Composite mantle cell lymphoma and T-cell prolymphocytic leukemia: a case report.

We encountered a patient with composite mantle cell lymphoma (MCL) and T-cell prolymphocytic leukemia (T-PLL) who presented with inactive disease to active T-PLL over 8 years. A 71-year-old man was diagnosed with MCL with an atypical T-cell population showing CD2+, CD3-, CD4+, CD7+, CD8-, and CD25+; however, the cause of the T-cell population could not be determined at the first MCL diagnosis. When MCL relapsed approximately 8 years after the initial treatment, T-PLL was definitively diagnosed using the T-PLL International Study Group criteria. MCL and T-PLL were determined to coexist in the l

[Transgenic mice expressing MTCP1: an animal model for T-cell prolymphocytic leukemia].

T-prolymphocytic leukemia (T-PLL) is a rare form of mature T-cell leukemia associated with chromosomal rearrangements of band 14q11, containing the gene TCRA/D, and bands 14q32.1 and Xq28, where the TCL1 and MTCP1 putative oncogenes have been identified. These genes encode two homologous proteins, p14(TCL1) and p13(MTCP1) respectively, which share no similarity with other known proteins. To determine the oncogenic role of MTCP1, transgenic mice with an expression of MTCP1 targetted to the T-cells were generated. A lymphoïd malignancy similar to Human T-PLL occurred in to independent transgenic

Burkitt-like mutations in the c-myc gene locus in prolymphocytic leukemia.

The c-myc oncogene recently shown to act as a transcription factor, is involved in cellular proliferation. Deregulation of this gene can be one step in malignant transformation. In Burkitt's lymphoma (BL) the c-myc gene is consistently involved in chromosomal translocations and the first exon of the gene has been found to be a frequent target of somatic mutations. These mutations are believed to interfere with normal transcriptional regulation of the gene. We demonstrate a case of the rare prolymphocytic leukemia (PLL), a variant of chronic lymphocytic leukemia (CLL), that shows multiple Burki

[Prolymphocytic B-cell leukemia with fatal acute liver failure. A case report].

Prolymphocytic leukemia (LPL) is a well defined entity with a relatively low incidence in our country. This disease usually is seen in patients over 50 years of age, and there is a very definite male preponderance. This lymphoproliferative disorder is characterized by very high white cells counts, massive splenomegaly, poor response to therapy and short term survival. The neoplastic cell in prolymphocytic leukemia usually is of B-cell origin (80% of cases). In our patient, affected by B-lineage prolymphocytic leukemia, a acute hepatic failure occurred, leading him to death in a short time. Aut

Prolymphocytic leukemia: a very satisfactory response to treatment with recombinant interferon alpha.

We report the case of a patient with prolymphocytic leukemia in whom a lengthy survival of 5 years was observed after treatment with splenic irradiation and chemotherapy. The patient obtained a surprising improvement with a significant reduction in the absolute count of the prolymphocytes and considerable reduction in splenomegaly after 3 months' therapy with interferon alpha.

Further characterization of prolymphocytic leukemia cells as a tumor of activated B cells.

Peripheral blood mononuclear cells from 24 patients with prolymphocytic leukemia (PLL) were isolated using a Ficoll-Hypaque gradient and stained by indirect immunofluorescence using a wide panel of monoclonal antibodies against B cell restricted and associated antigens, including HLA DR (Ia), CD19, CD21 (C3dR) surface membrane immunoglobulin (Slg), CD10 (CALLA), C3b, B5, CD25 (TAC), PCA1, T9, and T10. The cells were also tested for the FMC7, defined previously on PLL cells and the RAB1, a newly described hairy cell leukemia antigen. Thirteen out of the 24 samples expressed with variable intens

A Rare Presentation of T-cell Prolymphocytic Leukemia With Abnormal Uterine Bleeding.

T-cell prolymphocytic leukemia (T-PLL) is a rare and clinically aggressive T-cell neoplasm, which is composed of lymphoid cells that are of post-thymic T-cell origin. This is a case of a 57-year-old female with no significant medical history, who presented with a 4-month history of facial swelling, peripheral edema, dyspnea, palpitations, and abnormal uterine bleeding (AUB). A complete blood count demonstrated lymphocytosis, and a computed tomography (CT) scan of the abdomen and pelvis showed diffuse lymphadenopathy, splenomegaly, and uterine fibroids. A bone marrow biopsy confirmed the presen

Alemtuzumab monotherapy for T-cell prolymphocytic leukemia: an observational study in Japan.

Alemtuzumab is recommended as first-line and second-line therapies for T-cell prolymphocytic leukemia (T-PLL). This study retrospectively evaluated the efficacy and safety of alemtuzumab in nine Japanese patients with T-PLL at five participating institutions who were treated between January 2015 and August 2023. The median age at first administration of alemtuzumab was 72 years (range, 39 to 78). Two patients were treatment naïve, and seven had been treated with a median of one (range, 1 to 3) prior systemic therapy. Six patients were refractory to their most recent therapy. Three patients com

Development of HDAC Inhibitors Exhibiting Therapeutic Potential in T-Cell Prolymphocytic Leukemia.

Epigenetic targeting has emerged as an efficacious therapy for hematological cancers. The rare and incurable T-cell prolymphocytic leukemia (T-PLL) is known for its aggressive clinical course. Current epigenetic agents such as histone deacetylase (HDAC) inhibitors are increasingly used for targeted therapy. Through a structure-activity relationship (SAR) study, we developed an HDAC6 inhibitor KT-531, which exhibited higher potency in T-PLL compared to other hematological cancers. KT-531 displayed strong HDAC6 inhibitory potency and selectivity, on-target biological activity, and a safe therape

An unusual case of T-cell prolymphocytic leukemia mimicking a cutaneous vasculitis.

T-cell prolymphocytic leukemia (T-PLL) is an aggressive post-thymic T-cell malignancy, which accounts for 2% of mature lymphocytic leukemias in adults. Though typically presenting with a brief history of B symptoms, hepatosplenomegaly, and marked lymphocytosis, erythematous or nodular skin rashes involving the trunk or limbs may be seen in 25% to 30% of patients, as well as a purpuric rash in a periorbital distribution. Cutaneous involvement typically presents in the context of patients with an established history of T-PLL, but it can less frequently present as an initial symptom heralding the

[Splenic rupture associated with aggressive conversion of indolent T-cell prolymphocytic leukemia].

A 65-year-old man was diagnosed with leukocytosis in a routine medical examination. Further laboratory examinations showed increased LDH and sIL-2R levels in the serum. There was no evidence of infiltrative lesions or organomegaly. Bone marrow aspiration revealed many atypical small-sized lymphocytes without apparent nucleoli. Flow cytometric analysis of atypical lymphocytes was positive for T-cell markers, and chromosome analysis showed a normal karyotype. He was diagnosed with the small cell variant of T-PLL. Approximately 34 months later, having received no treatment, his cervical lymph nod

We report a 79-year-old woman with T-cell prolymphocytic leukemia (T-PLL) who was successfully treated with fludarabine monophosphate. She was admitted to our hospital because of dyspnea on effort. On admission, anemia and hepatosplenomegaly were apparent but lymphadenopathy was absent. Peripheral blood examination showed anemia and leukocytosis with 29.5% abnormal lymphocytes. The bone marrow was infiltrated with 84.1% abnormal lymphocytes. The nucleolus was visible in some of these abnormal cells. These cells were positive for CD2, CD3, CD4, CD5, CD7, CD38, CD52, and negative for CD8, CD10,

[Treatment of T-cell prolymphocytic leukemia with monoclonal anti- CD52 antibody (alemtuzumab].

INTRODUCTION: T-cell prolymphocytic leukaemia (T-PLL) is a rare post-thymic T-cell malignancy with an aggressive clinical course. It has generally been resistant to alkylating chemotherapy, but some effect has been observed with the purine analog 2-deoxycoformicin with documented partial or complete response rates in up to 45% of patients. Treatment with monoclonal antibodies against CD 52 has been shown to be highly effective in T-PLL with response rates of up to 76%. This may allow for further consolidating treatment with high-dose chemotherapy with autologous stem cell support or allogeneic

Successful treatment of B-cell prolymphocytic leukemia with monoclonal anti-CD20 antibody.

We report a patient with B-cell prolymphocytic leukemia (PLL) who was treated successfully with the monoclonal anti-CD20 antibody (rituximab). The patient had recurrent infections due to relative neutropenia, secondary to bone marrow infiltration. After treatment with monoclonal anti-CD20 antibodies (rituximab) 375 mg/m(2) weekly for 4 weeks, complete remission was obtained. It was documented by normalization of peripheral blood counts, disappearance of organomegaly, and by molecular cytogenetics-fluorescent in situ hybridization (FISH) on bone marrow cells. She remains in complete remission 8

Nucleolated variant of mantle cell lymphoma with leukemic manifestations mimicking prolymphocytic leukemia.

Chronic lymphoproliferative disorders sometimes can be difficult to classify. We report 4 cases characterized by large cells with distinct central nucleoli, reminiscent of prolymphocytic leukemia, but shown on further workup to represent mantle cell lymphoma. At initial examination, the patients had generalized lymphadenopathy, splenomegaly, and a leukemic blood picture. The peripheral blood showed many large cells with round to slightly irregular nuclei, single central nucleoli, and a fair amount of pale cytoplasm. The picture was not typical of prolymphocytic leukemia because of the presence

Human T-lymphotropic virus type I provirus and T-cell prolymphocytic leukemia.

T-cell prolymphocytic leukemia (T-PLL) is a rare type of post-thymic T-cell neoplasm, the etiology of which is unknown. Patients with T-PLL have been found to be seronegative for human T-lymphotropic virus type-I (HTLV-I), and their leukemia cells do not retain monoclonally integrated HTLV-I provirus. Recently, we have demonstrated the presence of defective HTLV-I provirus by polymerase chain reaction in the DNA extracted from peripheral blood cells or affected lymph nodes of T-PLL patients. Although there is a possibility, from our observation, that an alternative mechanism is operating in HT

[Leukemic B-cell colony formation in patients with B-cell prolymphocytic leukemia].

We performed in vitro B cell colony assay after coculture with phytohemagglutinin-P (PHA-P) and some cytokines (IL-2, IL-4, IL-6 and PHA-MTCM) in B leukemic cells from B-PLL and B-CLL patients. The number of colonies generated with PHA and PHA-MTCM was significantly smaller in patients with B-PLL than B-CLL. The surface markers of colony cells from a PLL patient were similar to those of circulating leukemic cells of the patient. IL-2, IL-4 and IL-6 did not enhance PHA-induced B cell colony formation in B-PLL. The expression of CD23 and CD25 antigens of the PLL cells was not enhanced after cocu

Bone marrow histological patterns in B-cell and T-cell prolymphocytic leukemia.

Bone marrow trephine biopsies from 30 cases of prolymphocytic leukemia (PLL), 16B and 14T were studied. The bone marrow infiltration by the neoplastic prolymphocytes showed the following four histological patterns: interstitial, mixed (interstitial-nodular), mixed (interstitial-diffuse), and diffuse. The most frequent pattern was the mixed (interstitial-nodular) seen in 40% of cases. In comparison with available data from patients with chronic lymphocytic leukemia, the bone marrow involvement in PLL differs by the absence of the pure nodular pattern and by the presence of a mixed (interstitial
