Closure of the left atrial appendage using percutaneous transcatheter occlusion devices.

Closure of the left atrial appendage (LAA) using percutaneous transcatheter occlusion devices is used for stroke prevention as an alternative for patients with a high risk and contraindications for long-term oral anticoagulation use. In this manuscript, we will discuss the practical aspects of four among the available devices that provide percutaneous intravascular closure of the LAA: Watchman, Amulet, WaveCrest, and LAmbre.

Incomplete Left Atrial Appendage Closure Successfully Treated With a Redo Procedure.

A 68 year's old man with permanent atrial fibrillation, underwent a left atrial appendage closure because of high bleeding risk. However, after one month, an incomplete occlusion of the left atrial appendage due to a too deep position of the device, was detected by a transesophageal echocardiogram. Considering the residual high risk of thrombi formation, a new different device was successfully implanted with a correct closure of the appendage.

Pulmonary artery perforation caused by a left atrial appendage closure device.

We describe a female patient who received hybrid catheter intervention and occlusion of the patent ductus arteriosus (PDA) and left atrial appendage (LAA). Four hours after the procedure, pericardial tamponade suddenly occurred. Surgical exploration of the heart found that a fixation hook of the WATCHMAN device, had protruded through the LAA wall and tore the dilated pulmonary artery (PA). This is the first case report to describe PA perforation caused by barbs of the WATCHMAN device, and more attention should be paid to the relationship between the LAA and its adjacent structures before LAA c

Left atrial appendage closure using Amulet device in a patient with prior percutaneous atrial septal defect closure.

Percutaneous left atrial appendage (LAA) occlusion is an alternative therapeutic option for stroke prevention in patients with atrial fibrillation and contraindications for oral anticoagulation. There are few data available regarding the technical success of percutaneous LAA closure in patients with the previous implantation of an atrial septal defect (ASD) or a patent foramen ovale closure device. This is the description of a case of a successful LAA closure performed with an Amplatzer Amulet occluder device (St. Jude Medical, Inc., St. Paul, MN, USA) in a patient with a previous history of p

Role of Endovascular Closure of the Left Atrial Appendage in Stroke Prevention for Atrial Fibrillation.

The pathophysiologic mechanism of thromboembolic stroke in the setting of non-valvular atrial fibrillation (AF) resides in the left atrial appendage (LAA). In this setting, approximately 90 % of all strokes originate from this structure. Percutaneous left atrial appendage occlusion (LAAO) therapy has recently emerged as an important strategy for prevention of stroke and systemic embolism in patients with non-valvular AF. Systemic anticoagulation therapy in this AF population, while effective, is associated with a significant bleeding risk, drug compliance issues, and limited reversal strategie

Hemodynamic impact of percutaneous left atrial appendage closure in patients with paroxysmal atrial fibrillation.

PURPOSE: Percutaneous left atrial appendage (LAA) closure has become a valid alternative to anticoagulation therapy for the prevention of thromboembolic events in patients with atrial fibrillation (AF). However, scarce data exist on the impact of LAA closure on left atrial and ventricular function. We sought to assess the acute hemodynamic changes associated with percutaneous LAA closure in patients with paroxysmal AF. METHODS: The study population consisted of 31 patients (mean age 73 ± 10 years; 49% women) with paroxysmal AF who underwent successful percutaneous LAA closure. All patients we

Effect of Left Atrial Appendage Closure in Combination With Catheter Ablation on Left Atrial Function for Persistent Atrial Fibrillation.

Background: A single procedure combining left atrial appendage closure (LAAC) plus catheter ablation (CA) has been proven to be safe and feasible for treating atrial fibrillation (AF). However, the influence of treatment modality on left atrial (LA) function is not thoroughly explained. Objective: We aimed to investigate the changes of LA function in persistent AF patients undergoing concomitant LAAC and CA. Methods: The study population comprised 65 patients who underwent combined AF ablation and Watchman LAAC (combined therapy group) in our center, and 65 participants of the AF simple ablati

Left atrial appendage closure for patients with atrial fibrillation at high intracranial haemorrhagic risk.

BACKGROUND AND OBJECTIVES: Although left atrial appendage closure (LAAC) is performed in patients with non-valvular atrial fibrillation (NVAF) at increased risk of intracranial haemorrhage (ICH), outcome data are scarce. We assessed the detailed neurological indications for LAAC and outcomes after LAAC in high ICH risk patients. METHODS: Study population included consecutive patients with NVAF who underwent LAAC in a single hospital network between January 2015 and October 2021 because of prior ICH or the presence of high ICH risk imaging markers on brain MRI (cerebral microbleeds (CMBs)). Left atrial appendage closure for stroke prevention in atrial fibrillation: current status and perspectives.

Atrial fibrillation (AF) is associated with an increased risk of stroke and systemic embolism, and the left atrial appendage (LAA) has been identified as a principal source of thromboembolism in these patients. While oral anticoagulation is the current standard of care, LAA closure (LAAC) emerges as an alternative or complementary treatment approach to reduce the risk of stroke or systemic embolism in patients with AF. Moderate-sized randomized clinical studies have provided data for the efficacy and safety of catheter-based LAAC, largely compared with vitamin K antagonists. LAA device iterati

Left atrial appendage closure for stroke prevention in nonvalvular atrial fibrillation: A current overview.

Atrial fibrillation (AF) is the most common arrhythmia encountered in clinical practice and its prevalence increases with age. AF causes palpitations, heart failure, and cardiogenic embolism. Of them, the most critical and fatal complication is a cardio-embolic event. Oral anticoagulation plays a central role in reducing stroke risk in AF patients. Recently, when oral anticoagulation is considered in patients with non-valvular AF who are eligible for direct oral coagulations, they are preferred to vitamin K antagonist based on accumulating evidence. Although many patients can tolerate oral ant

Left atrial appendage closure: a percutaneous transcatheter approach for stroke prevention in atrial fibrillation.

Atrial fibrillation is a frequent cause of stroke; in the elderly, more than 20% of strokes are attributed to this common arrhythmia. Anticoagulation with warfarin reduces the risk of stroke by ∼60%; however, a large proportion of patients with atrial fibrillation do not receive this treatment because of relative/absolute contraindications. Moreover, patients often discontinue warfarin for a variety of reasons and chronic warfarin administration rates remain suboptimal. Although the compliance with anticoagulation may improve with novel anticoagulants and bleeding risk can be somewhat reduced

Impact of HAS-BLED Score on outcome after percutaneous left atrial appendage closure: insights from the German Left Atrial Appendage Occluder Registry LAARGE.

AIM: Percutaneous left atrial appendage (LAA) closure has been established as alternative stroke prophylaxis in patients with non-valvular atrial fibrillation (AF) and high bleeding risk. However, little is known regarding the outcome after LAA closure depending on the HAS-BLED score. METHODS: A sub-analysis of the prospective, multicenter, Left-Atrium-Appendage Occluder Register-GErmany (LAARGE) registry was performed assessing three different groups with respect to the HAS-BLED score (0-2 [group 1] vs. 3-4 [group 2] vs. 5-7 [group 3]). New Measurements Using Computed Tomography Predict Success of Left Atrial Appendage Closure.

BACKGROUND: Preprocedural screening using transesophageal echocardiography (TEE) and computed tomography (CT) is widely performed in patients undergoing left atrial appendage (LAA) closure (LAAC). However, established methods for accurately predicting procedural device success (DS) remain unclear. AIMS: This study aimed to evaluate whether a novel preprocedural CT-based assessment focusing on the landing zone and presence of an internal septum could improve DS prediction. METHODS: We analyzed 560 patients from a Japanese multicenter registry who underwent LAAC with Watchman FLX (Boston Scien

Threading a needle: percutaneous closure of an incompletely ligated left atrial appendage.

Atrial fibrillation increases the risk of ischemic stroke by increasing left atrial appendage thrombus formation. Previously surgically ligated atrial appendages can recanalize, which increases stroke risk. Percutaneous ligation of such appendages is technically challenging due to altered morphology but feasible. We present a case of a successful percutaneous closure of a previously incompletely ligated atrial appendage using a WATCHMAN FLX device and discuss our approach to similar cases with challenging anatomy. Thorough pre-procedural imaging for case planning is indispensable and dictates
