MAGNET: Counterfactual samples synthesizing for mitigating hallucination in large language models.

Hallucinations are widely recognized as a significant drawback of large language models. Several attempts have been made to reduce the intensity of hallucinations. Among the various attempts, our research has been directed towards mitigating hallucinations caused by the co-occurrence statistics of pre-training corpora. We introduce Model-AGNostic countErfacTual synthesis and adaptive fine-tuning framework (MAGNET), a fine-tuning method that can mitigate the bias of co-occurrence statistics on large language models pre-training data when generating sentences. Our pipeline generates the counterf

Beyond LLaVA-HD: Diving Into High-Resolution Multimodal Large Language Models.

Seeing clearly with high resolution is a foundation of Multimodal Large Language Models (MLLMs), which has been proven to be vital for visual perception and reasoning. Existing works usually employ a straightforward resolution upscaling method, where the image consists of global and local branches, with the latter being the sliced image patches but resized to the same resolution as the former. This means that higher resolution requires more local patches, resulting in exorbitant computational expenses, and meanwhile, the dominance of local image tokens may diminish the global context. In this

Cultural bias in large language models' ability to follow neuroradiology guidelines.

OBJECTIVES: Large language models (LLMs) are increasingly explored as decision-support tools in medical imaging. However, their ability to align with country-specific guidelines, which often diverge, remains uncertain. We set out to evaluate the geographic neutrality of three state-of-the-art LLMs-GPT-o3, Mistral Large, and DeepSeek R1-and a biomedical LLM (MedGemma 1.5 4B), when applied to neuroradiology scenarios with conflicting U.S. and non-U.S. MATERIALS AND METHODS: Vignettes derived from contradictory international guidelines were presented to each model under two conditions: an implic

Scoring Physician Risk Communication in Prostate Cancer Using Large Language Models.

Effective risk communication is essential to shared decision-making in prostate cancer care. However, the quality of physician communication of key concepts varies widely in real-world consultations. Manual evaluation of communication is labor-intensive and not scalable. We present a structured, rubric-based framework that uses large language models (LLMs) to automatically score the quality of risk communication in prostate cancer consultations. Using transcripts from 20 clinical visits, we curated and annotated 487 physician-spoken sentences that referenced five key concepts for shared decisi

Enabling interoperability across disparate health data sources using Large Language Models.

The heterogeneity of health data systems remains a major barrier to interoperability and data integration, and a challenge for clinical decision support and AI-driven research. The coexistence of multiple Common Data Models (CDMs) complicates integration, requiring complex manual or rule-based schema mapping, which is labor-intensive, error-prone, and difficult to scale. We evaluated three leading Large Language Models (LLMs): ChatGPT, Gemini, and DeepSeek, for their ability to match data elements from four widely used CDMs (PCORnet, OMOP, Sentinel, and i2b2) to FHIR R4 via web-based chat inte

Adaptive level modification via player skill classification and large language models.

Maintaining player engagement in video games requires a careful balance between challenge and player competence. Static difficulty settings fail to account for individual skill variation, while existing dynamic difficulty adjustment systems are limited to tuning low-level game parameters rather than restructuring level content. This paper presents an adaptive level modification framework that personalizes gameplay by continuously inferring player skill and applying targeted structural modifications to level content in real-time. A hybrid behavioral dataset is constructed by combining agent-gen

Pseudocode Generation from Clinical Protocol Flowchart using Large Vision-Language Models.

A key component of developing clinical decision support systems (CDSS) involves translating clinical protocols into executable code linking to the backend Electronic Health Record (EHR) systems - a challenging, time-consuming, and error-prone task, particularly for flowchart-based protocols with complex logic and role-specific actions. This study investigates the potential of large vision-language models (LVLMs) to automate pseudocode generation from clinical protocols as an initial step in this process. Using emergency medical services (EMS) as a case study, we evaluated state-of-the-art LVLM

Generative AI and Large Language Models in Rehabilitation: A Scoping Review.

Generative artificial intelligence (AI) and large language models (LLMs) are increasingly evaluated in rehabilitation, yet their clinical validity, reproducibility, and safety remain uncertain. This scoping review mapped peer-reviewed studies of generative AI/LLMs across rehabilitation assessment, clinical reasoning, decision support, planning, education, and functional classification. Following JBI methodology and PRISMA-ScR, five databases were searched for English-language studies published from 1 January 2015 to 24 July 2026. Two reviewers independently conducted study selection, data extr

Fast information and slow evidence in the large language models era.

LLMs accelerate medical information work, but speed of synthesis does not justify reliance. As T.S. Eliot asked, "Where is the knowledge we have lost in information?" Using a data-information-evidence-practice hierarchy, LLM outputs enter clinical setting as information and become evidence only through appraisal, validation, and contextual judgement. We highlight how LLMs can support evidence infrastructures, clarify evidence boundaries, and reshape clinical expertise to strengthen rather than replace evidence-based medicine.

Evaluating large language models in specialized myopia knowledge and clinical reasoning.

BACKGROUND: Myopia is a major global public health concern, and the growing use of LLMs for health consultation makes the accuracy of their myopia-related responses increasingly important. Existing ophthalmic LLM studies have largely focused on image-based applications and mixed ocular diseases, with limited evaluation of myopia-specific reasoning. Yet myopia-related inquiries span both breadth and depth, from specialized differential considerations to common issues in prevention, control, and public education. This study aimed to assess mainstream LLMs across multiple levels of myopia-related

Diagnostic Accuracy of Multimodal Large Language Models in Retinal Fundus Photography.

PURPOSE: Artificial intelligence in ophthalmology has mostly used task-specific convolutional neural networks. The performance of general-purpose multimodal large language models (LLMs) for fundus image diagnosis remains largely unexplored. This study evaluated and compared the performance of four multimodal LLMs in diagnosing retinal diseases from color fundus photographs. METHODS: A total of 800 images (100 per category across 8 conditions) were randomly sampled and evaluated by ChatGPT-4o, Claude 3.5 Sonnet, Gemini 1.5, and Grok-2. Each model received a standardized prompt with a single fu

Large language models as versatile predictive engines for notifiable infectious diseases.

Accurate forecasting of infectious disease cases and deaths is crucial for public health decision-making. Traditional statistical and machine learning approaches may be challenged by complex temporal patterns and heterogeneous surveillance data. We evaluated large language models (LLMs) for infectious disease forecasting. We collected monthly reported cases and deaths data from China National Notifiable Diseases Surveillance System (NNDSS) from January 2009 to February 2025, and monthly reported cases from the United States NNDSS from 2016 to 2023, covering 79 notifiable infectious diseases an

Aligning, Autoencoding and Prompting Large Language Models for Novel Disease Reporting.

Given radiology images, automatic radiology report generation aims to produce informative text that reports diseases. It can benefit current clinical practice in diagnostic radiology. Existing methods typically rely on large-scale medical datasets annotated by clinicians to train desirable models. However, for novel diseases, sufficient training data are typically not available. We propose a prompt-based deep learning framework, i.e., PromptLLM, to align, autoencode, and prompt the (large) language model to generate reports for novel diseases accurately and efficiently. Our method includes thr

Large language models for clinical decision support in gastroenterology and hepatology.

Clinical decision making in gastroenterology and hepatology has become increasingly complex and challenging for physicians. This growing complexity can be addressed by computational tools that support clinical decisions. Although numerous clinical decision support systems (CDSS) have emerged, they have faced difficulties with real-world performance and generalizability, resulting in limited clinical adoption. Generative artificial intelligence (AI), particularly large language models (LLMs), are introducing new possibilities for CDSS by offering more flexible and adaptable support that better

Enhancing Substance Use Detection in Clinical Notes with Large Language Models.

Identifying substance use behaviors in electronic health records (EHRs) is challenging because critical details are often buried in unstructured notes that use varied terminology and negation, requiring careful contextual interpretation to distinguish relevant use from historical mentions or denials. Using MIMIC-III/IV discharge summaries, we created a large, annotated drug detection dataset to tackle this problem and support future systemic substance use surveillance. We then investigated the performance of multiple large language models (LLMs) for detecting eight substance use categories wit

Leveraging Prompt Engineering in Large Language Models for Accelerating Chemical Research.

Artificial intelligence (AI) using large language models (LLMs) such as GPTs has revolutionized various fields. Recently, LLMs have also made inroads in chemical research even for users without expertise in coding. However, applying LLMs directly may lead to "hallucinations", where the model generates unreliable or inaccurate information and is further exacerbated by limited data set and inherent complexity of chemical reports. To counteract this, researchers have suggested prompt engineering, which can convey human ideas formatively and unambiguously to LLMs and simultaneously improve LLMs' r

DrugGen enhances drug discovery with large language models and reinforcement learning.

Traditional drug design faces significant challenges due to inherent chemical and biological complexities, often resulting in high failure rates in clinical trials. Deep learning advancements, particularly generative models, offer potential solutions to these challenges. One promising algorithm is DrugGPT, a transformer-based model, that generates small molecules for input protein sequences. Although promising, it generates both chemically valid and invalid structures and does not incorporate the features of approved drugs, resulting in time-consuming and inefficient drug discovery. To address
