[Acute interstitial pneumonia in patient with rheumatoid arthritis treated with leflunomide].

Leflunomide (LEF) is an isoxazole derivative used as disease-modifying anti-rheumatic drug (DMARD) in the treatment of rheumatoid arthritis (RA). It is effective and safe in patients with active RA, in whom standard treatment is insufficient or contraindicated, but it can cause interstitial lung disease (ILD). Identified risk factors for LEF-induced ILD include pre-existing ILD, cigarette smoking, low body weight, and use of loading dose. LEF should be avoided in patients with pre-existing ILD. We present a case of 59-year-old male with RA and a history of smoking and methotrexate (MTX) treatm

Pharmacokinetics and bioequivalence evaluation of leflunomide tablets in Korean healthy volunteers.

Leflunomide is a disease-modifying antirheumatic drug. The purpose of this study was to evaluate the bioequivalence of a test drug (CJ leflunomide) and a commercially available reference drug (Arava®) at 2 doses (10 and 20 mg) in healthy Korean volunteers. This was a single-dose (28 individuals enrolled at each dose group), randomized, open-label, 2-way crossover study. The 2 treatment periods were separated by a 56-day wash-out interval. Blood sampling was conducted until 672 h after drug administration. Plasma teriflunomide (active metabolite of leflunomide) concentrations were determined, a

Effect of leflunomide on liver regeneration after partial hepatectomy in rats.

PURPOSE: Partial hepatectomy (PH) can be an inevitable surgical therapy in some conditions, such as hepatic malignancies, trauma or partial liver transplantation. Its capacity for regeneration distinguishes the liver from other essential organs. Regeneration is a complex process involving growth factors, cytokines, transcription factors, hormones, and oxidative stress products. In the event of ineffective or total absent liver regeneration, the life threatening picture of acute liver failure may supervene. In the present research, we studied the effect of leflunomide, a novel immunosuppressive

Leflunomide in subacute cutaneous lupus erythematosus - two sides of a coin.

BACKGROUND: Subacute cutaneous lupus erythematosus (SCLE), a distinct clinical subset of lupus erythematosus, remains a therapeutic challenge, especially in cases resistant to topical and standard systemic therapy. Leflunomide, a novel antirheumatic drug, has shown efficacy in the treatment of systemic lupus erythematosus in pilot studies. METHODS: We report two patients with SCLE who demonstrated the spectrum of possible clinical responses to leflunomide therapy. RESULTS: One patient experienced a complete clinical remission of symptoms, whereas the other developed a massive skin reaction w

Evaluation of the anti-inflammatory effect of ectoine-leflunomide combination in adjuvant-induced arthritis in rats, and its colonic protective action against leflunomide-induced gastrointestinal injury.

Rheumatoid arthritis (RA) is an autoimmune disease with an intriguing pathophysiological mechanism that primarily affects the joints and leads to extra-articular manifestations. Leflunomide is one of the most effective disease-modifying anti-rheumatic drugs (DMARDs), utilized in RA treatment, but its use comes with frequent gastrointestinal adverse effects, which might necessitate therapy discontinuation. The current work aimed to explore the antirheumatic and gastroprotective effects of ectoine, a compatible solute found in nature, on adjuvant-induced arthritis (AIA) in rats, and to compare i

Pulmonary Nodules After Leflunomide Therapy in Rheumatoid Arthritis: A Case Report.

Rheumatoid arthritis (RA) is a systemic autoimmune disorder with both articular and extra-articular manifestations, including rare pulmonary complications. We report a case of a 65-year-old male with long-standing RA who developed multiple cavitary pulmonary nodules following prolonged leflunomide therapy. Diagnostic evaluation excluded infectious, neoplastic, and autoimmune causes. Bronchoscopy and biopsy confirmed an acute suppurative inflammatory process with central necrosis. Discontinuation of leflunomide resulted in the resolution of pulmonary nodules. This case highlights the need for v

Repurposing leflunomide for relapsed/refractory multiple myeloma: a phase 1 study.

The inexpensive, well-tolerated, immunomodulatory agent leflunomide, used extensively for the treatment of rheumatoid arthritis, has been shown to produce significant activity against multiple myeloma (MM) in pre-clinical studies. We conducted a phase 1 study (clinicaltrials.gov: NCT02509052) of single agent leflunomide in patients with relapsed/refractory MM (≥3 prior therapies). At dose levels 1 and 2 (20 and 40 mg), no dose-limiting toxicities (DLTs) were observed. At dose level 3 (60 mg), one patient experienced elevated alanine aminotransferase; an additional three patients were enrolled

Leflunomide-induced subacute cutaneous lupus erythematosus with erythema multiforme-like lesions.

Leflunomide is an immunosuppressive agent that acts by inhibiting pyrimidine synthesis in lymphocytes and other rapidly proliferating cells, as well as by suppressing tumor necrosis factor-alpha-induced cellular responses. A number of leflunomide-related adverse events have been reported. Among cutaneous side effects, a few cases of subacute cutaneous lupus erythematosus have been described. We report a previously undocumented reaction to leflunomide, manifesting as subacute cutaneous lupus erythematosus and erythema multiforme-like lesions, in a young woman treated with this drug for ankylosi

[Skin ulceration after leflunomide treatment in two patients with rheumatoid arthritis].

Leflunomide is a disease-modifying antirheumatic drug for the treatment of active rheumatoid arthritis and psoriatic arthritis. Skin ulcerations are not listed as a side effect for this drug, and there has been only one case report on leflunomide associated skin ulcerations. We report on two females, 59 and 63 years old respectively, who were treated with leflunomide for rheumatoid arthritis and subsequently developed severe skin ulcerations. After discontinuation of the drug the skin ulcerations healed complete even if very slowly.

Leflunomide-related lung injury in patients with rheumatoid arthritis: imaging features.

Imaging findings of 26 cases of leflunomide (Arava)-related acute lung injury were analyzed. Thirteen cases had pre-existing interstitial pulmonary disease on chest X-ray or computed tomography. The main features of clinically determined leflunomide-induced acute lung injury were similar to those caused by other drugs: diffuse or widespread patchy ground-glass opacities and/or consolidation, frequently accompanied by septal thickening and intralobular reticular opacities. We categorized these findings into four patterns: diffuse alveolar damage (DAD), acute eosinophilic pneumonia, hyperreactio

Innovative treatment approaches for rheumatoid arthritis. Cyclosporin, leflunomide and nitrogen mustard.

Cyclosporin A (CSA) Cyclosporin inhibits IL-2 release and T-cell activation and, secondarily, affects B-cell function. It also inhibits bone resorption, at least in vitro. This drug's bio-availability averages 25-35% but is highly variable. Food and grapefruit juice enhance bio-availability and newer formulations may make its absorption more reliable. It is highly concentrated in fatty tissues and red blood cells but does not cross the blood-brain barrier. CSA is metabolized to numerous metabolites by the liver and its elimination half-life is 6-12 hours in the absence of severe liver disease.

Regulatory effects of leflunomide on gut microecology during IgA nephropathy treatment.

Growing evidence suggests that the gut-kidney axis may contribute to the pathogenesis of IgA nephropathy (IgAN). However, the effects of immunosuppressants on the intestinal microbiome remain unclear. We investigated how different therapeutic strategies influence gut microbial composition in IgAN patients. We enrolled 46 patients with IgAN and 37 healthy controls (HC). Patients were stratified by treatment regimen into a supportive care group or an immunosuppressive therapy group, with subgroups defined by the use of leflunomide and systemic glucocorticoids. Fecal samples from patients in clin

QbD-enabled leflunomide-transfersomal gel for topical treatment of rheumatoid arthritis.

This study aimed to create innovative transfersomes (TRFs) consisting of a lipid blend and a chemical permeation enhancer for efficient transdermal delivery of leflunomide (LFN). Leflunomide, a dihydroorotate dehydrogenase inhibitor, is mainly used to manage rheumatoid arthritis (RA). Oral consumption of LFN for RA can lead to adverse systemic effects; hence, local application is advisable. To develop topical dosage form of LFN, transfersomes with improved skin permeation capabilities were fabricated. The vesicle diameter of LFN-TRF was determined to be 181.2 ± 2.17 nm, zeta potential of - 29.

Leflunomide-Mediated Immunomodulation Inhibits Lesion Progression in a Vitiligo Mouse Model.

Autoimmune CD8+ T cell-driven melanocyte destruction constitutes a key pathogenic mechanism in the development of vitiligo. Therefore, the pharmacological inhibition of CD8+ T cell effector functions and skin trafficking is a clinically viable therapeutic strategy. This study investigates leflunomide (LEF), an immunomodulatory drug with established safety in autoimmune diseases, for its therapeutic potential in a tyrosine-related protein (TRP) 2-180-induced vitiligo mouse model. Through flow cytometry, immunofluorescence, ELISA, and histopathological analyses, we systematically evaluated LEF's

Hepatoprotective effects of carvedilol and crocin against leflunomide-induced liver injury.

Leflunomide-induced liver injury has been an important problem since its approval. Although, severe cases of leflunomide-induced liver injury leading to hospitalization are rare, the risk is higher with concurrent liver disease or use of other hepatotoxic drugs. The current study was conducted to investigate the potential protective effects of carvedilol and crocin alone and in combination against leflunomide-induced hepatic injury and to clarify the possible mechanism(s) through which carvedilol and crocin may elicit their effects. Fifty male albino mice were allocated into five groups: norma

Leflunomide therapy for BK virus allograft nephropathy after pediatric kidney transplantation.

The BK virus (BKV) can be reactivated with immunosuppressive treatment in renal allograft recipients, which can result in interstitial nephritis (BKV-associated nephropathy, BKVAN) and lead to renal allograft failure. Recently, leflunomide has been reported in some case series of BKVAN with favorable results. Most studies have included only adult patients, we herein report a pediatric case and include a literature review. The patient was a nine-yr-old female with end-stage renal disease due to hypoxic kidney injury. A deceased donor renal transplant was performed and good initial allograft fun
