Hyoid bone and larynx movements during electrical stimulation of motor points in laryngeal elevation muscles: a preliminary study.

OBJECTIVES: This study aimed to determine the laryngeal elevation muscle motor points, evaluate the movement of hyoid bone and larynx during stimulation of the motor points, and examine the potential for treating severe dysphagia by functional electrical stimulation. METHODS: The motor points of the laryngeal elevation muscles were anatomically determined from four cadavers. Those motor points in two healthy subjects and one lateral medullary syndrome patient were electrically stimulated by surface or implanted electrodes. RESULTS: The movements elicited by electrical stimulation of the moto

[Myopathology of the intrinsic laryngeal muscles in neurodegenerative diseases, with reference to the mechanism of vocal cord paralysis].

To investigate the mechanism of the vocal cord abductor paralysis (VCAP) in the neurodegenerative diseases, the intrinsic laryngeal muscles (the crycothyroid, the interarytenoid, and the posterior crycoarytenoid muscles) from 41 autopsied cases were histologically examined: 10 cases of amyotrophic lateral sclerosis (ALS), 10 of Parkinson's disease (PD), 9 of multiple system atrophy (MSA), 4 of Machado-Joseph disease (MJD), 4 of progressive supranuclear palsy (PSP), 1 of familial amyloidotic polyneuropathy (FAP), and 3 of cerebrovascular diseases as a control. Coactivation of the Laryngeal Muscles in Pigs Without External Neural Control Indicates Existence of an Intrinsic Neuronal Network.

Electrophysiological studies of the larynx expose the mechanisms by which voice production is controlled. Previous studies have revealed certain phenomena during laryngeal oscillations that suggest a complex control mechanism. Starting from the principle of agonist-antagonist muscular pairing, the aim of this study was to gain a deeper insight into the function of the cricothyroid (CT) and thyroarytenoid (TA) muscles, both central to voice production. Electromyographic recordings were used to determine the response of the two muscles to different stimulation situations in an ex vivo animal mod

The study of laryngeal muscle activity in normal human subjects and in patients with laryngeal dystonia using multiple fine-wire electromyography.

The normal human larynx performs numerous complex tasks with nearly complete reliability. These tasks require precise timing of movements that are effected by the laryngeal muscles. The most specific method to examine these muscles is by electromyography. Although many studies on laryngeal electromyography have been reported using multichannel recordings, none has provided a detailed analysis of each laryngeal muscle's role during a variety of common tasks and the spectrum of normative values. Simultaneous eight-channel, fine-wire electromyographic recordings were made in 11 human subjects. Th

Skeletal muscle metastasis of early glottis laryngeal carcinoma: a case report.

Glottis cancer, a prevalent subtype of laryngeal malignancies, is frequently encountered in clinical practice in LMIC (Lower Middle Income Countries). This case report presents an extraordinary occurrence of T1 glottis cancer, shedding light on a rare clinical scenario. Typically, early-stage glottis cancers, including T1 tumours, exhibit a remarkable rarity of occult lymph node involvement, with estimates indicating a prevalence of less than five percent. However, in this significant case, seen at the Cyberknife and Tomotherapy Centre, Jinnah Postgraduate Medical Centre (JPMC), Karachi, in 20

Laryngeal muscle tension in patients with sinonasal diseases: prevalence and clinical significance.

OBJECTIVE: To determine the prevalence of laryngeal muscle tension in patients with sinonasal diseases. METHODS: The medical records and video-recordings of patients with a history of sinonasal disease were reviewed to identify one of four muscle tension patterns during phonation. A control group with no history of sinonasal diseases was matched according to age and gender. RESULTS: Seventy-seven patients were divided into a study group (n = 47) and a control group (n = 30). In the study group, 29 patients had at least one muscle tension pattern compared with only 9 in the control group (p =

Amount and spatial arrangement of muscle fibers in the human laryngeal Musculus ventricularis.

Textbooks and atlases of human macroscopic and microscopic anatomy of the larynx generally provide, if at all, only sparse information on the laryngeal Musculus ventricularis. However, several studies indicate that this muscle takes over the function of vestibular (ventricular) fold phonation after denervation of the Musculus vocalis. In the present study, 29 laryngeal specimens were coronally dissected at different levels, i.e. the anterior (L1), middle (L2), and posterior third of the vestibular fold (L3), and they underwent histological analysis. In all specimens the vestibular folds of bot

Phonation Threshold Pressure Revisited: Effects of Intrinsic Laryngeal Muscle Activation.

OBJECTIVES/HYPOTHESIS: Phonation threshold pressure (Pth ) is the minimum subglottic pressure required to reach phonation onset and is considered a marker for vocal efficiency and health. We investigated the effects of intrinsic laryngeal muscle (ILM) activation on Pth . STUDY DESIGN: In vivo animal study. METHODS: In an in vivo canine phonation model, laryngeal adductor muscles were activated together by stimulation of the recurrent laryngeal nerves (RLNs) and individually via stimulation of respective terminal nerve branches. Cricothyroid (CT) muscles were activated via stimulation of the

Experience with laryngeal reinnervation using nerve-muscle pedicle in pediatric patients.

OBJECTIVE: Vocal fold paralysis (VFP) in adducted position remains a challenge for airway surgeons. Alternatives to tracheostomies such as lateralization, cordectomy, and posterior rib grafting disrupt the laryngeal tissue or framework and carry an increased risk of aspiration. Laryngeal reinnervation using nerve-muscle pedicle (NMP), carries the distinct advantage of preserving the larynx, sparing the recurrent laryngeal nerve, and obtaining an active VF abduction. The aim of this study was to evaluate the success and complications of laryngeal reinnervation using nerve-muscle pedicle (NMP) i

Myosin heavy chain composition in normal and atrophic equine laryngeal muscle.

The myosin heavy chain (MHC) composition of a given muscle determines the contractile properties and, therefore, the fiber type distribution of the muscle. MHC isoform expression in the laryngeal muscle is modulated by neural input and function, and it represents the cellular level changes that occur with denervation and reinnervation of skeletal muscle. The objective of this study was to evaluate the pattern of MHC isoform expression in laryngeal muscle harvested from normal cadavers and cadavers with naturally occurring left laryngeal hemiplegia secondary to recurrent laryngeal neuropathy. The effects of oesophageal distension on diaphragm and laryngeal muscle activity in the anaesthetized cat.

The electromyographic (EMG) activities of diaphragm and laryngeal muscles were recorded during oesophageal distension in anesthetized cats. The responses to distension of the thoracic oesophagus differed from those evoked by distension of the cervical oesophagus. The crural component of the diaphragm (CD) was inhibited by distension of the thoracic oesophagus; distension of the cervical oesophagus did not affect CD EMG. Thyroarytenoid (TA) muscle EMG increased markedly and consistently in response to distension of the cervical oesophagus. Distension of the thoracic oesophagus only produced sta

Long-term results of nerve-muscle pedicle reinnervation for laryngeal paralysis.

Between 1976 and 1986, 214 patients with bilateral vocal fold paralysis and 73 patients with unilateral vocal fold paralysis were managed by the author using the nerve-muscle pedicle technique for reinnervation. Follow-up of at least 2 years has been obtained on 202 of the bilaterally and 70 of the unilaterally involved patients. Long-term success has been achieved in 74% of the bilateral group and 88% of the unilateral group. Successful reinnervation of unilateral paralyses usually maintains voice correction indefinitely, but there is late (2 to 5 years postsurgery) deterioration of successfu

Experimental laryngeal reinnervation by phrenic nerve implantation into the posterior cricoarytenoid muscle.

Under general anaesthesia, 5 dogs underwent sectioning of the right recurrent nerve followed by implantation of the phrenic nerve into the posterior cricoarytenoid (PCA) muscle. Some 6-7 months later the dogs were sacrificed after registration of vocal cord motility. Still photographs and movie film of the larynx were taken during quiet and forced respiration and at electrical stimulation of the implanted phrenic nerve. The PCA and vocal muscles were removed for histochemical studies. We found practically no abductory movement of the vocal cord on the reinnervated side, either during quiet or

Assessment of the efficacy of an abductor muscle prosthesis for treatment of laryngeal hemiplegia in horses.

Four variations of abductor muscle prosthesis for treating laryngeal hemiplegia were evaluated in 153 horses by questionnaire, and in the 100 Thoroughbred racehorses in this group survival analysis was used to compare their racing performances and earnings with those of 400 control horses. The questionnaire indicated that the technique which included a ventriculectomy and 2 prostheses was regarded as being the most successful (P less than 0.01) and resulted in the least residual stertor (P less than 0.001). Survival analysis showed that there was no significant difference between the treated g

Exercise-induced laryngeal obstruction in athletes treated with inspiratory muscle training.

BACKGROUND: Exercise-induced laryngeal obstruction (EILO) is common in athletes and presents with dyspnoea, chest tightness, inspiratory stridor and sometimes panic reactions. The evidence for conservative treatment is weak, but case reports suggest effects from inspiratory muscle training (IMT). We aimed to explore effects from IMT used in athletes with EILO. METHOD: Twenty-eight athletes, mean age 16.4 years, diagnosed with EILO at our clinic, participated in a 6-week treatment programme, using a resistive flow-dependent IMT device (Respifit S). Four athletes competed at international level

Atrophy of the posterior cricoarytenoid muscle as an indicator of recurrent laryngeal nerve palsy.

BACKGROUND AND PURPOSE: The posterior cricoarytenoid (PCA) muscle is one of the intrinsic muscles of the larynx innervated by the recurrent laryngeal nerve. As such, recurrent laryngeal nerve palsy should not only result in paralysis of the true vocal cord or thyroarytenoid muscle but also in a similar change in the PCA muscle. The ability of CT and MR imaging to depict denervation atrophy in the PCA muscle in patients with recurrent laryngeal nerve palsy was evaluated. METHODS: Two investigators reviewed the CT and/or MR studies of 20 patients with a clinical history of vocal cord paralysis.

Laryngeal muscle activity during speech breaks in adductor spasmodic dysphonia.

To determine the laryngeal muscle activation abnormalities that are associated with speech symptoms in adductor spasmodic dysphonia (ADSD), electromyographic measures of extrinsic and intrinsic laryngeal muscles during speech compared 1) muscle activity when ADSD patients had breaks in words with when they produced the same words without breaks; and 2) muscle activity in ADSD patients during speech without voice breaks with normal control producing phonetically similar words. Simultaneous electromyographic recordings were made from the thyroarytenoid (TA), cricothyroid (CT), sternothyroid (ST)
