An Unusual Cause of Bleeding on the Floor of Mouth: Leech Infestation.

Leech infestation is a very rare phenomenon in humans. It mostly occurs in humans when rural untreated water is drunk or while swimming in streams or lakes. When leeches adhere to the mucous membrane, they ingest blood. Thus, they can sometimes cause severe anemia that may require blood transfusion. We report a case that was referred to emergency service with bleeding in the floor of the mouth. A 10-year-old child was referred to the emergency service of a city hospital with a complaint of swelling in the floor of the mouth and spitting of blood. The patient was promptly taken to the operating

An unusual cause of dysphonia with hemoptysis: a laryngeal live leech.

INTRODUCTION: Foreign bodies in the upper airway are one of the most challenging otolaryngology emergencies and have various presentations depending on their physical properties and location. Leeches are blood-sucking hermaphroditic worms that vary in color, length, and shape. They usually reside in fresh-water streams and lakes. When rural untreated water is drunk, leeches may localize in the nose, pharynx, and esophagus, or rarely in the larynx. CASE REPORT: This case is a man who was referred to our otolaryngology clinic with a complaint of hemoptysis and mild respiratory distress. The pat

Rectal bleeding due to leech bite: a case report.

We present herein a case with a four-day history of fresh rectal bleeding due to leech bite. The cause was found to be a leech in the rectum by anoscope. This pathological condition is extremely rare in urban areas. Leech endoparasitism, although rare, may cause serious, even lethal, complications. Suspicion of leech infestation should be kept in mind when faced with intermittent or severe rectal bleeding in humans, and should be investigated surgically as with all other foreign bodies. In the current study, we present a patient who admitted with rectal bleeding due to leech bite. The leech wa

Aquatic leech as a rare cause of respiratory distress and hemoptysis.

This paper presents a 73 years old male with three weeks history of intermittent hemoptysis, dysphagia, dyspnea, stridor, and suffocations sensation. By means of fibrotic bronchoscopic examination, the cause was found to be a leech in the glottis. It was removed by injection of 4 ml lidocaine 2%. Infestation into the respiratory tract by a leech may become lethal because of hypoxia and death secondary to airway obstruction. A high index of suspicion of leech infestation sould be considered in patients presenting with hemoptysis, hoarseness and respiratory distress and a history of recent conta

Unusual cause of unilateral epistaxis: nasal leech infestation.

Common causes for unilateral nasal bleeding in adults are benign or malignant tumors and deviated nasal septum. Here we present two cases of recurrent unilateral nasal bleeding due to leech infestation. Though it is not a common cause for nasal bleeding, a clinician should suspect leech infestation for a recurrent nasal bleeding specially in tropical countries like Nepal.

Voltage-dependent Ca2+ influx into identified leech neurones.

We determined the relationships between the intracellular free Ca2+ concentration ([Ca2+]i) and the membrane potential (Em) of six different neurones in the leech central nervous system: Retzius, 50 (Leydig), AP, AE, P, and N neurones. The [Ca2+]i was monitored by using iontophoretically injected fura-2. The membrane depolarization evoked by raising the extracellular K+ concentration ([K+]o) up to 89 mM caused a persistent increase in [Ca2+]i, which was abolished in Ca(2+)-free solution indicating that it was due to Ca2+ influx. The threshold membrane potential that must be reached in the diff

Expression of surface glycoproteins early in leech neural development.

Cell migration and axon growth during neural development rely upon cell-cell and cell-matrix interactions mediated by surface glycoproteins. The surface glycoprotein recognized on leech neurons by monoclonal antibody Lan3-2 has previously been implicated in the process of axon fasciculation during regeneration in adults. In adult leeches, Lan3-2 binds to a carbohydrate epitope of a 130 kD protein. The present study demonstrates that in embryos the antibody binds to the same carbohydrate epitope of glycoproteins with molecular weights of 130 kD and higher. As a first step in evaluating a possib

A calcium reversible action of barbiturates on the leech Retzius cell.

The Na salts of phenobarbital, barbital, pentobarbital, secobarbital, methohexital and thiopental in concentrations of 1 to 5 mM all caused prolonged action potentials in leech Retzius cells. The prolongation was favored by low Ca and was reversible by elevation of Ca. Barbiturate-prolonged action potentials were not affected by 50 micronm tetrodotoxin or replacement of Cl by propionate, but they were dependent on external Na. Their amplitudes were increased by steady hyperpolarization and input resistance was reduced during them. They were of shorter duration when elicited at a rapid rate. Th

Characterization of the natural peptidome of four leeches by integrated proteogenomics and pseudotargeted peptidomics.

Animal-derived drugs are an indispensable part of folk medicine worldwide. However, their chemical constituents are poorly approached, which leads to the low level of the quality standard system of animal-derived drugs and further causes a chaotic market. Natural peptides are ubiquitous throughout the organism, especially in animal-derived drugs. Thus, in this study, we used multi-source leeches, including Hirudo nipponica (HN), Whitmania pigra (WP), Whitmania acranulata (WA), and Poecilobdella manillensis (PM), as a model. A strategy integrating proteogenomics and novel pseudotargeted peptido

Distribution and morphology of nociceptive cells in the CNS of three species of leeches.

The present study describes the segmental variation in the distribution and morphology of nociceptive neurons (N cells) in the central nervous system of the leech. N cells of midbody ganglia can be segregated into lateral and medial types. We show that monoclonal antibodies specific for N cells can distinguish between the two populations. The monoclonal antibodies were used to map the complete distribution of the cells along the nervous cord. There are two pairs of the medial and lateral nociceptive neurons in the midbody ganglia, one pair of the medial type in the sex ganglia (5 and 6), and a

Acute postoperative infection with Aeromonas hydrophila after using medical leeches for treatment of venous congestion.

INTRODUCTION: Venous convulsion after reconstructive microsurgery procedures is one major complication a surgeon has to deal with. Today, especially in the field of reconstructive microsurgery, medicinal leech therapy enjoys a renaissance. The potential risks such as infections associated with leech therapy are generally underestimated and not sufficiently discussed in literature. METHOD/PATIENTS: We present five male patients with an average age of 47 years. All patients suffered from a trauma incident, which had to be treated as an emergency. Three patients showed, postoperatively, a venous

A multidimensional strategy for exploring active peptides combining chemical analysis and bioassays for medicinal leeches.

ETHNOPHARMACOLOGICAL RELEVANCE: Leeches were first recorded in Shennong Bencao Jing, and they have been traditionally used to treat blood stasis-related disorders through decoction and oral administration more than 2000 years in China. This practice involves heating and gastrointestinal transformation. However, whether it converts native constituents into pharmacologically active substances remains unresolved, representing a critical gap in understanding bioactive substance formation in traditional medicine. AIM OF THE STUDY: To elucidate the component transformation of Hirudo nipponia (HN) a

Multi-Target Anticoagulant Regulation by Peptides from Medicinal Leeches: A Unique Natural Strategy against Thrombosis.

BACKGROUND: Thrombotic diseases remain a major global health burden. Current anticoagulants are often limited by bleeding risks and narrow therapeutic windows, largely due to their single-target mechanisms. In contrast, medicinal leeches secrete diverse peptides that naturally and synergistically modulate multiple steps of the hemostatic system. METHODS: This paper systematically reviews published biochemical, structural, functional, and omics studies on leech-derived anticoagulant peptides, classifying them according to their molecular targets and antithrombotic mechanisms. RESULTS: Leech-d

The use of dendrograms to describe the electrical activity of motoneurons underlying behaviors in leeches.

The present manuscript aims at identifying patterns of electrical activity recorded from neurons of the leech nervous system, characterizing specific behaviors. When leeches are at rest, the electrical activity of neurons and motoneurons is poorly correlated. When leeches move their head and/or tail, in contrast, action potential (AP) firing becomes highly correlated. When the head or tail suckers detach, specific patterns of electrical activity are detected. During elongation and contraction the electrical activity of motoneurons in the Medial Anterior and Dorsal Posterior nerves increase, re

Possible detection of physiological state disorders with the help of medicinal leeches Hirudo verbana Carena, 1820.

In the experiment, 160 medicinal leeches of the species Hirudo verbana Carena, 1820 were studied. Medicinal leeches were fed on the blood of animals and people (conditionally healthy and diseased). Four leeches were taken from each animal/person. The animals were studied for 3 weeks. Mortality was mostly observed in the first days after feeding on the blood of the host. We noted mortality, the appearance of constrictions on the leeches' body, the intensity of the host blood spitting from their body. The host's blood was taken from their stomach on the first day after feeding. Hematological and

Scrotal Hirudiniasis: The Mysterious Migration of a Leech Into the Scrotum: A Case Report.

Hirudiniasis, or leech infestation, is a rare parasitic condition typically involving external skin or mucosal surfaces. We report a case of scrotal hirudiniasis, an 11 cm long leech discovered within the scrotal sac of a 15-year-old male. He initially developed hematuria due to periurethral leech attachment, which resolved following catheterization and bladder irrigation. A week later, he presented with acute scrotal pain and swelling. Ultrasound revealed a tubular, mobile structure near the left vas deferens. Surgical exploration identified an 11 cm leech within the tunica vaginalis, without

Monomeric and Oligomeric Decorsins of the Asian Medicinal Leech Hirudinaria manillensis.

Hematophagous leeches rely on a broad diversity of bioactive factors that interfere with the host's defense systems to secure a successful blood meal. The most prominent examples of such factors are the antithrombotics that address either the primary hemostasis (the platelet aggregation) or the secondary hemostasis (the blood coagulation). Whereas the inhibitors of platelet aggregation mainly cause continuous blood flow, coagulation inhibitors mainly keep the blood fluid within the stomach of the leech. The critical dependency of hematophagous leeches on the accurate action of all antithrombot

Airway leech infestation masquerading as a tumor: a rare case report.

Airway obstruction is a ventilatory dysfunction caused by internal and external diseases of the respiratory tract, which is usually considered a life-threatening clinical emergency. Traditionally, airway obstruction can be caused by respiratory tumors, trauma, and foreign bodies, among which tumors are the most common. Consequently, physicians may prioritize tumor-related diagnoses, potentially leading to the misdiagnosis of rare non-neoplastic conditions. Here, we report a case of a 61-year-old adult traveler who initially presented with mild dyspnea and a bronchial space-occupying lesion on
