Prolonged second stage of labor and risk of adverse maternal and perinatal outcomes: a systematic review.

BACKGROUND: Safe and effective management of the second stage of labor presents a clinical challenge for laboring women and practitioners of obstetric care. This systematic review was conducted to evaluate evidence for the influence of prolonged second stage of labor on the risk of selected adverse maternal and neonatal outcomes. METHODS: Articles were searched using PubMed, Cochrane Library, and CINAHL from 1980 until 2005. Studies were included according to 3 criteria: if they reported duration of the second stage of labor, if they reported maternal and/or neonatal outcomes in relation to p

Effect of Postpartum Outcomes in Mother's Upright Position During the Second Stage of Labor: Systematic Review.

PURPOSE: The purpose of this study was to determine whether upright position is effective in labor through systematic review in randomized controlled trials. METHODS: We established the PICO (Patient-Intervention-Comparator-Outcome) strategy, and reviewed 282 literatures from national and international electronic databases, and finally selected 9 references based on inclusion and exclusion criteria. We evaluated the quality of references and carried out a meta-analysis. RESULTS: The maternal outcomes showed that the duration of their second-stage labor was 2.29 minutes shorter than that of t

Why do supportive birth attendants become directive of maternal bearing-down efforts in second-stage labor?

A supportive approach to care for women during the second stage of labor that primarily relies on the laboring woman's involuntary expulsive urges has been advocated. We aimed to learn about the clinical circumstances surrounding the caregiver shift from being primarily supportive to directing women regarding their bearing-down efforts. This research analyzed the communications of 10 birth attendants and women during the expulsive phase of labor using videotapes recorded from two studies carried out between 1986 and the present. The occasions when a birth attendant shifted verbalizations were

Supine position compared to other positions during the second stage of labor: a meta-analytic review.

The routine use of the supine position during the second stage of labor can be considered to be an intervention in the natural course of labor. This study aimed to establish whether the continuation of this intervention is justified. Nine randomized controlled trials and one cohort study were included. A meta-analysis indicated a higher rate of instrumental deliveries and episiotomies in the supine position. A lower estimated blood loss and lower rate of postpartum hemorrhage were found in the supine position, however it is not clear whether this is a real or only an observed difference. Heter

Active pushing versus passive fetal descent in the second stage of labor: a randomized controlled trial.

OBJECTIVE: To compare perinatal outcomes among women with epidural anesthesia who were encouraged to push at complete dilatation with those who had a period of rest before pushing began. METHODS: After a power analysis to determine appropriate sample size (based upon an alpha error rate of.05% and 80% power), a prospective randomized trial of 252 women with epidural anesthesia was conducted. Patients were randomized to a rest period or immediate pushing at complete dilatation. Variables measured included rate of fetal descent, length of time of pushing, the number and type of fetal heart rate

Warm perineal compresses during the second stage of labor for reducing perineal trauma: A meta-analysis.

OBJECTIVE: Perineal trauma may have a negative impact on women's lives as it has been associated with perineal pain, urinary incontinence and sexual dysfunction. The aim of this systematic review and meta-analysis of randomized controlled trials was to evaluate the effectiveness of warm compresses during the second stage of labor in reducing perineal trauma. METHODS: Electronic databases were searched from inception of each database to May 2019. Inclusion criteria were randomized trials comparing warm compresses (i.e. intervention group) with no warm compresses (i.e. control group) during the

Midwives' Perspectives on a Program Supporting Spontaneous Pushing During the Second Stage of Labor: A Qualitative Study.

BACKGROUND: Maternal pushing during the second stage of labor plays a critical role to labor progression, with professional guidelines recommend supporting a natural, "spontaneous" approach to pushing. Midwives are key to facilitating this practice; however, there is limited of evidence regarding their perceptions of spontaneous pushing. This study was designed to explore midwives' perceptions of a program that supported a woman's spontaneous pushing during labor. METHOD: This study employed a descriptive qualitative approach using individual semi-structured interviews. Data were analyzed usi

The Impact of Prolonged Deceleration During the Second Stage of Labor on Umbilical Artery Blood Gas Analysis.

INTRODUCTION: Wrist-worn accelerometers can capture stepping behavior passively, continuously, and remotely. Methods utilizing peak detection, threshold crossing, and frequency analysis have been used to detect steps from wrist-worn accelerometer data, but it remains unclear how different approaches perform across a range of walking speeds and free-living activities. In this study, we evaluated the performance of four open-source methods for deriving step counts from wrist-worn accelerometry data, when applied to data from a range of structured locomotion and free-living activities. In additio

Duration of the second and third stages of labor and risk of postpartum hemorrhage: a cohort study stratified by parity.

BACKGROUND: Postpartum hemorrhage (PPH) remains a leading cause of maternal morbidity and mortality worldwide. It is therefore important to improve our understanding of the risk factors for PPH according to parity, in particular, those linked to modifiable obstetric practices. The aim of this study was to assess the risk of PPH by the duration of the second and third stages of labor, stratified by parity. METHODS: This study was based on secondary analysis of data from participants in a randomized controlled trial. A sample of women from three university hospitals in France aged at least 18 y

Management of the active second stage of labor in waterbirths compared with conventional births - a prospective cohort study.

BACKGROUND: The management of the active second stage in labor and perineal protection varies between countries and is rarely described regarding waterbirths. The objective of this study was to describe how midwives manage the active second stage of labor in waterbirths compared to conventional births. A secondary aim was to compare clinical outcomes between the two groups. METHODS: A prospective cohort study, based on 323 women who gave birth at three clinics in Sweden, between Dec 2015-May 2019. The women were both primiparous and multiparous; 153 gave birth in water and 170 had a conventio

Is there a correlation between pelvic floor dysfunction symptoms during pregnancy and the duration of the second stage of labor?

BACKGROUND: Pelvic floor dysfunction (PFD) most commonly results from weakened or injured muscles and ligaments whose purpose is to support the pelvic floor. Many studies have placed vaginal delivery and prolonged second stage of labor (SSL) as major risk factors for PFD, supposedly through generating enhanced pressure in the pelvic area. Although many studies describe the effects of vaginal delivery and labor on structure and function of the pelvic floor, not much is known regarding PFD deriving from pregnancy and its prevalence and severity in the postpartum. We aimed to evaluate whether a c

Maternal and neonatal outcomes following a prolonged second stage of labor: A meta-analysis of observational studies.

Several articles investigated the impact of prolonged second stage of labor on maternal and neonatal outcomes; however, strict consensus is still lacking. The purpose of the present meta-analysis is to investigate risk factors that contribute to the pathophysiology of prolonged labor as well as effect sizes of maternal and neonatal morbidity. We searched Medline, Scopus, Clinicaltrials.gov, EMBASE, Cochrane Central Register of Controlled Trials CENTRAL and Google Scholar database. Observational studies (prospective and retrospective) were considered eligible for inclusion in the present meta-a

The impact of extending the second stage of labor to prevent primary cesarean delivery on maternal and neonatal outcomes.

BACKGROUND: A low rate of primary cesarean delivery is expected to reduce some of the major complications that are associated with a repeat cesarean delivery, such as uterine rupture, adhesive placental disorders, hysterectomy, and even maternal death. Since 2014, and in alignment with the American College of Obstetricians and Gynecologists and the Society for Maternal-Fetal Medicine, we changed our approach to labor dystocia, defined as abnormal progression of labor, by allowing a longer duration of the second stage of labor. OBJECTIVE: To examine the effect of prolonging the second stage of

Effect of the birth chair on duration of second stage labor, fetal outcome, and maternal blood loss.

The effect of delivering in a birth chair on duration of second stage labor, fetal outcome, and maternal blood loss was examined in a retrospective study. The sample consisted of 60 primiparous women, 37 to 41 weeks gestation with a normal pregnancy and labor, 30 delivering on a traditional delivery table and 30 delivering in a birth chair. Comparisons were made between groups for mean duration of second stage labor, mean Apgar scores at one and five minutes, and mean maternal hemoglobin and hematocrit values during the pre- and post-partum periods. No significant difference was found between

What changes in the fetal heart rate are associated with neonatal acidosis during the second stage of labor?

OBJECTIVE: To describe fetal heart rate (FHR) patterns associated with neonatal acidosis during the second stage of labor. METHOD: This single-center (Lille, France) retrospective cohort study was conducted between September 2022 and January 2023. Fetal heart rate (FHR) during maternal expulsive efforts was interpreted for all low-risk women who delivered vaginally after 37 weeks' gestation. Low risk was defined by the absence of any maternal history, obstetric complications, and a normal fetal heart rate in the hour before the onset of expulsive efforts. A baseline increase was defined as a

Painless esophageal rupture in the second stage of labor presenting as surgical emphysema of the neck: a case report.

BACKGROUND: Pneumomediastinum in pregnancy has been described in around 200 cases, with an incidence of 1:100,000 pregnancies. The etiology is presumed to be either alveolar barotrauma secondary to the Valsalva maneuver (Hamman's syndrome) during the second stage of labor, or spontaneous esophageal rupture (Boerhaave's syndrome). The latter has a much higher morbidity/mortality and classically arises from esophageal rupture during excessive vomiting. Boerhaave's syndrome is described as a triad of chest pain, vomiting, and surgical emphysema, though all symptoms are seen in only one-third of c

Association between the second-stage duration of labor and perinatal outcomes in women with a prior cesarean delivery.

BACKGROUND: The cesarean delivery (CD) rate has been increasing globally. Trial of labor after cesarean delivery (TOLAC) has been used as a key method for the reduction of the CD rate. Little is known, however, about the association between the second-stage duration of TOLAC and adverse maternal and neonatal outcomes. This study evaluated the association between perinatal outcomes and the duration of second-stage labor in women undergoing TOLAC. METHODS: A 10-year retrospective cohort study was performed at the Department of Obstetrics and Gynecology, Third Affiliated Hospital of Guangzhou Me
