The Use of a Stylet to Aid the Lifting of the Epiglottis With a Video Laryngoscope.

This study involved 154 anesthetized patients requiring endotracheal intubation, in whom the epiglottis was visible but not lifted, which restricted the view of the glottis. A simple stylet was used to lift the epiglottis, thus improving the view of the glottis with the Glide Scope®, allowing tracheal tubes to be inserted into the patients. The stylet easily resolved the difficulty of lifting the epiglottis when using a video laryngoscope, thus facilitating endotracheal intubation in patients undergoing general anesthesia.

Clinical experiences of NBI laryngoscope in diagnosis of laryngeal lesions.

Endoscopy is essential for the diagnosis and treatment of cancers derived from the larynx. However, a laryngoscope with conventional white light (CWL) has technical limitations in detecting small or superficial lesions on the mucosa. Narrow band imaging especially combined with magnifying endoscopy (ME) is useful for the detection of superficial squamous cell carcinoma (SCC) within the oropharynx, hypopharynx, and oral cavity. A total of 3675 patients who have come to the outpatient clinic and complained of inspiratory stridor, dyspnea, phonation problems or foreign body sensation, were enroll

Successful intubation with McCoy laryngoscope in a patient with ankylosing spondylitis.

Involvement of cervical spine in ankylosing spondylitis limits neck movement resulting in difficult intubation. We report a case of 45-yr-old lady of ankylosing spondylitis with restricted neck movement planned for emergency laparotomy. Grade 3 Cormack and Lehane laryngoscopic view was observed with Macintosh laryngoscope and gum elastic bougie could not be negotiated. At second attempt with McCoy blade, the posterior commissure of the vocal cord could be visualized and intubation was successful. Thus, in absence of flexible laryngoscopes and when awake intubation is not feasible, McCoy blade

Real-time laryngoscopic measurements of vocal-fold vibrations.

We have developed a high-frame-rate laryngoscope that can measure the vibration distribution of a human vocal fold in real time at hundreds of hertz. Our laryngoscope can extract a vocal-fold contour at 4000 fps as 20 pairs of its left and right border points from 256 × 512-pixel images to quantify left-right asymmetry of vocal-fold vibrations. Experiments on artificial vocal-fold-like vibrations of a silicon rubber membrane were performed to confirm the laryngoscope's effectiveness, and the vocal folds of human subjects, including patients with laryngeal diseases, were examined under clinical

Phonomicrosurgery for posterior glottic lesions using triangular laryngoscope.

It is important to fully expose the posterior glottis to achieve adequate phonomicrosurgical resection of lesions in the posterior glottis. However, it is often difficult to obtain a sufficient view of the posterior glottis by ordinary direct laryngoscopy. We attempted to expose posterior glottic lesions using a triangular laryngoscope for adequate achievement of the surgery. We have performed phonomicrosurgery for 14 lesions in the posterior glottis including 5 early cancers, 1 dysplasia, 1 hemangioma, and 7 granulomatous lesions. Under general anesthesia, a triangular laryngoscope was set la

Comparison of two Macintosh laryngoscope blades in 300 patients.

BACKGROUND: There are two forms of Macintosh laryngoscope blade. Compared with the standard blade, the English blade is longer, its curve is more continuous across the entire length of the blade, the flange of the blade continues much closer to the blade tip, and the height of the flange is shorter. METHOD: We studied 300 patients to compare the ease of laryngoscopy with each type of Macintosh laryngoscope blade. In a random crossover design, after induction of anaesthesia and neuromuscular block, the two blades were inserted in turn, and the views of the glottis at laryngoscopy (Cormack and

[Laryngoscopic findings in dysphonia following traumatic midbrain syndrome].

26 patients with traumatic voice disorders were examined laryngoscopically. During the initial mute stage, reflex vocal cord movement could be detected but no intentional movement. A gradual approximation of the vocal cords led to a narrowing of the glottic aperture so that whispering but not voiced phonation, was possible (stage of whispering). The third stage was characterized by the rapid development of a laryngeal (and pharyngeal) spasticity, identifiable by shortened and thickened vocal folds, an increased approximation of the ventricular folds and in a more dorsal position of the epiglot

Posterior commissure laryngoscope for carbon dioxide laser surgery.

A laryngoscope has been developed for microlaryngeal laser surgery in the posterior commissure of the larynx. The tip has been designed to facilitate exposure of one entire arytenoid and the posterior commissure. Other features include a concavity on the superior lip of the tip to hold the endotracheal tube anteriorly, a bifurcated smoke evacuation channel and a nonreflective finish. It is anticipated that this laryngoscope will be available in pediatric and adolescent sizes in addition to the adult size that we have worked with.

Comparison of the C-MAC D-blade video laryngoscope and the McCoy laryngoscope for double-lumen endotracheal tube intubation: A prospective randomized controlled study.

BACKGROUND: Inserting a double-lumen endotracheal tube (DLT) poses more challenge than inserting a single-lumen tube. The C-MAC D-blade videolaryngoscope is a useful alternative to the direct laryngoscope. However, no study has compared its performance with that of the McCoy laryngoscope, which has a hyperangulated blade tip similar to that of the C-MAC D-blade. We aimed to compare the performance of the C-MAC D-blade videolaryngoscope with that of the McCoy laryngoscope in DLT intubation. METHODS: In this prospective randomized controlled study, 90 patients requiring DLT intubation were rand

Comparative Evaluation of Linscope and King Vision Video Laryngoscopes in Tracheal Intubation: A Randomized Controlled Trial.

Introduction Indirect laryngoscopy has become a widely accepted method for learning the techniques of airway management. The incorporation of small, less expensive, and yet more reliable video cameras in laryngoscopes has given the process of laryngoscopy and intubation a big leap. The King Vision video laryngoscope (Medline Industries, USA) has demonstrated promise in several settings while the Linscope video laryngoscope (Centrum, Turkey) is a newly launched device and no literature is available to the best of our knowledge. Therefore, we aimed to compare the performance of the Linscope vide

Force and pressure distribution using Macintosh and GlideScope laryngoscopes in normal airway: an in vivo study.

BACKGROUND: Forces applied on oropharyngeal soft tissues by direct laryngoscopy may cause damage to the patients. The aim of this study was to measure the forces applied during the manoeuvres to achieve glottis visualization and tracheal intubation, comparing direct laryngoscopy and videolaryngoscopy in vivo. METHODS: Thirty adult patients (ASA physical status 1 or 2, BMI between 18 and 30 kg/m2, no difficulty to intubate) were randomly and blindly assigned to one of two groups. Forces and pressure distribution applied during glottis visualization and intubation were measured using film press

Comparison of video laryngoscopes with direct laryngoscopy for tracheal intubation: a meta-analysis of randomised trials.

BACKGROUND: Video laryngoscopes have been introduced in recent years as an alternative choice to facilitate tracheal intubation. We conducted a meta-analysis to assess their value when compared with direct laryngoscopy. METHODS: PubMed and EMBASE were searched up until 24 September 2010. Randomised trials that reported data on the comparison of video laryngoscopes with direct laryngoscopy for tracheal intubation were included. RESULTS: Eleven trials with a total of 1196 participants were identified. During tracheal intubation, video laryngoscopes can achieve a better view of the glottis and

Tracheal intubation of morbidly obese patients: a randomized trial comparing performance of Macintosh and Airtraq laryngoscopes.

BACKGROUND: The Airtraq laryngoscope is designed to allow visualization of the glottis without alignment of the oral, pharyngeal, and laryngeal axes. We hypothesized that this new airway device would facilitate tracheal intubation of morbidly obese patients. We compared tracheal intubation performance of standard Macintosh laryngoscope with the Airtraq laryngoscope in morbidly obese patients. METHODS: One hundred and six consecutive ASA I-III morbidly obese patients undergoing surgery were randomized to intubation with the Macintosh laryngoscope or the Airtraq laryngoscope.
