Disconnection between periodic leg movements and cortical arousals in spinal cord injury.

OBJECTIVE: In this study we examine the temporal connection between periodic leg movements (PLMs) and cortical arousals, as well as the treatment effect of pramipexole, in a clinical case with spinal cord lesion. METHODS: A patient with complete cervical spinal cord injury and PLMs during sleep underwent two baseline sleep recordings, one recording with dopaminergic treatment, and one recording with adaptive servoventilation. RESULTS: The PLMs were temporally dissociated from cortical arousals as well as from respiratory or heart rate events. PLMs were suppressed by pramipexole and persisted

Acute isolated lateral compartment syndrome of the leg after a noncontact sports injury.

Compartment syndrome of the leg typically affects the anterior compartment or multiple compartments and can be traumatic or exertional (chronic) in nature. Isolated lateral (peroneal) compartment syndrome of the leg is a less typical and lesser-known condition that can potentially escape diagnosis if not considered in the workup of a painful leg. This article presents a case of isolated lateral compartment syndrome of the leg without gross muscle rupture or hematoma. The patient presented 21 hours after injury while playing soccer. Isolated lateral compartment syndrome was diagnosed based on h

Biofidelity of TRL Legform Impactor and Injury Tolerance of the Human Leg in Lateral Impact.

In nonfatal car-pedestrian accidents, lower extremities are the most commonly injured body parts. The test device used to evaluate the car-front aggressiveness regarding the risk of these injuries is a legform impactor. Injury-related factors causing AIS 2+ injury in the human lower leg when exposed to a lateral impact representing a pedestrian accident should be identified. One of the test devices commonly used to evaluate the risk is the legform impactor developed by the Transport Research Laboratory (TRL). However, information about the biofidelity of this impactor and leg injury tolerance

An alternative method to free flap for distal leg and foot defects due to electrical burn injury: distally based cross-leg sural flap.

BACKGROUND: High voltage electrical injuries can cause devastating results especially in distal extremities. Although free flaps are the golden standards for the reconstruction of these defects, sometimes local flap alternatives are more useful. One of the most favorable local flap is distally based sural flap (DBSF), which can be used in cross-leg fashion when ipsilateral extremity is affected by a high voltage electrical injury. The purpose of this study was to evaluate long term results of eleven patients who underwent a reconstruction to the lower extremity with cross-leg DBSF due to high

Analysis of Foot-Ankle-Leg Injuries in Various Under-Foot Impact Loading Environments With a Human Active Lower Limb Model.

Lower limb injuries caused by under-foot impacts often appear in sport landing, automobile collision, and antivehicular landmine blasts. The purpose of this study was to evaluate a foot-ankle-leg model of the human active lower limb (HALL) model, and used it to investigate lower leg injury responses in different under-foot loading environments to provide a theoretical basis for the design of physical dummies adapted to multiple loading conditions. The model was first validated in allowable rotation loading conditions, like dorsiflexion, inversion/eversion, and external rotation. Then, its sens

Ankle dorsiflexion deficit in the back leg is a risk factor for shoulder and elbow injuries in young baseball players.

The relationship between ankle joint function and throwing-related injuries has not been demonstrated. We hypothesized that limited ankle joint range of motion (ROM) was related to risk factors for shoulder and elbow injuries in young baseball players. This 12-month prospective cohort study evaluated the age, height, weight, playing position, shoulder, elbow, and ankle function of 228 enrolled baseball players. Shoulder and elbow injuries were tracked during the season. Univariate and multivariate analyses were performed to identify risk factors for shoulder and elbow injuries among participan

Hamstring activation deficits during Double-Leg Jump-Landing tasks in athletes with hamstring strain injuries using EMG Time-Frequency analysis.

This study aimed to investigate the hamstring activation in athletes with hamstring strain injuries in vertical jump tasks using an EMG time-frequency analysis. Fifteen injured athletes and 15 matched controls were recruited. The EMG signals of the lateral hamstring (LH) and medial hamstring (MH) were assessed during countermovement, squat, and drop vertical jump tasks. Wavelet analysis was used to analyze the hamstring activation during takeoff, flight, and landing phases. The kinetics outcomes were also investigated. The results showed a significantly lower mean frequency of the LH and MH fo

The attenuating effect of a dynamic cushion on the development of leg edema in wheelchair users with spinal cord injuries.

BACKGROUND: Prolonged wheelchair sitting is known to lead to venous stasis and results in leg edema. OBJECTIVE: To clarify how a dynamic cushion affects leg edema in people with spinal cord injuries (SCI) evoked by wheelchair sitting, we measured the changes in leg volume induced during wheelchair sitting with a dynamic air cushion or a static cushion. METHODS: Seven wheelchair users with SCI participated in this study. Leg edema during wheelchair sitting was evaluated with strain gauge plethysmography (the gauge was placed 10 cm proximally from the medial malleolus). Following a period of r

The effect of leg fracture on pedestrian kinematic response and injury severity in car-pedestrian collisions.

This study aims to clarify the relationship between pedestrian leg bone fracture, the head, chest, abdomen and pelvis kinematic responses, and knee joint injury severity in car-pedestrian collisions, based on a coupled FE model with two scenarios: leg bone fracture and non-fracture. It finds leg fractures cause abdominal and pelvic upward Z-axis shifts, reduce knee ligament elongation rates and reduce femur/tibia peak bending moment, but have no significant effects on head and chest kinematics and injuries. This study demonstrates that kinematic responses of abdomen and pelvis and knee injury

Conservative treatment for leg oedema and the effect on nocturnal polyuria in patients with spinal cord injury.

OBJECTIVES: To evaluate the possible influence of non-pharmacological interventions, such as compressive bandages and intermittent pneumatic compression (IPC), on leg oedema and nocturnal polyuria (NP), and the possible interrelation between both pathologies in patients with spinal cord injury (SCI), as patients with SCI often have leg oedema and during the night the oedema decreases as a result of natural drainage mechanisms that can cause NP. PATIENTS AND METHODS: Patients with SCI who followed their first rehabilitation after their SCI with bilateral leg oedema and/or with as much or a lar

Effects of stroke injury on the shear modulus of the lower leg muscle during passive dorsiflexion.

Contractures are common complications of a stroke. The spatial location of the increased stiffness among plantar flexors and its variability among survivors remain unknown. This study assessed the mechanical properties of the lower leg muscles in stroke survivors during passive dorsiflexions. Stiffness was estimated through the measurement of the shear modulus. Two experiments were independently conducted, in which participants lay supine: with the knee extended ( experiment 1, n = 13 stroke survivors and n = 13 controls), or with the knee flexed at 90° ( experiment 2, n = 14 stroke survivors

Quantitative measurement of intra-compartmental pressure of the leg in acute traumatic injury: As a routine trend.

BACKGROUND: Experience and literature regarding complications of lower extremity compartment syndrome led us to hypothesize that delayed diagnosis and limb loss are potentially preventable events. Clinical examination does play a role, but quantification of compartment pressure reading serves as confirmation and determines the need for surgical intervention and provides the only objective data in case of conflict. METHODS: We performed a prospective study of all cases of closed tibial fractures presenting to our trauma centre over a 3-year period (January 2009-June 2012). Variables reviewed i
