optimum treatment and prognosis. A multitraumatized patient with a fractured femur and occult knee ligament damage presents a difficult diagnostic challenge. This paper retrospectively reviews 52 patients with 54 midshaft femoral fractures. The high incidence (48%) of ipsilateral knee ligament damage, particularly of a severe nature (30%) evident at followup (mean 24.5 months) is alarming. In the 26 cases of knee ligament damage, the mean time from injury to documentation of instability is 12.8 months. Motor ve

Acute knee injury. How to determine if the knee is stable.

Knee injuries are common in patients of all ages. A detailed history concerning the mechanism of injury and symptoms is helpful in establishing a tentative diagnosis, and the physical examination can easily provide confirmation. X-ray films occasionally are helpful, but stress views and arthrograms should not be relied on for definitive diagnosis. Arthroscopy may be required for definitive evaluation of internal derangement of the knee and can be used therapeutically in meniscal injuries and other problems. Proper rehabilitation is mandatory for a good outcome; therefore, physical therapy is e

Multiple Ligament Knee Injuries: Does the Knee Dislocation Classification Predict the Type of Surgical Management?

This study aims to evaluate relationships among multiple ligament knee injury (MLKI) patterns as classified according to the knee dislocation (KD) classification and the types of surgical management pursued. We hypothesized that the KD classification would not be predictive of the types of surgical management, and that categorizing injuries according to additional injury features such as structure, chronicity, grade, and topographic location would be predictive of the types of surgical management. This is a Retrospective cohort study. This study was conducted at a level I trauma center with a

[Overuse injury syndromes of the knee].

Overuse injuries are frequent in the knee joint. The reason for this is that the knee joint is engaged in all sports activities. Furthermore, the joint area has numerous attachment points for muscles and tendons and numerous bursae. Another reason is that the specific joint between the patella and femur (patellofemoral joint) constitutes a part of the knee joint. Speaking in general terms, all overuse injuries in the knee joint can be divided in four groups according to the aspect: anterior aspect--patellofemoral pain syndrome, patellar tendinitis (jumper's knee), Osgood-Schlatter disease, Sin

Rehabilitation following knee injury.

Over the past decade, there has been a resurgence of interest in techniques of rehabilitation of the injured knee. Technologic advances have provided instruments that yield objective measurements so that appropriate staging can be done as part of the rehabilitation plan. Research in multidisciplinary fields has provided data that currently allow a rational physiologic approach to progressive care of the injured lower extremity. No longer is the adage "work it out and let's see how it goes" appropriate care for the injured knee. Starting with the proper diagnosis, rehabilitation begins at the t

MRI of knee joint injury.

Most knee injuries are the result of rotational, varus, valgus, hyperextension, or translational forces or a combination of these. The pattern of abnormalities seen on magnetic resonance imaging depends on the mechanism of trauma and the position of the joint at the time of injury. Knee injuries can be classified under two broad categories of direct and indirect injuries.

Simultaneous bilateral multiligamentous knee injuries are associated with more severe multisystem trauma compared to unilateral injuries.

PURPOSE: To compare the knee ligamentous injury patterns and associated multisystem trauma in patients who have sustained simultaneous bilateral knee multiligamentous injuries (MLI) to patients with unilateral MLIs. METHODS: Patients with simultaneous bilateral and unilateral knee MLIs after motor vehicle accidents were identified from 2007 to 2014 at a single institution. Chart and radiographic reviews were performed to identify patient demographics, extremity fractures and associated head, thoracic, abdominal and spine injuries. The MLIs were characterized by ligamentous injury pattern and

Disparities in Evaluation, Treatment, and Outcomes of Pediatric Knee Injuries.

Similar to other fields, there is growing evidence of disparities in pediatric sports medicine. For children and adolescents with knee injuries, disparities persist along the lines of sex, gender, race, ethnicity, insurance, socioeconomic status, and other determinants. These factors influence the evaluation, treatment, and outcomes for a variety of knee pathologies, including anterior cruciate ligament ruptures, meniscus tears, osteochondritis dissecans, cartilage injuries, and tibial spine fractures. Inequities in care may have long-term functional and psychosocial implications. Additional r

Tissue Augmentation Techniques in the Management of Ligamentous Knee Injuries.

Despite early reports of high failure rates in knee ligament repair techniques resulting in favor of reconstruction, newer advances in surgical technology have shifted the attention back to repair with the addition of various tissue augmentation techniques. Ligament repair preserves proprioceptors in the native ligament and avoids autograft tendon harvest, minimizing the complications associated with donor site ruptures in reconstruction techniques. Tissue augmentation has been successfully used in knee ligamentous and tendon repair procedures, as well as in some upper extremity procedures. Th

MR Imaging of Acute Knee Injuries: Systematic Evaluation and Reporting.

Acute knee injury ranges among the most common joint injuries in professional and recreational athletes. Radiographs can detect joint effusion, fractures, deformities, and malalignment; however, MR imaging is most accurate for radiographically occult fractures, chondral injury, and soft tissue injuries. Using a structured checklist approach for systematic MR imaging evaluation and reporting, this article reviews the MR imaging appearances of the spectrum of traumatic knee injuries, including osteochondral injuries, cruciate ligament tears, meniscus tears and ramp lesions, anterolateral complex
